Aqueous humor flow rate in normal cats and the effect of topical 2% dorzolamide on aqueous humor flow and intraocular pressure

Master of Science / Department of Clinical Sciences / Amy Rankin / Objective - To establish the aqueous humor flow rate in normal cats via fluorophotometry, utilizing a noninvasive method previously established in other species and to evaluate the effect of topical 2% dorzolamide on aqueous humor flow rate and intraocular pressure (IOP) in normal cats. Animals - 20 clinically normal domestic shorthair cats. Procedures – Topical administration of 10% sodium fluorescein was performed using a 3-drop protocol to establish its use in this species. Aqueous flow was measured using fluorophotometry in the right and left eyes. The subjects were then divided into 2 groups: the first received topical 2% dorzolamide (Trusopt®) and the second received topical artificial tear solution (control). The study was divided into two phases: a 3 day acclimation phase (no treatments given) and a 5 day treatment phase (treatments given three times daily). IOP measurements were taken at 7am, 10am, 1pm, 5pm, and 9pm throughout all phases of the study. Fluorophotometry was performed to measure the aqueous flow rate just prior to and at the end of the treatment phase (days 3 and 9 respectively). Results - The calculated aqueous humor flow rate for normal cats in the right, left, and both eyes was 5.94 ± 2.30 μl/min, 5.05 ± 2.06 μl/min, and 5.51 ± 2.21 μl/min, respectively. No significant differences were noted between the right and left eyes. In the dorzolamide group, the average flow rate during treatment (3.47 + 1.50 μl/min ) was significantly lower than prior to treatment (5.9 + 2.20 μl/min)(P < 0.001). The mean IOP during treatment (11 + 3 mmHg) was significantly lower than the mean IOP prior to treatment (15 + 3 mmHg)(P < 0.001). In the control group, there were no significant differences in aqueous humor flow or IOP values before or during treatment. Conclusions - The technique utilized for this study met the standard for accurate fluorophotometric calculation of aqueous humor flow. The average aqueous humor flow rate for normal cats calculated in this study was 5.51 ± 2.21 μl/min. Topical 2% dorzolamide significantly lowers aqueous humor flow (a 41% reduction) and IOP (a 26% reduction) in normal cats.

O peixe dourado é extensivamente utilizado em pesquisas sobre visão devido a diversos fatores: é um animal pecilotermo, o que possibilita manter sua retina ex-vivo viável para manipulação experimental por mais tempo; apresenta uma organização retiniana semelhante à de mamíferos; possui neurônios grandes, facilitando estudos eletrofisiológicos e morfológicos e, diferentemente de mamíferos, é dotado de neurogênese constante de alguns tipos celulares retinianos. A produção, composição e escoamento dos meios oculares nesta espécie, fundamentais para a formação de imagens sobre a retina, são, entretanto, pouco conhecidos. A variedade padrão desta espécie apresenta desenvolvimento ocular normal, enquanto a variedade demekin é extremamente míope por possuir crescimento ocular exagerado (buftalmia), condição característica de alguns tipos de glaucoma em mamíferos. O presente trabalho investigou aspectos morfofisiológicos do sistema de produção e drenagem do humor aquoso e a composição do humor vítreo nestas duas variedades de peixe dourado, com o objetivo de melhor compreender as causas subjacentes ao crescimento ocular do demekin. Para estudar a drenagem do humor aquoso, foram realizadas injeções de corante na câmara anterior de animais das duas variedades, com processamento histológico posterior em intervalos de tempo pré-determinados (5, 15, 30, 60, 120 minutos e 1, 2, 3 4 e 7 dias, n <= 3 olhos de cada variedade por grupo experimental). Não foram observadas diferenças na drenagem de humor aquoso entre as duas variedades. Em ambas, o humor aquoso é drenado principalmente pela malha trabecular contida na porção ventral do ângulo de drenagem, entre a íris e o ligamento anular. Duas vias secundárias de drenagem foram encontradas nas duas variedades de peixe dourado estudadas: (i) a via irídica e (ii) a drenagem através de vasos epirretinianos. Estas vias são aparentemente mais lentas do que a via trabecular, uma vez que foi encontrado corante nessas regiões até quatro dias após as injeções de corante na câmara anterior. Foi realizado também estudo ultraestrutural da retina e epitélio ciliar em ambas as variedades através de microscopia eletrônica de transmissão. O epitélio pigmentado da retina do demekin apresentou grânulos de melanina mais escassos, e observamos certa desorganização da camada de fotorreceptores, bem como redução da sua densidade. Através de aparato invasivo construído por Joselevitch e Smith (1997), foi mensurada a pressão intraocular (PIO) de 14 olhos de cada variedade. O demekin apresentou valores de PIO ligeiramente aumentados em relação ao animal padrão (3,6 ± 0,9 mmH2O para o demekin e 1,9 ± 0,6 mmH2O para o peixe dourado padrão). A composição proteica do corpo vítreo foi estudada através de eletroforese em gel de poliacrilamida utilizando um pool de humor vítreo de 4 animais da variedade padrão e 2 da variedade demekin. As proteínas de baixa massa molecular (entre 15 a 25 kDa) estavam expressas em ambas - embora com maior expressão na variedade padrão - e outras, de maior massa molecular, expressas apenas no humor vítreo do demekin (50-75 kDa e 150 kDa). Os resultados obtidos sugerem, portanto, que há diferenças importantes na ultraestrutura retiniana, na composição proteica do humor vítreo e na pressão intraocular do demekin. Para melhor compreensão do crescimento ocular desta variedade faz-se necessária a realização de um estudo proteômico, identificando quais são as proteínas do humor vítreo evidenciadas através da eletroforese, bem como estudos morfológicos em animais de diferentes tamanhos visando a quantificação dos danos retinianos decorrentes do seu crescimento ocular / The goldfish is extensively used in vision research due to several factors: it is a poikilothermic animal, which allows ex-vivo experiments using its retina for longer periods of time; its retinal organization is very similar to that of mammals; it has large neurons, which facilitates electrophysiological and morphological studies and, unlike mammals, it is endowed with constant neurogenesis of some retinal cell types. However, the production, composition and outflow of ocular media in this species, which are fundamental for image formation onto the retina, are poorly understood. The standard goldfish has normal eye development, while the Black Moor goldfish is extremely myopic due to excessive eye growth (buphthalmos), a condition that is present in certain types of mammalian glaucoma. The present study investigated morphophysiological aspects of the aqueous humor production and drainage system and vitreous humor composition in these two varieties of goldfish to better understand the causes underlying the eye growth of the Black Moor. To study the drainage of the aqueous humor, anterior chamber dye injections and histological processing were performed in animals of both varieties after predetermined time intervals (5, 15, 30, 60, 120 minutes and 1, 2, 3, 4, and 7 days, n <= 3 eyes of each variety for each experimental group). No differences were observed in the drainage of aqueous humor between the two varieties. In both, the aqueous humor is drained mainly by the trabecular meshwork contained in the ventral portion of the drainage angle, between the iris and the annular ligament. Two secondary drainage routes were found in the two goldfish varieties studied: (i) the iridic route and (ii) drainage through epiretinal vessels. These pathways are apparently slower than the trabecular route, since the dye was found in these regions 4 days after the injections. An ultrastructural study of the retina and ciliary epithelium was also carried out in both varieties through transmission electron microscopy. The retinal pigment epithelium of the Black Moor presented more scarce melanin granules and we observed some disorganization of the photoreceptor layer, as well as a reduction in its density. Through an invasive device constructed by Joselevitch and Smith (1997), the intraocular pressure (IOP) of 14 eyes of each variety was measured. The Black Moor presented slightly increased IOP values compared to the standard animal (3.6 ± 0.9 mmH2O for demekin and 1.9 ± 0.6 mmH2O for the standard goldfish). The protein composition of the vitreous body was studied by polyacrylamide gel electrophoresis using a vitreous humor pool of 4 animals of the standard variety and 2 of the Black Moor variety. Low molecular weight proteins (15 to 25 kDa) were expressed in both - although with greater expression in the standard variety - and others of higher molecular mass, expressed only in the vitreous humor of the Black Moor (50-75 kDa and 150 kDa). These results suggest, therefore, that there are important differences in the retinal ultrastructure, the protein composition of the vitreous humor and the intraocular pressure of the Black Moor. To better understand the ocular growth in this variety, it is necessary to carry out a proteomic study, identifying which are the vitreous humor proteins evidenced through electrophoresis, as well as morphological studies in animals of different sizes aiming the quantification of the retinal damages caused by its ocular growth

Blood circulation and aqueous humor flow in the eye : multi-scale modeling and clinical applications

Indiana University-Purdue University Indianapolis (IUPUI) / Glaucoma is a multi-factorial ocular disease associated with death of retinal ganglion cells and irreversible vision loss. Many risk factors contribute to glaucomatous damage, including elevated intraocular pressure (IOP), age, genetics, and other diseases such as diabetes and systemic hypertension. Interestingly, alterations in retinal hemodynamics have also been associated with glaucoma. A better understanding of the factors that contribute to these hemodynamic alterations could lead to improved and more appropriate clinical approaches to manage and hopefully treat glaucoma patients. In this thesis, we develop several mathematical models aimed at describing ocular hemodynamics and oxygenation in health and disease. Precisely we describe: (i) a time-dependent mathematical model for the retinal circulation that includes macrocirculation, microcirculation, phenomenological vascular regulation, and the mechanical effect of IOP on the retinal vasculature; (ii) a steady-state mathematical model for the retinal circulation that includes macrocirculation, microcirculation, mechanistic vascular regulation, the effect of IOP on the central retinal artery and central retinal vein, and the transport of oxygen in the retinal tissue using a Krogh cylinder type model; (iii) a steady-state mathematical model for the transport of oxygen in the retinal microcirculation and tissue based on a realistic retinal anatomy; and (iv) a steady-state mathematical model for the production and drainage of aqueous humor (AH). The main objective of this work is to study the relationship between IOP, systemic blood pressure, and the functionality of vascular autoregulation; the transport and exchange of oxygen in the retinal vasculature and tissue; and the production and drainage of AH, that contributes to the level of IOP. The models developed in this thesis predict that (i) the autoregulation plateau occurs for different values of IOP in hypertensive and normotensive patients. Thus, the level of blood pressure and functionality of autoregulation affect the changes in retinal hemodynamics caused by IOP and might explain the inconsistent outcomes of clinical studies; (ii) the metabolic and carbon dioxide mechanisms play a major role in the vascular regulation of the retina. Thus, the impairment of either of these mechanisms could cause ischemic damage to the retinal tissue; (iii) the multi-layer description of transport of oxygen in the retinal tissue accounts for the effect of the inner and outer retina, thereby improving the predictive ability of the model; (iv) a greater reduction in IOP is obtained if topical medications target AH production rather that AH drainage and if IOP-lowering medications are administrated to patients that exhibit a high initial level of IOP. Thus, the effectiveness of IOP-lowering medications depend on a patient’s value of IOP. In conclusion, the results of this thesis demonstrate that the insight provided by mathematical modeling alongside clinical studies can improve the understanding of diseases and potentially contribute to the clinical development of new treatments.

Progression of retinal ganglion cell loss observed as a result of anterior segment dysgenesis following conditional deletion of activating protein-2 in cranial neural crest cells

Our lab has shown that conditionally disrupting the tcfap2beta gene, responsible for the activating protein-2beta (AP-2beta) transcription factor, exclusively in the craniofacial neural crest cells, leads to anterior segment dysgenesis. Subsequent loss of the corneal endothelium results in the adherence of the iris to the corneal stroma, causing closure of the iridocorneal angle. The activating protein-2beta neural crest cell knockout (AP-2beta NCC KO) model involves a complete blockage of the both the conventional (through the trabecular meshwork) and non-conventional (uveoscleral) pathways for aqueous humor drainage, and therefore it could be used as a powerful experimental model for glaucoma. As shown by our previous work, elevated intraocular pressure (IOP) and a 35% decrease in the number of cells in the retinal ganglion cell (RGC) layer was observed in AP-2beta NCC KO mice by 2 months; 6 to 11 months sooner than other reported mouse models of glaucoma. These observations suggested that the AP-2beta NCC KO mouse could be a novel and cost-effective experimental model for glaucoma if the RGC loss occurred progressively rather than due to a congenital defect. The purpose of this research project was to investigate how the retinal ganglion cell layer and macroglial activity changes with respect to age in the AP-2beta NCC KO mutant through immunofluorescence. Specifically, it was investigated whether the loss of RGCs was progressive and due to the increased IOP caused by the blockage of the uveoscleral drainage pathway. A significant decrease in the number of RGCs was observed between P4 and P10 in the retinal periphery of both WT and AP-2beta NCC KO mice (p<0.05), which is indicative of the programmed cell death that occurs due to retinal pruning during development. No statistical difference between WT and AP-2beta NCC KO mice phenotypes was observed at postnatal day 4 (P4), suggesting that no developmental defect resulted in the significant loss of RGCs at 2 months. In all other time points investigated, while no statistical difference was found between WT and the AP-2 NCC KO mutant, a clear downward trend was present in the AP-2 NCC KO mutant retinal ganglion cell layer from P10 to P40. There was also an expression of glial fibrillary acidic protein (GFAP) by Müller cells, indicating the presence of neuroinflammation at P35 and P40. This substantiates the potential P42 starting point of neurodegeneration our lab previously observed. This was further corroborated with Müller cell-associated expression of GFAP at P35 and P40 exclusively in the AP-2beta NCC KO mouse. Overall, we have shown that the retinal damage observed in our AP-2beta NCC KO mouse is not due to a developmental defect, but rather occurs over time. Thus, this mouse model, which appears to block both the conventional and unconventional uveoscleral pathways, has a profound effect on aqueous humor drainage. As a result, the model requires relatively little time to observe an increase in IOP and subsequent RGC loss. Our findings suggest that the AP-2beta NCC KO mouse can be a novel, powerful, and extremely cost-effective experimental model for glaucoma. / Thesis / Master of Science (MSc)

Aqueous humor concentration and prostaglandin E2 suppression efficacy of topically applied ophthalmic ketorolac 0.5% and diclofenac 0.1% solutions in dogs with cataract

Background: Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used for their analgesic, anti-pyretic and anti-inflammatory properties in both human and veterinary patients. Topical ophthalmic NSAIDs are commonly employed in the management of intraocular inflammation (uveitis), corneoconjunctival inflammatory disease and pre-operatively to prevent intraoperative miosis during cataract surgery. Despite their routine application in these clinical scenarios, little is known regarding the corneal penetration and relative anti-inflammatory efficacy of the available topical ophthalmic NSAIDs in the dog. Decisions regarding which of these agents to employ are therefore based upon factors such as cost and ease of acquisition as opposed to established efficacy. Objectives: To investigate the relative intraocular penetration and anti-inflammatory efficacy of two commonly utilized topical ophthalmic NSAIDs in dogs, diclofenac 0.1% and ketorolac 0.5%. Animals: Twenty-two client owned dogs (22 operated eyes) presenting to the VTH ophthalmology service for routine cataract surgery for mature or hypermature cataract. Methods: Subjects were randomized to be treated with either topical ketorolac 0.5% or topical diclofenac 0.1% ophthalmic solutions at specified times in the 24-hour period pre-operatively. Aqueous humor samples were obtained intra-operatively and stored for subsequent evaluation of drug concentrations and prostaglandin E2 (PGE2) concentrations via ultra performance liquid chromatography-mass spectrometry (UPLC-MS/MS) and enzyme-linked immunoassay (ELISA) analysis, respectively. Results: Median aqueous humor drug concentrations were significantly higher in dogs treated with ketorolac 0.5% (1311.6 ng/mL) compared to those treated with diclofenac 0.1% (284.9 ng/mL). There was no significant difference in aqueous humor PGE2 concentrations between the two treatment groups. No significant association was determined between aqueous humor drug concentration and PGE2 concentration. There was no significant association between diabetic status and aqueous humor drug concentration or PGE2 concentration in either group. Conclusions and clinical importance: This study suggests that topical ketorolac 0.5% and diclofenac 0.1% are efficacious in decreasing aqueous humor PGE2 concentrations and are equally suitable for use based on their comparable anti-inflammatory profiles. The results of these assays provide clinically relevant information regarding intraocular penetration and anti-inflammatory efficacy of these medications in dogs with cataract. / Master of Science / Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used for their analgesic, anti-pyretic and anti-inflammatory properties in both human and veterinary patients. Topical ophthalmic NSAIDs are commonly employed in the management of intraocular inflammation (uveitis), corneoconjunctival inflammatory disease and pre-operatively to prevent intraoperative miosis during cataract surgery. Despite their routine application in these clinical scenarios, little is known regarding the intraocular penetration and relative anti-inflammatory efficacy of the available topical ophthalmic NSAIDs in the dog. Decisions regarding which of these agents to employ are therefore based upon factors such as cost and ease of acquisition as opposed to established efficacy. Efficacy of topical anti-inflammatory medications in controlling intraocular inflammation is primarily related to the ability of the medication to penetrate the cornea and its efficacy at suppressing inflammatory mediators. The purpose of this study, therefore, is to investigate the relative intraocular penetration and anti-inflammatory efficacy of two commonly utilized topical ophthalmic NSAIDs in dogs, diclofenac 0.1% and ketorolac 0.5%. Twenty-two dogs presenting to the VTH ophthalmology service for routine cataract surgery with the presence of a mature or hypermature cataract were enrolled in a prospective, randomized clinical trial. Subjects were treated with either topical ketorolac 0.5% or topical diclofenac 0.1% ophthalmic solutions at specified times in the 24-hour period pre-operatively. Aqueous humor samples were obtained intra-operatively and stored for subsequent evaluation of drug concentrations (n=22) and prostaglandin E2 (PGE2) concentrations (n=19) via ultra performance liquid chromatography (UPLC) and enzyme-linked immunoassay (ELISA) analysis, respectively. Treatment with topical ketorolac 0.5% resulted in higher median aqueous humor drug concentrations when compared to treatment with diclofenac 0.1% (1311.6 ng/mL vs. 284.9 ng/mL). However, there was no significant difference in anti-inflammatory efficacy when comparing PGE2 concentrations between the two groups. Furthermore, no significant association was determined when drug concentration was directly compared with PGE2 concentration. The results of these assays suggest that topical ketorolac 0.5% and diclofenac 0.1% are equally suitable for use based on their comparable anti-inflammatory profiles, and provides clinically relevant information regarding intraocular penetration and anti-inflammatory efficacy of these medications in dogs with cataract.

Development of Liquid-based Separation Techniques using Tailored Surfaces for Analysis of Biological Samples

Development and improvement of analytical techniques are vital in analytical chemistry research. This thesis describes the development and use of tailored surfaces for bioanalytical applications. In sample preparation, solid phase extraction is often used and the development of a protocol for extraction on a molecular imprinted polymer (MISPE) directly from plasma sample is presented. Molecular imprinted polymers (MIP) offer selective sorbents for the imprinted analyte. MISPE has mainly been used in organic phase but in this thesis the development of a protocol for direct extraction of the analyte form an aqueous phase is described. For analysis of complex samples a separation step is often needed. The growing interest in analysis of biological samples and analysis of the human proteome and potential biomarkers has increased the interest in developing new separation techniques. Capillary electrophoresis (CE) has evolved into an important technique for use in analysis of body fluids. In this thesis a novel polyamine coating named PolyE 323 tailored for minimizing the adsorption of basic proteins to the surface is introduced. A straightforward coating protocol, with four simple rinsing steps, was developed. The coating was highly reproducible and useable over a wide pH range. Successful protein separations on PolyE-323-coated capillaries coupled to electrospray ionization mass spectrometry (ESI-MS) were demonstrated. The coated capillaries were also used in studies of protein content of aqueous humor samples from cataract patients as a complement to capillary liquid chromatography. In the studies presented the protein content of aqueous humor samples from two clinical groups was compared. By using capillary liquid separation techniques coupled to matrix-assisted laser desorption/ionization mass spectrometry (MALDI-MS) and MS/MS in combination with isobaric tags for relative and absolute quantitation (iTRAQ) the identity and relative concentrations of proteins in the samples were evaluated. Earlier studies of the proteins in these kinds of samples have mainly been done with techniques using immunological detection where the proteins of interest were chosen in advance. In this thesis it was shown that liquid-based separation techniques are a good complement and by using the mass spectrometry approach presented the protein content of the samples could be evaluated without bias.

Os grupos saudável controle (SC) e toxoplasmose controle (TC) foram compostos respectivamente por gatos saudáveis e com sorologia positiva para toxoplasmose, enquanto os grupos saudável firocoxib (SF) e toxoplasmose firocoxib (TF) foram compostos respectivamente por gatos saudáveis e com sorologia positiva para toxoplasmose que receberam tratamento prévio com firocoxib (5 mg/kg), por via oral, 24 e uma hora antes da indução da uveíte experimental, através da paracentese da câmara anterior. Após indução anestésica colheu-se 0,2 mL de humor aquoso basal. Decorrido uma hora, realizou-se nova paracentese para colheita de 0,2 mL de humor aquoso inflamado. As amostras de humor aquoso foram acondicionadas a -80°C para posterior mensuração dos níveis de prostaglandina E2 (PGE2) e proteínas totais. No humor aquoso dos grupos TC e TF, realizou-se ainda, titulação para anticorpos IgG anti-Toxoplasma gondii. Mediante análise das amostras, observou-se aumento significativo dos níveis de PGE2 e proteína total do humor aquoso basal para o inflamado (p<0,05). No humor aquoso basal, os níveis de PGE2 no grupo TC, assim como os níveis de proteína total nos grupos SF e TF foram significativamente superiores aos demais (p<0,05). Os grupos TC e TF não apresentaram títulos IgG anti-Toxoplasma gondii no humor aquoso basal. No humor aquoso inflamado, os níveis de PGE2 e proteína total, bem como os títulos IgG anti-Toxoplasma gondii, não diferiram significativamente entre os grupos (p>0,05). Admite-se que gatos com títulos de anticorpos IgG anti-Toxoplasma gondii possuem níveis de PGE2 no humor aquoso basal superiores aos apresentados por indivíduos saudáveis. Entretanto, a concentração de PGE2 no humor aquoso destes indivíduos não é suficiente para ensejar a quebra da barreira hematoaquosa e causar uveíte anterior. O firocoxib não é capaz de evitar a quebra da barreira hematoaquosa em gatos saudáveis e naqueles soropositivos para toxoplasmose. / The objective was to evaluate the efficacy of firocoxib in preventing the blood-aqueous barrier breakdown in healthy cats and those with positive serology for toxoplasmosis. Thirty-two cats divided into four groups (n=8/each). The healthy control (HC) and toxoplasmosis control (TC) groups, were composed respectively of healthy cats and cats with positive serology for toxoplasmosis, while healthy firocoxib (HF) and toxoplasmosis firocoxib (TF) groups, were composed respectively of healthy cats and cats with positive serology for toxoplasmosis who had received previous treatment with orally firocoxib (5 mg/kg), twenty-four and one hour before the experimental induction of uveitis. Under anesthesia 0.2 mL of baseline aqueous humor was collected via aqueocentesis. One hour later, the same procedure was repeated and inflamed aqueous samples were collected. Aqueous samples were conditioned at -80°C for subsequent measurement of the levels of prostaglandin E2 (PGE2) and total proteins. In the aqueous samples of TC and TF groups, anti-Toxoplasma gondii IgG specific antibodies were titrated. PGE2 and total protein levels increased significantly in inflamed aqueous humor in comparison to baseline aqueous samples (p<0.05). At baseline aqueous humor, levels of PGE2 in TC group and total protein in HF and TF groups were increased significantly, in comparison with the other groups (p<0.05). Anti-Toxoplasma gondii IgG specific antibodies were found only in inflamed aqueous humor, and aqueous titers did not change significantly between TC and TF (p=0.1051). Although we have observed that aqueous humor PGE2 levels were significantly higher in seropositive cats in baseline aqueous humor, such increase was not able to break the blood-aqueous barrier and cause anterior uveitis. Firocoxib did not prevent intraocular inflammation after aqueocentesis, in healthy and toxoplasmosis-seropositive cats.
