Neurocognitive outcomes in HIV and childhood trauma

Thesis (PhD)--Stellenbosch Univesity, 2011. / ENGLISH ABSTRACT: It is well established that South African women are disproportionately affected by HIV/AIDS and gender based violence. Research to date has provided evidence for neurocognitive decline in individuals infected with HIV/AIDS and in individuals who have experienced early life trauma. However, many gaps remain in our knowledge about the neurocognitive profile of HIV and childhood trauma in South African women. The present study focused on the neurocognitive effects of HIV infection and childhood trauma, both separately and in combination in South African women. The primary aim of the study was to assess neurocognitive functioning in HIV-positive and matched HIVnegative controls, with and without a history of childhood trauma. Moreover, the study sought to assess the synergistic relationship between HIV and childhood trauma in influencing neurocognitive outcomes, a relationship which has not yet been investigated. A neuropsychological battery sensitive to HIV-related impairments was administered to 83 HIV-positive and 47 matched HIV-negative women with histories of childhood trauma. A history of childhood trauma was assessed using the Childhood Trauma Questionnaire short form (CTQ-SF). Forty eight of the 83 HIV-positive women were exposed to childhood trauma. Among the control subjects, a total of twenty women were exposed to childhood trauma. Findings of the present study revealed neurocognitive deficits in memory and executive functions. Results demonstrated significant HIV effects in memory (HVLT-R learning and delay trials), and executive functions (Halstead Category test). Similarly, a trauma effect was evident in delayed recall (HVLT-R delay). Moreover, results revealed a significant interaction effect between HIV status and trauma status on the WAIS-III Symbol Search Task, a task of psychomotor speed. However, HIV-negative controls with a history of childhood trauma scored the highest on this task. Although this finding was unexpected, it may suggest that psychomotor speed may not be a sensitive or discriminating test of childhood trauma in healthy adults. The present study demonstrated evidence for HIV and trauma effects in the ability domains of learning and delayed recall and executive functions. Although the present study did not find evidence for a synergistic relationship between HIV and trauma, it did provide evidence for both HIV and trauma effects on neurocognition, a finding in keeping with previous studies. Future research should be prospective in nature and should better delineate the nature, severity, and temporal relationship of childhood trauma to neurocognitive outcomes, as well as the mediators and moderators of these outcomes. / AFRIKAANSE OPSOMMING: Dit is alombekend dat Suid-Afrikaanse vroue buite verhouding swaar deur MIV/vigs en geslagsgebaseerde geweld getref word. Hierdie studie konsentreer op die neurokognitiewe uitwerking van MIV-infeksie en kindertrauma, afsonderlik sowel as gesamentlik, op Suid-Afrikaanse vroue. Die hoofdoel van die studie was om neurokognitiewe funksionering by MIV-positiewe vroue te bepaal en dit met gepaste MIV-negatiewe kontrolepersone te vergelyk, met én sonder 'n geskiedenis van kindertrauma. Daarbenewens wou die studie die sinergistiese verwantskap tussen MIV en kindertrauma in hul impak op neurokognitiewe uitkomste bepaal – 'n verwantskap wat tot dusver nog nie ondersoek is nie. Daarbenewens het die WAIS-II- (Wechsler-volwassene-intelligensieskaal) simboolsoekopdrag – 'n psigomotoriese spoedtoets – 'n beduidende wisselwerkingseffek tussen MIV-status en traumastatus getoon. Tog het MIV-negatiewe kontrolepersone met 'n geskiedenis van kindertrauma die beste in hierdie opdrag gevaar. Hoewel hierdie bevinding verrassend was, kan dit daarop dui dat psigomotoriese spoed dalk nie 'n gevoelige of diskriminerende toets van kindertrauma by gesonde volwassenes is nie. Die studie het bewys gelewer van MIV- en traumaverwante uitwerkings op korttermyngeheue en uitvoerende funksies. Hoewel die ondersoek nie bewyse van 'n sinergistiese verwantskap tussen MIV en trauma kon vind nie, het dit wél bevestig dat MIV en trauma neurokognitiewe werking beïnvloed – 'n bevinding wat in pas is met vorige studies. Toekomstige navorsing behoort ondersoekend te wees en die aard, felheid en tydgebondenheid van die verwantskap tussen kindertrauma en neurokognitiewe uitkomste, sowel as die mediator- en moderatorveranderlikes van hierdie uitkomste, beter te omskryf.

Women on executive and board levels in South Africas finance sector : why so few

Thesis (MBA)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: The study presents an empirical investigation that was conducted on why the number of women on executive and board levels in companies is not consistent with the number of women in the workforce. The barriers which women face in career progression were explored by focusing on three main themes, namely internal, societal and organisational barriers. The research method used to evaluate previous and current research was content analysis which provides a definitive correlation between current challenges that women face and how they have progressed over the past 20 years. The research further provides insight in what changes companies and government can incorporate to ensure that the gender gap is closed over time. Analysis shows that although the number of women on executive and board level has improved during the past ten to 15 years, it remains disconcertingly low. The type of barriers which hindered women from reaching the upper management positions some time ago, are still the same reasons that keep them in the lower ranks in the business. Findings have indicated that the strongest internal barrier is that women struggle to balance family obligations with the severe requirements of top positions in business. It was also found that some women do not perceive the male-dominated structures and discrimination in companies as a threat but rather see it as a challenge which strong women can overcome if they believe in and equip themselves. Although some companies have made progress in supporting women and invite women onto their boards and executive teams, the attempt is still not enough to close the gender gap. The present study concludes with recommendations on what women, male colleagues, companies and government bodies can do to increase the number of women on upper levels in the organisations.

The feminisation of poverty and female headship in post-apartheid South Africa, 1997-2006.

A large and growing body of scholarship has suggested that income poverty has recently decreased in post-apartheid South Africa. Evidence for an overall drop in poverty rates notwithstanding, there has been very little work which has examined the gendered nature of poverty. There have, however, been important changes over the period which might suggest that poverty trends have been gendered. On the one hand, for example, the post-apartheid period has seen the expansion of several grants to support the care-givers of children and the elderly as well as employment growth for women. On the other hand, this same period has been characterised by declining marital rates, rising rates of female unemployment, and women increasingly overrepresented in low-wage work, changes which would be expected to have negative implications for women's economic well-being. This thesis uses nationally representative household survey data from the October Household Surveys (1997 and 1999) and the General Household Surveys (2004 and 2006) to investigate gendered trends in income poverty in several different ways. It examines first, whether females are more likely to live in poor households than males, and whether this has changed over time; and second, how poverty has changed among female- and male-headed households. The thesis also considers why females and female-headed households are more vulnerable to poverty and why the poverty differential between males and females (and female- and male-headed households) may have widened over time. Given the criticism of headship based analyses of income poverty, the thesis also investigates poverty and female headship in greater detail by adopting several alternative definitions of female headship that are commonly used in the literature. / Thesis (Ph.D.)-University of KwaZulu-Natal, Durban, 2011.

The primary concern of the study is to examine leadership challenges confronted by women leaders in Eastern Cape municipalities. International organisations has made a number commitments encouraging gender parity and eliminating imbalance against women in the past three decades, through international platforms. In the World Summit held in 2005, it was recommended that a Platform for Action (1995) and the resolutions of the Twenty Third special session of the General Assembly as a crucial baseline in attaining then internationally agreed development goals, as well as those contained in the Millennium Declaration. (www.ilo.org) In the above forums national decision makers worldwide were urged to apply strategies and programmes which would improve gender fairness, in leadership positions, giving women complete and comparable share in economic, social, cultural and political decision-making as men. South Africa as a role player in the international space also joined the world in endorsing various programmes and strategies which seek to address gender parities and strive to eliminate women oppression especially in public sector.

Prevalence and affective outcomes of prenatal obsessive compulsive disorder amongst clinic attendees in the Capricorn District, Limpopo Province

(Clinical Psychology)) University of Limpopo, 2017 / The study investigated the prevalence of Obsessive-Compulsive Disorder (OCD) symptoms and their relationship with pregnancy-related anxiety, prenatal depression and clinical anger among African pregnant women. The sample consisted of 206 pregnant women attending their antenatal check-ups at the Mankweng, Nobody and Rethabile clinics, and Mankweng hospital in the Capricorn District, Limpopo Province. When correlational analysis was conducted, the patient characteristics of age, having undergone a medical check-up, and having previously delivered a live baby generally did not correlate with any of the main scales measuring OCD, namely, perinatal depression, pregnancy-related anxiety and clinical anger (p > 0.05). Findings from the study indicated that almost 81% of the pregnant women could be classified as obsessive-compulsive disordered, when using the Foa et al. (2002) cut-off score of 21. Furthermore, findings from the regression analyses indicated that higher age, the number of gestation weeks, having previously experienced pregnancy-related complications, perinatal depression, pregnancy-related anxiety and clinical anger were variably positive predictors of OCI-R measured OCD symptoms. The predictors are specific to each of the symptoms. It can be concluded from the study that there is a relationship between OCD symptoms and all the independent variables used. / National Research Foundation

Health and fitness of young, healthy adult females and the effect of an eight week pilates intervention

Purpose: The first phase of this study aimed to investigate the health and fitness status of young adult females in the local community. The second phase of this study investigated the impact of an eight week progressive Pilates intervention on selected health and fitness parameters in this cohort. Methods: Healthy young adult females aged 18-26 years (n=96), from the local community, partook in once-off tests including anthropometric measures (stature, body mass, Body Mass Index and waist circumference) as well as fitness parameters including balance, flexibility, muscular strength and endurance, and lastly lumbo-pelvic stability. Health measures investigated included blood pressure and spirometry. Where possible, results were compared to those of similar populations from national surveys or published normative data. Sedentary volunteers from this cohort were then randomised into a Pilates Exercise (PEx, n=12) or an inactive Control (Con, n=11) group, with their results from Phase 1 serving as baseline measures. Pilates classes were held twice weekly (60 minutes per session). All the measures from phase 1 were repeated at weeks 4 and 8. An additional intervention test included Transversus abdominis recruitment. Participants maintained habitual dietary intake and energy expenditure throughout. Nine PEx group and eight Con group participants completed the intervention. Results: The current sample (phase 1) was found to be healthier than comparative populations from national surveys, and significant differences (p<0.05) were found for all comparisons except Forced Expiratory Volume (FEV1), (p=0.64). Physical activity levels (230 min.week-1) exceeded that of the recommend weekly threshold (150 min.week-1). BMI, waist circumference, blood pressure and spirometry measures were all found to be within suggested healthy normal ranges. Pilates significantly improved lumbo-pelvic stability in the PEx group at weeks 4 (p<0.005) and 8 (p<0.002). Similarly, abdominal (p=0.00, d=1.1), upper limb (p=0.037, d=0.9) and lower limb endurance (p=0.02, d=1.0, between group d=0.73 for PEx) also improved with no changes in the Con group. PEx energy expenditure significantly increased from baseline to weeks 4 (p=0.007, d=10.7) and 8 (p=0.027, d=0.64), however body mass was maintained throughout. Conversely, Minute Ventilation decreased in the PEx cohort (p=0.010, d=0.95) from weeks 4 to 8. The Con group showed significant increases in body mass (p=0.018), leg strength (within-group Cohen’s d=-1.08 between weeks 0-8; d=-2 between weeks 4-8) and dynamic balance (p=0.01, d=-0.5). While no within-group changes were observed, Protein intake was significantly greater (p=0.036, d>0.8 at baseline and week 8) in the PEx group throughout the intervention. Medium between-group effect sizes (d>0.5) were noted for PEx BMI and waist circumference measures at all time points. Further, although not significant, the large within-group effect size (d=-0.84) between baseline and week 8 for PEx systolic blood pressure, suggested the 9 mm Hg was meaningful. The same time period also indicated a large within-group effect size (d=-0.8) for PEx dynamic balance, and a medium Cohen’s d for (d=0.57) PEx static balance. Conclusion: The local population of young adult females was found to be significantly healthier than those of comparable national samples. Further, Pilates participation significantly improved lumbo-pelvic stability and muscular endurance with meaningful changes in systolic blood pressure, and balance in previously sedentary young, adult females. Body mass was also maintained.

A lot of previous research survey about how SMMEs can improve women’s socio-economic challenges did not properly include those women living in the rural places such as KwaZulu-Natal. This study aims to analyse the use of SMMEs as a strategic tool for women’s socio-economic empowerment in rural northern KwaZulu-Natal. The study was conducted within the rural areas of northern KwaZulu-Natal using qualitative method. The sample for the study consisted of 250 respondents of rural women SMMEs. The respondents were selected using quota sampling. Respondents were asked to complete 2 page questionnaires with an interviewer present to assist. The data analysis was done by using SPSS version 24.0. The results were presented through tables and bar graphs. The main aim of the study was to investigate and describe the use of SMMEs as a strategic tool, to identify factors influencing the use and to examine to what extent they affect the use of SMMEs as a strategic tool for women empowerment. The findings of the study reveals that rural women SMMEs are mostly affected by external factors while on the other hand they also indicated that lack of proper training and entrepreneurial education have impact on the day- to -day running of their businesses. The study was limited by the exploratory nature and small sampling size. Therefore, generalisation of the findings should be done with care and further research is encouraged and should include other places in the area. / M

A reflection on the coping mechanisms of abused female breadwinners in the Vhembe District of Limpopo, South Africa

PhD (Sociology) / Department of Sociology / The purpose of the study was to explore and understand the circumstances, which influence abused female breadwinners to stay in nuptial contracts and to model coping strategies for their plight. The study was qualitative in nature and an explorative case study design of abused married female breadwinners who had reported cases at Thohoyandou Victim Empowerment Programme (TVEP) trauma centers in the Vhembe District of Limpopo Province, South Africa was used. A critical case purposive sample was used to draw a cross-sectional sample of 12 participants from a total population of 1800 female breadwinners who reported cases between 2010 and 2015. The data collection methods were; in-depth interviews, focus group discussions from the same participants and key informants‟ interviews from three TVEP female managers. The three data collection techniques ensured triangulation for more complete and well-validated outcomes of the study. The data analysis method was the Van Mann. The study found out that a plethora of circumstances that involves the following; culture, the desire to take care of children, culture, religion, social needs, social status and entrapment influences abused married female breadwinners to stay in abusive marriages. As a result, they are forced to devise some coping strategies that include; problem solving strategies, reporting to the police, protection orders, seeking refuge, alcohol abuse and dependence, counselling, religious intervention, social support to cope with their plight. Establishing the reasons was critical in coming up with intervention methods that may help to curb abuse and coping strategies that may assist them in their plight. / NRF
