Psychosocial predictors of posthumous organ donation intention: a comparison among Chinese, Japanese and American adults. / CUHK electronic theses & dissertations collection

By path analysis, it was found that only self-efficacy and subjective norm, but not general attitudes, significantly predicted individuals' BID across all samples. Moreover, in contrast to the hypothesis, results showed that after-death anxiety predicted self-efficacy but not attitudes. Low after-death anxiety, accurate knowledge regarding organ donation, and high subjective norm promoted self-efficacy. Altruism was the main determinant of attitudes but its indirect impact on BID was weak. The applicability of the model on all four samples was acceptably high, and the configural invariance of the model was generally supported across three ethnic groups. / Previous studies have shown that the general public exhibits favorable attitudes toward posthumous organ donation, but a low donor card signing rate. Hence, positive attitudes may not be a good predictor of the behavioral commitment to donate organs. This dissertation was a cross-cultural study on the actual behaviors of signing or taking away the donor card (i.e. b&barbelow;ehavioral i&barbelow;ntentions to d&barbelow;onate organs after death (BID). It aimed at proposing an integrative, content-specific but culture-general model for posthumous organ donation. The model was composed of two levels of influences on BID, namely, the proximal level (including self-efficacy toward signing the donor card, general attitudes toward posthumous organ donation, and subjective norm for posthumous organ donation) as well as the distal level (including knowledge regarding posthumous organ donation, altruism, and after-death anxiety). The model was first tested and refined among 517 Chinese college students (Study 1), and was then further validated with 290 Chinese community adults (Study 2) in Hong Kong. The applicability of the model was also examined with a Western control sample of 217 Caucasian American college students (Study 3) and a non-Chinese Asian sample of 670 Japanese college students (Study 4). Psychosocial characteristics of three college samples were then compared, and ethnic differences on predictive values of psychosocial factors on BID were studied (Study 5). / The ANOVA results challenged the old assumption of underlying similarities in psychosocial characteristics across Asian ethnic groups. As expected, Americans were the most likely to show BID, followed by Chinese, while Japanese had the lowest tendency to show BID. Papel da angiotomografia no diagnóstico de morte encefálica: revisão sistemática / Role of computed tomography angiography in diagnosing brain death: a systematic review

Introdução: Transplantes de órgãos ocorrem principalmente devido a doações provenientes de pacientes que apresentam morte encefálica (ME). Algumas situações limitam o diagnóstico de ME baseado apenas no exame neurológico, sendo necessário utilizar um exame de imagem ou gráfico para esta confirmação. No Brasil, o exame complementar é obrigatório por lei para todos os casos suspeitos. A maioria dos métodos complementares utilizados para confirmação de ME não está disponível em muitos locais do Brasil. Neste contexto, a angiografia por tomografia computadorizada (ATC) representaria uma alternativa, devido à presença de equipamentos de tomografia em diversos hospitais brasileiros. Porém, a capacidade de este exame reconhecer a interrupção da circulação intracraniana é desconhecida. Métodos: Realizou-se revisão sistemática para verificar evidência na literatura sobre o uso de ATC como teste avaliador de ME. Foram seguidas diretrizes de busca e extração de dados, sendo o QUADAS 2 utilizado para verificar risco de vieses e qualidade dos estudos. Os dados foram sumarizados para produzir metanálise. Resultados: Dez estudos com alto risco de vieses foram encontrados. Devido à falta de estudos controlados, não se obteve dados de especificidade. Trezentos e vinte e dois pacientes foram elegíveis para metanálise, a qual revelou 84,7% de sensibilidade. Houve variação de protocolos de avaliação das imagens de ATC entre os estudos, sobre a definição de resultados positivos ou negativos. Conclusão: ATC apresenta alta sensibilidade para detectar interrupção de circulação intracraniana entre pacientes com avaliação clínica compatível com ME. Este nível de evidência é similar ao de outros métodos utilizados no mundo. Há falta de estudos bem desenhados neste tema / Background: Organ transplantation depends more often of donation from brain dead (BD) individuals. Several complications make the diagnosis of BD medically challenging and a complimentary method is needed for confirmation. Additionally, in Brazil, the complimentary diagnosis is mandatory by law, despite there are still many areas where these methods are not available. In this context, computed tomography angiography (CTA) could represent a valuable alternative, because of its widespread presence. However, the reliability of CTA for confirming brain circulatory arrest remains unclear. Methods: A systematic review was performed to identify relevant studies regarding the use of CTA as ancillary test for BD confirmation. Guidelines for online search were followed, and the QUADAS 2 tool was used to verify study quality. Data from the studies retrieved were extracted aiming to perform the meta-analysis. Results: Ten low quality studies were found. Due to the absence of controls in all studies, specificity could not be calculated. Three hundred twenty-two patients were eligible for the meta-analysis, which exhibited 84,7% sensitivity. CTA image evaluation protocol exhibited variations between medical institutions regarding which intracranial vessels should be considered to determine positive or negative test results. Conclusions: For patients who were previously diagnosed with BD according to clinical criteria, CTA demonstrated high sensitivity to verify intracranial circulatory arrest. The current evidence that supports the use of CTA in BD diagnosis is comparable to other methods applied worldwide. Método: Efetuou-se um estudo observacional retrospectivo em 402 pacientes transplantados entre 1997 a 2012. Excluíram-se os pacientes que realizaram técnica tradicional (standard), uso de enxertos duplos ou reduzidos e os com dados incompletos. Avaliaram-se os dados do doador e do receptor. Para cada paciente foi calculado o escore BAR por meio do site http://www.assessurgery.com/bar-score/bar-score-calculator, cuja escala varia de 0 a 27 pontos.Os pontos de corte para BAR foram definidos por meio da área sob a curva ROC. A análise estatística foi efetuada usando-se método de Kaplan-Meier (teste log-rank) para SV, teste de Mann-Whitney para comparação entre grupos e análise regressão logística múltipla. Resultado: A SV aos três meses para BAR ? 11 foi de 46% e para BAR < 11 de 77% (p = 0,001); a SV aos 12 meses para BAR ? 11 foi de 44% e para BAR < 11 de 69% (p = 0,001). A análise de regressão múltipla mostrou como fatores determinantes para SV < três meses: escore BAR ? 11 pontos (OR = 3,08; IC95% = 1,75 - 5,42; p = 0,001) e uso de concentrado de hemácias(CH) acima de 6 unidades(u) no intraoperatório (OR = 4,49; IC95% = 2,73 - 7,39; p = 0,001) e para SV < 12 meses: escore BAR ? 11 pontos (OR = 2,94; IC95% = 1,67 - 5,16; p = 0,001) e CH > 6u intraoperatório (OR = 2,99; IC95% = 1,92 - 4,64; p = 0,001). Conclusão: Este estudo demonstra uma habilidade moderada do escore BAR, e realça a importância da hemotransfusão como fator preditor de sobrevida nesta população / Abstract: Background: The balance of risk (BAR) is a prediction system after liver transplantation. Methods: To assess the BAR system a retrospective observational study was performed for 402 patients who had transplant surgery between 1997 and 2012. BAR score was computed for each patient. Receiver operating characteristic (ROC) curve analysis with the Hosmer-Lemeshow test were used to calculate sensitivity, specificity and model calibration. Cutoff value were selected with the best Younden index. Statistical analysis employed the Kaplan-Meier method (log-rank test) for survival, the Mann-Whitney test for group comparison, and multiple logistic regression analysis. Results: 3-month survival was 46% for BAR ? 11 and 77% for BAR < 11 (p = 0.001); 12-month survival was 44% for BAR ? 11 and 69% for BAR < 11 (p = 0.001). Factors of survival < 3 months were BAR ? 11 (OR = 3.08; IC95% = 1.75 - 5.42; p = 0.001), and intrasurgical use of packed red blood cells (RBC) above 6 units (OR = 4.49; IC95% = 2.73 - 7.39; p = 0.001). For survival < 12 months, factors were BAR ? 11 (OR = 2.94; IC95% = 1.67 - 5.16; p = 0.001), and RBC > 6 units (OR = 2.99; IC95% = 1.92 - 4.64; p = 0.001). Introdução: As doenças da córnea são responsáveis por cerca de 5% da cegueira reversível no mundo e o transplante de córnea (TC) é importante para o tratamento dessas enfermidades. A partir de fontes de dados oficiais e públicas, foram analisados o progresso e as dificuldades relacionados ao TC no Brasil nos últimos 16 anos, bem como desigualdades regionais, gastos do Sistema Único de Saúde (SUS) e indicadores de qualidade dos bancos de tecido ocular (BTOs). Métodos: Estudo retrospectivo e analítico com dados sobre TCs e BTOs no Brasil, no período de janeiro de 2001 a dezembro de 2016, divulgados pelo Sistema Nacional de Transplantes (SNT), pela Associação Brasileira de Transplante de Órgãos (ABTO) e pela Agência Nacional de Vigilância Sanitária (ANVISA). Para verificação de existência de tendência, comparação de médias entre as regiões e verificação da diferença de médias, foram utilizados o teste de Cochran-Armitage, a Análise de Variância e as comparações múltiplas de Duncan, respectivamente. Em todos os testes foi utilizado um nível de significância de 5%. Resultados: No Brasil, houve aumento: de 2,4 vezes no número de TCs (de 6.193 - 35,2 pmp para 14.641 - 71,0 pmp - p < 0,001); de 50,7% na eficácia do atendimento à demanda populacional por TCs (de 35,3% para 53,2% - p < 0,001); de 27,8% no número de globos e córneas in situ doados (de 24.608 - 127,1 pmp para 31.450 - 152,6 pmp - p < 0,001); de 31,7% nas córneas preservadas (de 21.012 para 27.674); de 2,4 vezes no gasto financeiro total com TCs (de R$ 9.179.688 para R$ 22.060.973); e de 2,2 vezes no gasto unitário com TC (de R$ 716 para R$ 1.603). A fila de espera para TC reduziu em 45,4% (de 23.549 - 123,0 pmp para 12.865 - 62,4 pmp - p < 0,001). As duas principais causas para a não doação foram as contraindicações médicas (média de 42,5%) e a recusa familiar (média de 36,6%). As principais causas de descarte de córneas foram a sorologia positiva para hepatite B (média de 33%), validade tecidual (média de 30,9%) e qualidade imprópria do tecido (16,8%). A Eficácia na Preservação de Córnea (EPC), o Coeficiente de Descarte de Córnea (CDC) e a Eficácia no Fornecimento de Córnea para Transplante (ECT) foram em média 88%, 37% e 63% ao longo dos anos, respectivamente. Os melhores índices foram apresentados nas regiões Sul, Centro-Oeste e Sudeste e os piores no Norte e Nordeste. Conclusão: No Brasil, o pequeno número de doações e a grande taxa de descarte de córneas são as principais dificuldades ao adequado atendimento à demanda populacional por TCs. The Cochran-Armitage test, the Analysis of Variance and the Duncan\'s multiple comparisons were used to verify the existence of trend, comparison of means between regions and verification of the mean difference, respectively. A significance level of 5% was used in all tests. Results: In Brazil, there was an increase: of 2.4 times in the number of CTs (from 6,193 - 35.2 pmp to 14,641 - 71.0 pmp - p < 0.001); of 50.7% in the efficacy of meeting the population demand for CTs (from 35.3% to 53.2% - p < 0.001); of 27.8% in the number of donated globes and corneas in situ (from 24,608 - 127.1 pmp to 31,450 - 152.6 pmp - p < 0.001); of 31.7% in preserved corneas (from 21,012 to 27,674); of 2.4 times in the total finance expense with CTs (from R$ 9,179,688 to R$ 22,060,973); and 2.2 times the unit expense with CT (from R$ 716 to R$ 1,603). The waiting list for CT decreased by 45.4% (from 23,549 - 123.0 pmp to 12,865 - 62.4 pmp - p < 0.001). The two main causes for non-donation were medical contraindications (mean of 42.5%) and family refusal (mean of 36.6%). The main causes of corneal discard were positive serology for hepatitis B (mean of 33%), tissue validity (mean of 30.9%) and inadequate tissue quality (16.8%). Efficacy in Corneal Preservation (EPC), Corneal Discarding Coefficient (CDC) and Efficacy Supply of Corneas for Transplantation (ECT) averaged 88%, 37%, and 63% over the years, respectively. The best indexes were presented in the South, Midwest and Southeast regions and the worst in the North and Northeast. Conclusion: In Brazil, the small number of donations and the high rate of discard of corneas are the main difficulties to the adequate attendance to the population demand by CTs. However, the country increased the ability to transplant corneas and reduced waiting lists in 16 years
