Efficacy of Methods Available to Remove Environmental Tobacco Smoke in Vehicles

Environmental tobacco smoke (ETS) is a serious public health threat. Residual ETS in vehicles unknowingly exposes future occupants to environmental tobacco smoke. Reducing or removing exposure to ETS has been proven to reduce long-term health complications. This quasi-experimental study investigated 5 cleaning methods and their effect on air particulate matter2.5 (PM2.5) along with the correlation between air nicotine levels and PM2.5 levels. Study variables included cleaning methods as the independent variable, and changes in air nicotine and PM2.5 levels as the dependent variables. This study is framed within primary prevention and risk reduction based on the harm reduction theory. The harm reduction theory professes that when a hazard cannot be completely removed, methods to reduce the social and personal costs associated with the hazard should be developed. Fifty vehicles were placed in 5 groups: car wash vacuumed, shop vac vacuumed, air change, hand held vacuumed, and Hepa filtration air cleaned. Nicotine and PM2.5 levels were measured before and after cleaning. A Wilcoxon ranked test analysis of the data showed all methods of cleaning studied had a statistically significant decrease in both air nicotine (Z = -6.154, p < .001) and PM2.5 levels (Z = -5.934, p < .001). Kruskall-Wallis analysis showed no statistical significance between cleaning methods. Correlation analysis determined no correlation between nicotine and PM2.5 (r value = <.3). Results of this study provides public health program professionals with information linking cleaning methods to reduction of exposure to ETS. Positive social change comes when programs are developed to training and education people to reduce their exposure, resulting in an increase in health and a decrease in medical costs.

Cardiovascular Outcomes in Nonsmokers Exposed to Secondhand Smoke: Results From The National Health and Nutrition Examination Survey (NHANES) 2015-2016

While the adverse health risks associated with smoking have been well-documented, few studies have examined the cardiovascular outcomes associated with secondhand smoking. The purpose of the study was to assess the distributions and association of cardiovascular diseases (CVDs) in nonsmokers exposed to secondhand smoke (SHS). Data were extracted from the National Health and Nutrition Examination Survey (NHANES) 2015-2016 cycle. Self-reported smoking status and cotinine levels were used to identify exposure groups (smokers, nonsmokers, and secondhand smokers), and medical history of several cardiovascular diseases such as coronary heart diseases and stroke were also collected via self-report survey. The association between exposure to SHS and seven cardiovascular outcomes were analyzed using chi-square analysis and odd ratios (OR) with 95% confidence intervals (CIs) were calculated using two logistic regression models. The data included 5,709 subjects including 18.5% smokers, 23.6% secondhand smokers, and 57.9% nonsmokers. There was statistically significant association between exposure to SHS and only two out of seven cardiovascular outcomes, hypertension (OR 1.554, 95% CI [1.066, 2.265]) and cholesterol levels (OR 1.213, 95% CI [1.017, 1.446]). This study is one of the first to determine an association between SHS and seven cardiovascular outcomes, thus highlighting the importance of reducing SHS exposure and can be used for further research on SHS and cardiovascular health.

NON-TRADITIONAL EXPOSURES AND CHILDHOOD DENTAL CARIES AMONG CHILDREN 1-5 YEARS OLD

Background: Early Childhood Caries (ECC) is one of the most common chronic illnesses among young children, affecting around 23% of children 2-5 years old, leading to pain, discomfort, and poor quality of life. It is a multi-factorial disease that develops through the combined effects of bacteria, tooth morphology, fermentable carbohydrates, time, and various social factors. Several studies have investigated the associations between dental caries and non-traditional factors acquired during the first years of life including; mode of delivery, breastfeeding, and Environmental Tobacco Smoke (ETS). However, the literature on these associations has been inconsistent. Objective: To investigate the unadjusted and adjusted associations between the presence of Early Childhood Caries (ECC) and Severe-ECC (S-ECC), and three non-traditional factors: breastfeeding, mode of delivery, and ETS, among children 1-5 years old. Methods: A cross-sectional design was employed, and a sample of 112 caregiver/ child dyads was recruited from the ongoing flow of patients at Temple University Maurice H. Kornberg School of Dentistry (TUKSoD). After consent, subjects completed a questionnaire and received a standard intra-oral examination and the American Academy of Pediatric Dentistry (AAPD) Caries-risk Assessment Tool (CAT) by a student doctor. The study was approved by the Temple University Institutional Review Board (Protocol # 23885). Chi-square tests, two-sample t-tests and bivariate logistic regressions were used to assess the unadjusted associations. Two multivariable logistic models were developed for ECC and S-ECC and included demographics, overall CRA, and the three non-traditional risk factors. Results: The prevalence of ECC and S-ECC were 61% and 30%, respectively. The following variables were significant in the unadjusted analysis for both ECC and S-ECC: child’s age, maternal educational attainment, overall AAPD CAT classification, sugary snacks per day, presence of plaque on child’s teeth and ETS. Exposure to ETS was associated with an increased adjusted odds ratio for ECC (aOR=5.39 [95% CI: 1.14-25.33], P=0.033), but not for S-ECC. Furthermore, C-section birth was associated with a decreased adjusted odds ratio for both ECC and S-ECC, respectively (ECC: aOR=0.132 [95% CI: 0.02-0.72], P=0.02; S-ECC: aOR=0.141 [95% CI: 0.026-0.748], P=0.021). With inclusion of the AAPD CAT, demographics, and the three non-traditional factors, the overall model accuracy at predicting ECC was 82.2%. Conclusions and Clinical Relevance: In this study of urban, predominantly African American, and low income children, ECC was found associated with two non-traditional factors, ETS and mode of delivery, suggesting that including them in CRA may improve prediction of future dental caries, and aid in the prevention and treatment of disease. Results from this study support the notion that ECC is a multi-factorial disease, and highlights the importance of adopting oral health education among caregivers. / Clinical Research and Translational Medicine

Tobacco smoking has been proved to be harmful to human health and is a known cause of many diseases. It has been shown that not only active smoking can cause health inconveniences - even an exposure to the environmental tobacco smoke (so called secondhand - SHS - or passive smoking) is a serious threat to human health, in some aspects nearly as severe as active smoking. Being aware of these risks, many countries have recently amended their policies which more or less restrict smoking in public places, especially workplaces and hospitality venues. In the Czech Republic, the smoke-free policy belongs to the less strict related to other countries, because it does not restrict smoking at all public places - restaurant and bar owners can choose whether their venue is smoke-free or not and their duty is only to label their venue both outside and inside. The law also enables the option of setting smoking and non-smoking spaces within the restaurant or bar. In a representative sample (over 100) of smoking and non-smoking sections of these restaurants and bars the concentrations of PM2.5, a marker of SHS concentrations, were measured with a photometer. Advancing Global Tobacco Control: Exploring Worldwide Youth Attitudes and Behaviors toward Tobacco Use and Control

Tobacco use continues to be the leading cause of preventable death in the world. The disproportionate increase in tobacco use in low- and middle-income countries needs immediate attention. Many smokers begin smoking as adolescents and are most likely to become permanent smokers. Moreover, youth are highly targeted by tobacco industry strategies. However, a gap exists in literature to understand worldwide youth tobacco use and control. The purpose of this study is to 1) identify factors that influence never-smoking youths‟ smoking susceptibility, 2) explore characteristics that influence youth exposure to environmental tobacco smoke (ETS) and 3) to delineate key determinants of youth support for smoke-free policies (SFPs). Data related to worldwide youth tobacco use was obtained from the Global Youth Tobacco Survey for the years 1999-2008. Simple and multiple logistic regression analyses were conducted, after the sample was weighted for design effect, nonresponse patterns and poststratification. Unadjusted and adjusted odds ratios along with 95% confidence intervals were reported. Around 14% of never-smoking youth were susceptible to smoking worldwide. Around 40% and 50% youth were exposed to ETS inside and outside the home respectively, and 78% supported SPFs globally. Parental and peer smoking was strongly associated with smoking susceptibility in never-smoking youth [AOR 2.63, 95% CI 2.43 to 2.84], and youth exposure to ETS inside [AOR 5.09, 95% CI 4.84 to 5.35] and outside [AOR 2.51, 95% CI 2.39 to 2.63] the home, while anti-smoking school education was negatively associated. Youth having knowledge about smoking harm [AOR 2.37, 95% CI 2.22 to 2.54] supported SFPs, while youth exposed to tobacco industry promotion [AOR 0.83, 95% CI 0.78 to 0.89] did not support. The study highlighted a number of modifiable factors that can be used for augmenting global tobacco control in youth. Well-executed anti-smoking campaigns, parental and peer education, inclusion of anti-tobacco education in school curricula, comprehensive SFPs, and comprehensive ban on tobacco industry strategies are important approaches to prevent tobacco use and advance global tobacco control in youth.

Exposición al humo ambiental del tabaco y medidas de control del tabaquismo

Antecedentes: En España, hasta 3.200 defunciones anuales son atribuidas al tabaquismo pasivo. La exposición al humo ambiental del tabaco (HAT) aumenta el riesgo de padecer enfermedades crónicas como cáncer de pulmón, enfermedades cardiovasculares y enfermedades respiratorias. Hipótesis: 1) Existe una relación entre la exposición al HAT medida mediante cuestionario y la concentración de cotinina en saliva. 2) La exposición al HAT y la concentración de cotinina en saliva es mayor en los trabajadores del sector de la hostelería que en la población general. 3) El aumento de medidas de control del tabaquismo reduce la prevalencia de exposición al HAT. Objetivos: 1) Caracterizar la exposición al HAT en la población general no fumadora mediante un cuestionario y biomarcador antes de la entrada en vigor de la ley 28/2005 de medidas frente al tabaquismo. 2) Comparar la concentración de cotinina en población general y en trabajadores del sector de la hostelería antes de la entrada en vigor de la ley 28/2005 de medidas frente al tabaquismo. 3) Evaluar la correlación entre las medidas de control del tabaquismo y la prevalencia de exposición al HAT en 27 países de la Unión Europea. Metodología: Para conseguir los objetivos marcados se ha realizado un estudio transversal de la población general de Barcelona, un estudio de una cohorte de trabajadores del sector de la hostelería en 5 Comunidades Autónomas antes de la entrada en vigor de la ley (octubre-diciembre 2005) y seguidos durantes 2 años después de la ley (octubre-diciembre 2007) y un estudio ecológico a partir de los datos del Eurobarometro sobre tabaco. Conclusiones: 1) La prevalencia de exposición al HAT entre los no fumadores en España antes de la entrada en vigor de la ley 28/2005 es elevada y tiene una asociación inversamente con la edad. La concentración de cotinina en saliva se asocia con el número de fumadores en casa y la cantidad de cigarrillos fumados en presencia del no fumador. 2) La prevalencia de exposición al HAT y la concentración de cotinina en saliva en España es mayor entre los trabajadores de la hostelería en comparación con la población general antes de la entrada en vigor de la ley 28/2005. 3) El apoyo a la ley 28/2005, la percepción de su cumplimiento y el acuerdo en prohibir definitivamente el consumo de tabaco en todos los lugares públicos, incluidos bares y restaurantes, ha aumentado entre los trabajadores del sector de la hostelería 2 años después de la entrada en vigor de la ley. 4) A nivel ecológico, los países de la Unión Europea con mayor implementación de políticas sanitarias frente al tabaquismo (medidas mediante la escala propuesta por Joossens y Raw) presentan menor prevalencia de consumo de tabaco y menor prevalencia de exposición al HAT en casa y en el trabajo. Además estos países con mayores políticas frente al tabaquismo tienen un mayor apoyo a las medidas de regulación del consumo de tabaco en todos los lugares públicos. / Background: In Spain, up to 3,200 deaths per year were attributable to secondhand smoke (SHS) exposure. SHS has been associated with a variety of health effects among non-smokers, especially lung cancer and ischemic heart disease, and respiratory diseases. Hypotheses: 1) There is a relationship between exposure to SHS measured by questionnaire and cotinine concentration in saliva. 2) Exposure to SHS and salivary cotinine concentration are higher in hospitality workers than in the general population. 3) The increasing of the tobacco control measures reduces the prevalence of exposure to SHS. Objectives: 1) To describe the exposure to SHS in non-smokers from the general population using a questionnaire and salivary cotinine concentration before the Spanish smoking ban. 2) To compare the salivary cotinine concentration between non-smokers from the general population and non-smokers hospitality workers before the Spanish smoking ban. 3) To assess the correlation between tobacco control policies and the prevalence of exposure to SHS in the 27 countries of the European Union. Conclusions: 1) The prevalence of exposure to SHS in general population was high before the Spanish smoking ban and it decreases with the age. Salivary cotinine concentration in the general population depends of the number of smokers and the number of cigarettes smoked in presence of non-smokers at home. 2) The prevalence of exposure and salivary cotinine concentration were higher among non-smoker hospitality workers than in the general population before the Spanish smoking ban. 3) Positive attitudes (in favour) towards smoking ban increased after the ban among hospitality workers. 4) At ecological level, the Tobacco Control Scale scores were strongly associated with a lower prevalence of smokers and a lower self-reported exposure to SHS. Moreover, countries with more tobacco control policies have higher support towards smoking bans in all workplaces (including restaurants, bars, pubs and clubs, and other indoor workplaces).

Cabin Air Quality in Commercial Aircraft : Exposure, Symptoms and Signs

<p>The objective of the dissertation was to study the cabin environment, and identify personal and environmental risk factors, associated with symptoms, and perception of cabin air quality. Another objective was to study if ban of smoking, and increased relative air humidity on intercontinental flights, could have a beneficial health effect. The studies were performed among Scandinavian cabin crew in one Airline Company. Office workers from the same company served as controls. Exposure differed between cruise and non-cruise conditions. Air humidity was very low during intercontinental flights (3-8%). Concentration of moulds, bacteria, formaldehyde, and ozone was low. Tobacco smoking increased respirable particles in the cabin air, from 3 to 49 mg/m3, and increased cotinine in urine. The ETS-exposure was highest in the aft part of the cabin. Symptoms and environmental complaints were more common among flight crew than office workers. We could identify personal factors of importance, and certain conditions that could be improved, to achieve a better cabin environment. There was an association between symptoms and environmental perceptions and work stress, lack of influence on working condition, and a history of atopy. After ban on smoking in aircraft, there was a decrease of ocular and general symptoms, and increased tear-film stability in aircrew. Air humidification reduced headache and ocular, nasal, and dermal dryness symptoms, increased tear-film stability, and increased nasal patency. Our result indicates that ETS and low air humidity are important environmental factors in aircraft, and that atopy, and work stress could be significant risk factors for symptoms and environmental perceptions.</p>
