Improving outcomes for young people with type 1 diabetesLockhart, Lorraine January 2016 (has links)
Aims: The thesis aimed to contribute to the current understanding of how to improve comprehensive health outcomes for children and adolescents with type 1 diabetes. Methods: A systematic review was undertaken to identify existing interventions designed to improve health-related quality of life in a paediatric diabetes population. The quality of identified studies was assessed and the effectiveness of the interventions was evaluated. Parent-adolescent dyads were also recruited via paediatric diabetes teams to participate in an empirical study. Participants were asked to complete questionnaires measuring psychological flexibility, mindfulness, perception of parental care and control, adherence to treatment and quality of life. Relationships were explored using correlation and regression analysis. Results: Twenty seven articles were identified in the systematic review. More than half were rated as “acceptable” or “high quality”. Quality of life was a primary treatment target in only three studies. Eight studies reported significant beneficial effects on health-related quality of life. In the empirical study, regression analysis found that both parent and adolescent diabetes-specific psychological flexibility predicted treatment adherence while adolescent mindfulness and insulin administration predicted quality of life. Conclusion: There is some evidence for the effectiveness of intensive structured education and coping skills training in improving health-related quality. However consideration should be given to developing theoretically informed interventions to target quality of life alongside other treatment related outcomes. The empirical study suggested psychological flexibility and mindfulness are useful constructs for understanding health outcomes in adolescents with type 1 diabetes. Acceptance and commitment, and mindfulness-based therapies may prove beneficial for improving outcomes in this population.

Literacy learning of adolescent students with intellectual disabilities : a case studyTwee, Pam, University of Western Sydney, Macarthur, Faculty of Education and Languages January 2001 (has links)
This study explores the learning, and specifically literacy learning, of intellectually disabled adolescent students. Factors that have influenced this learning throughout the adolescent's education are identified. Nineteen case studies of the literacy development of adolescent students with mild and moderate intellectual disabilities were developed through observations in their classrooms, and of related activities. The results of the study show that there were three main influences on literacy development for these students. These were the unique nature of the learner, home and family factors and the impact of school on the literacy learning of these students from their early learning and through their years of formal education. This research adds to the current research on the learning of adolescent students with intellectual disabilities by looking specifically at literacy development and using qualitative approaches to search within and beyond the classroom for issues which affect their learning.Practitioners in education and in the care of disabled children can use the study's findings to build a framework of knowledge to develop appropriate educational placements, programs and support for learning by drawing on significant aspects of the child's personal, social and educational development. / Doctor of Philosophy (PhD)

Night shift or non-standard work continues to grow throughout the economy (Beers, 2000). Women's roles in society are changing as they find themselves having to join the work force due to economic need. South African women most of whom are mothers are no exception in this regard. Many of these mothers are obliged to do night shift work. Sectors like manufacturing and public-oriented industries often use shift work to ensure efficient continuous operation and uninterrupted response to the needs of society. This article aims to explore the role that night shift work plays in the relationships between mothers and their adolescent children; whether the mutual perceptions of night shift working mothers and their adolescent children regarding their relationships differ from those of non-shift working mothers and their adolescent children; and whether there is a difference between the night shift working group and the non-shift working group with regard to the perceptions of their relationships. Lastly, the article aims to determine the reliability of the measuring instrument that was used in this study, namely the Parent-Adolescent-Relationship-Questionnaire (PARQ).
Available literature shows that shift work has a negative impact on health, for example
stress-related illnesses due to lack of sleep (Akerstedt, 1998, 2003; Fletcher & Dawson, 1997, 2001; Presser, 2005). Family life is also affected and relationship difficulties have been reported (Chang, Wang, & Liu, 1993; Holland, 2004; Grosswald, 2003, 2004; Presser, 1995, 2000, 2003, 2005), while shift work is seen as a threat to family cohesion. Adolescence is a critical developmental stage and mothers have an important role to enhance healthy adolescent development in terms of issues like self-identity and autonomy. The mother's unusual working hours can have a negative impact on the mother-adolescent relationship.
A cross-sectional design was used in this study. There were two groups: 35 night shift working mothers and their adolescent children, and 35 non-shift working mothers and their adolescent children. Participants were requested to complete a PARQ questionnaire. Data was analysed by means of SAS and SPSS programmes. Descriptive statistic methods such as central tendency, mean and median, variability, range and standard deviation, skewness and kurtosis were used to explore data. Independent and dependent t-tests were used to determine differences between the mean scores of the night shift working and non-shift working mothers and their adolescent children. The effect size was used to provide an objective measure of a practical effect.
The findings indicate that PARQ is a reliable measure for this study as it showed good internal consistency. The skewness and kurtosis indicate acceptable normality. Adolescent children of non-shift working mothers perceive communication with their mothers as more efficient than adolescent children of night shift working mothers, and there is an indication of a significant practical effect. Adolescent children of non-shift working mothers have a more positive perception of the Mothers' Problem Solving than adolescent children of night shift working mothers, with indications of a significant practical effect between the two groups. There is no statistically significant difference between and no significant practical effect in any of the variables concerning the two groups of mothers, as reflected in Table 4. However, Table 5 shows a difference between the perceptions of the two groups of adolescents. There is a statistically significant difference between Cohesion for non-shift working mothers and for their adolescent children, with an indication of a significant practical effect. Tables 5 and 6 show a statistically significant difference between Conventionalisation of the two groups, with an indication of a significant practical effect. Both night shift working and non-shift working mothers have a more positive perception of Conventionalisation than their adolescent children. Findings also indicate a statistically significant difference in Global Distress between night shift working mothers and their adolescent children, with an indication of a significant practical effect. Night shift working
mothers experience higher levels of Global Distress than their adolescent children. For both groups -night shift working mothers and their adolescent children; and non-shift working mothers and their adolescent children there is a statistically significant difference between the mothers' and the adolescents' perception of Ruination.
This study has several limitations and it is recommended that future studies use a larger sample size and include longitudinal studies. Future research should also explore the construct of the night shift working mother's parenting style and their coping strategies. Father-adolescent relationships should also be a research focus.

Grounded theory analysis of therapeutic interventions practiced by professionals in India and the UK with child and adolescent survivors of sexual abuseNarang, Javita January 2016 (has links)
There is a high prevalence of child sexual abuse (CSA) in India and the UK (Laccino, 2014). However, there is a lack of research on culturally-specific aspects of psychotherapy offered to child and adolescent survivors of CSA in both these countries. Therapeutic interventions with sexually abused children raise complex concerns due to the heterogeneity based on the developmental stage of children; varied impact, presenting difficulties and needs; and characteristics of abuse, age, gender, ethnicity and cultural factors. Numerous therapeutic interventions for CSA have been documented, with varying theoretical constructs, structure, content and outcome (Reavey & Warner, 2001), ranging from cognitive behavioural, psychoanalytic, humanistic and feminist paradigm (Misurell, Springer, & Tryon, 2011). Most of these approaches do not provide sufficient guides to therapy (Reavey & Warner, 2001). Few of these are widely studied and clinically accepted, while most lack empirical evidence. Although existing studies conclude that therapy is better than no treatment, there is lack of consensus on treatment characteristics important in child and adolescent sexual abuse therapy (Hetzel-Riggin, Brausch, & Montgomery, 2007). Further, little is known about culturally-specific holistic and coherent responses to CSA. A Constructivist Grounded Theory (Charmaz, 2006) study was conducted to investigate therapeutic approaches practiced by professionals in India and the UK with child and adolescent survivors of CSA, and to understand the factors that govern the choice of an approach or model adopted. In-depth, semi-structured interviews were undertaken with 32 professionals (16 each in India and the UK) from different settings including statutory, voluntary and private sector. NVIVO 10 was used for data management and analysis of the digitally recorded and transcribed interviews. Although there is a growing emphasis on evidence-based therapies, with most empirical studies focussing on traditional forms of psychotherapy, the actual practice of the professionals interviewed reflected a more fluid, flexible, multi-modal, ecological and integrative approach to CSA-therapy. The findings indicate that the therapeutic interventions progress through four different phases, based on the goals identified by professionals. The goals in turn are influenced by the socio-cultural context, structural factors, and/or trauma understanding of the professionals. Four phases identified in the study are: 1. Social Action Framework, with the goal to identify silenced and invisible children, particularly in a culture of silence and suppression in India; 2. Stabilisation and Resilience Building Framework, found to be practiced in India and the UK, with the goal to ensure safety and build the foundation for ensuing phase of therapy; 3. CSA-Trauma Resolution Framework, where the goal is to uncover the sexual abuse details to facilitate recovery and reintegrate the traumatised child, more visible in therapeutic practice in the UK; and 4. Maintenance and Relapse Prevention Framework, with the goal to prevent relapse and protect from revictimisation and future developmental difficulties. These phases may be mutually exclusive, follow a linear trajectory, or there may be a back and forth movement from one phase to the other. These four phases were integrated to construct a culturally relevant, ecological-based theoretical model of CSA therapy with child and adolescent survivors.

Homelessness has become a common phenomenon across the world. And its impact is felt in both the developed and developing countries. The situation in the Kingdom of Swaziland is more devastating due to the inability of the Government and other stakeholders to control and adequately mitigate the constant expansion of the phenomenon as a result of HIV/AIDS, poverty and inadequate care (Baggerly, 2006:162; National Coalition for the Homeless, 2004; Hlatshwayo, 1997).
The research problem was about the psycho-social challenges homeless adolescent children go through in a shelter in Manzini, Swaziland. It was deemed necessary and important to find out how they cope without professional and psycho-social support in the shelter. The goal of the study was to explore the psycho-social experiences of homeless adolescent children in a shelter in Manzini, Swaziland. The research approach was qualitative and the research type was applied. Data was collected by means of semi-structured interviews with eight homeless adolescent children in a shelter in Manzini, Swaziland.

Social Skills Training with High-Functioning Autistic AdolescentsEversole, Amy 08 1900 (has links)
Social skills training is a need among autistic adolescents. This investigation examined a social skills training program involving several teaching strategies. Specific social skills were targeted for improvement. Attempts to decrease negative social behaviors were made. Five autistic adolescents participated in the program and five were selected for the no-treatment group. Two measures were used. A survey addressing the skills targeted in the program was completed by parents and teachers before and after the program. A test conversation with a stranger and a peer was conducted with each subject before and after the program. Anecdotal information was obtained from therapists, teachers, and parents. Results provided information on the effectiveness of this social skills program. The benefits and limitations of the program were discussed.

Validity of Two Childhood Autism Rating Instruments for Use with Autistic AdolescentsMcCallon, Denise 05 1900 (has links)
It is now known that autism is a lifelong handicapping condition. While some of the characteristic behaviors of autistic children remain unchanged in adolescence and adulthood, there is evidence that other behaviors change as a function of development. Assessment instruments for identifying autism are generally intended for use with. young children and may not accurately assess autism in adolescents. Two studies compared autistic adolescents with matched autistic children and nonautistic adolescents on two autism rating scales. The validity of the Childhood Autism Rating Scale for use with adolescents was supported while the validity of the Prescreening Checklist was questioned. The findings were discussed in relation to the age-related changes which occur in autistic adolescents.
