Bakgrund: Bröstcancer är idag den vanligaste cancerdiagnosen hos kvinnor i Sverige och drabbar cirka 20 kvinnor om dagen. Cancerbehandling har många biverkningar vilket försämrar livskvaliteten hos de drabbade. Syfte: Syftet med denna litteraturstudie är att undersöka om evidens finns för att fysisk aktivitet har positiva effekter på biverkningar och livskvalitet hos kvinnor med bröstcancer. Metod: Litteraturstudien baseras på 15 vetenskapliga artiklar och resulterar i 4 kategorier, nämligen fysisk aktivitet minskar biverkningar vid bröstcancerbehandling, fysisk aktivitet ökar kvinnornas livskvalitet under behandling, fysisk aktivitet ökar kvinnornas livskvalitet efter behandling och sjuksköterskans råd om fysisk aktivitet. Resultat: Evidens finns för att fysisk aktivitet har positiva effekter på biverkningar och livskvalitet hos kvinnor med bröstcancer. All fysisk aktivitet som studerats reducerar biverkningar som följer en cancerbehandling och ökar därmed livskvaliteten. Kombinationen av pulshöjande fysisk aktivitet och annan aktivitet ger bäst långvarig effekt på livskvaliteten. Slutsats: Resultatet kan motivera sjuksköterskan att rekommendera fysisk aktivitet till kvinnor med bröstcancer då det är ett säkert och effektivt tillvägagångssätt för att öka kvinnornas livskvalitet. / Background: Breast cancer is now the most common cancer diagnosed in women in Sweden and it affects about 20 women a day. Cancer treatment has many side effects, which impairs the quality of life of those affected. Aim: The aim of this study is to investigate whether there is evidence that physical activity has a positive impact on the side effects and quality of life in women with breast cancer diagnosis. Method: The literature study is based on 15 scientific articles and results in four categories: Physical activity reduces the side effects of breast cancer treatment, Physical activity increases patients' quality of life during treatment, Physical activity increases patients' quality of life after treatment and Nurse advice about physical activity. Results: There is evidence that physical activity has a positive impact on the side effects and quality of life in women with breast cancer diagnosis. Any physical activity that has been studied reduces the side effects that follow a cancer treatment and it increases the quality of life among women. The combination of pulse-pounding physical activity and other activity provides the best long-term impact on quality of life. Conclusion: The results can motivate nurses to recommend physical activity to women with breast cancer because there is a safe and effective approach to increase women's quality of life.

Análise do real valor do índice preditor de resposta terapêutica na quimioterapia neoadjuvante em portadores de carcinoma mamário localmente avançado por cintilografia

Orientador: Sonia Marta Moriguchi / Resumo: O câncer de mama é o tipo mais comum entre as mulheres no Brasil e no mundo, com 28% de novos casos a cada ano. O diagnóstico tardio pode identificar carcinoma de mama localmente avançado (CMLA) que necessita tratamento sistêmico quimioterápico pré-operatório que visam reduzir o tamanho do tumor e da carga tumoral. A quimiorresistência pode estar relacionada à expressão de glicoproteína P (GpP), que promove o efluxo celular da droga, diminuindo o tempo de permanência da droga na célula e, consequentemente a apoptose celular. O sestamibi-99mTc utilizado na realização da cintilografia de mamas (CM) é um substrato dessas proteínas, similar às drogas quimioterápicas, sendo o único método de imagem a representar o tempo de permanência dessa droga. O índice preditor da resposta quimioterápica por cintilografia tem sido utilizado para prever o tempo de permanência dessas drogas nessas células. O objetivo desse estudo foi verificar se o índice preditor de resposta quimioterápica neoadjuvante obtido por cintilografia de mamas com sestamibi-99mTc proposto por Alonso e cols. relacionou-se com resposta quimioterápica do estudo anatomopatológico da mama pós-cirurgia (AP), considerado padrão ouro, analisando-se os índices isolados: índice precoce (IP) e índice tardio (IT) e associados (IP+IT). Delayed diagnosis can identify locally advanced breast carcinoma (LABC), which requires preoperative systemic chemotherapy that aims to reduce the size and load of the tumor. The chemo-resistance may be related to expression of glycoprotein P (GpP) that promotes cellular efflux of the drug, diminishing not only its time within the cell but consequently cellular apoptosis. Sestamibi99mTc, utilized in performing scintimammography (SMM), is a substrate of these proteins, similar to chemotherapeutic drugs, being the only imaging method to represent the permanence time of this drug. The predictive index of chemotherapeutic response by SMM has been utilized to predict the permanence time of chemotherapeutic drugs into these cells. När diagnosen är bröstcancer : En litteraturöversikt om kvinnors hantering av beskedet / When the diagnosis is breast cancer : A literature review on women's coping of the news

Bakgrund: Bröstcancer är idag den vanligaste cancerformen bland kvinnor. Att få ett besked om en livshotande sjukdom kan generera starka känslor. Hur kvinnorna hanterar ett sådant besked beror på omedvetna försvarsmekanismer och medvetna copingstrategier vilka kan vara mångfacetterade. Sjuksköterskan har en viktig roll i att stödja dessa copingstrategier för att främja en god omvårdnad.                 Syfte: Syftet var att belysa kvinnors känslor och copingstrategier efter ett bröstcancerbesked. Metod: En litteraturöversikt gjordes utifrån tolv vetenskapliga artiklar vilka analyserades enligt Fribergs metod. Artiklarna är författade mellan år 2010 och år 2018 och var av kvalitativ-, kvantitativ- och mixad metod. Utifrån artiklarnas resultat identifierades likheter och skillnader vilket resulterade i två kategorier och åtta subkategorier. Resultat: Resultat visade att vanligt förekommande känslor och copingstrategier vid ett bröstcancerbesked var olika känslor i form av chock, rädsla och ångest och oro samt copingstrategier vilka visade sig hos kvinnorna i form av förnekelse, ändra fokus, positiv attityd, social stöttning och tro. Diskussion: Diskussionen berör att ångest kan förekomma innan, under och efter att ett bröstcancerbesked givits. Ångesten är således existentiell och kan påverka livskvalitén under längre tid än vid diagnostillfället. Ändringen av fokus och prioritera saker som var betydelsefulla diskuterades ge en känsla av kontroll. Social stöttning var en copingstrategi vilken diskuterades kunna ha en positiv effekt och negativ effekt för kvinnorna. / Background: Breast cancer is the most common form of cancer among women. Receiving news regarding a life-threatening disease can cause strong emotions. How women handle such news is due to defense mechanisms and conscious coping strategies that can all be multifaceted. The nurse has an important role to support these coping strategies and to promote good caregiving. Aim: The purpose was to highlight women’s feelings and coping strategies after breast cancer diagnosis. Method: Twelve scientific articles were analyzed in the present literature review according to the method recommended by Friberg. The articles were written between year 2010 and 2018, which were of qualitative, quantitative and mixed methods. Based on the results of the articles, similarities and differences were identified which became two head categories and eight sub-categories. Results: The result showed that common kinds of feelings and coping strategies when receiving the news of breast cancer were feelings such as shock, fear and anxiety and worries. It also showed coping strategies such as denial, change of focus, positive attitude, social support and faith. Discussion: The discussion concerns that anxiety can occur before, during and after a breast cancer message. Anxiety is therefore existential and can affect quality of life for longer time than at the time of diagnosis. The change of focus and prioritizing matters that were significant were discussed to give a sense of control. Social support was a coping strategy that was discussed to have a positive and negative effect for the women.
