Tratamento de retrações gengivais múltiplas adjacentes com a matriz colágena xenogênica comparado ao enxerto de tecido conjuntivo subepitelial: ensaio clínico aleatório de não-inferioridade / Treatment of multiple adjacent gingival recessions with collagen matrix compared with subephitelial connective tissue graft: non-inferiority randomized clinical trial

Rodrigo Carlos Nahás de Castro Pinto 16 September 2016 (has links)
The aim of this study was to compare the 6 months outcome of root coverage, dentin hypersensitive, pain after surgery and aesthetics using the modified coronal advanced flap (mCAF) associate with connective tissue graft (CTG) or collagen matrix (CM) in the treatment of gingival recession (GR). Fifteen patients with superior multiple recessions in canines and pre-molars were selected for this non-inferority randomized clinical trial, which was divided in control group (mCAF+CTG) and test group (mCAF+CM). The residual GR (primary outcome) was lower for control group (0,3 ± 0,6 mm) comparing with test group (0,6 ± 1,0 mm) (p = 0,008). The root coverage percentage was 89,3% for control group and 77,7% for test group. Both groups demonstrated gain of keratinized tissue in 6 months (p < 0,05) statically different between groups (p = 0,031). The probing depth and gain of clinical attachment level were similar between the groups in all evaluated periods. Dentin hypersensitive was reduced in all groups. The pain after surgery was higher in control group comparing with test group. Aesthetic satisfaction evaluated by patient and periodontist was high independently of group with no statistical difference, however, the scar formation at papillae base and visual differences were visually higher using mCAF+CTG. Considering the clinical outcomes balanced with patient related outcomes and aesthetic satisfaction, the use of mCAF+CM is a alternative to mCAF+CTG in the treatment of multiple gingival recession.

Avaliação da morbidade pós-operatória e qualidade de vida no recobrimento de retrações gengivais múltiplas através do uso de matriz colágena xenogênica (Mucograft®) comparada ao enxerto de tecido conjuntivo subepitelial: ensaio clínico randomizado cego / Postoperative assessment and quality of life in multiple recession coverage using xenogeneic collagen matrix (Mucograft®) compared to subepithelial connective tissue graft: a randomized clinical trial

Luis Marcelo Monteiro Calderero 05 December 2016 (has links)
There are few randomized controlled clinical trials mentioning the oral health-related quality of life (OHRQoL) effect and morbidity in treatments of multiple gingival recession coverage. This study aims to compare the OHRQoL and morbidity in two groups of patients: A. surgical intervention with connective tissue graft (CTG) removed from the palate; B. surgical intervention using a collagen matrix (CM - no graft from the palate) in the treatment of multiple gingival recessions. Methods: A split-mouth randomized clinical trials was conducted. Fifteen patients with bilateral multiple recessions in maxillary canines and pre-molars were selected. Quality-of-life was assessed by the Oral Health Impact Profile-14 (OHIP-14) questionnaire through multinível regression. The morbidity included pain, quantify of medication, healing and inflammation/edema. Parameters were evaluated at baseline, 7, 15, 30, and 180 days. Results: OHIP-14 in 7 and 15 days showed better OHRQoL in CM group when compared with CTG (p=0,002 and p=0,001 respectively). Correlation was stablished in OHIP-14 subscales after treatment with CM: physical pain, psychological discomfort, physical disability, psychological disability, and handicap (p<0,05). There were statistically significant differences for pain and amount of medication, favoring CM group (p<0,05). Conclusion: This study shows that surgical treatment with CM for multiple gingival recessions Miller Class I was positively associated with psychological and physical impacts on patient\'s quality of life, when compared with CTG removed from the palate.

Retinal Pigment Epithelium Cell Alignment on Nanostructured Collagen Matrices

Ulbrich, Stefan, Friedrichs, Jens, Valtink, Monika, Murovski, Simo, Franz, Clemens M., Müller, Daniel J., Funk, Richard H. W., Engelmann, Katrin 04 March 2014 (has links) (PDF)
We investigated attachment and migration of human retinal pigment epithelial cells (primary, SV40-transfected and ARPE-19) on nanoscopically defined, two-dimensional matrices composed of parallel-aligned collagen type I fibrils. These matrices were used non-cross-linked (native) or after riboflavin/UV-A cross-linking to study cell attachment and migration by time-lapse video microscopy. Expression of collagen type I and IV, MMP-2 and of the collagen-binding integrin subunit α2 were examined by immunofluorescence and Western blotting. SV40-RPE cells quickly attached to the nanostructured collagen matrices and aligned along the collagen fibrils. However, they disrupted both native and cross-linked collagen matrices within 5 h. Primary RPE cells aligned more slowly without destroying either native or cross-linked substrates. Compared to primary RPE cells, ARPE-19 cells showed reduced alignment but partially disrupted the matrices within 20 h after seeding. Expression of the collagen type I-binding integrin subunit α2 was highest in SV40-RPE cells, lower in primary RPE cells and almost undetectable in ARPE-19 cells. Thus, integrin α2 expression levels directly correlated with the degree of cell alignment in all examined RPE cell types. Specific integrin subunit α2-mediated matrix binding was verified by preincubation with an α2-function-blocking antibody, which impaired cell adhesion and alignment to varying degrees in primary and SV40-RPE cells. Since native matrices supported extended and directed primary RPE cell growth, optimizing the matrix production procedure may in the future yield nanostructured collagen matrices serving as transferable cell sheet carriers. Extracellular Matrix Synthesis and Remodeling by Mesenchymal Stromal Cells Is Context-Sensitive

Burk, Janina, Sassmann, Anna, Kasper, Cornelia, Nimptsch, Ariane, Schubert, Susanna 16 January 2024 (has links)
Matrix remodeling could be an important mode of action of multipotent mesenchymal stromal cells (MSC) in extracellular matrix (ECM) disease, but knowledge is limited in this respect. As MSC are well-known to adapt their behavior to their environment, we aimed to investigate if their mode of action would change in response to healthy versus pathologically altered ECM. Human MSC-derived ECM was produced under different culture conditions, including standard culture, culture on Matrigel-coated dishes, and stimulation with the pro-fibrotic transforming growth factor-1 (TGF1). The MSC-ECM was decellularized, characterized by histochemistry, and used as MSC culture substrate reflecting different ECM conditions. MSC were cultured on the different ECM substrates or in control conditions for 2 days. Culture on ECM increased the presence of surface molecules with ECM receptor function in the MSC, demonstrating an interaction between MSC and ECM. In MSC cultured on Matrigel-ECM and TGF1-ECM, which displayed a fibrosis-like morphology, gene expression of collagens and decorin, as well as total matrix metalloproteinase (MMP) activity in the supernatant were decreased as compared with control conditions. These results demonstrated that MSC adapt to their ECM environment, which may include pathological adaptations that could compromise therapeutic efficacy.
