Prevalência e susceptibilidade aos antifúngicos de isolados de leveduras do gênero candida da cavidade bucal de pacientes com hanseníase

Foram utilizados enxágües bucais de trinta e oito indivíduos com idade entre 10 a 89 anos, diagnosticados como portadores de hanseníase clinicamente e por exame baciloscópico, que estejam sob poliquimioterapia por no mínimo 45 dias. Para o grupo controle o mesmo número de indivíduos saudáveis e com perfil semelhante (quanto à idade, gênero e condições bucais) aos pacientes do grupo em estudo. As amostras foram processadas e semeadas em ágar Sabouraud dextrose acrescido de cloranfenicol. Após o crescimento, as colônias foram examinadas quanto às características morfológicas, contadas as unidades formadoras de colônias (ufc). As colônias representativas de cada morfologia diferente foram isoladas. A identificação foi realizada por provas de formação de tubo germinativo, produção de hifas/pseudohifas e clamidoconídeos, fermentação, assimilação de carboidratos. Para identificação presuntiva de Candida dubliniensis foi realizado o teste à temperatura diferencial de 45ºC. Também foram realizadas provas de susceptibilidade aos antifúngicos anfotericina B, fluconazol, cetoconazol e 5- fluorocitosina. As contagens de leveduras nos grupos hansenianos e controle foram comparadas estatisticamente por ANOVA teste de Mann-Whitney (5%). O porcentual de pacientes hansenianos positivos para leveduras foi superior aos de indivíduos controle; porém não foi observada diferença significativa entre as contagens de leveduras nos grupos estudados. Houve maior prevalência de C. albicans e C. tropicalis em ambos os grupos estudados. Apenas uma amostra de C. tropicalis foi resistente à anfotericina B e todas as demais cepas foram sensíveis aos antifúngicos testados. / The present study aimed to evaluated the presence of Candida genus yeasts in the oral cavity of patients with hanseniasis, comparing the results with control individuals. Mouth rinses of thirty eight individuals aged between 10 to 89 years, diagnosed clinically and by baciloscopic examinations as hanseniasis positive, and who were under multi-drug therapy for at least 45 days were included. Control group was constituted by the same number of healthy individuals and same profile (in relation to age, gender and oral conditions) in relation to the study-group. Samples were processed and plated on Sabouraud dextrose agar supplemented with chloramphenicol. After the growth, colonies were examined in relation to their morphologic characteristics and the number of colony-forming units were counted. The colonies representative of each morphology were isolated. The identification was performed by germ tube formation test, production of hyphae/pseudohyphae and chlamidoconidea, fermentation and assimilation of carbohydrates. Candida dubliniensis presuntive identification was performed by growth at 45°C test. Antifungal susceptibility was tested with amphotericin B, fluconazole, ketoconazole and flucytosine. Counts of yeasts in the hanseniasis and control groups were compared by ANOVA Mann-Whitney’s test (5%). The porcentage of hanseniasis patients positive to yeasts was higher in relation to the control group, however no significant differences were observed between the counts of yeasts in the studied groups. Higher prevalence of C. albicans and C. tropicalis was observed in both groups. Only one isolate of C. tropicalis was resistant to amphotericin B and all the other isolates were susceptible to the antifungal drugs tested.
