The first manifestations of this disease are usually depressive states that can be frequently mistaken by unipolar depression. An incorrect diagnosis at this point can turn into a problem as it delays the specific treatment for bipolar disease, or even worse, an inappropriate treatment can aggravate the clinical presentation. Currently differential diagnosis between bipolar disorder and unipolar depression holds on the presence of a maniac or hippomaniac episode. This study aims to evaluate social demographics and clinical aspects of young adults with age between 18-29 years, living in the city of Pelotas, Rio Grande do Sul, Brazil, searching for factors that can differentiate both affective diseases. / O transtorno afetivo bipolar é uma das doenças mais incapacitantes do mundo. As primeiras manifestações dessa doença geralmente são quadros depressivos que podem ser facilmente confundido com um transtorno depressivo unipolar. Um erro no diagnóstico pode acarretar em atraso no tratamento específico e até mesmo em um tratamento incorreto que pode levar a um agravamento do quadro clínico. Atualmente o diagnóstico diferencial entre transtorno afetivo bipolar e transtorno depressivo é feito apenas pela existência de um episódio maníaco ou hipomaníaco nos pacientes bipolares. O presente estudo busca avaliar aspectos sócios demográficos e clínicos de pacientes adultos jovens com idade entre 18-29 anos, residentes no município de Pelotas, Rio Grande do Sul, Brasil, a fim de identificar fatores que possam diferenciar ambos os quadros afetivos.

ASSESSMENT OF THE SERUM AMYLOID A ASSAY FOR DIAGNOSING DISEASE IN NEONATAL FOALS

Diagnosing disease in equine neonates poses a challenge for the equine industry because of the nonspecific manifestations of many diseases and the rapid deterioration that occurs. The differential diagnostic procedure requires many laboratory tests, whose results take days to receive. Serum amyloid A (SAA) is the only major acute phase protein identified in the horse; it exists in low levels in the healthy horse and increases over 100 fold in response to inflammatory stimulus 6-8 hours post stimulus. A point of care test allows veterinarians to obtain a SAA concentration within minutes that indicates the existence of infection. Being able to test and quantify this protein at the onset of illness may reduce the time before treatment is initiated and therefore increase the chance of survival for the equine neonate, which would greatly help a large problem in the industry.

Automated Detection and Differential Diagnosis of Non-small Cell Lung Carcinoma Cell Types Using Label-free Molecular Vibrational Imaging

Lung carcinoma is the most prevalent type of cancer in the world, considered to be a relentlessly progressive disease, with dismal mortality rates to patients. Recent advances in targeted therapy hold the premise for the delivery of better, more effective treatments to lung cancer patients, that could significantly enhance their survival rates. Optimizing care delivery through targeted therapies requires the ability to effectively identify and diagnose lung cancer along with identifying the lung cancer cell type specific to each patient, \textit{small cell carcinoma}, \textit{adenocarcinoma}, or \textit{squamous cell carcinoma}. Label free optical imaging techniques such as the \textit{Coherent anti-stokes Raman Scattering microscopy} have the potential to provide physicians with minimally invasive access to lung tumor sites, and thus allow for better cancer diagnosis and sub-typing. To maximize the benefits of such novel imaging techniques in enhancing cancer treatment, the development of new data analysis methods that can rapidly and accurately analyze the new types of data provided through them is essential. Recent studies have gone a long way to achieving those goals but still face some significant bottlenecks hindering the ability to fully exploit the diagnostic potential of CARS images, namely, the streamlining of the diagnosis process was hindered by the lack of ability to automatically detect cancer cells, and the inability to reliably classify them into their respective cell types. More specifically, data analysis methods have thus far been incapable of correctly identifying and differentiating the different non-small cel lung carcinoma cell types, a stringent requirement for optimal therapy delivery. In this study we have addressed the two bottlenecks named above, through designing an image processing framework that is capable of, automatically and accuratly, detecting cancer cells in two and three dimensional CARS images. Moreover, we built upon this capability with a new approach at analyzing the segmented data, that provided significant information about the cancerous tissue and ultimately allowed for the automatic differential classification of non-small cell lung carcinoma cell types, with superb accuracies.

Avaliação do papel dos marcadores CD200, CD43, CD52 e CD123 no diagnóstico diferencial das doenças linfoproliferativas crônicas b

Introdução: as doenças linfoproliferativas de células B maduras correspondem a cerca de 80% das neoplasias linfoides, são caracterizadas pela proliferação clonal de uma célula B precursora em diferentes estágios de diferenciação. A semelhança imunofenotípica a um dado estágio maturativo é relevante para o diagnóstico diferencial através da imunofenotipagem por citometria de fluxo, embora a sobreposição de expressões possa dificultar a identificação correta de cada linfoproliferação. Alguns marcadores são pouco conhecidos nas diferentes linfoproliferações B e há pouca literatura analisando os dados quantitativamente, sendo grande parte qualitativa. Objetivos: este trabalho avalia a expressão quantitativa em intensidade de fluorescência média (IFM) dos anticorpos CD200, CD43, CD52 e CD123 no diagnóstico diferencial das doenças linfoproliferativas B crônicas. Métodos: estudo transversal, com 124 amostras de pacientes em investigação diagnóstica de doenças linfoproliferativas que realizaram imunofenotipagem em um centro de referência em neoplasias hematológicas no período de outubro de 2014 a junho de 2015. Foram analisadas as IFMs de cada marcador nas onze diferentes doenças diagnosticadas. Resultados: as neoplasias dos 124 pacientes analisados foram: 81 leucemias linfocíticas crônicas (LLC), 17 linfomas de zona marginal (LZM), 9 linfomas linfoplasmocíticos (LLPL), 6 linfomas do manto (LM), 2 tricoleucemias (TRL), 2 tricoleucemias variantes (TRLv), 5 linfomas foliculares (LF), 1 linfoma de Burkitt e 1 linfoma difuso de grandes células B (LDGCB). A expressão mediana do CD200 foi de 46,8 (intervalo: 1,5-334). A expressão mediana de CD200 foi maior na TRL (incluindo a TRLv) e na LLC (85 e 61,2, respectivamente). A expressão de IFM do CD200 na TRLv diferiu quando comparado à TRL na sua forma clássica (mediana: 36,1 versus 220,3). A mesma diferença foi observada na expressão do CD123 quando comparada a TRL à TRLv. Verificamos, que casos de LZM demonstraram IFM medianas de CD43 de 7,1 (intervalo: 1,1-106), em comparação a casos de LM e LLC, nos quais as medianas de IFM do CD43 foram 90 e 176, respectivamente. A comparação da intensidade de CD52 entre amostras demostrou diferença estatisticamente significativa entre LLC e LZM com medianas de IFM de 775,5 versus 1297,0 (P=0,04). Conclusões: nossos resultados sugerem que a citometria de fluxo quantitativa desses marcadores pode ser uma ferramenta adicional útil na identificação de alguns tipos de DLPCBs. / Background: lymphoproliferative disorders of mature B cells account for about 80% of the lymphoid malignancies. Objectives: this study evaluated the expression of CD200, CD43, CD52 and CD123 by flow cytometry on B-cell chronic lymphoproliferative disorders (BCLDs) differential diagnosis. Methods: cross-sectional study, of 124 samples from patients for diagnostic investigation of lymphoproliferative disorders who underwent immunophenotyping in a referral center for hematologic malignancies from October 2014 to June 2015. MFIs were analyzed for each marker in eleven different diagnosed pathologies. Results: the diseases of the 124 patients investigated comprised: 81 chronic lymphocytic leukemia (CLL), 17 marginal zone lymphoma (MZL), 9 lymphoplasmacytic lymphoma (LPL), 6 mantle cell lymphoma (MCL), 2 hairy cell leukemia (HCL), 2 hairy cell leukemia variant (HCLv), 5 folicular lymphoma (FL), 1 Burkitt lymphoma (BL) e 1 diffuse large B cell lymphoma (DLBCL). The CD200 median MFI expression was 46,8 (range: 1,5-334). CD200 was higher in HCL (including HCLv) and CLL cells (85 and 61,2 respectively). HCLv difference in CD200 MFI, when compared to classical HCL (median 36,1 versus 220,3). The same difference in CD123 expression was observed when comparing HCL versus HCLv. O estudo trata de infecções fúngicas por Histoplasma, Cryptococcus gattii e Paracoccidioides e por Actinomyces e Nocardia que simularam neoplasia a partir de achados radiológicos. O objetivo deste trabalho é demonstrar, através de casos do banco de dados do Laboratório de Micologia da Irmandade Santa Casa de Misericórdia de Porto Alegre, a importância de um diagnóstico diferencial destas infecções com doenças neoplásicas. Identificamos os aspectos epidemiológicos e clínico-laboratoriais desses pacientes e revisamos criticamente a literatura. A casuística foi constituída por 83 casos, extraídos de uma população de 1.368 pacientes, de 1981 a 2012. As características clínicas e radiológicas de neoplasia foram observadas principalmente na histoplasmose, pela formação de histoplasmoma, na criptococose por C. gattii pela capacidade de formar criptococoma; na actinomicose e na nocardiose pela produção de massas. O que reforça o fato de que a síndrome clínica não é patognomônica da doença neoplásica e que as doenças infecciosas, especialmente granulomatosas devem ser consideradas no diagnóstico diferencial. / The study comes to infections by Histoplasma, Cryptococcus gattii, Paracoccidioides, and also Actinomyces and Nocardia simulating neoplasia from radiological findings. The objective of this study is to demonstrated, through case database of Mycology Laboratory of Irmandade Santa Casa de Misericordia de Porto Alegre, the importance of a differential diagnosis of these infections with neoplastic disease. We identified the epidemiological, clinical and laboratorial datas of these cases and review the literature critically. The casuistic consisted of 83 cases, drawn from a population of 1,368 patients, from 1981-2012. The clinical and radiological features of malignancy were observed mainly in histoplasmosis by manifestation as histoplasmoma, in cryptoccosis by C.gattii to the ability to form crytococcoma; actinomycosis and nocardiosis by the production of masses. Diagnósticos diferenciais de hérnias umbilicais em Suínos no abate / Differential diagnoses of umbilical hernias in slaughter pigs

Existe a necessidade de um diagnóstico preciso sobre a causa dos aumentos de volume na área umbilical de suínos por ocasião do abate, para que sejam adotados critérios adequados durante a inspeção das carcaças. Foi realizada uma avaliação deste tipo e alterações foram observadas e retiradas da carcaça, para abertura e categorização. Através da descrição das características anatomopatológicas externas e internas, e classificadas as diferentes formas de aumento de volume na região umbilical. Foram diagnosticados 515 aumentos de volume entre 96.599 suínos abatidos (0,53%), destes 235 foram avaliados e categorizadas 19 condições patológicas, com diversos tipos de associações. Os diagnósticos obtidos foram: hérnias umbilicais, flaps, cistos, fibrose e abscessos. Hérnias, associadas ou não, representaram o diagnóstico mais frequente, 155 casos. A seguir foram os "flaps", cistos, fibrose e abscessos, respectivamente. No exame histopatológico do saco herniário, foi observada proliferação de tecido conjuntivo denso, adjacente à área de tecido conjuntivo frouxo, revestida por mesotélio. Ainda, áreas de necrose, mineralização e hemorragia focalmente extensa. A composição dos flaps foi basicamente tecido conjuntivo frouxo com acentuada quantidade de formações vasculares, revestido por mesotélio. Os cistos apresentaram proliferação de tecido conjuntivo frouxo com acentuada quantidade de formações vasculares e dilatações císticas com material amorfo eosinofílico no interior, revestido por mesotélio e proliferação de tecido conjuntivo denso perifericamente com área focal de infiltrado de linfócitos. Em relação ao destino das 235 carcaças, 135 (57,44%) foram liberadas para mercado interno, 91 (38,72%) submetidas ao tratamento térmico e 9 (3,82%) enviadas à graxaria. Durante a análise dos casos de aumento de volume na área umbilical pela inspeção federal, não foram adotados os critério de condenação considerando a caracterização precisa das lesões, proposta no presente trabalho. A adoção dos mesmos poderia contribuir para a redução de perdas, sem comprometer critérios que visam a garantir a segurança alimentar. / It is important to reach a precise diagnosis of causes of increase in the volume in the umbilical area of pigs at slaughter, to enable decisions on the use of adequate criteria of carcass inspection. A study to evaluate this problem was carried out observing and removing lesions from the carcass to dissection and characterization. Through description of internal and external anatomopathologic characteristics, different forms of increase in the volume in the umbilical area were classified. Between 96,599 carcasses, 515 increases in the volume were diagnosed (0,53%), among them 235 were assessed and categorized and 19 pathological conditions with different associations were found. The main diagnosis were: umbilical hernias, flaps, cysts, fibrosis and abscesses. Hernias, in isolation or in association, were the most frequent diagnosis, 155. The following were the "flaps", cysts, abscesses and fibrosis, respectively. Using histopathological examination of the hernial sac, it was found proliferation of dense conjunctive tissue, adjacent to an area of loose conjunctive tissue, covered by mesothelium. Additionally, areas of necrosis, mineralization and focally extensive hemorrhage. The composition of flaps was basically loose conjunctive tissue with decreased amount of vascular formations, covered by mesothelium. Cysts showed proliferation of loose connective tissue with accentuated amount of vascular formations and with amorphous eosinophilic material inside, covered by mesothelium and proliferation of dense connective tissue with focal areas of inflammatory infiltrate composed by lymphocytes in the periphery. With respect to the destiny of the 235 carcasses, 135 (57,44%) were released for consumption in the internal market, 91 (38,72%) submitted to heat treatment and 9 (3,82%) sent to meat-and-bone-meal production. During the analysis of cases of increase in the volume in the umbilical area by the Federal Inspection, criteria proposed in the present work were not fully adopted. Its use could contribute to reduce unnecessary losses, without compromising a criterion which would maintain guarantees of food safety.

Os diagnósticos obtidos foram: hérnias umbilicais, flaps, cistos, fibrose e abscessos. Hérnias, associadas ou não, representaram o diagnóstico mais frequente, 155 casos. A seguir foram os "flaps", cistos, fibrose e abscessos, respectivamente. No exame histopatológico do saco herniário, foi observada proliferação de tecido conjuntivo denso, adjacente à área de tecido conjuntivo frouxo, revestida por mesotélio. Ainda, áreas de necrose, mineralização e hemorragia focalmente extensa. A composição dos flaps foi basicamente tecido conjuntivo frouxo com acentuada quantidade de formações vasculares, revestido por mesotélio. Os cistos apresentaram proliferação de tecido conjuntivo frouxo com acentuada quantidade de formações vasculares e dilatações císticas com material amorfo eosinofílico no interior, revestido por mesotélio e proliferação de tecido conjuntivo denso perifericamente com área focal de infiltrado de linfócitos. Em relação ao destino das 235 carcaças, 135 (57,44%) foram liberadas para mercado interno, 91 (38,72%) submetidas ao tratamento térmico e 9 (3,82%) enviadas à graxaria. Durante a análise dos casos de aumento de volume na área umbilical pela inspeção federal, não foram adotados os critério de condenação considerando a caracterização precisa das lesões, proposta no presente trabalho. A adoção dos mesmos poderia contribuir para a redução de perdas, sem comprometer critérios que visam a garantir a segurança alimentar. / It is important to reach a precise diagnosis of causes of increase in the volume in the umbilical area of pigs at slaughter, to enable decisions on the use of adequate criteria of carcass inspection. A study to evaluate this problem was carried out observing and removing lesions from the carcass to dissection and characterization. Through description of internal and external anatomopathologic characteristics, different forms of increase in the volume in the umbilical area were classified. Between 96,599 carcasses, 515 increases in the volume were diagnosed (0,53%), among them 235 were assessed and categorized and 19 pathological conditions with different associations were found. The main diagnosis were: umbilical hernias, flaps, cysts, fibrosis and abscesses. Hernias, in isolation or in association, were the most frequent diagnosis, 155. The following were the "flaps", cysts, abscesses and fibrosis, respectively. Using histopathological examination of the hernial sac, it was found proliferation of dense conjunctive tissue, adjacent to an area of loose conjunctive tissue, covered by mesothelium. Additionally, areas of necrosis, mineralization and focally extensive hemorrhage. The composition of flaps was basically loose conjunctive tissue with decreased amount of vascular formations, covered by mesothelium. Cysts showed proliferation of loose connective tissue with accentuated amount of vascular formations and with amorphous eosinophilic material inside, covered by mesothelium and proliferation of dense connective tissue with focal areas of inflammatory infiltrate composed by lymphocytes in the periphery. With respect to the destiny of the 235 carcasses, 135 (57,44%) were released for consumption in the internal market, 91 (38,72%) submitted to heat treatment and 9 (3,82%) sent to meat-and-bone-meal production. During the analysis of cases of increase in the volume in the umbilical area by the Federal Inspection, criteria proposed in the present work were not fully adopted. Its use could contribute to reduce unnecessary losses, without compromising a criterion which would maintain guarantees of food safety.
