Postoj sester a porodních asistentek k preventivním prohlídkám / Attitude to Prophylactic Check-ups among Nurses and Midwives

Abstract The thesis focuses on an insight into the attitude to prophylactic check-ups among nurses and midwives. The theoretical part deals with health, prevention, prophylactic check-ups, characteristics of attitudes and education. The role of nurses and midwives in prevention, definition of competences and lifestyle problems in relation to healthcare job performance is not omitted. The aim of the thesis was to find out whether nurses and midwives themselves regularly have prophylactic check-ups and how they approach the information on necessity of prophylactic check-ups in the public. Quantitative method was applied to the research. An anonymous questionnaire research among nurses and midwives working in primary care and in hospitals was used as the data collection technique. The total number of questionnaires returned from the respondents was 270, 91 were returned from primary care and 179 from hospitals. None of the questionnaires had to be rejected for incompleteness. A pilot study was performed within the research, where 10 questionnaires were handed out evenly to the primary care sector and to the hospital sector to verify whether no serious problems with their completion occured. Thanks to their correct completion they were included into the main research. The monitored hypotheses were statistically evaluated by means of the Pearson's Chi-Square Test and Fisher?s Test with low values in contingency tables. The fact whether participation of nurses and midwives in prophylactic check-ups differs with age was evaluated. The frequency of regular prophylactic check-ups was 62 per cent at the age below 40 and 98 per cent at the age over 40. Age has significant influence on medical prophylactic check-up attendance. Education level does not have statistically significant influence on prophylactic check-up attendance. The research has not completely refuted difference in prophylactic check-up attendance according to workplace type. Nurses and midwives working in hospitals have prophylactic check-ups less regularly than those working in primary care. Nurses and midwives in primary care are demonstrably older and they go to prophylactic check-ups more regularly. The difference in approach to prophylactic check-ups among nurses and midwives according to workplace type was monitored. Nurses and midwives have positive attitude to prophylactic check-ups regardless of the type of their workplace. They belong to the most frequent activities they apply to maintenance and improvement of their health condition. Nurses and midwives are first of all motivated to regular visits of prophylactic check-ups by the care of their own health. Statistical level was achieved in the tests for the total number of nurses and midwives regularly attending prophylactic check-ups and educating patients. The research has confirmed that nurses and midwives that regularly visit prophylactic check-ups have more positive approach to providing information to clients-patients than those that do not undergo the prophylactic check-ups. Nurses and midwives in hospitals pay more attention to the disease itself and to prevention of possible complications in patients than to general prevention and prophylactic check-ups in their educational activities. With regard to the set goals the final evaluation of the research has confirmed that nurses and midwives have positive approach to prophylactic check-ups and try to visit them regularly. Respondents from the primary care are older and visit the prophylactic check-ups more regularly. Prophylactic check-ups have priority in health condition support and maintenance for the respondents. The research has shown that the respondents working in primary care as well as those from hospitals educate themselves in the field of prevention and prophylactic check-ups and are knowledgeable about legislation applicable to prophylactic check-ups.

Hälsokontroller och beteendeförändring hos byggarbetare : en intervjustudie / Health surveillance and behavior change among construction workers : an interview study

Förekomsten av ohälsa är mer vanligt i byggarbetabranschen än i andra branscher, p.g.a. arbetsolyckor och övriga förhållanden i arbetet. En av de viktiga resurserna till att förebygga skada och upprätthålla hälsa i arbetslivet, är företagshälsovård. Syftet med denna kvalitativa intervjustudie var att beskriva byggarbetares upplevelser om deras arbetshälsa/arbetsmiljö och om vad företagshälsovårdens hälsokontroller har haft för betydelse för deras beteendeförändring.   Fem byggarbetare som genomgått en hälsokontroll via företagshälsovård och åstadkommit en potentiell beteendeförändring som förbättrat hälsan, intervjuades. Den tematiska analysen av intervjuerna resulterade i fyra huvudteman: ”möjliggörande faktorer till en god arbetshälsa i byggarbetarens yrke”, ”begränsade faktorer till god arbetshälsa i byggarbetarens yrke”, ”konkreta resultat av företagshälsovårdens hälsokontroller” och ”fördelarna med företagshälsovårdens hälsofrämjande arbete”. Studiens resultat visar att byggarbetare upplevde sin arbetshälsa/arbetsmiljö som både positivt och negativt. Det positiva med arbetet var den sociala kontakten med kollegor och arbetsgivare och det negativt var arbetsmiljön som bidragit till sämre hälsa p.g.a. av muskuloskeletala skador och stress. Hälsokontroller bidrog till beteendeförändring som kan förebygga sjukdomar bland byggarbetarna.   Hälsokontrollen upplevdes som meningsfull då ett gott bemötande möjliggjorde en beteendeförändring samtidigt som en ökad insikt av att förbättra sin egen hälsa skett, efter negativa provsvar. Vidare forskning behövs om hur byggarbetares arbetshälsa kan förbättras genom företagshälsovårdens insatser. / The incidence of illness among construction workers is higher than average in other industries, because of accidents and other conditions at work. One of the most important resources to prevent injury and maintain health in the workplace, is the occupational health care. The aim of this qualitative study was to describe the construction workers experiences of their occupational health and work environment and about whether occupational health check-ups have had any significance for their behavior change. Five male construction workers who had underwent a health check and after that performed a behavior change, was recruited via an occupational care center. Data was collected by face to face and telefon interviews, that were subsequently transcribed and analysed by thematic analyse. The tematic analyse resulted in four main themes: "enabling factors for good health of the construction workers professional”, ”restricted factors to the good health of the construction worker's occupation", "benefits of occupational health promotion" and "concrete results of occupational health check-ups".   The construction workers experienced their occupational health and work environment as both positive and negative. The positive about work was the social contact with colleagues and employers, the negative was that the work environment contributed to poor health due musculoskeletal of injury and stress. Health check-ups contributed to behavior change and pontential prevention of diseases among construction workers.   The health check-ups was considered as being meaningful and enabled a change in behavior after understanding that they must change their lifestyle to avoid disease. Further research is needed on how to improve construction workers occupational health through occupational health services.

Siekiant nustatyti veiksnius, kurie sukelia ginčus, būtina analizuoti ginčytinus profesinių ligų atvejus. Profilaktiniai sveikatos tikrinimai padeda nustatyti profesines ligas ankstyvose stadijose, darbo medicinos gydytojams suteikia svarbią, papildomą informaciją apie pacientą, profesinės ligos nustatymo metu. Todėl nagrinėjant ginčytinus profesinių ligų atvejus svarbu įvertinti profilaktinių sveikatos tikrinimų įtaką. Darbo tikslas. Išanalizuoti Lietuvos ginčytinų profesinių ligų atvejų (GPLA) 2005–2009 m. charakteristikas bei jų sąsajas su profilaktiniais sveikatos tikrinimais. Uždaviniai. Nowadays the burden occupational diseases is discussed very often all around the world. To identify problems causing disputes about occupational disease diagnosis it is necessary to analyze the controversial cases of occupational diseases. Annual health check ups provide important additional information for occupational doctors, who are determining the diagnosis of occupational disease, also helps to identify diseases in early stages. Since the efectivness of annual health check ups is discussed it is necessary to evaluate how annaul health check ups influence changes in the controversial cases of occupational diseases (CCOD). The study describes 52 patients' experiences and feelings towards theirillness and the care received in connection with it. The main purpose ofthe study is to gather knowledge on cancer patients' need of psychosocialcare and from this insight draw up a proposition for a clinically usefulmethod for psychosocial care. I have met and spoken with the patients,each suffering from a different form of cancer, on one occasion. They areall patients o'f the Department of General Surgery, Östersund Hospital.The taped conversations constitute the most important source for thisthesis, which has its crucial point in the empirical part. My theoreticalframe of reference has a psychodynamic and existential perspective.To distinguish and give as clear a picture as possible on "what it'slike to get a cancer and live with it", different phases have been drawnup showing the process of a cancer. Living with a cancer means adjustingto constant uncertainty and a fear of recurrence.
