Clinical learning experiences of university male student nurses during their placement in a clinical setting

Magister Curationis - MCur / An increasing number of males is entering the nursing profession. The researcher in his position as a clinical supervisor at the School of Nursing at the University of the Western Cape (UWC), through informal ward rounds with student nurses in the wards, has received concerns raised by male student nurses regarding their dissatisfaction with their clinical learning. Given the paucity of literature about the experiences of males working in a profession dominated by females, the researcher embarked on this study to understand how male student nurses experienced the clinical learning environment. The aim of the study was to explore and describe the lived clinical learning experience of male student nurses during their experiential learning in the clinical setting. A descriptive phenomenological design was used. Purposive sampling was used to select participants from the second, third and fourth year of their study. Three focus group discussions, consisting of six participants per group were used to collect data. One open-ended question guided the interviews. Focus group discussions were audio-recorded and transcribed verbatim. Data analysis was conducted by means of Colaizzi`s (1978) seven steps method of qualitative analysis. Three major themes identified focused on the experiences regarding the constraints in the learning environment, the impact on the self and social support of students working in a female dominated profession. The participants in this study were male students only, but after looking at the findings and literature, the problem of not being given opportunities to practise clinical skills in a clinical learning environment, particularly according to their level of study, is a problem that faces both male and female students. The findings indicate that male nurses do have different experiences compared to female nurses because of their masculinity, hence they are limited in the care that they can provide to female patients.

Exploring the role of the physiotherapy clinical educators in the clinical setting in Uganda

Clinical education involves translation of theoretical knowledge into practical skills, facilitated by clinical educators whose roles vary from place to place. The clinical educators’ roles have a number of facilitators as well as barriers and these have been minimally evaluated globally and locally, including Uganda. Therefore, the aim of the study was to explore and describe the perception of the physiotherapists regarding their role as clinical educators and to identify the barriers to and facilitators for physiotherapy clinical education at the national and regional referral hospitals in Uganda. / 2022

O brincar na clínica psicanalítica de crianças com autismo / Playing in the psychoanalytic clinic with autistic children

O presente trabalho tem como objetivo estudar o brincar como ferramenta para o analista na clínica psicanalítica de crianças com autismo. Autores como Winnicott, Rodulfo e Guerra defendem que o brincar acompanha pari passu o desenvolvimento subjetivo, de forma que as perturbações que aparecem na estruturação psíquica do bebê podem ver-se refletidas no desenvolvimento da capacidade para o brincar. Essas dificuldades são observadas de forma clara nas crianças com autismo, que apresentam tanto a incapacidade para o brincar quanto perturbações em sua constituição subjetiva. Dessa maneira, o tratamento psicanalítico visa o desenvolvimento do brincar dessas crianças, tendo em vista seus efeitos constitutivos. Este trabalho analisou vinhetas clínicas de crianças em atendimento onde se puderam verificar possibilidades do desenvolvimento do brincar. O material clínico discutido foi organizado em três grandes eixos teóricoclínicos: encontro analítico, estereotipias e objeto tutor (conceito formulado por Victor Guerra e apropriado neste trabalho como elemento que inauguraria a possibilidade do brincar no encontro analítico). O eixo vinculado ao encontro analítico buscou discutir a importância da constância do setting para o atendimento de crianças com autismo. O eixo ligado às estereotipias abordou a importância de intervenções analíticas que propiciam desdobramentos e continuidades em relação a movimentos inicialmente repetitivos e disfuncionais, apresentando a dimensão do novo e da criatividade, além de trazer a abertura para a presença do outro (analista) e do brincar compartilhado. Além disso, apresentamos a ideia de que as estereotipias apresentam alguma complexidade, podendo ser identificadas como uma forma da criança investigar o mundo à sua volta, responder à demanda do analista ou ainda de propor a retomada de brincadeiras. O último eixo de discussão apresentou alguns caminhos que poderiam levar à criação do objeto tutor na clínica do autismo, seja por meio da transformação do objeto autístico ou de propostas de continuidades para os movimentos estereotipados, levando em consideração a presença e a participação ativa por parte do analista. As vinhetas analisadas mostraram caminhos possíveis para a criação do brincar compartilhado por meio de intervenções terapêuticas. Pudemos observar que o desenvolvimento do brincar compartilhado no encontro analítico é um importante aspecto constitutivo no processo de estruturação psíquica das crianças com autismo / This study aims at investigating the role of playing as a clinical tool for the psychoanalyst in his practice with autistic children. Authors such as Winnicott, Rodulfo and Guerra state that playing progresses alongside emotional development, and thus disturbances in the babies psychical structure development will reflect in his capacity to play. These difficulties are clearly observed in children with autism, who present both the incapacity to play and the disturbances in their subjective constitution. Thus, psychoanalytical treatment aims at developing the capacity to play in these children, bearing in mind its aforementioned effects to healthy subjective constitution. The author studies clinical scenes of children undergoing clinical treatment in which possibilities for the development of the capacity to play are present. This clinical material was discussed and organized in three major theoreticalclinical axes: the analytical encounter, stereotypes and tutoring object (the latter being a concept developed by Victor Guerra and mobilized in this study as an element that would develop the capacity to play in the analytical encounter). In the axis concerning the analytical encounter we propose to discuss the importance of the settings constancy for the clinical treatment of children with autism. In the second axis, regarding stereotypes, we evaluate the importance of the analytical interventions that make it possible for stereotypical behavior to develop out of its initial dysfunctionality and purposelessness into communicative and relational behavior, presenting thereby the possibility of innovation and creativity and the openness to the presence of the analyst and to shared play. We present additionally the idea that stereotypes present complexity, being potentially a means for the child to investigate the world, to answer to an appeal or even to propose an engagement in interactive play. The last axis of investigation presented some strategies that could lead to the establishment of tutoring objects in clinical practice with children with autism, might it be through the transformation of an autistic object into a tutoring object or through the proposition of enactments that emerge from the stereotypical behavior into meaningful and communicative behavior, always bearing in mind the active presence and participation of the analyst. The clinical scenes presented aim at shedding light into possible paths for the creation of shared play activity, developed by therapeutic intervention. The author considers that the development of the capacity by the child to engage in shared play activities with the analyst in the clinical encounter is a crucial aspect in the process of psychical development of children with autism

Implementing Child Maltreatment Prevention into the Clinical Setting: an On-Line Learning Tutorial for Advanced Practice Nurses

Child maltreatment in the pediatric population is becoming more prevalent in today’s society and is being seen more frequently in the primary care setting. Universal prevention of child maltreatment plays a significant role in nursing practice. The American Academy of Pediatrics set forth guidance for pediatricians to practice when assessing a pediatric patient for maltreatment; however, such guidance is not available for advanced practice nurses and little to no training is provided in the clinical setting. In order to accurately identify the child who is a suspected victim of maltreatment, advanced practice nurses must possess the ability to assess, evaluate, refer, investigate, and provide appropriate outcomes for the child and his/her families. The purpose of this Doctor of Nursing (DNP) project was to create an on-line learning tutorial for advanced practice nurses regarding the prevention of child maltreatment in the clinical setting. The goal of the on-line learning tutorial was to address the crucial role advanced practice nurses have in the prevention of child maltreatment and promote the well-being and safety of children. This project used a quasi-experimental, one-group, pre-test/post-test design to determine the effectiveness of an on-line learning tutorial related to child maltreatment. The pre-test and post-test would determine the inferences on the effect of the intervention by examining the differences in the pre- and post-test results. This study provided significant evidence demonstrating that an on-line learning tutorial on the implementation of child maltreatment prevention in the clinical setting was an effective means for increasing knowledge of nurse practitioner graduate students on child maltreatment. The results demonstrated a significant increase in the test scores of the participants after viewing the on-line learning tutorial, indicating the tutorial was effective.

The Role of the Art Therapist: A Multi-Faceted Approach

This research explores the role of the art therapist in a nonclinical setting. The research dissects the experiences of nine art therapists who participated in an artist residency that was informed by art therapy but was not clinically based. The spectrum of literature reviewed focused on the professional identity of art therapists and therapists as well as social action art therapy and working with Native American cultures. A qualitative approach was utilized through the conduction of a focus group along with four individual interviews, which were both enhanced by a parallel art making process. Analysis of the data resulted in four significant themes: illuminating strengths and activating existing resources, containment, chaos, and finding hope amidst inadequacy. The findings were triangulated with the art therapy literature reviewed on the identity of the art therapist as well as leading art therapists’ visions for the field’s future. The research accentuates the complexity and significance of art therapists participating culturally informed, with underserved communities and redefining their role in order to carry out that purpose. In sum, the research offers insight into how art therapists can creatively and with great sensitivity, “meet clients where they are at”.

Factors influencing clinical teaching of midwifery students in a selected clinical setting in Tanzania

A quantitative, descriptive non-experimental design was used to identify the factors that influence clinical teaching of midwifery students in selected postnatal clinical settings in Tanzania. Structured questionnaires were used to collect data from midwifery tutors/preceptors. The major findings of the study showed that both the professional and educational qualification of tutors was low; tutors and preceptors were overworked due to shortage of staff; the school skills laboratory and postnatal wards lacked equipment and necessary supplies; there were no clinical accompaniment guidelines, and overcrowding of postnatal patients. Recommendations included developing the clinical teaching guidelines, employing more tutors, preceptors and clinical staff; improving the tutors'/ preceptors' educational and professional qualifications and updating the qualifications of ward supervisors and clinical facilitators. / Health Studies / M.A. (Health Studies)

Clinical teaching by registered nurses

The purpose of this quantitative, descriptive study was to describe how registered nurses utilise teachable moments to enhance students' learning in the clinical setting. The research questions were: What are the problems faced by registered nurses when they teach students in the clinical setting? and How do registered nurses utilise teachable moments to teach students? A pilot study was conducted in the clinical setting involving the respondents who had similar characteristics than the population. The sample consisted of 45 registered nurses who provided direct patient care in the clinical setting. A self­ administered, structured questionnaire was used to collect data. Data analysis was done by using descriptive statistical tests. The findings brought to light strengths and weaknesses with regard to the utilisation of teachable moments by registered nurses, and problems faced by registered nurses with regard to clinical accompaniment in general. Recommendations to improve clinical accompaniment by registered nurses through effective utilisation of teachable moments were made. The impact of the rights and obligations of nurses on patient care in a critical setting in Gauteng Province

This study explored and described the impact of the rights and obligations of nurses on the delivery of quality patient care in a clinical setting. Quantitative research approach was utilised. Data was collected using a self-administered questionnaire. The research results revealed that respondents who participated in this study were aware of their rights and obligations, the effects and impact of factors influencing such rights and obligations on patient care. It was established that “patient abandonment” observed when nurses embark on a strike as their constitutionally enshrined right is not due to a lack of insight about their rights and obligations, but on how such rights and obligations are implemented. The researcher recommends that further research be undertaken to explore the causes of nurses embarking on strike actions despite their high level of knowledge concerning the impact of such actions on patient care in a clinical setting / Health Studies / M.A. (Health Studies)

It is justified as part of a larger project, which aims at proposing a differentiated setting to be used in institutional contexts and which has sport as playful mediation. It is methodologically organized around the use of the Themed Drawing-Stories Procedure in a collective psychological interview with nineteen teenagers who attend a NGO dedicated to professionalizing teenagers from lowincome families. This interview was registered through a transferential narrative encompassing interactions, happenings, and the researcher?s subjective impressions. Considering the production under the psychoanalytical method allows for the creation of four affective-emotional meaning fields: ?Sport is success?, ?Sport is courage?, ?Sport is healthy? and ?Sport is dangerous?. The overall picture indicates the predominance of a favorable, though utilitarian, view on sport, which does not disfavor its use in psychological clinic. / A presente pesquisa tem como objetivo investigar o imagin?rio coletivo de adolescentes sobre o esporte. Justifica-se como etapa de um projeto maior de proposi??o de enquadre diferenciado, a ser utilizado em contextos institucionais, que toma o esporte como media??o brincante.
