Využití prvků terapeutické komunity v práci s rizikovou mládeží / Running terapeutic community for youth with risky behaviours

The master thesis deals with the applicability of the model of therapeutic community in the work with youth at risk. The aim of the thesis is to provide information about possible connection of these two phenomena based on theory and Czech practice. The thesis has a theoretical and an empirical part. The chapters of the theoretical part describe target groups from the view of adulthood development, the model of therapeutic community and its possible application in various diagnostic circles. The last chapter of the theoretical part describes the employment of the model of therapeutic community in the work with youth at risk in a historical perspective. Next, it suggests that the model of therapeutic community is compatible with the attainment of developmental goals during maturing. The empirical part maps the usage of elements of therapeutic community model in selected facilities which work with youth at risk. It informs whether these elements occur and if so, what shape they take in practice.

This is a sub-study project on the effect of ADHD on selected mental functioning, life skills and quality of life of clients recovering from addiction in therapeutic communities (Miovský et al ., 2014), which aims, among other things, describe the negative effects of ADHD on the treatment process in the therapeutic community. The incidence of ADHD symptoms in the treatment of methamphetamine dependence is a major complication of treatment. The aim of this thesis is to describe methamphetamine sub-group specifics in the research sample, their assessment in the TC's treatment process and ADHD diagnosis specifity in this group, constitutes up to 70 % of all treatment demand clients (Mravčík, et al., 2014). The method used for data acquisition are: questionnaire, test, interview, observation, computerization of data (IBM SPSS Statistics 20 was used). For data analysis was used descriptive statistics (descriptive and mathematical-statistical analytical processing). Qualitative data were analyzed by using method of clustering, text coloring and simple enumeration. The research sample consisted oh 116 respondents (83 men and 32 women) aged 16-47 years. The thesis describes a demonstration of the use of drama and psychodrama approach as a method when working with a drug dependent client during the treatment in a therapeutic community. In the theoretical part I focus on clarification of terms associated with the name of thesis, especially with the concepts of dramatherapy, psychodrama, catharsis, abreaction, corrective emotional experience. Furthermore, the theoretical section contains information on individual methods and possibilities of their use. The main objective of thesis was to illustrate a specific therapeutic method by the analysis of particular work, in my case dramatherapy and psychodrama. In the practical part I analysed from available documents and observing to which extent dramatherapy and psychodrama may be beneficial for a client in the treatment in the therapeutic community and demonstrated their impact on the client. The outcome of thesis is a description of each session, which can serve as an inspiration for starting dramatherapists working with drug dependent clients. Furthermore, it is also the analysis of client's development, a progress in the treatment and a thorough analysis during and after the sessions. The HR refers to policies and support programs whose purpose is to reduce the risks related to the use of AD, without necessarily decrease individual consumption. This research aim was to analyze the HR conceptions and practices at two specialized institutions for AD users: 1) Psychosocial Care Center Alcohol and Drugs III (CAPSad III); 2) Therapeutic Community Fazenda da Esperan?a (FE) Dom Bosco. This is a qualitative research that used the following methodological tools: semi-structured interview with 21 professionals; socio demographic file and rounds of conversation with 63 participants users; participant observation and field journal. The interviews with professionals have characterized HR as a less complex and cheap treatment. At FE the HR proposal does not make part of their actions, being considered ―against the human being dignity‖. At CAPSad III is understood as an ―inevitable‖ guideline to service, once users do not remain abstinent. The users understand RD as an improvement in healthy conditions, social relations and work that occurs with the decreasing consumption of AD. They use the HR when they avoid situations that facilitate AD consumption, share relapse prevention strategies and, exclusively at CAPSad III, decrease psychotropic consumption. Stands out as an analyzer the HR comprehension as a less efficient treatment that opposes to the objective of both institutions, which is abstinence. Cultural variables affecting client/therapist consonance : the perception of efficacy in arts therapies group treatment

This thesis addresses the hypothesis “ Intragroup cultural differences between client and therapist will adversely affect client – therapist consonance in their perception of arts therapies group treatment” The literature review of intercultural psychotherapy, arts therapies and congruence research is contextualised in a discussion of the arts therapies in the UK, in particular group therapy in psychiatry. The discussion of the evolution of a multi modal research design incorporates an ethnographic perspective. The researcher shows how the setting and two pilot studies as well as the Evidence Based Practice initiative influenced the design. The main concepts in the research question are defined and the sample analysed within its local context. Helping and hindering factors in arts therapies group sessions are identified through cluster analysis of questionnaires and focus groups. The next stage of the analysis examines which client, therapist and treatment variables are shown to affect dissonance. Five case studies show the interaction of these variables for individual clients. The concluding chapter discusses the findings and critiques the methodology, as well as providing recommendations for further research. The hypothesis of the research is found invalid; cultural background variables alone do not create client-therapist dissonance. The findings show that client, therapist and treatment variables interact to create dissonance. Client diagnosis, stage of treatment and cultural background interact with their experience of the arts therapies medium. In an arts therapy group context the structuring of the group and the interpretation of the arts expression as symbol or index, will interact with client and therapist cultural background variables. The intragroup variations are migration history, nationality, religious orientation and first language spoken. Cultural difference with the therapist affecting dissonance was evident for those clients who were third generation English / British and who had grown up and were still resident in an non-urban area (small town or village in a predominantly agricultural region) with little cultural diversity. Intergroup difference affected attrition for one client, influenced more by peer than therapist dissonance. Treatment interruptions, the theoretical orientation of the therapists and peer dissonance interact with the client-therapist dissonance. Recommendations for practice are formulated from these findings. These concern adjusting practice to allow for a greater emphasis on expression and play, differing client perceptions about symbolism and the establishing of an early therapeutic alliance.

The Religious Working Group (WG) was created by the State Programme of STD/AIDS in Sao Paulo to be an area in which the various religious traditions could talk and think about prevention strategies with the technical staff responsible for the health of the population. The booklet was published by public funds under the supervision of professionals in the Reference and Training Centre STD/AIDS . Through multipliers workshops for the public of various churches, the "AIDS and the Churches" has been used as a pedagogical tool for orientation and training. We explore the contents of this material, by analysing the most relevant issues, such as their goals and methodology. We emphasize the meaning of the term "therapeutic community" suggested by the ecumenical organisation, as a model of reception and care to people living with HIV/AIDS. We examine how the issues of AIDS, Sexuality and Dogma are related in the publication. We also assess which praxis used by Koinonia in the application of the booklet in the workshops for the multipliers. In the final considerations, we emphasize the cooperation between the State and Koinonia, through the publication of the booklet "AIDS and the Churches: A Call to Action," to overcome stigma and discrimination against people living with HIV / AIDS. / No ano de 2005 foi publicado o Caderno AIDS e Igrejas: um Convite à Ação , idealizado pela organização KOINONIA- Presença Ecumênica e Serviço e tendo como objetivo instigar nas igrejas protestantes, respostas de prevenção e de cuidado diante da pandemia de HIV/AIDS. O Grupo de Trabalho Religiões (GT) foi destinado pelo Programa Estadual de DST/AIDS para ser um espaço no qual as diversas matrizes religiosas pudessem dialogar e pensar em estratégias de prevenção juntamente com técnicos responsáveis pela saúde da população. O Caderno foi publicado mediante recursos públicos com a supervisão dos profissionais do Centro de Referência e Treinamento em DST/AIDS. One of the difficulties to competent and humane care of these users in the Unified Health System - SUS has been shown through studies that users of alcohol and other drugs, as being the target of stigma and prejudicial attitudes within the health services. The unpreparedness regarding training for this kind of attention is another barrier to be overcome. Sought to question the role of the psychologist as a worker overlaps in these health institutions, as this professional has been in increasing numbers among the components of multidisciplinary teams of health facilities. The relevance of this research is to contribute to a reflection on the inclusion of psychologists in public policy healthcare for users of alcohol and other drugs seeking to bring elements that contribute to the improvement of profession. The objective of this research was to understand the relationship between the meanings attributed by psychologists to alcohol and other drugs and its implications in the treatment of users of Psychosocial Care Centers - CAPS ad and the Therapeutic Communities. Was then carried out a search using qualitative interviews with three psychologists these institutions. The interviews were analyzed using the technique of content analysis and had the help of the software Atlas - Ti 5.2 in the preparation of the results. Based on interviews of psychologists surveyed, it was found that the substances are seen by them as being essentially negative properties that would have the ability to subjugate the subject to a state of illness and dependency. Furthermore, the meanings attributed to drug use as a negative imply ideological differences on the issue of Harm Reduction, while policy carefully since this is understood by psychologists as a strategy for reducing the use of substances. The major implication of these senses occurs in the field of practice in which one realizes that most of the activity is intended to provide strategies for reducing the use of substances, not being included interventions in complex contexts involving the process of drug addiction. Realized that even acknowledging stigma user, interventions to reduce this stigma need to be more effective. Most practices are performed by psychologists in individual and group consultations indicate that the bias predominantly clinical formation. It takes the aim of these practices become increasingly broad in order to intervene in complex contexts involving the health of drug addicts. / A atenção aos problemas decorrentes do uso e dependência de drogas tem sido um desafio atual para a saúde pública brasileira. Uma das barreiras para a assistência competente e humanizada destes usuários no Sistema Único de Saúde – SUS tem sido evidenciada através de estudos que apontam os usuários de álcool e outras drogas, como sendo alvo de estigmatização e posturas preconceituosas no interior dos serviços. O despreparo com relação a formação para este tipo de atenção é outra barreira a ser superada. Buscou-se problematizar o papel do psicólogo, enquanto trabalhador imbricado nestas instituições, visto que este profissional tem estado em número cada vez maior entre os componentes das equipes multiprofissionais de estabelecimentos de saúde. A relevância desta pesquisa consiste em contribuir com uma reflexão sobre a inserção do psicólogo nas políticas públicas de assistência à saúde aos usuários de álcool e outras drogas buscando trazer elementos que contribuam para o aprimoramento da profissão. O objetivo desta pesquisa consistiu em compreender a relação existente entre os sentidos atribuídos por psicólogos ao uso de álcool e outras drogas e suas implicações no tratamento de usuários dos Centros de Atenção Psicossocial – CAPS ad e nas Comunidades Terapêuticas. Realizou-se então uma pesquisa de natureza qualitativa utilizando de entrevistas com três psicólogos destas instituições. As entrevistas foram analisadas com base na técnica de Análise de Conteúdo e tiveram o auxílio do software Atlas – Ti 5.2 na elaboração dos resultados. Com base nas entrevistas dos psicólogos, percebeu-se que as substâncias são vistas por eles como tendo propriedades essencialmente negativas que teriam a capacidade de submeter os sujeitos a um estado de enfermidade e dependência. Além disso, os sentidos atribuídos ao uso de drogas como algo negativo implicam em divergências ideológicas quanto à questão da Redução de Danos, enquanto política de cuidado, visto esta ser compreendida pelos psicólogos como uma estratégia de redução do uso de substâncias. A maior implicação destes sentidos se dá no campo das práticas onde se percebe que a maior parte das atividades tem como objetivo oferecer estratégias para a redução do uso de substâncias, não sendo incluídas intervenções nos complexos contextos que envolvem o processo da dependência de drogas. Percebeu-se que mesmo reconhecendo a estigmatização do usuário, as intervenções para a redução deste estigma carecem de serem mais efetivadas. A maioria das práticas realizadas pelos psicólogos são atendimentos individuais e grupais que indicam o viés predominantemente clínico de sua formação. É preciso que o objetivo destas práticas tornem-se cada vez mais amplos a fim de intervirem nos complexos contextos que envolvem a saúde dos dependentes de drogas.

Apesar de muitos estudiosos entenderem a etiologia da dependência química como multidimensional, sendo os fatores sociais os elementos mais determinantes neste processo, Comunidades Terapêuticas tendem a entender a etiologia como unidimensional, seguindo o modelo de doença. Assim como tem sido feito com os fatores de risco, há hoje uma crescente preocupação em se estudar os fatores de prevenção e proteção contra o abuso de álcool e outras drogas, fatores esses encontrados no campo individual, familiar e social do indivíduo. As comunidades terapêuticas, modelos de tratamento informais para pessoas com dependência de álcool e outras drogas, surgiram no Brasil antes mesmo que houvesse qualquer política pública sobre álcool e outras drogas e são, habitualmente, baseados no Modelo Minnesota, em preceitos religiosos ou em uma combinação de ambos. Participaram da pesquisa nove egressos de uma comunidade terapêutica. Os instrumentos utilizados na coleta de dados foram: entrevista semiestruturada, no qual foi feita a análise de conteúdo; questionário socioeconômico e teste para triagem do envolvimento com fumo, álcool e outras drogas, no qual foi feita análise quantitativa a partir da estatística descritiva para caracterização da amostra. Os resultados apontam a busca religiosa/espiritual como o principal dos três grupos de ferramentas apreendidas durante o tratamento na comunidade terapêutica, seguido da importância dos novos vínculos criados/suporte do novo grupo de apoio e do suporte psicológico recebido durante a internação. A religiosidade/espiritualidade está presente entre as estratégias e é endossada como muito importante pela maioria dos entrevistados. Há influência da religiosidade/espiritualidade durante o tratamento, sendo percebida por todos os participantes como foco principal do mesmo. Essa influência interferiu na religiosidade/espiritualidade da maioria destes egressos, trazendo uma “conversão”, mudança na doutrina anterior ao tratamento e maior envolvimento, busca de relacionamento com Deus após o tratamento. Quanto à representação da comunidade terapêutica para esses egressos, todos concordam com a importância dessa modalidade de tratamento, e referenciam o lugar como excelente, local de aprendizagem, refúgio e abrigo. Os mesmos elogiam o tratamento recebido, através do amor e carinho percebidos na execução do trabalho dos psicólogos e monitores. / This research aimed to study the mechanisms of coping in relapse prevention in alcohol and other drugs in a therapeutic community graduates in the city of Juiz de Fora. Currently dependence on alcohol and other drugs is concern widely disseminated and discussed, being a serious social and public health. The etiology of addiction is multidimensional, but social factors are the most crucial in this process. As has been done with the risk factors, there is now a growing concern in studying the factors of prevention and protection against abuse of alcohol and other drugs, these factors are found in the individual, familiar and social fields. Therapeutic communities, informal treatment models for people with alcohol and other drugs' problems, emerged in Brazil even before there was any public policy on alcohol and other drugs and are usually based on the Minnesota Model in religious precepts or a combination both. This research counted with Nine participants. The instruments used in data collection were semi-structured interview; socioeconomic questionnaire and screening test for the involvement with tobacco, alcohol and other drugs. Quantitative analysis from descriptive statistics to characterize sample. The results show the religious/spiritual search as the main of the groups of tools seized during treatment in the therapeutic community, followed by the importance of the new groups/support established and psychological support received during treatment. The religiosity/spirituality is present between the strategies and is endorsed as very important by most subjects. There is influence of religiosity/spirituality during treatment, being perceived by all participants as the main focus of it. This influence interfered in religiosity/spirituality of the majority of these participants, bringing a change in doctrine after treatment and greater involvement, seeking relationship with God after treatment. Regarding the representation of the therapeutic community for these graduates, all agree on the importance of this treatment modality, and refer the place as excellent learning place, refuge and shelter. They rave about the treatment received by the love and affection in the perceived performance of the work of psychologists and monitors.

Evaluace výsledků léčby v terapeutické komunitě pro drogově závislé Martin Šefránek Abstrakt: Východiska: Výzkumem podložené závěry prokazující efektivitu léčby v terapeutických komunitách pro léčbu závislostí a umožňující mezinárodní srovnání jsou v ČR ojedinělé. "Evaluace výsledků léčby v terapeutické komunitě pro drogově závislé" je v ČR první prospektivní výzkumná studie hodnoticí efektivitu léčby uživatelů drog v terapeutických komunitách. Cil: Cílem studie je vyhodnotit, jak se změnilo užívání drog, kriminalita, osobní a sociální fungování a kvalita života uživatelů drog 1 rok po ukončení léčby. Metody: Data byly sbírány prostřednictvím strukturovaných interview s klienty terapeutických komunit. Studie porovnává změny v problémovém chování klientů ve 30 dnech před začátkem léčby a ve 30 dnech po 1 roce od jejího ukončení. Základním výzkumným nástrojem je the Maudsley Addiction Profile, který jsme doplnili o další měření. Soubor: Výzkumný soubor tvoří 176 uživatelů drog, kteří v letech 2007 a 2008 zahájili léčbu v rezidenční terapeutické komunitě. 1 rok po ukončení léčby byly data získány od 77.8% (N=137) klientů.
