Mechanical Flow Restoration in Acute Ischemic Stroke: A Model System of Cerebrovascular Occlusion: A Dissertation

Stroke is the third most common cause of death and a leading cause of disability in the United States. The existing treatments of acute ischemic stroke (AIS) involve pharmaceutical thrombolytic therapy and/or mechanical thrombectomy. The Food and Drug Administration (FDA)-approved recombinant tissue plasminogen activator (tPA) administration for treatment of stroke is efficacious, but has a short treatment time window and is associated with a risk of symptomatic hemorrhage. Other than tPA, the Mechanical Embolus Removal in Cerebral Ischemia (MERCI) retriever system and the Penumbra Aspiration system are both approved by the FDA for retrieval of thromboemboli in AIS patients. However, the previous clinical studies have shown that the recanalization rate of the MERCI system and the clinical outcome of the Penumbra system are not optimal. To identify the variables which could affect the performance of the thrombectomy devices, much effort has been devoted to evaluate thrombectomy devices in model systems, both in vivo and in vitro, of vascular occlusion. The goal of this study is to establish a physiologically realistic, in vitro model system for the preclinical assessment of mechanical thrombectomy devices. In this study, the model system of cerebrovascular occlusion was mainly composed of a human vascular replica, an embolus analogue (EA), and a simulated physiologic mock circulation system. The human vascular replica represents the geometry of the internal carotid artery (ICA)/middle cerebral artery (MCA) that is derived from image data in a population of patients. The features of the vasculature were characterized in terms of average curvature (AC), diameter, and length, and were used to determine the representative model. A batch manufacturing was developed to prepare the silicone replica. The EA is a much neglected component of model systems currently. To address this limitation, extensive mechanical characterization of commonly used EAs was performed. Importantly, the properties of the EAs were compared to specimens extracted from patients. In the preliminary tests of our model system, we selected a bovine EA with stiffness similar to the thrombi retrieved from the atherosclerotic plaques. This EA was used to create an occlusion in the aforesaid replica. The thrombectomy devices tested included the MERCI L5 Retriever, Penumbra system 054, Enterprise stent, and an ultrasound waveguide device. The primary efficacy endpoint was the amount of blood flow restored, and the primary safety endpoint was an analysis of clot fragments generated and their size distribution. A physiologically realistic model system of cerebrovascular occlusion was successfully built and applied for preclinical evaluation of thrombectomy devices. The recanalization rate of the thrombectomy device was related to the ability of the device to capture the EA during the removal of the device and the geometry of the cerebrovasculature. The risk of the embolic shower was influenced by the mechanical properties of the EA and the design of the thrombectomy device.

Tratamento endovascular das dissecções e pseudoaneurismas da artéria vertebral. / Endovascular treatment of dissections and pseudoaneurysms of the vertebral artery.

As dissecções da artéria vertebral causam acidentes vasculares cerebrais isquêmicos e hemorrágicos. A dissecção arterial é a ruptura da sua parede com formação de hematoma intramural. Podem ser espontâneas, acometendo a artéria vertebral extra ou intracraniana. O tratamento em geral é clínico, porém em alguns casos está indicada intervenção. A via endovascular é uma importante alternativa, permitindo o tratamento específico da lesão em alguns casos, mas na maioria sacrificando a artéria vertebral, após teste de tolerância à oclusão. Com o objetivo de analisar os aspectos clínicos e técnicos do tratamento endovascular, estudamos de forma prospectiva 15 pacientes. Três apresentavam dissecções traumáticas (todas extracranianas) e 12 espontâneas, dos quais dois tiveram traumatismos menores como desencadeantes. Cinco pacientes apresentaram dissecções extracranianas, oito, intracranianas e dois, combinadas. No grupo das extracranianas, a principal indicação de tratamento foi a presença de fístula arteriovenosa, em três dos cinco pacientes. No grupo da intracranianas, foi a presença de hemorragia meníngea. Nos quatro pacientes com acidente vascular isquêmico, a indicação de tratamento deveu-se à presença de pseudoaneurismas que não involuíram com tratamento clínico. Nesse grupo, dois pacientes tinham dissecção extracraniana, um, intra e um, combinada. Um paciente apresentou intolerância à oclusão e foi encaminhado para tratamento conservador. Dos 14 pacientes tratados, um teve como estratégia a oclusão seletiva da lesão, 11 a oclusão da artéria vertebral proximal à lesão e dois oclusão acima e abaixo da lesão. Os materiais utilizados foram balões destacáveis em sete pacientes, molas de destaque livre em 6 e molas eletricamente destacáveis associadas a molas de destaque livre em 1 paciente. Dois pacientes apresentaram complicações do tratamento, e um paciente, recidiva de fístula arteriovenosa, todos resolvidos sem seqüelas. A angiografia controle revelou oclusão total do segmento dissecado ou do pseudoaneurisma em 9 pacientes, reversão do fluxo em quatro e preservação da artéria vertebral com oclusão da lesão em um. Num período de seguimento de 8,6 meses não se registraram recorrências. O tratamento foi eficiente na prevenção de ressangramentos e na trombose dos pseudoaneurismas e apresenta segurança em relação a complicações. / Vertebral artery dissections can cause brain ischemia and hemorrhage. Arterial dissection consist of mural tears with subsequent intramural hematoma formation. They may occur either spontaneously or as a consequence of traumatism, in the extracranial or intracranial vertebral artery. The treatment is usually clinical, but in some instances intervention is indicated. The endovascular approach is an important tool, allowing specific treatment of the lesion in some cases, but sacrificing the vertebral artery in most cases. With the aim of analyze the clinical and technical aspects of the endovascular treatment, we studied prospectively 15 patients treated by endovascular approach. Three presented traumatic dissections (all extracranial) and 12 spontaneous dissections, two of which after minor traumatic events. Five patients had extracranial dissections, eight, intracranial and two, combined. In the extracranial dissection group, the main indication for treatment was the presence of an arteriovenous fistula (three of five patients). In the intracranial group, it was subarachnoid hemorrhage. Four patients presenting with brain isquemia were treated because of pseudoaneurysms that did not resolve in clinical treatment. In this group 2 patients had extracranial dissections, one had intracranial and one had both. One patient did not tolerate occlusion and was treated clinically. Fourteen patients were treated by endovascular means, one with selective lesion occlusion, 12 with proximal vertebral artery occlusion and two with proximal and distal vertebral artery occlusion. The embolic material were detachable balloons in 7 patients, platinum microcoils in 6 patients and electrically detachable platinum microcoils and platinum microcoils in one patient. Two patients presented complications, and one presented recurrence of an arteriovenous fistula, all resolved without sequelae. Angiographic controls disclosed total occlusion of the segment with dissection or of the pseudoaneurysm in 9 patients, retrograde flow in 4 and vertebral artery preservation with selective lesion occlusion in 1. During a mean follow-up period of 8,6 months no recurrence was observed. The treatment was efficient in preventing recurrent hemorrhage and promoting pseudoaneurysms thrombosis, besides it was a safe treatment option.

Hyperglycemia and Focal Brain Ischemia : Clinical and Experimental Studies

<p>Diabetes is a major risk factor for ischemic stroke and is associated with increased mortality. Additionally, hyperglycemia, a common complication in acute stroke, is associated with poor outcome.</p><p>In order to identify the correlation between blood glucose and early mortality, multiple logistic regression analyses were used and odds ratios calculated in a retrospective study of 447 stroke patients. Eighty-one patients (18%) had diabetes. The odds ratios for 30-day case-fatality and blood glucose were 1.9 and 1.6 in diabetic and non-diabetic patients respectively. Optimal blood glucose concentrations in respective group were 10.3 and 6.3 mmol/L, as determined by receiver operator characteristic (ROC) curves.</p><p>Cerebral ischemia triggers different signaling pathways including mitogen-activated protein kinases (MAPK) which regulate fundamental cell functions. In an experimental rat model of combined hyperglycemia and transient middle cerebral artery occlusion (MCAO), the activation pattern of one such MAPK, extracellular signal-regulated kinase (ERK) was studied along with infarct volumes and neurological function. Hyperglycemia resulted in markedly increased ERK activation and approximately three-fold increase of infarcts compared with controls. </p><p>Based on the increased ERK activation, further experiments were conducted to limit the hyperglycemic-ischemic damage by interfering with ERK and supposedly related mechanisms. Consequently, rats were given U0126 (inhibiting ERK activation), PBN (anti-oxidative), PP2 (inhibiting src-family kinases), or vehicle. PBN reduced infarcts and improved neurological function compared with controls while no statistically significant effects were observed for U0126 or PP2. However, when the dose was doubled, U0126 significantly reduced infarcts and improved neurological function after 1 day in hyperglycemic rats. Post-ischemic ERK activation was completely inhibited by U0126 as demonstrated with Western immunoblotting. The findings suggest that ERK is an important mediator of hyperglycemic-ischemic brain injury and possible target for future interventions.</p>

Desempenho motor de pacientes com acidente vascular cerebral em um jogo baseado em realidade virtual

Objective: This study was aimed to evaluate the performance of stroke patients in chronic stage using a virtual reality game. Method: 20 patients (10 with injury to the left and 10 to the right side), right-handed, average age 50.6 ? 9.2 years, and 20 healthy subjects with average age of 50.9 ? 8.8, also right-handed participated. The patients had a motor (Fugl-Meyer) and muscle tone assessment (Ashworth). All participants made a kinematic evaluation of the drinking water activity and then underwent training with the table tennis game on XBOX 360 Kinect?, 2 sets of 10 attempts for 45 seconds, 15 minutes rest between sets, giving a total of 30 minutes session. After training the subjects underwent another kinematic evaluation. The patients trained with the right and left hemiparect upper limb and the healthy ones with the right and left upper limb. Data were analyzed by ANOVA, t Student test and Pearson correlation. Results: There was significant difference in the number of hits between the patients and healthy groups, in which patients had a lower performance in all the attempts (p = 0.008), this performance was related to a higher level of spasticity (r = - 0.44, p = 0.04) and greater motor impairment (r = 0.59, p = 0.001). After training, patients with left hemiparesis had improved shoulder and elbow angles during the activity of drinking water, approaching the pattern of motion of the left arm of healthy subjects (p < 0.05), especially when returning the glass to the table, and patients with right hemiparesis did not obtain improved pattern of movement (p > 0.05). Conclusion: The stroke patients improved their performance over the game attempts, however, only patients with left hemiparesis were able to increase the angle of the shoulder and elbow during the functional activity execution, better responding to virtual reality game, which should be taken into consideration in motor rehabilitation / O Acidente Vascular Cerebral (AVC), por ser uma das principais causas de incapacidade motora em adultos e idosos necessita de interven??es eficazes que contribuam para a recupera??o motora. Resultados: Houve diferen?a significativa no n?mero de acertos entre o grupo de pacientes e saud?veis, no qual os pacientes apresentaram um desempenho inferior em todas as tentativas realizadas (p=0,008), estando esse desempenho relacionado a um maior n?vel de espasticidade (r= -0,44; p=0,04) e a um maior comprometimento motor (r= 0,59; p=0,001). Ap?s o treino, os pacientes com hemiparesia esquerda tiveram melhora na angula??o de ombro e cotovelo durante a atividade de beber ?gua, se aproximando do padr?o de movimento do membro superior esquerdo dos saud?veis (p<0,05), principalmente no momento de retornar o copo a mesa; j? os pacientes com hemiparesia direita n?o obtiveram melhora do padr?o de movimento em rela??o aos saud?veis (p>0,05). Cerebral venous outflow resistance and interpretation of cervical plethysmography data with respect to the diagnosis of chronic cerebrospinal venous insufficiency

No / PURPOSE: To investigate cerebrospinal fluid (CSF) dynamics in the aqueduct of Sylvius (AoS) in chronic cerebrospinal venous insufficiency (CCSVI)-positive and -negative healthy individuals using cine phase contrast imaging. MATERIALS AND METHODS: Fifty-one healthy individuals (32 CCSVI-negative and 19 age-matched CCSVI-positive subjects) were examined using Doppler sonography (DS). Diagnosis of CCSVI was established if subjects fulfilled >/=2 venous hemodynamic criteria on DS. CSF flow and velocity measures were quantified using a semiautomated method and compared with clinical and routine 3T MRI outcomes. RESULTS: CCSVI was associated with increased CSF pulsatility in the AoS. Net positive CSF flow was 32% greater in the CCSVI-positive group compared with the CCSVI-negative group (P = 0.008). This was accompanied by a 28% increase in the mean aqueductal characteristic signal (ie, the AoS cross-sectional area over the cardiac cycle) in the CCSVI-positive group compared with the CCSVI-negative group (P = 0.021). CONCLUSION: CSF dynamics are altered in CCSVI-positive healthy individuals, as demonstrated by increased pulsatility. This is accompanied by enlargement of the AoS, suggesting that structural changes may be occurring in the brain parenchyma of CCSVI-positive healthy individuals.

Assessment of cerebral venous return by a novel plethysmography method

BACKGROUND: Magnetic resonance imaging and echo color Doppler (ECD) scan techniques do not accurately assess the cerebral venous return. This generated considerable scientific controversy linked with the diagnosis of a vascular syndrome known as chronic cerebrospinal venous insufficiency (CCSVI) characterized by restricted venous outflow from the brain. The purpose of this study was to assess the cerebral venous return in relation to the change in position by means of a novel cervical plethysmography method. METHODS: This was a single-center, cross-sectional, blinded case-control study conducted at the Vascular Diseases Center, University of Ferrara, Italy. The study involved 40 healthy controls (HCs; 18 women and 22 men) with a mean age of 41.5 +/- 14.4 years, and 44 patients with multiple sclerosis (MS; 25 women and 19 men) with a mean age of 41.0 +/- 12.1 years. All participants were previously scanned using ECD sonography, and further subset in HC (CCSVI negative at ECD) and CCSVI groups. Subjects blindly underwent cervical plethysmography, tipping them from the upright (90 degrees ) to supine position (0 degrees ) in a chair. Once the blood volume stabilized, they were returned to the upright position, allowing blood to drain from the neck. We measured venous volume (VV), filling time (FT), filling gradient (FG) required to achieve 90% of VV, residual volume (RV), emptying time (ET), and emptying gradient (EG) required to achieve 90% of emptying volume (EV) where EV = VV - RV, also analyzing the considered parameters by receiver operating characteristic (ROC) curves and principal component mathematical analysis. RESULTS: The rate at which venous blood discharged in the vertical position (EG) was significantly faster in the controls (2.73 mL/second +/- 1.63) compared with the patients with CCSVI (1.73 mL/second +/- 0.94; P = .001). In addition, respectively, in controls and in patients with CCSVI, the following parameters were highly significantly different: FT 5.81 +/- 1.99 seconds vs 4.45 +/- 2.16 seconds (P = .003); FG 0.92 +/- 0.45 mL/second vs 1.50 +/- 0.85 mL/second (P < .001); RV 0.54 +/- 1.31 mL vs 1.37 +/- 1.34 mL (P = .005); ET 1.84 +/- 0.54 seconds vs 2.66 +/- 0.95 seconds (P < .001). Mathematical analysis demonstrated a higher variability of the dynamic process of cerebral venous return in CCSVI. Finally, ROC analysis demonstrated a good sensitivity of the proposed test with a percent concordant 83.8, discordant 16.0, tied 0.2 (C = 0.839). CONCLUSIONS: Cerebral venous return characteristics of the patients with CCSVI were markedly different from those of the controls.
