Comparison of Normal- and High-Tension Glaucoma: Nerve Fiber Layer and Optic Nerve Head Damage

Purpose: The aim of this study was to investigate differences in the nerve fiber layer and glaucoma-induced structural optic nerve head (ONH) damage in patients with normal- (NTG) and high-tension (HTG) glaucoma. Methods: In this retrospective pair-matched comparative study, 22 NTG and 22 HTG eyes were matched according to the same glaucomatous damage based on rim volume, rim area and disk size, as measured by Heidelberg retinal tomography (HRT III). Visual fields (VF) were assessed by Humphrey perimetry, and nerve fiber layer thickness was determined both by scanning laser polarimetry (GDxVCC) and spectral-domain optical coherence tomography (SD-OCT). Comparisons of all measured parameters were made between NTG and HTG groups. Results: Based on HRT results, both NTG and HTG eyes displayed comparable structural damage to the ONH (NTG/HTG, mean: disk area, 2.30/2.31 mm 2 , p = 0.942; rim area, 1.02/0.86 mm 2 , p = 0.082; rim volume, 0.19/0.17 mm 3 , p = 0.398). NTG eyes had significantly less VF damage than HTG eyes (NTG/HTG, mean deviation: –4.23/–12.12 dB, p = 0.002; pattern standard deviation: 5.39/8.23 dB, p = 0.022). The inferior nerve fiber layer of NTG patients was significantly thicker than that of HTG patients (NTG/HTG, mean: GDx inferior: 53.5/46.3 μm, p = 0.046). SD-OCT revealed a significantly thicker nerve fiber in NTG compared with HTG patients in all quadrants (NTG/HTG, total mean: 72.72/58.45 μm, p = 0.002). Conclusion: At comparable glaucomatous stages, nerve fiber loss was more advanced in HTG patients compared with NTG patients.

Métodos: Trinta e seis pacientes com glaucoma primário de ângulo aberto e perda de campo visual em um hemicampo (afetado) e ausência de perda no hemicampo oposto (não afetado), e 36 controles pareados por idade tiveram o olho de estudo examinado com Stratus-OCT (Carl Zeiss Meditec Inc., Dublin, Califoprnia, USA) e o 3DOCT-1000 (Topcdon, Tokyo, Japan). As medidas de espessura da CFNR peripapilar e a classificação normativa fornecida pelos aparelhos foram registrados para análise. A média aritmética dos valores do mapa total deviation em cada hemicampo (mean deviation do hemicampo) foi calculada para cada indivíduo. \"Ìndices de assimetria\" para o campo visual e para a CFNR foram calculados como a razão entre o mean deviation dos hemicampos afetado e não-afetado, e como razão entre a espessura da CFNR das hemirretinas afetada e não-afetada, respectivamente. As variáveis contínuas foram comparadas usando os testes de Mann-Whitney, Kruskal-Wallis ou Wilcoxon, quando apropriados. As variáveis categóricas foram comparadas usando o teste qui-quadrado de Pearson. O coeficiente de correlação de Spearman foi usado para testar as correlações entre as medidas de espessura da CFNR fornecidas pelos OCTs. A presença de afinamento da CFNR foi estabelecida com base nos dados normativos fornecidos pelos softwares dos OCTs. As espessuras de CFNR fora do intervalo de previsão de 95% para a mesma faixa etária foram consideradas anormais. Resultados: As medidas de CFNR corespondentes a hemicampos não-afetados [média (DP) 87,0 (17,1) um e 84,3 (20,2) um, para TD e SD-OCT, respectivamente] foram menores do que as dos controles [média (DP) 119,0 (122,2)um e 117,0 (17,7) um, para TD e SD-OCT, respectivamente, P < 0,001, para ambos]. O banco de dados normativo classificou como alterado 42% e 67% das hemirretinas correspondentes a hemicampos não-acometidos com TD e SD-OCT, respectivamente (P = 0,01). As medidas da CFNR foram consistentemente mais espessas com TD comparadas com SD-OCT. Os índices de assimetria da CFNR em pacientes com glaucoma foram semelhantes entre TD [média (DP) 0,76 (0,17)] e SD-OCT [média (DP) 0,79 (0,12), P = 0,89] e significantemente maiores do que o índice de assimetria do campo visual [média (DP) 0,36 (0,20), P < 0,001]. Peripapillary RNFL thickness measurements and normative classification were recorded for analysis. Total deviation values were averaged for each hemifield (hemifield mean deviation) for ecch subject. The RNFL thicknesses outside the prediction interval of 95% for the same age group were considered abnormal. <p>Докторска дисертација припада области информационих система, са јасним акцентом на употребу неуронских мрежа за решавање проблема вишеструких зависних временских серија&nbsp; који је у&nbsp; овом докторату дефинисан.Основни циљ дисертације је креирање система у форми едукативног софтвера путем којег ће се обучавати младе кошаркашке судије</p><p>Један од кључих елемената овог&nbsp; доктората јесте симулација хоризонталног видног поља на основу којег се&nbsp; утврђује да ли је резоновано кретање кошаркашких судија било адекватно&nbsp; или није. Стога развијени софтвер поседује споменуту едукативну примену.</p><p>Како би се реализовао споменути софтвер спроведено је истраживање које је обухватило обучавање великог броја традиционалних вишеслојних перцептрона као и формирање посебне&nbsp; LTR&nbsp; &ndash;&nbsp; MDTS структуре неуронске мреже за коју се сматра да је погодна за решавање постојећег проблема. The main objective of the&nbsp; thesis is to create a system in the form of educational software that will be used druring the training of young basketball referees.One of the key elements of this doctorate is a simulation of a horizontal field of&nbsp; vision&nbsp; on the basis of which it is determined whether the movement of&nbsp; reasoned&nbsp; basketball referees was adequate &nbsp; or not.&nbsp; Therefore developed software has&nbsp; aforementioned educational use. In order&nbsp; to realize the aforementioned software, a&nbsp; research&nbsp; was conducted that included training of a large number of traditional multilayer perceptron neural networks and the&nbsp; formation of special LTR&nbsp; -&nbsp; MDTS&nbsp; neural network&nbsp; structure which is considered to be&nbsp; suitable&nbsp; for solving the presented problem. For the realization of the simulation&nbsp; of the horizontal field of vision a large number of algorithms in the field of computer graphis was considered&nbsp; and Sweep and Prune algorithm partially provided the basis for the developed and&nbsp; currently implemented algorithm.</p>

Influence de la dépendance au champ visuel dans la construction et le maintien d’une posture verticale inversée en milieux terrestres et aquatiques / Influence of visual field dependence in building and maintaining an upside-down posture in terrestrial and aquatic environments

La contribution relative des différentes entrées sensorielles dans le contrôle postural a souvent été étudiée dans le cadre de la posture érigée fondamentale. L’objectif de nos travaux a été de déterminer cette contribution dans deux postures relativement proches dans leur configuration : l’appui tendu renversé (ATR) et la verticale inversée (VI) en milieu aquatique. Si la vision est souvent considérée comme information principale dans le contrôle postural de la station érigée, la configuration structurelle (champ visuel restreint en ATR, immersion de l’œil en VI) de ces deux postures laisse imaginer un fonctionnement différent. La perturbation des différents capteurs sensoriels impliqués dans le contrôle postural a permis d’observer la réorganisation mise en place par le système nerveux central (SNC) pour y remédier. De plus la prise en compte d’un facteur perceptif comme la dépendance au champ visuel nous a paru être un élément pertinent pour tenter d’observer d’éventuelles différences interindividuelles dans les comportements des sujets. La perturbation du contrôle postural a été évaluée au travers d’une analyse cinématique et d’une analyse stabilométrique de l’ATR (analyse classique et non-linéaire). Les résultats de ces analyses laissent entrevoir des différences de stratégie entre les sujets dépendants et indépendants au champ visuel dans le contrôle de l’appui tendu renversé, ce qui ne semble pas être le cas en verticale inversée / The relative contribution of the different sensory inputs in erect postural control has often been studied. The aim of this work is to determine this contribution in two positions relatively close in their configuration: the handstand and the upside-down posture in water. If vision is often considered as the main information in postural control, the structural configuration (restricted visual field in handstand, eye’s immersion in upside-down posture) of these two postures lets imagine a different operation. Disruption of sensory receptors involved in postural control has allowed observing the reorganization implemented by the central nervous system (CNS). In addition, the visual field dependence appeared to be a relevant factor to observe interindividual behavioral differences. Disturbance of postural control was assessed through a kinematic analysis and a stabilometric analysis of the handstand (classical analysis and non-linear analysis). Results of these analyzes suggest different strategy according to visual field dependence in the control of the handstand, which does not seem to be the case in upside-down posture
