Cuban intervention in Angola and Ethiopia, 1975-1980 : the question of Soviet influence on Cuba

Cuba's role in Africa in the 1970s has been the subject of numerous and diverse interpretations. Students of Cuban politics sometimes tend to take a general view of Cuban policy toward Angola, and so important differences between each issue or situation are overlooked. This study offers separate accounts of the role of Cuba in Africa and in Ethiopia since 1975, with the purpose of exploring the possibility of an influence relationship between Moscow and Havana. Influence analysis is an extremely subjective task. That is, it appears to be virtually impossible to document Soviet influence on Cuba. Nevertheless, it remains possible to examine available evidence with respect to each situation, and then to construct what appears to be the most coherent argument regarding the possibility of Soviet influence on Cuba. I shall conclude that the body of evidence suggests that Cuba did not intervene in Angola chiefly to conform to Soviet preferences or interests. In this sense then, Cuba probably was not influenced by the Soviets to any great degree with respect to the Angolan episode. In contrast, Cuba's role in each of the two Ethiopian incidents seems to exemplify the rather strong possibility of Soviet influence on Cuba. Improving tuberculosis case finding among household contacts of tuberculosis patients by using community based model in Addis Ababa, Ethiopia

Introduction: World Health Organization recommends screening of household contact as a key to improve detection of tuberculosis cases. Ethiopia’s current tuberculosis household contact investigation strategies rely on symptomatic contacts attending health facilities for investigation. This approach has not led to the detection of additional tuberculosis (TB) cases; alternative approaches have to be considered. The purpose of the research was to develop guidelines in endorsing the implementation of a community based household contact investigation program in Addis Ababa. Methods: A mixed method research using sequential exploratory design was conducted in Addis Ababa. In the first phase, qualitative data collection and analysis methods were used to formulate intervention approach and in the second phase, a quantitative random controlled trial was conducted, with the purpose of comparing the proposed intervention measures with routine household contact tuberculosis investigation. Frequencies and logistic regression analyses were used to determine the relative risk and associated factors. Thematic analysis was used for qualitative data analysis. Results: The in-depth interview and focus group discussion findings identified themes namely household contact investigation (HHCI) implementation, misconceptions on HHCI, challenges with HHCI Approaches, opportunities for HHCI provision, contributing factors associated with household involvement, strategies for effective HHCI service and partnerships with health bureau. In phase two, the study reported that the prevalence of TB was 7.1% among the intervention group compared to 1.9% in the control groups at the end of first year follow-up. Nine guidelines were developed to support the household contact investigation system. Conclusion: The passive case detection strategy of contact investigation did not find more cases, and tuberculosis patients and their family contacts were not satisfied with this method. However, the proposed community-based strategy shows that more TB cases can be detected by using existing medical staff. Therefore, an approach that makes the service more accessible is significant and the recommended community based TB household contact tracing approaches needs to be scaled up for its performance towards identified missed cases and enhance patient and their household contacts involvement. / Health Studies / D. Perceptions of nurses on disclosure of children's HIV positive status in Addis Ababa, Ethiopia

A quantitative, descriptive, explorative survey was conducted to explore and describe nurses’ perceptions of disclosure to children of their HIV positive status in Addis Ababa. 100 nurses working in six conveniently sampled health centres participated by completing a self-administered questionnaire. The findings revealed that the majority of participants were of the opinion that children have the right to know their HIV status, participate in their own treatment, and that disclosure contributes towards improved adherence. Forty-one of the participants said that it is nurses’ role to support caregivers in the disclosure process. But 56.3% felt they lacked the training to disclose to children that they are infected with HIV. Accordingly, it is recommended that relevant and applied training is required to equip nurses with the knowledge and skills to disclose to children their status. The importance of nurses’ proactive role in disclosure to children of their HIV status needs to be emphasised. / Health Studies / M.A. (Public Health)

Indigenous play as a psychotherapeutic technique with young adolescents experiencing socio-emotional and behavioural difficulties in Hawassa City, Ethiopia

The study aimed to examine the extent of indigenous play or teret-teret, as a psychotherapeutic technique to assist young adolescents in their adjustment from socio-emotional and behavioural difficulties. Following a mixed-methods, the study was conducted in three phases. The qual then QUANT in the main study followed by QUANT - QUANT and finally qual was employed. Two hundred ninety-nine participants, with 13 in Phase 1; 221 in Phase 2, and 65 in Phase 3, were involved in the study. Thirteen participants were purposively selected based on their experiences and merits for Phase 1, while 221 were selected through multilevel probability sampling techniques for Phase 2. Among the 65 participants for Phase 3, five were selected randomly whereas 60 were selected purposely. Three data-gathering instruments, comprising in-depth interviews, archive analysis, and questionnaires were employed. This design explored the quality of 62 manifestations of indigenous teret-teret for use in psychotherapy. The qualitative phenomenological study confirmed teret-teret as a relevant psychotherapeutic technique, practised in Ethiopia for the adjustment of young adolescents who experience socio-emotional and behavioural difficulties. The intervention study revealed statistically significant differences between the participants who received teret-teret psychotherapy and those who did not receive teret-teret psychotherapy. It was found that teret-teret psychotherapy advanced the socio-emotional and behavioural competencies of the participants. The cross-sectional survey study verified a 43% prevalence rate of composite SEBD, with 50% being anxiety/depression, 45% somatic complaints, 44% attention problems, 43% delinquency, and 42% aggression, respectively. Alcohol use amongst psychiatric in-patients in a mental hospital in Ethiopia

The purpose of this study was to explore alcohol use among psychiatric in-patients in a mental hospital in Ethiopia. A quantitative, descriptive, cross-sectional study design was used. Data were collected through face to face structured interviews and a document analysis checklist. A researcher-modified interview-version of the Alcohol Use Disorder Identification Test (AUDIT) questionnaire was used to measure alcohol use. The study population comprised of 70 psychiatric in-patients. Data analysis showed that being male, living in an urban area, being diagnosed with schizophrenia, and having parents who drink alcohol had a statistical significant association with the alcohol use of the respondents. Respondents reported that it was difficult to abstain or stop using alcohol. They also felt discriminated against when forbidden by relatives to drink. It was alarming to find that thioridazine, which has been discontinued in most countries for the treatment of psychosis, was still being prescribed in Ethiopia. The study highlighted the need for health education to strengthen patients’ perceptions about the negative consequences of alcohol use. Care should be taken when prescribing psychotropic drugs such as thioridazine to psychiatric in-patients because of possible cardiotoxic effects. / Health Studies / M.A. (Public Health)

Perceptions and experiences of people in Gambella region of Ethiopia on Guinea worm disease eradication interventions

Introduction: Guinea worm disease (GWD) is a painful, disabling disease caused by the parasite dracunculus medinensis. The implementation of the global GWD eradication campaign resulted in a decrease of global burden of disease and several countries have been proclaimed free of the disease. Though, Ethiopia has implemented the GWD eradication programme since 1992 the disease remains endemic in the Gambella region. Purpose of the study: The purpose of this study was to explore the perceptions and experiences of the community of Gambella region on Guinea worm disease (GWD) eradication interventions. Methodology: A qualitative study paradigm with a phenomenological research design was employed to collect data using focus group discussions, in-depth individual interviews, document reviews, and observations. The study participants were determined by purposive sampling. Qualitative data analysis involved transcription, data entry, and management of verbal information followed by coding, categorizing into themes. Interpretation of the data was done using triangulation methodology with appropriate mechanisms for validity and trustworthiness. Result: The study found that there are gaps in the perceptions and understanding of the community and programme implementers about the achievement and application of the various interventions on the eradication of Guinea worm disease. Health education programme was not fine-tuned to the eradication of GWD by community participation. Community perception was not at the required level for the eradication of GWD due to the low success of the educational intervention. Conclusion: continuous health education targeting the community with clear objectives of helping to eradicate GWD was found to be limited in its coverage. In addition, provision of water filters necessary for GWD eradication was inadequate. Moreover, the community did not have clear understanding of the causes of GWD and its eradication strategy. The aim of this research is to determine what physical evidence can be found on the body of the perpetrator to link the suspect with the crime of rape. Regarding this, the researcher was trying to introduce a number of important concepts such as forensic investigation, chain of custody, evidence, information, identification, individualization, crime scene and the Locard Principle. This research will explain the meaning of forensic investigation, the objective of criminal investigation, the difference between information and evidence and individualization and identification. An important principle which all investigators and prosecutors should know is the Locard Principle. It says: "Every contact leaves a trace". The principle further explains that, whenever two objects come into contact with one another, material from the first would be transferred to the second and material from the second would be transferred to the first. Although this principle is important for investigators as well as prosecutors, most of them do not know the principle. The other important thing is the consideration of the body of the perpetrator as a crime scene to prove a crime. A crime scene is defined as an area where a criminal act has taken place. If we get evidence from the body of the perpetrator, we can then consider it as a crime scene. Lastly, regarding the legal rights to collect evidence on the body of the perpetrator, there should be specific laws in Ethiopia. However, there is no specific law. In addition to this, there is no DNA investigation in Ethiopia. Hence, this research will enhance the skills of investigators and prosecutors will benefit from what I have discovered through the research and I am very pleased to be the first Ethiopian in the introduction of the field of forensic investigation to the readers. / Forensic Investigations / M. Barriers to compliance to exclusive breastfeeding and timely introduction of complementary feeding practices in Ethiopia

Text in English / This study aimed at exploring Barriers to compliance with exclusive breastfeeding and timely introduction of complementary feeding practices in Ofla District, Tigray Region in Ethiopia. A quantitative exploratory descriptive study was conducted to explore and describe the barriers that restrict mothers /caregivers to comply with exclusive breast feeding practice until six months and with introduction of solid, semi-solid and soft foods at six months in Ofla District. Data were collected using structured questionnaire, from a total of 112 samples of which 75 mothers and care givers with children aged 0-5 months and 38 children aged 6-8 months participated. The data were entered into a computer and analysed though the Statistical Package for Social Sciences (SPSS) software. The findings revealed that there are barriers related Doer mothers and Non-Doer mothers perceived susceptibility, perceived severity, perceived benefits, perceived barriers, perceived self-efficacy, cues for actions, perceived social acceptability and positive and negative attitudes towards exclusive breastfeeding and initiation of complementary feeding. On the other hand, the participant’s perception of both groups with regard to perception of Divine (God’s) Will on two child feeding practices was insignificantly the same. Urban and peri-urban EcoHealth markers and health promotion intervention in Addis Ababa Ethiopia

The purpose of this study was to explore and describe the EcoHealth markers and health promotion interventions in the behaviour and practices of vegetables growers in Addis Ababa. The objectives were to assess the urban and peri-urban community members’ knowledge and practices on the existing environmental health policies/regulations/guidelines, assess the perspectives of women and youth on urban and peri-urban EcoHealth promotions and protections and evaluate the development and implementation of the health promotion activities on waste water use by applying an ecological model aimed at changing behaviour and by providing Bio-sand filter to promote hand wash practice which helps to reduce potential health risks among urban vegetable growers. Both quantitative and qualitative descriptive researches were conducted to identify the urban and peri urban community members’ knowledge and practices of the existing environmental health regulators and the perspectives of women and the youth on EcoHealth. Additional quasi experimental method; multiple baseline survey along with the EcoHealth Stress Process promotion method was employed to identify stressors, measure changes in environmental health promotion intervention and use of hand wash (biosand filter for hand wash). Six woredas from two subcities of the Addis Ababa City Administration were purposely selected, and a total of 845 (98.9% response rate) households participated in the quantitative study, while 142 community members (public sector offices, factories and establishments’ management and employee), participated in the qualitative survey. For quasi experimental method, six settings/blocks (of which 3 sites were provided with a bio-sand filter for hand washing) cultivated by 6 to 8 vegetable growers and their family members, were included in the intervention (multiple baseline survey). The findings revealed that about 77 percent of participants reported, knowledge of one or more of the selected environmental health and EcoHealth regulations. Ownership of assets and education tend to influence the public awareness of selected EcoHealth and environmental health regulations. Women and youth found to be significant contributors to better urban and peri-urban EcoHealth conditions and were the ones primarily affected by environmental hazards. The intervention study results also indicated evidence-based IEC interventions and ecological health promotion methods proved useful in promoting EcoHealth in an urban environment. Moreover, the biosand filter used to provide evidence of health promotion was found to be valuable for wastewater treatment and significantly reduced chemical, physical and biological contaminates from the wastewater. / Health Studies / D.
