Remifentanil preconditioning reduces post-ischaemic myocardial infarction and improves left ventricular performance via activation of the JAK/STAT signal pathway and subsequent inhibition of GSK3β in rats

Remifentanil is an ultra-short-acting phenylpiperidine opioid analgesic that is rapidly metabolized by nonspecific blood and tissue esterases. In clinical practice, remifentanil is now more commonly used during both cardiac and non-cardiac surgery than classic opioid agonists such as morphine, since it can be given in higher doses, is more titratable and enables fast recovery of patients in the postoperative period. Remifentanil preconditioning (RPC), achieved by intravenous remifentanil infusion interspersed with infusion-free periods before indexed ischaemia, attenuates cardiac ischaemia-reperfusion injury (IRI). This is experimentally manifested by reduced postischaemic myocardial infarct size (IS) and diminished markers of cardiac failure and apoptosis, and, clinically, by reduced release of biomarkers of myocardial cellular injury after cardiac surgery. However, the underlying mechanisms by which RPC has a cardioprotective effect need to be further explored. It’s generally considered that the Reperfusion Injury Salvage Kinase (RISK) pathway, triggering the expression of phosphatidylinositol 3-kinase (PI3K) as well as Akt, exerts a pivotal role in both classic ischaemic preconditioning (IPC) and pharmacological preconditioning induced cardioprotection. Moreover, recent studies show that the Survivor Activating Factor Enhancement (SAFE) signalling pathway, which involves signal transducers and activators of transcription-3 (STAT3) and janus activated kinase-2 (JAK2), also has an essential role in IPC. Although cross-talk has been found between the RISK and SAFE pathways, the SAFE pathway can function independently of the RISK to confer cardioprotection. However, the roles of JAK/STAT and PI3K/Akt signalling and, in particular, their relative importance in RPC-mediated cardioprotection have not been studied. I explored whether RPC confers cardioprotection via the JAK/STAT or PI3K/Akt pathway and its relationship with GSK3β inhibition. In first part of my study, I explored relative role of the JAK/STAT and PI3K/Akt which were involved in RPC cardioprotection using JAK2 and PI3K inhibition. Male Sprague-Dawley rats were either sham operated or randomly assigned to receive I/R alone or as well as RPC. Pretreatment with the JAK2 inhibitor AG490 or the PI3K inhibitor wortmannin was induced before ischaemia in rats. RPC reduced myocardial infarction and haemodynamic dysfunction induced by IRI accompanied with increased phosphorylation of STAT3 but not Akt or eNOS phosphorylation. AG490 but not wortmannin cancelled RPC’s cardioprotection. In addition, RPC attenuated hypoxia/reoxygenation induced cardiomyocyte apoptosis while STAT3 knock-out abolished the protective effects of RPC. These findings suggest that RPC confers cardioprotection primarily via activation of the JAK/STAT signalling but not the PI3K/Akt signalling pathway. The second study further investigated the role of GSK3β in RPC cardioprotection using the GSK3β inhibitor SB216763. I found that SB restored the ability of RPC to reduce the extent of myocardial infarction and CK-MB release despite the presence of AG490. The phosphorylation of GSK3β was increased by RPC. In addition, GSK3β gene knock-out with siRNA preserved RPC’s cardioprotection regardless of STAT3 abrogation indicating that GSK3β inhibition plays a critical role as a downstream effector in RPC mediated cardioprotection. Taken together with the evidence from this two part study, I conclude that RPC confers cardioprotection by activating the JAK/STAT and, subsequently, inhibiting GSK3β, a critical downstream effector of RPC cardioprotection. / published_or_final_version / Anaesthesiology / Master / Master of Philosophy

Objetivos: Avaliar a influencia do estresse sobre os pesos maternos, placentários e fetais, as alterações histológicas placentárias e a expressão gênica dos fatores angiogênicos em ratas prenhes. Métodos: De setembro de 2007 a julho de 2008, foi realizado um estudo experimental do tipo casocontrole, em que 6 ratas prenhes foram submetidas a estímulos estressantes crônicos e agudos enquanto que 6 animais constituíram o grupo controle. No 20o dia de prenhez, todas as ratas foram sacrificadas. Os seguintes dados foram analisados e comparados entre os grupos: a. pesos maternos, placentários e fetais; b. alterações histológicas placentárias; e, c. expressão gênica dos fatores angiogênicos (VEGF-A e PlGF), dos seus receptores (VEGFR-1 e VEGFR-2) e do fator induzido por hipoxia (HIF-1). Resultados: Os pesos maternos, placentários e fetais foram significativamente menores no grupo de ratas prenhes-estressadas em relação ao controle. Histologicamente foram encontradas a presença de núcleos picnóticos no grupo prenhe-estressado. Observou-se expressão gênica significativamente maior de VEGF-A no grupo rata prenhe-estressada assim como redução significante da expressão gênica de PlGF, VEGFR-1, VEGFR-2, quando comparadas ao controle. Não houve alterações entre os grupos para o gene HIF-1. Conclusões: No modelo animal de estresse estudado, observou-se alterações significativas no peso placentário e da expressão gênica dos fatores angiogênicos placentários em ratas submetidas ao estresse. / Objectives: to evaluate the influence of the stress on the maternal, placental and fetal weights, on the histological findings and on the genetic expression of the angiogenic factors in pregnant rats. INTRODUÇÃO: A síndrome compartimental abdominal (SCA) é uma complicação frequente da cirurgia de controle de dano, podendo ocorrer em até 33% dos casos. A medida da pressão intra-abdominal para diagnóstico da SCA é avaliada através da pressão intravesical. OBJETIVO: Avaliar, através de modelo experimental, as repercussões da colocação de compressas sobre a pressão intravesical e em diferentes regiões do abdome. MÉTODO: Foram utilizados 10 porcos machos, entre 45 e 60 dias, com peso entre 15 e 18 kg, submetidos a laparotomia exploradora para colocação de compressas e avaliação de medidas de pressão no espaço supra-hepático (Psh), espaço infra-hepático (Pih), espaço subfrênico esquerdo (Psfe), veia cava inferior (Pvci), veia porta (Pvp) e bexiga (Pv). Foi também avaliada a pressão arterial média (PAM) e o débito cardíaco. As diferentes medidas de pressão foram comparadas no mesmo animal com diferentes tipos de fechamento da parede abdominal: bolsa de Bogotá (CCB), total da aponeurose (CCAF), pele (CCPF) e total da pele e aponeurose (CF). Os resultados foram submetidos a análise estatística. RESULTADOS: Não houve diferença (p>0,05) entre as medidas de PAM em todas as fases de fechamento do abdome. A pressão de veia cava inferior e a pressão intravesical foram diferentes (p<0,05) entre os grupos CF e CCAF. Psfe, Psfd e Psh foram diferentes (p<0,05) entre CF e CCB, CF e CCPF e CF e CCAF. Foram diferentes (p<0,05) Pv e Psfd, Pv e Psh e Psfe e Psfd nos grupos CCB, CCPF e CCAF. No grupo CCAF ainda foram diferentes (p<0,05) Pv e Psfe e Psh e Psfd. A avaliação de Pvp mostrou-se diferente (p<0,05) em relação ao grupo controle (CF) nos grupos CCB, CCPF e CCAF, e ainda entre CCB e CCAF. CONCLUSÃO: A pressão intra-abdominal não é uniforme quando são colocadas compressas na cavidade peritoneal. A pressão vesical é menor que em outras regiões da cavidade peritoneal. Diagnosis of abdominal compartment syndrome depends on measurement of abdominal pressure which is usually assessed through intravesical pressure. OBJECTIVE: Evaluate the consequences of liver packing with sponges to the intravesical pressure and to pressures in different sites of the abdomen in an animal experimental model. METHODS: 10 adult male pigs, aged 45 to 60 days, weighing 15 to 18 kg, underwent laparotomies for liver packing and evaluation of pressures in suprahepatic space (Psh), infrahepatic space (Pih), left subphrenic space (Psfe), inferior vena cava (Pvci), portal vein (Pvp) and bladder (Pv). Other variables such as mean arterial pressure and cardiac output, were also measured. Different pressure measurements were compared in the same animal with different types of closures of the abdominal wall: Bogota bag (CCB), total closure of aponeurosis (CCAF), skin closure (CCPF) and total skin and aponeurosis closure (CF). Results were analyzed statistically. RESULTS: There was no difference (p>0,05) between mean arterial blood pressure in all phases of abdominal closure. Pressure in inferior vena cava and intravesical pressure were different (p<0,05) in groups CF and CCAF. Psfe, Psfd and Psh were different (p<0,05) between CF vs CCB, CF vs CCPF and CF vs CCAF. Pv vs Psfd, Pv vs Psh and Psfe vs Psfd were different (p<0,05) in groups CCB, CCPF and CCAF. In group CCAF, Pv vs Psfe and Psh vs Psfd were also different (p<0,05). Evaluation of Pvp has shown to be different (p<0,05) when compared to control group (p<0,05) in groups CCB, CCPF and CCAF, and also between CCB and CCAF. CONCLUSION: Intra-abdominal pressure is not uniform when sponges are placed in the abdomen. Intravesical pressure is lower than pressures in other sites of the peritoneal cavity. Translocação bacteriana na pancreatite aguda: efeito da administração de indometacina / Bacterial translocation in acute pancreatitis: effect of indomethacin administration

A ocorrência de translocação bacteriana durante a pancreatite aguda tem sido descrita como o principal fator responsável pela ocorrência de infecção pancreática. O objetivo deste trabalho foi determinar o efeito da administração da indometacina na ocorrência de translocação bacteriana e infecção pancreática. Foi utilizado um modelo experimental de pancreatite aguda com taurocolato de sódio 2,5%. Noventa ratos wistar machos foram divididos em três grupos: Sham, Pancreatite e Indometaciana. Foram analisadas, a ocorrência de translocação bacteriana e a incidência de infecção pancreática. A indometacina foi capaz de reduzir a população bacteriana e a incidência de infecção pancreática na pancreatite aguda experimental, no entanto a indometacina não foi capaz de alterar os índices de mortalidade / Increased gut bacterial translocation (BT) in AP has been correlated with pancreatic infection. Thepurpose of this study was to determinate if the BT and pancreatic infection can be reduced in severe AP after Indomethacin administration. An experimental model of severe AP by injection taurocholic acid 2.5%. Ninety male wistar rats were divided in 3 groups: Sham, Pancreatitis, and Indomethacin. We analyzed the occurrence of BT to the pancreas and pancreatic infection with bacterial cultures. Bacterial translocation or bacterial accumulation was not observed in the Sham group. We concluded based in our data that the administration of Indomethacin may reduce the bacterial population in the pancreas and the occurrence of pancreatic infection. Moreover Indomethacin could not reduce the mortality rate in this experimental model

A necessidade de cirurgia para substituição da aorta ascendente e valva aórtica com reimplante coronariano em grupos especiais de pacientes onde a anticoagulação é indesejável, como nos idosos, é crescente nos últimos anos. Este estudo objetiva desenvolver e avaliar em modelo animal o comportamento de um conduto valvado aórtico feito com pericárdio bovino tratado pelo glutaraldeído (CVAP). Para tanto, CVAPs foram implantados em 8 ovinos jovens e explantados após serem sacrificados com 150 dias de pós-operatório. Realizou-se estudo angiográfico e hemodinâmico no préoperatório e antes do explante. Ecodopplercardiogramas foram realizados nos dias 30 e 150 de pós-operatório (teste) e também em 5 ovinos não operados (controle). Após explantados, submetemos os CVAPs à avaliação macroscópica, radiológica e histológica por microscopia óptica e eletrônica de transmissão. A análise estatística foi feita com teste não-paramétrico de Mann-Whitney, teste não-paramétrico de Wilcoxon e teste exato de Fisher. O nível de significância utilizado foi 5%. Na análise hemodinâmica houve acréscimo (p>0,05) das pressões arterial e capilar pulmonar entre os dias 0 e 150. Na análise ecodopplercardiográfica, o grupo teste apresentou valores maiores (p>0,05) dos diâmetros diastólicos e sistólicos do ventrículo esquerdo. No grupo teste entre os dias 30 e 150 houve acréscimo (p>0,05) de: peso, espessura das paredes do ventrículo esquerdo, gradiente transvalvar máximo, gradiente transvalvar médio, diâmetro diastólico do ventrículo esquerdo e decréscimo da fração de ejeção. Dois animais com endocardite podem explicar essas diferenças. A macroscopia demonstrou calcificação de grau variável, além de abaulamento na região dos seios em todos os CVAPs, sem aumento de diâmetro. A microscopia óptica revelou dados similares ao da literatura com o uso do pericárdio bovino tratado pelo glutaraldeído. A quantificação realizada com a microscopia eletrônica de transmissão mostrou percentual maior (p>0,05) de colágeno nos seios e nas cúspides e maior conteúdo hídrico na porção mais distal do CVAP. Estes dados indicam que a bioprótese aqui estudada permite a realização desse tipo de experimento no modelo proposto e que os resultados hemodinâmicos encontrados se assemelham aos parâmetros fisiológicos. / The need for replacement surgery of the ascending aorta and aortic valve with coronary reimplantation in special patient groups where anti-coagulation in undesirable, such as elderly, is increasing in the past years. This study aims to develop and evaluate the performance of an aortic valved conduit made with glutaraldehyde treated bovine pericardium (AVCP) in an animal model. Therefore, AVCPs were implanted in 8 young ovine and explanted after being euthanized at 150 days of the post-operative period. An angiographic and hemodynamic study was performed at pre-operative and prior the explant. EchoDoppercardiograms were performed at day 30 and 150 of post-operative (test) as well as in 5 non-operated on ovines (control). Following the explant, AVCPs were submitted to a macroscopical, radiological and histological evaluation by optic and electronic transmission microscopy. A statistics analysis was performed with a Mann-Whitneys nonparametric test, Wilcoxons non-parametric test and Fishers exact test. The significance level used was 5%. In the hemodynamic analysis an increase (p>0,05) of arterial and pulmonary capillary pressure occurred between day 0 and 150. In the echoDoppercardiographic analysis, the test group presented higher values (p>0,05) in the diastolic and systolic diameters of the left ventricle. In the test group, between day 30 and 150, occurred an increase (p>0,05) of weight, thickness of the left ventricle walls, maximum transvalvar gradient, medium transvalvar gradient, left ventricle diastolic diameter and a decrease in the ejection function. Two animals with endocarditis could explain those differences. Macroscopy showed a calcification in variable degrees besides a bulging of the sinus region in all AVCPs, without an increase of diameter. Optic microscopy revealed data similar to literature with the use of glutaraldehyde treated bovine pericadium. The quantification performed by electronic transmission microscopy showed a higher percentage (p>0,05) of collagen in sinus and cusps, and a higher watery content in the most distal portion of the AVCP. These data indicate that the bioprosthesis hereby studied allows the performance of this kind of experiment in the proposed model and that the hemodynamic outcomes found are similar to physiological parameters.
