Timpanometria em lactentes utilizando sonda de multifrequência / Multifrequency tympanometry in infantsTâmyne Ferreira Duarte de Moraes 28 February 2011 (has links)
O objetivo deste trabalho foi realizar a caracterização das medidas de imitância acústica de lactentes de zero a três meses de idade utilizando timpanometria de multifrequência. Foram avaliados 54 lactentes sem queixas auditivas e com presença de emissões otoacústicas evocadas transientes, por meio de entrevista audiológica, inspeção visual do meato acústico externo e medidas de imitância acústica nas frequências de 226Hz, 678Hz e 1000Hz, coletando-se os registros timpanométricos relacionados ao efeito de oclusão, à forma da curva timpanométrica, à pressão de pico timpanométrico, ao volume equivalente do meato acústico externo e ao pico compensado da admitância acústica estática. Os resultados indicaram presença de efeito de oclusão nas três frequências: 2,88% em 226Hz; 4,81% em 678Hz e 3,85% em 1000Hz; predomínio de curva timpanométrica em pico único (65,35% em 226Hz, 81,82% em 678Hz e 77,00% em 1000Hz); pressão de pico timpanométrico variando de -150 a 180daPa, com valores considerados normais; aumento do volume equivalente do meato acústico externo com aumento da frequência da sonda (0,64ml em 226Hz, 1,63mmho em 678Hz e 2,59mmho em 1000Hz), assim como os valores do pico compensado da admitância acústica estática (0,51ml em 226Hz, 0,55mmho em 678Hz e 1,20mmho em 1000Hz). Foram classificados como normais 93,06% dos timpanogramas realizados com 226Hz, 80,81% em 678Hz e 82,00% em 1000Hz. Por meio destas avaliações e resultados encontrados foi possível caracterizar as medidas de imitância acústica dos lactentes. / The aim of this study was to characterize acoustic immittance measures of infants from zero to three months of age using multifrequency tympanometry. 54 infants without hearing complaint and with transient evoked otoacoustic emissions presence were evaluated by an audiologic interview, a visual inspection of the ear canal and measures of acoustic immittance in the frequencies of 226Hz, 678Hz and 1000Hz, where tympanometric records of the occlusion effect, tympanometric curve type, tympanometric peak pressure, equivalent ear canal volume and peak compensated static acoustic admittance were collected. The results indicated presence of occlusion effect in three frequencies: 2.88% at 226Hz, 4.81% at 678Hz and 3.85% at 1000Hz; predominance of tympanometric curve with single peak ( 65.35% at 226Hz, 81.82% at 678Hz and 77.00% at 1000Hz), tympanometric peak pressure ranging from -155 to 180daPa with normal limits, equivalent ear canal volume increase with the frequency of the probe increase (0.64ml at 226Hz, 1.63mho at 678Hz and 2.59mmho at 1000Hz) and the increase of the peak compensated static acoustic admittance values with the frequency increase (0.51ml at 226Hz, 0.55mmho at 678Hz and 1.20mmho at 1000Hz). 93.06% of tympanograms were classified as normal at 226Hz, 81.82% at 678Hz and 77.00% at 1000Hz. Considering these evaluations and results it was possible to characterize the acoustic immittance measures of infants.

A Tutorial: Use of the WHO ICIDH-2 for Determining Aural Rehabilitation GoalsPatterson, Nancy Muscato 25 April 2001 (has links)
The purpose of this project was to implement the newly revised International Classification of Functioning, Disability and Health (ICIDH-2) developed by the World Health Organization (WHO), to establish specific aural rehabilitation goals. Five graduate clinicians in speech language pathology and audiology interviewed ten participants with adult onset hearing loss. A modified version of the General Questions for Participation and Activities (i.e., a structured interview technique) from the ICIDH-2 Checklist, was developed. Prior to completing this checklist, the students attended a brief training session to become familiar with the major components of the ICIDH-2, specifically the ICIDH-2 Checklist. Completion of the ICIDH-2 Checklist, Version 2.1a, clinician form (prefinal draft, December 2000), allowed the students to classify and qualify disability and health according to the constructs of Activity and Participation, (i.e., what a person can and cannot do as a result of hearing loss and what a person does and does not do as a result of hearing loss, respectively). Following completion of Parts 2 (Activities and Participation) and 3 (Environmental Factors) of the Checklist for each of the ten clients interviewed, aural rehabilitation goals were developed. Four participants are highlighted to illustrate how the ICIDH-2 is used to objectify the impact of hearing loss and to establish specific treatment goals. The results support the use of the modified version of the General Questions for Participation and Activities in development of aural rehabilitation goals for clients with adult onset hearing loss. Graduate clinicians demonstrated the ability to complete the checklist with little assistance, suggesting that the use of the ICIDH-2 by experienced clinicians should be a relatively easy task. Goal development was also a relatively easy task using the checklist ratings, and the ratings related directly to the individual participant's quality of life in their current situation.

Development of the Word Auditory Recognition and Recall Measure: A Working Memory Test for Use in Rehabilitative AudiologySmith, Sherri L., Pichora-Fuller, M. Kathleen, Alexander, Genevieve 01 November 2016 (has links)
Objectives: The purpose of this study was to develop the Word Auditory Recognition and Recall Measure (WARRM) and to conduct the inaugural evaluation of the performance of younger adults with normal hearing, older adults with normal to near-normal hearing, and older adults with pure-tone hearing loss on the WARRM. Design: The WARRM is a new test designed for concurrently assessing word recognition and auditory working memory performance in adults who may have pure-tone hearing loss. The test consists of 100 monosyllabic words based on widely used speech-recognition test materials. The 100 words are presented in recall set sizes of 2, 3, 4, 5, and 6 items, with 5 trials in each set size. The WARRM yields a word-recognition score and a recall score. The WARRM was administered to all participants in three listener groups under two processing conditions in a mixed model (between-subjects, repeated measures) design. The between-subjects factor was group, with 48 younger listeners with normal audiometric thresholds (younger listeners with normal hearing [YNH]), 48 older listeners with normal thresholds through 3000 Hz (older listeners with normal hearing [ONH]), and 48 older listeners with sensorineural hearing loss (older listeners with hearing loss [OHL]). The within-subjects factor was WARRM processing condition (no additional task or with an alphabet judgment task). The associations between results on the WARRM test and results on a battery of other auditory and memory measures were examined. Results: Word-recognition performance on the WARRM was not affected by processing condition or set size and was near ceiling for the YNH and ONH listeners (99 and 98%, respectively) with both groups performing significantly better than the OHL listeners (83%). The recall results were significantly better for the YNH, ONH, and OHL groups with no processing (93, 84, and 75%, respectively) than with the alphabet processing (86, 77, and 70%). In both processing conditions, recall was best for YNH, followed by ONH, and worst for OHL listeners. WARRM recall scores were significantly correlated with other memory measures. In addition, WARRM recall scores were correlated with results on the Words-In-Noise (WIN) test for the OHL listeners in the no processing condition and for ONH listeners in the alphabet processing condition. Differences in the WIN and recall scores of these groups are consistent with the interpretation that the OHL listeners found listening to be sufficiently demanding to affect recall even in the no processing condition, whereas the ONH group listeners did not find it so demanding until the additional alphabet processing task was added. Conclusions: These findings demonstrate the feasibility of incorporating an auditory memory test into a word-recognition test to obtain measures of both word recognition and working memory simultaneously. The correlation of WARRM recall with scores from other memory measures is evidence of construct validity. The observation of correlations between the WIN thresholds with each of the older groups and recall scores in certain processing conditions suggests that recall depends on listeners' word-recognition abilities in noise in combination with the processing demands of the task. The recall score provides additional information beyond the pure-tone audiogram and word-recognition scores that may help rehabilitative audiologists assess the listening abilities of patients with hearing loss.

Com o surgimento do registro das Emissões Otoacústicas e sua presença no repertório de testes de avaliação auditiva, foi possível ao clínico fazer o diagnóstico de NA. Assim sendo, é possível que alguns indivíduos que foram diagnosticados como portadores de uma perda auditiva neurossensorial, tivessem, na verdade, NA. Objetivos: O objetivo deste estudo foi caracterizar o tipo e grau da deficiência auditiva em uma população de deficientes auditivos da cidade de São Paulo; verificar a época da suspeita pelos pais da deficiência auditiva bem como do diagnóstico audiológico nestes indivíduos; identificar os participantes cujas avaliações comportamentais são incompatíveis com as avaliações eletrofisiológicas, visando identificar casos de Neuropatia Auditiva e realizar estudo qualitativo de casos dos participantes com incompatibilidade de respostas nas avaliações comportamentais e eletrofisiológicas. Método: Foram avaliados 89 deficientes auditivos de três escolas especiais da cidade de São Paulo e 11 do setor de Audiologia Educacional da Faculdade de Medicina da Universidade de São Paulo através de alguns testes audiológicos: imitanciometria, audiometria tonal limiar (ATL), emissões otoacústicas (EOA) e potencial evocado auditivo do tronco-encefálico (PEATE). Resultados: Dos 100 participantes deste estudo, 99% apresentaram uma deficiência auditiva do tipo neurossensorial e em 50% o grau predominante da deficiência auditiva foi profundo, um deles não conseguiu realizar a ATL. A média de idade da suspeita dos pais foi de 15,52 meses e o diagnóstico clínico foi de 25,07 meses, em todos os grupos a suspeita dos pais foi anterior ao diagnóstico. Apenas um participante apresentou avaliações comportamentais incompatíveis com as eletrofisiólogicas, todavia, no decorrer da pesquisa o quadro foi modificado, sendo que a flutuação da audição foi o fator mais marcante observado. Conclusões: Os resultados sugerem que a NA é um transtorno raro mesmo na população dos deficientes auditivos e alertam para a necessidade de acompanhar longitudinalmente os casos de suspeita do transtorno / Introduction: Auditory Neuropathy/Auditory Dys-synchrony (AN) is a disorder identified only 20 years ago. Due to diagnosis failure, patients with this disorder were diagnosed as hearing impaired. AN diagnosis was made possible upon the appearance of OAE\'s recordings and its presence in hearing evaluation battery. Therefore, it is possible that some individuals who have been diagnosed as suffering from sensorioneural hearing loss had indeed AN. Purpose: The purpose of this study was to characterize the type and degree of auditory impairment in a population of the hearing impaired in the city of São Paulo; to verify when parents had the suspicious of the hearing impairment and when was the audiological diagnosis done in these individuals; to identify the subjects whose behavioral evaluations are not compatible with the electrophysiological evaluations in order to identify Auditory Neuropathy events and carry out a qualitative study of the cases of subjects with incompatibility of responses in the behavioral and electrophysiological evaluations. Method: 89 hearing impaired individuals from three school for the deaf in the city of São Paulo, and 11 from the Educational Audiology Division of the Medicine School of the University of São Paulo were evaluated in some audiological tests: imitanciometry, audiometer evaluation (AE), otoacoustic emissions (OAE) and Auditory Brainstem Response (ABR). Results: Out of the 100 subjects in this study, 99% presented a sensorioneural hearing loss, and in 50% the predominant degree of auditory loss was profound, one of the subjects was not able to perform AE. The average age of parents suspicion was 15.52 months and the clinical diagnosis was 25.07 months, in all the groups the parents suspicion was prior to the formal diagnosis. Only one subject presented behavioral evaluations incompatible with the electrophysiological, however, in the course of the research the chart was changed, whereas the hearing fluctuation was the most remarkable factor observed. Conclusions: The results suggest that AN is a rare disorder even in the hearing impaired community and alert the need of long term follow up in the cases of suspected disorder

It occurs then the characterization of the auditory loss (degree, type and configuration), the determination of the necessities and expectations, the election of the hearing aid, being chosen its physical and eletroacustics characteristics. After that, the verification of the amplification prescribed during the programming of the hearing aid is carried through and, finally, the validation, to verify the effect in the auditory perception and the development of speaks. With this they stand out the importance of the diagnosis and the establishment of necessary auditory thresholds in the indication and adaptation of the hearing aid in children and the necessity of a knowledge deepened on the subject, that subsidizes the improvement of the politics and services of attention to the auditory health. Objetivo.Discribed and to argue the process of lapsing, verification and validation in the adaptation of hearing aid (Device of Individual Sonorous Amplification) for children of 3 the 36 months of age, inserted in a service of auditory health. Methodology. They had been selected and considered for it analyzes 10 citizens that were in attendance in the Centro Audição na Criança (CeAC), had been collected the data in the handbook of the service on the audiológico diagnosis, the indication and adaptation of the amplification and had been gotten given through measures in situ, audiometria with visual reinforcement with AASI and test of perception of speaks; in each specific case. From it analyzes had been chosen 2 citizens that had better allowed the quarrel of: Establishment of the auditory thresholds for the lapsing of characteristics acoustics of the AASI.The process of verification of the lapsing of the characteristics of the AASI: waited and the found one. Results. The results gotten with the quarrels of the two cases had disclosed that the determination of the audiométricos thresholds in the studied cases was determinative in the lapsing of the characteristics of the AASI. We presented different values of RECD in the two ears, being justified by the asymmetry of external acoustic meato that was visualized through the auricular mold. In the studied cases the value indicated in the software of the company does not correspond to the values gotten in the process of verification in coupler 2 cc. Reaction for the sounds was observed of says in both the casesConclusion. This study it allowed some reflections on the determination of the thresholds for the lapsing of the amplification. The mensuração of the RECD must be measured in the two ears when the anatomical difference will be visualized in the auricular mold. Adjustments of until 20dB had been necessary so that the parameters prescribed for software were gotten in the coupler 2cc. These results confirm the necessity of the process of verification in the coupler in the routine of the indication of AASI. The organization of a protocol with all the carried through procedures made possible one better understanding of the process and was basic in the decisions in such a way in the establishment of the thresholds as in the verification and validation of the characteristics of the AASI. Ocorre então a caracterização da perda auditiva (grau, tipo e configuração), a determinação das necessidades e expectativas, a seleção do AASI, sendo escolhidas suas características físicas e eletroacústicas. Em seguida, é realizada a verificação da amplificação prescrita durante a programação do AASI e, por último, a validação, para se verificar o efeito na percepção auditiva e no desenvolvimento de fala.Com isso ressaltam a importância do diagnóstico e do estabelecimento de limiares auditivos precisos na indicação e adaptação do AASI em crianças e a necessidade de um conhecimento aprofundado sobre o tema, que subsidie o aprimoramento das políticas e serviços de atenção à saúde auditiva. Foram selecionados e considerados para analise 10 sujeitos que estavam em atendimento no Centro Audição na Criança (CeAC), foram coletados os dados no prontuário do serviço sobre o diagnóstico audiológico, sobre a indicação e adaptação da amplificação e foram obtidos dados através de medidas in situ, audiometria com reforço visual com AASI e teste de percepção de fala; em cada caso específico.A partir da analise foram escolhidos 2 sujeitos que melhor permitiram a discussão de: Estabelecimento dos limiares auditivos para a prescrição de características acústicas do AASI.O processo de verificação da prescrição das características do AASI: o esperado e o encontrado.Resultados. Os resultados obtidos com as discussões dos dois casos revelaram que a determinação dos limiares audiométricos nos casos estudados foi determinante na prescrição das características do AASI. We apresentou valores de RECD diferentes nas duas orelhas, sendo justificada pela assimetria de meato acústico externo que foi visualizado através do molde auricular. Nos casos estudados o valor indicado no software da empresa não correspondem aos valores obtidos no processo de verificação no acoplador 2 cc.Foi observada reação para os sons de fala em ambos os casos Conclusão.Este estudo permitiu algumas reflexões sobre a determinação dos limiares para a prescrição da amplificação. A mensuração do RECD deve ser medido nas duas orelhas quando for visualizado no molde auricular a diferença anatômica..Ajustes de até 20dB foram necessários para que os parâmetros prescritos pelo software fossem obtidos no acoplador 2cc.Estes resultados confirmam a necessidade do processo de verificação no acoplador na rotina da indicação de AASI. A organização de um protocolo com todos os procedimentos realizados possibilitou uma melhor compreensão do processo e foi fundamental nas decisões tanto no estabelecimento dos limiares como na verificação validação das características do AASI. Possibilitou a organização de uma sugestão de protocolo que pode contribuir para a interpretação clínica dos exames disponíveis no primeiro ano de vida

Outcomes of an audiologic rehabilitation programme for working adults with hearing impairment who do not wear amplificationGrosskreutz, Jessica Susanne Gabriele January 2013 (has links)
Hearing impairment is a chronic health condition that affects increasingly younger age groups. Prevalence rates in the working population are estimated to be between four and nine percent when defined by audiometric loss, and between 30 – 40% when using self-report of hearing problems.
Hearing impairment can limit and threaten the social functioning of the affected person. It interferes with oral communication, causing activity limitations and participation restrictions. Additionally, a stigma is attached to hearing loss that can lead to feelings of embarrassment, guilt, anxiety and social exclusion. The stigma also poses a threat to the identity of the hearing impaired person who, in return, manages this threat by concealing or disclosing their hearing impairment depending on the social implications. As a consequence, help–seeking is delayed by a considerable amount of time. Although proven to be an effective intervention, amplification is often rejected by working adults.
Another available effective intervention is participating in audiologic rehabilitation (AR) programmes. These programmes focus on stigma reduction and communication strategies. Most existing programmes target an elderly population that had been fitted with hearing aids. No programme for working adults who do not wear amplification is published in the literature.
The new AR programme “See it! Hear it! Say it!” had been designed for adults who do not wear amplification and previously trialled in the USA. The purpose of this study was to evaluate the short and mid-term outcomes of a version adapted for the New Zealand context, specifically changes in health related quality of life (HRQoL) and cognitive anxiety.
Thirteen participants in two groups participated in the study. The design was a quasi–randomised pre-test/post-test/follow-up test with waitlist design. Outcomes were measured with the International Outcome Inventory – Alternative Interventions (IOI-AI), the Hearing Handicap Inventory for Adults (HHIA), the Cognitive Anxiety Scale (CAS) and a non-standardised online questionnaire.
Results demonstrated statistically significant differences between pre-group and follow-up assessment outcomes. Effect sizes ranged between 0.606 and 2.114. Participants reported implementing communication strategies in a number of adverse listening environments.
These findings provide evidence that the New Zealand specific version of “See it! Hear it! Say it!” is effective in improving HRQoL and reducing cognitive anxiety.

Apenas um participante apresentou avaliações comportamentais incompatíveis com as eletrofisiólogicas, todavia, no decorrer da pesquisa o quadro foi modificado, sendo que a flutuação da audição foi o fator mais marcante observado. Conclusões: Os resultados sugerem que a NA é um transtorno raro mesmo na população dos deficientes auditivos e alertam para a necessidade de acompanhar longitudinalmente os casos de suspeita do transtorno / Introduction: Auditory Neuropathy/Auditory Dys-synchrony (AN) is a disorder identified only 20 years ago. Due to diagnosis failure, patients with this disorder were diagnosed as hearing impaired. AN diagnosis was made possible upon the appearance of OAE\'s recordings and its presence in hearing evaluation battery. Therefore, it is possible that some individuals who have been diagnosed as suffering from sensorioneural hearing loss had indeed AN. Purpose: The purpose of this study was to characterize the type and degree of auditory impairment in a population of the hearing impaired in the city of São Paulo; to verify when parents had the suspicious of the hearing impairment and when was the audiological diagnosis done in these individuals; to identify the subjects whose behavioral evaluations are not compatible with the electrophysiological evaluations in order to identify Auditory Neuropathy events and carry out a qualitative study of the cases of subjects with incompatibility of responses in the behavioral and electrophysiological evaluations. Method: 89 hearing impaired individuals from three school for the deaf in the city of São Paulo, and 11 from the Educational Audiology Division of the Medicine School of the University of São Paulo were evaluated in some audiological tests: imitanciometry, audiometer evaluation (AE), otoacoustic emissions (OAE) and Auditory Brainstem Response (ABR). Results: Out of the 100 subjects in this study, 99% presented a sensorioneural hearing loss, and in 50% the predominant degree of auditory loss was profound, one of the subjects was not able to perform AE. The average age of parents suspicion was 15.52 months and the clinical diagnosis was 25.07 months, in all the groups the parents suspicion was prior to the formal diagnosis. Only one subject presented behavioral evaluations incompatible with the electrophysiological, however, in the course of the research the chart was changed, whereas the hearing fluctuation was the most remarkable factor observed. Conclusions: The results suggest that AN is a rare disorder even in the hearing impaired community and alert the need of long term follow up in the cases of suspected disorder
