Avaliação das propriedades mecânicas de ossos de coelhas submetidas à administração de glicocorticóides / Evaluation of mechanical properties of the bone´s of rabbits induced glucocorticoid administration

O corticóide é usado nas várias especialidades médicas por ser um fármaco de potente efeito anti-inflamatório e imunossupressor, no entanto, é capaz de produzir algumas alterações metabólicas quando administrado por uso prolongado. Desta forma o presente estudo teve como objetivo avaliar as possíveis alterações nas propriedades mecânicas de osso cortical e trabecular de coelhas albinas, quando submetidas à administração de corticoesteróide em altas doses e por tempo prolongado, através de ensaio mecânico de flexão em três pontos em tíbia e fêmures avaliados como estrutura, além de ensaios mecânicos em corpos de prova por flexão em três pontos em fêmures e compressão em vértebras (L5). Para o estudo foram utilizados 39 animais, divididos aleatoriamente em dois grupos, experimental (GE) e controle (GC). Estes grupos foram divididos em quatro subgrupos, sendo 2 experimentais e 2 controles. Na investigação estrutural foram avaliadas as tíbias e fêmures direito por ensaios mecânicos de flexão em 3 pontos, e na investigação como material foram realizados ensaios mecânicos de flexão em 3 pontos para fêmures e compressão para vértebras (L5) através de corpos de prova. A metodologia utilizada para esta pesquisa foi igual para todos os animais, no entanto, o grupo experimental foi administrado durante 21 dias com o medicamento metilprednisolona (Solumedron) diluída em solução salina numa proporção de 2 mg/Kg/dia, e o grupo controle foi somente administrada solução salina na mesma dosagem. Todos os ensaios mecânicos foram realizados na Máquina Universal de Ensaio do Laboratório de Bioengenharia - FMRP/USP. Durante os ensaios foram registrados os valores das deflexões e as respectivas cargas aplicadas, sendo posteriormente confeccionadas curvas carga versus deflexão para cada osso ensaiado. As propriedades mecânicas determinadas para os fêmures e tíbias analisadas de forma estrutural foram: a carga aplicada no limite de proporcionalidade, deflexão no limite de proporcionalidade, carga máxima, rigidez e a resiliência de cada osso. Já para os fêmures e vértebras analisados como material ósseo, foram avaliados suas tensões máximas através da curva carga versus deflexão. Comparações estatísticas foram realizadas entre os resultados encontrados, através do teste T-Student, com nível de significância estabelecido em 5% para todos os parâmetros analisados. Nas análises estatísticas entre os grupos investigados não foram observadas diferenças significativas em nenhuma das comparações, porém observou-se ao final deste estudo uma perda de peso corporal significativa do grupo experimental. Desta forma foi evidenciado através dos resultados encontrados nesta pesquisa que as propriedades mecânicas investigadas não apresentaram alterações significativas com o protocolo de administração de medicamento, sugerindo-se que novos experimentos sejam realizados, com modificações de dosagem e tempo de aplicação. / Corticoteroids are used in many clinical conditions because they present strong anti-inflammatory and imunessupressor activities. But, at the same time, they can cause many metabolic alterations and side effects mainly when there is prolonged. In the present research we studied the possible alterations caused by steroids on the mechanical properties of lamellar and trabecular bone of rabbits. The mechanical properties were assessed by bending tests performed on intact femurs and tibial as well as in samples of cortical bone. Compression tests were performed in L5 vertebral. Thirty-seven female rabbits were randomly distributes in one experimental group (EG-animals) and control (CG animals). Such groups were divided into four subgroups: two experimentals and two controls. The experimental animals received 2mg/kg/day of methylprednisolone (Solumedron ® ) during three weeks. The control animals received the same volume of intramuscular injections of saline, once a day, during three weeks. From the load x deformation curves the load and deflexion were obtained at the yielding point. The ultimate load as well as resilience were also obtained for the intact bones. When the specimens were analysed the ultimate tension was determined. The statisitical analyses did not show any difference between treated and untreated animals for the mechanical properties. But the treated animals showed a significant loose of body weight. We ful that such results require a deepening in the investigation.

O objetivo neste estudo foi avaliar a possível relação entre os estágios de maturidade óssea avaliados em radiografias de mão e punho e das vértebras cervicais em telerradiografias em norma lateral, em indivíduos com idade média de 13 anos e 6 meses e desvio padrão de 2 anos e 3 meses (variando de 8 anos e 6 meses a 16 anos e 11 meses). A casuística foi composta por 209 conjuntos radiográficos (radiografias de mão e punho e telerradiografias em norma lateral) tomados na mesma data para cada indivíduo . Dois avaliadores, devidamente treinados e calibrados classificaram por estágios de maturidade óssea todas as radiografias, as de mão e punho segundo Helm et al, e a avaliação das vértebras cervicais nas telerradiografias em norma lateral, segundo O’Reilly e Yaniello (1988) e Baccetti, Franchi e McNamara (2002). Considerando a classificacão de Helm et al (1971) como padrão áureo, teste de correlação de Spearman foi aplicado para verificar relação com os dois métodos que avaliam as vértebras cervicais. Os resultados indicam que houve forte correlação entre os métodos de avaliação das vértebras cervicais e de mão e punho (Rs= 0,906 para O’Reilly e Yaniello(1988) e Helm et al. (1971) e Rs= 0,889 para Baccetti, Franchi e McNamara Jr (2002) e Helm et al. (1971), para todos p<0,001). A análise estatística descritiva indicou que houve maior número de ocorrências concordantes na identificação do início e do pico máximo do surto. Com base nesses resultados pode-se concluir que a avaliação da maturidade óssea pelas vértebras cervicais oferece confiabilidade para a identificação do início e pico do surto de crescimento puberal, mas não para a identificação dos estágios na fase descendente, portanto, continuar usando a radiografia de mão e punho ainda se faz imprescindível quando é necessária a identificação de algum potencial de crescimento restante . / The purpose of this study was to evaluate the possible relationship between the stages of skeletal maturation analyzed by means of hand-wrist radiography and cervical vertebrae in lateral cephalograms, from individuals with mean age of 13 years and 6 months, S.D. 2 years and 3 months (ranging from 8 years and 6 months to 16 years and 11 months). The sample was composed by 209 radiography sets (hand-wrist radiography and lateral cephalograms) taken in the same date for each individual. Two investigators, properly trained and calibrated, classified all the hand-wrist radiography relating them to the skeletal maturation according to Helm et al. The cervical vertebrae evaluation was performed in lateral cephalograms by means of the O'Reilly and Yaniello and Baccetti, Franchi and McNamara method. Considering the classification of Helm et al as “gold standard’, Spearman test correlation was applied to verify relationship with the two methods that evaluate the cervical vertebrae. The results indicated that there was strong correlation among the methods of evaluation of the cervical vertebrae and hand-wrist radiography (Rs =0,906 for O'Reilly and Yaniello and Helm et al and Rs = 0,889 for Baccetti et al and Helm et al, for all p <0,001). The descriptive statistical analysis indicated that there was larger number of concordant occurrences in the identification of the onset and maximum peak of the adolescent growth spurt. Based on these results, it can be concluded that the evaluation of the skeletal maturation by means of the cervical vertebrae method can offer reliability only for the identification of the onset and peak of adolescent growth spurt. Thus, is still indispensable to continue using the hand-wrist radiography when it is necessary the identification of some potential remaining growth.

As cobras são um grupo de vertebrados extremamente bem sucedido. Apesar da aparente simplicidade, elas conseguiram ocupar uma grande diversidade de habitats. O esqueleto axial das serpentes apresenta relação com vários aspectos da vida que puderam ter tido influência direta em seu sucesso evolutivo. Além disso, a forma dos elementos vertebrais possui valor taxonômico, fato que desperta particular interesse, visto que a maior parte do registro fóssil deste grupo é constituído por elementos vertebrais desarticulados. A morfometria geométrica utiliza informações sobre a posição de pontos em duas ou três dimensões, localizados sobre estruturas anatômicas para análise estatística da variação da forma. Esse conjunto de técnicas tem se mostrado eficaz em avaliar variações da forma que não seriam diagnosticadas por métodos tradicionais. O presente trabalho buscou avaliar as vértebras do esqueleto pré-cloacal de Serpentes no contexto da morfometria geométrica, considerando as influências ecológicas e filogenéticas sob estas estruturas, e investigando ainda a utilidade taxonômica da forma vertebral. Os resultados apresentados aqui sugerem a existência de uma padronização do esqueleto axial de serpentes que é conservada ao longo da história evolutiva desse grupo. Análises ecomorfológicas demonstram que a forma vertebral é fortemente influenciada por hábitos de vida como fossorialidade e constrição. A despeito da ausência de sinal filogenético, as análises de classificação de elementos vertebrais dissociados demonstram que a forma vertebral pode ser utilizada para a diagnose de vértebras isoladas. Esses resultados reforçam a necessidade de realização de análises de variação longitudinal para a identificação correta de material fóssil referente a este grupo. / Snakes are an extremely successful group of vertebrates. Despite their apparent morphological simplicity, they managed to occupy a great diversity of habitats. The axial skeleton of snakes has relationships with various aspects of their life history that could have had direct influence in the evolutionary success of these animals. Furthermore, the shape of the vertebrae is recognized as being of taxonomic value, a fact that elicits a special interest, since the majority of the fossil record of this group is composed of disarticulated vertebral elements. Geometric morphometrics methods use information about the position of landmarks in two or three dimensions, placed over anatomical structures for statistical analyses of shape variation. This group of techniques has been shown to identify shape variations that would not be diagnosed through traditional methods. The present work sought to evaluate the vertebrae of the pre-cloacal skeleton of Serpentes in the context of geometric morphometrics, evaluating the influences of ecology and phylogeny over the shape of these structures, and also investigating the taxonomic usefulness of vertebral shape. Results presented here suggest the existence of a patterning of the axial skeleton that is conserved throughout the evolutionary history of the group. An ecomorphological analysis demonstrates that the vertebral shape is highly influenced by life-history traits such as fossoriality and constriction. Despite the absence of phylogenetic signal, analyzes of classification of dissociated vertebral elements show that the vertebral shape can be used to diagnose isolated vertebrae. These results reiterate the necessity of performing studies of longitudinal variation in order to correctly identify fossil material associated with this group.

Structure and function of the elastic fibre network of the human lumbar anulus fibrosus.

Degeneration of the lumbar intervertebral disc, a condition widely implicated in the cause of low back pain among adult humans, is typically characterised by progressive biochemical and structural changes to the extracellular matrix. Comprehensive descriptions of the structural and functional inter-relationships within the extracellular matrix are therefore critical to understanding the degenerative process and developing effective treatments. In the anulus fibrosus, this matrix has a complex, hierarchical architecture comprised of collagens, proteoglycans, and elastic fibres. Elastic fibres are critical constituents of dynamic biological structures that functionally require elasticity and resilience. Studies to date of elastic fibre network structure in the anulus fibrosus have been qualitative and limited in scope. Additionally, there is poor understanding of the structural and functional associations between elastic fibres and other matrix constituents such as collagen, and, critically, there have been no studies directly examining the nature and magnitude of the contribution made by elastic fibres to anulus fibrosus mechanical behaviour. In this thesis, multiple experimental studies are described that specifically examine each of these areas. Novel imaging techniques were developed and combined with histochemistry and light microscopy to facilitate the visualisation of elastic fibres at a level of detail not previously achieved. Examination of elastic fibre network structure revealed architectural differences between the intralamellar and interlamellar regions, suggesting that elastic fibres perform functional roles at distinct levels of the anulus fibrosus structural hierarchy. The density of elastic fibres within lamellae was found to be significantly higher in the lamellae of the posterolateral region of the anulus than the anterolateral, and significantly higher in the outer regions than the inner, suggesting it may be commensurate with the magnitude of the tensile strains experienced by each region of the disc in bending and torsion. The nature of the structure-function associations between elastic fibres and collagen was then examined with respect to the reported structural mechanisms of collagen matrix tensile deformation. Histological assessment of collagen crimp morphology in specimens from which elastic fibres had been enzymatically removed revealed no observable differences when compared with controls, suggesting that any contribution made by elastic fibres to maintaining crimp is minimal. Elastic fibres in anulus fibrosus specimens subjected to radial tensile deformations exhibited complex patterns of re-arrangement, suggesting that they maintain cross-collagen fibre connectivity. Elastic fibres were also observed to maintain physical connections between consecutive lamellae undergoing relative separation. Finally, the nature and magnitude of the contribution made by elastic fibres to anulus fibrosus mechanical properties at the tissue level was investigated using a combination of biochemically verified enzymatic treatments and biomechanical tests. Targeted degradation of elastic fibres resulted in a significant reduction in both the initial modulus and the ultimate modulus, and a significant increase in the extensibility, of radially oriented anulus fibrosus specimens. Separate treatments and mechanical tests were used to account for any changes attributable to non-specific degradation of glycosaminoglycans. These results suggest that elastic fibres enhance the mechanical integrity of the anulus fibrosus extracellular matrix in the direction perpendicular to the plane containing the collagen fibres. In summary, the results of the studies presented in this thesis provide important new insights into the structure and function of the anulus fibrosus elastic fibre network, and highlight its potential importance as a contributing or ameliorating factor in the progression of the structural and mechanical changes associated with intervertebral disc degeneration.
