Developing a paper decision aid for contraceptive counseling that reflects patients’ values, aligns with decision aid standards, and meets literacy and accessibility guidelines: an exploratory sequential mixed-methods study

BACKGROUND: Contraception is an effective, preference-sensitive intervention that supports quality of life, management of health, and self-determination. Contraception is used by 99% of people assigned female at birth with an average of 3.4 methods used across their lifespan. Providers counsel patients on contraception and patients want to be counselled. Shared decision-making frameworks promote using decision aids (DA) during counseling as best practice. Existing DAs lack transparency in their development methodologies and evaluation results and may not facilitate patient-centered care. AIM: Create a contraception DA and accompanying contraception method information sheets (MIS) that are informed by patients’ values, align with international standards, meet health literacy and accessibility guidelines, and are evaluated by patients and providers to be acceptable, quality, and feasible to use during contraceptive counseling. METHODOLOGY: To create the DA/MIS, (1) literature was reviewed on contraceptive counseling frameworks, DAs, patients’ contraceptive preferences, health literacy, accessibility, user-centered design, and validated patient education material quality measures, (2) results were reviewed from a patient focus group and provider meeting where they defined user requirements, and (3) evidence-based contraceptive information was synthesized. Once created, an exploratory sequential mixed-methods study iteratively refined the DA/MIS after each data collection phase: (1) provider focus groups and survey, (2) patient focus groups and survey, (3) observed patient testing during counseling followed by an interview and survey, (4) expert patient and provider review, and (5) provider field testing in clinic followed by an interview and survey. DA/MIS readability levels were assessed. RESULTS: Quantitatively, the DA/MIS were acceptable, quality, and feasible to use during counseling. Qualitatively, the DA/MIS were preferred to verbal-only counseling and other DA/MIS, centered patients’ preferences, increased knowledge, focused on patient autonomy, challenged bias/coercion, improved counseling satisfaction and quality, offered a novel design, and were appealing, inclusive, and accessible. CONCLUSION: This DA/MIS had positive quantitative and qualitative results, offered a novel design aligned with international standards, and had a transparent, rigorous development process aligned with frameworks and validated tools. Third Child (Un)lucky : A WPR Analysis of The United Kingdom’s Two-Child Limit

This thesis analyses the two-child limit policy in the United Kingdom which is codified in section 10 of the Welfare Reform Act 2012, amended in section 14 of the Welfare Reform and Work Act 2016, by utilising Carol Bacchi’s "What’s the Problem Represented to be?" (WPR) approach. The WPR approach draws on Foucauldian discourse theory and investigates the policy's implications by studying the problem presentations within it. The analysis identifies three core problems in the Welfare Reform and Work Bill’s impact assessment: the necessity to address the deficit by restructuring the welfare state, fairness towards working taxpayers versus benefit recipients, and the escalation of tax credit expenditure. It further uncovers five assumptions embedded within the policy, suggesting that UK citizens incorporate the policy into family planning, anticipate future social security needs, and make fertility decisions based on financial considerations, with the policy aimed at improving children's future prospects.Additionally, the study traces the ideological roots shaping welfare reform, highlighting the influence of Conservative Party ideologies from the Thatcher era and subsequent policies that reduced welfare support, contrasting with New Labour's interventionist approach and the media’s role in reinforcing negative welfare perceptions. The analysis uncovers several problem presentations in the policy and concludes that the government and media can craft narratives that shift the responsibility for child poverty from the state to parents, with enduring impacts on social discourse and policy direction, challenging future policy reversals.

Men's knowledge and attitude towards vasectomy in east Wollega zone of Oromia region, Ethiopia

The purpose of this study was to assess men’s knowledge and attitude towards vasectomy as a family planning method options available to men in East Wollega zone of Oromia Region. Male sterilisation in sub-Saharan countries including Ethiopia is very much limited due to lots of reasons despite its many advantages than other family planning methods. Quantitative, descriptive cross-sectional research was used to describe level of knowledge and attitude towards vasectomy. Data were collected using structured questionnaire in which a total of 150 respondents, who were selected using non-random purposive sampling technique participated in the study. The data were analysed using SPSS version 20. Hence the findings revealed the lack of knowledge and low interest on vasectomy among respondents. The concerted effort from all stakeholders and use of multiple strategies to educate the community will raise awareness which in turn improves vasectomy service uptake / Health Studies / M.A. (Public Health)

Factors affecting contraceptive use among women of reproductive age in northern Jordan : a framework for health policy action

Jordan has a higher fertility rate (3.8) than the averages of countries similar in income to Jordan (2.2) and compared to the Middle East and North Africa region as a whole (2.8) (WHO, WB, UNICEF, & DHS, 2011). The findings of the 2009 Jordanian Population and Family Health Survey demonstrated that the total fertility rate (TFR) has stopped declining in the country since 2002 (DOS, 2010b; USAID, 2010). The prevalence of contraceptive use has also shown little change in Jordan over the last decade (DOS, 2010b; USAID, 2010). Given that contraception is one of the proximate determinants of fertility (Rahayu et al., 2009), the main purpose of this study was to investigate which factors are contributing to women's current contraceptive behavior and intention for future contraceptive use. Research questions were developed in a comprehensive framework that considers women's intention and actual behavior as outcomes of various interactive factors within a socio-cultural context. In particular, the study's framework was directed by a theoretical basis adapted from Ajzen and Fishbein's Theory of Reasoned Action (TRA) and an extensive review of the available literature in the research area. Obviously, the social set-up and cultural norms in the study setting, together with attitudes toward children and family, represent a traditional scenario that could help explain the consistency of fertility and contraceptive use in the country. Further, the influences of background characteristics on women's contraceptive behaviors and intentions provide another scenario that could help assess the current situation of family planning (FP) in Jordan. In this study, demographic factors, spousal communication variables and healthcare system-related factors are all defined as background characteristics. Attitudes and social norms reflect the women's behavioral determinants and represent the main constructs of the TRA. In fact, involving a set of factors related to women's beliefs and social norms in the study's framework provided an opportunity to explore how these factors might promote or inhibit a woman's intentions and behaviors in respect to contraceptive use. In a three-manuscript format, this research was designed to achieve a number of objectives. The first manuscript aimed at identifying the major factors associated with the current use of contraception among women of childbearing age in northern Jordan. The second manuscript focused on investigating the main factors that are associated with women's contraceptive method preference (e.g. the choice of modern contraceptives as effective methods in preventing pregnancy versus the choice of traditional contraceptives as methods with high failure rates). The third manuscript attempted to explore the key factors associated with women's intention for future contraceptive use since the existence of such an intention would consequently translate into an actual behavior later. In 2010, original cross-section data were collected by means of a face-to-face interview using a structured pre-tested survey. The study sample included women who were currently married and were between 18 and 49 years old. Applying a systematic random sampling procedure, all respondents were recruited from the waiting rooms of five randomly selected Maternal and Child Health (MCH) centers in the Governorate of Irbid, northern Jordan. Using a list provided by the Ministry of Health, all centers in the Governorate were stratified according to the region (urban vs. rural) and randomly selected in proportion to their number in each region. The final sample size for this research consisted of 536 women surveyed, giving a response rate of 92.4 percent. Utilizing logistic regression analyses, the results of the dissertation manuscripts indicate that women's behaviors and intentions toward the use of contraception are affected by a number of factors at the individual, familial and institutional levels. The findings that emerged from the three manuscripts provide health professionals and policy makers with important information to assist in the design of FP programs and campaigns aimed at increasing current contraceptive use, enhancing the adoption of modern contraception and motivating the intention for future contraceptive use. This research strongly suggests that health professionals develop health policies that both expand the availability of MCH centers and strengthen the role of healthcare providers to dispel the numerous rumors and misconceptions surrounding the use of contraceptives, particularly modern ones. Health workers at the MCH centers need to ensure that women have sufficient information about the benefits and side effects of different types of contraception by offering proper FP counseling. The messages that religious leaders can use in advocating for FP would also help make contraceptive use socially acceptable since their opinions are often followed by the majority. This would be a key step toward removing the barriers to contraceptive use. Moreover, to design effective FP interventions, planners should take into account women's attitudes toward the use of contraceptive methods and the components of those attitudes (e.g. women's approval of contraceptive use for birth spacing and perceptions regarding the value of large family sizes and the importance of having male children in Jordanian families). / Graduation date: 2012 / Access restricted to the OSU Community at author's request from May 9, 2012 - May 9, 2013

Text in Afrikaans / Summaries in English and Afrikaans / In an exploratory study variables influencing the intention to use contraceptives and contraceptive use of teenagers and the association between these variables were determined.A random quota sample consisting of 231 and 224 school boys and girls (14 - 18 years) were selected and the final investigating group were 315 sexually active teenagers. According to loglinear analyses the following variables showed a significant relationship with the intention to use contraceptives: attitudes towards the use of contraceptives, perceptions of accessibility and locus of control. Variables that showed a significant relationship with actual contraceptive use were gender, knowledge about reproduction and contraceptives, beliefs and attitudes towards the use of contraceptives, traditional values such as proving fertility before marriage and family size, perceptions of social approval of contraceptive use as well as perceptions of accessibility to contraceptives. Reproductive Decision Making Among Zambian Couples: Agreement and Conflict

Fertility studies have often focused on the behavioral and attitudinal attributes of women with regard to fertility. Until recently, the role of men in fertility studies have often been ignored within much of the literature concerning fertility decisions. The focus of this study will examine if differences exist between husbands and wives with regard to the following four aspects of fertility decisions: spacing of children, methods of family planning, sex preference, and desired family size. The data were collected from 125 households in Kitwe, Zambia. Identical questionnaires were submitted to the husbands and wives during separate interviews. Content analysis was used to analyze the data. Overall, no significant differences exist among husbands and wives with regard to the four aspects of fertility decisions being researched.

Relations entre le niveau de vie, la fécondité et les besoins non satisfaits en matière de planification familiale en Guinée

Tous mes travaux ont été réalisés à l'aide du logiciel stata 11. / La présente thèse étudie la relation entre le niveau de vie, la fécondité des femmes en union et leurs besoins non satisfaits en matière de planification familiale. Les données qui ont servi aux analyses proviennent des fichiers individuels de deux enquêtes démographiques et de santé (DHS) réalisées en Guinée en 1999 et 2005. Le niveau de vie est mesuré par un indicateur composite créé à partir des caractéristiques du logement et certains biens possédés par le ménage. Nous avons adopté deux stratégies d’analyse : analyses descriptives et multi-variées. Nous avons examiné à cet effet les niveaux et les tendances de ces deux comportements de fécondité dans les grandes villes, petites villes et villes secondaires, en milieu rural et à l’échelle nationale. Des méthodes statistiques appropriées ont été utilisées : (modèle de régression de Poisson et des modèles de régression logistique binaire et multinomiale). Les résultats des analyses descriptives montrent une faible fécondité chez les femmes issues des classes riches par rapport aux femmes pauvres et une diminution de leurs besoins non satisfaits en matière de planification familiale. Ces relations qui se sont révélées très statistiquement significatives en 1995-1999 quelque soient les milieux de résidence, ont disparu presque dans les analyses multi-variées, après avoir contrôlé certaines caractéristiques socio-économiques, démographiques et culturelles. Dans ces analyses multi-variées, l’éducation de la femme et la pratique contraceptive se sont révélées particulièrement déterminantes pour la fécondité. Par ailleurs, l’éducation de la femme, a été également un facteur important pour les besoins non satisfaits en matière de planification familiale. Il en est de même pour son âge, sa parité, sa religion, la discussion du couple sur la planification familiale, l’attitude du conjoint vis-à-vis de la planification familiale et de l’exposition aux messages sur la planification familiale qui restent aussi d’importants facteurs des besoins non satisfaits en matière de planification familiale. Le terme d’interaction groupe d’âge et le nombre d’enfants en vie, s’est avéré, lui aussi fortement significatif indiquant que l’effet de la parité sur les besoins non satisfaits en matière de planification familiale ne prend pas le même sens selon le groupe d’âge auquel appartient la femme. En définitive, nos analyses, nos analyses ont mis en évidence que la relation négative entre le niveau de vie, la fécondité et les besoins non satisfaits se vérifie mais avec des degrés variés selon les milieux de résidence et les périodes d’enquête. Les écarts entre les groupes contigus sont également très faibles. / The goal of this thesis is to study the relationship between the married women standard of living, their fertility and their unmet need for family planning. The data used for the analysis have been taken from two individual demographic survey files (DHS) carried out in Guinea in 1999 and 2005. The standard of living is measured using a composite indicator established from the housing characteristics and some capital goods of the family. We have adopted two strategies of analysis: Descriptive and Multivariate Analysis. Thus, we have studied the levels and tendencies of these two behaviors of fertility in the large cities, small or secondary cities, rural places, and at the national scale. Statistics appropriate methods of regression (Poisson regression method, logistic binary method and multinomial method). The results of descriptive analysis have confirmed a weak fertility in women from rich classes in comparison with poor women and a reduction of their unmet need for family planning. The relationship which proved very statistically significant in 1995-1999 whichever the place of residence was not observed with the multivariate analysis method, after controlling some socioeconomic, demographic and cultural characteristics. In the multivariate analysis, the woman education and her behavior in terms of contraception were particularly important in her fecundity. Moreover, the woman’s education, her age, parity (married status), her religion, the discussion of the couple about the family planning, the husband attitude concerning the family planning constitute some important factors of unmet need for family planning. The term of Interaction age group and number of living children turns out strongly significant, indicating that the parity (man-woman) effect can change according to the age group of the woman.   In fact, our analysis have shown with clearly that the negative relationship between the standard of living, the fertility and the unmet need is confirmed with various degrees according to the place of residence and the survey period. The differences between the closely related groups are too weak.

O presente estudo teve por objetivo investigar as práticas e as representações ligadas à vida reprodutiva entre jovens universitários da Universidade de São Paulo (USP), especialmente aquelas dirigidas à contracepção e à prevenção de doenças sexualmente transmissíveis, no âmbito das relações heterossexuais.A pesquisa foi dividida em duas fases. Na primeira, foi elaborada uma amostra representativa dos estudantes matriculados em cursos de graduação da USP, na Cidade de São Paulo, no ano de 2000, e foram entrevistados 952 alunos e alunas com idade entre 17 e 24 anos. Na segunda, foram gravadas entrevistas em profundidade com 33 estudantes que se ofereceram voluntariamente para continuar participando da pesquisa. O uso do condom é freqüente entre os estudantes universitários, principalmente na primeira relação sexual. No entanto, o uso desse método apresenta descontinuidades. Além da primeira relação sexual, o condom é usado especialmente nas relações casuais e no início dos relacionamentos sexuais com um parceiro ou uma parceira novos. Os jovens negligenciam o condom no contexto do namoro, tendendo a substituí-lo pela pílula. A contracepção é cercada por descuidos, erros e esquecimentos e os estudantes mencionam que utilizam métodos de baixa eficácia, como o coito interrompido e a abstinência periódica para regular a fecundidade. Os estudantes manifestam um forte desejo de adiar a fecundidade, mas o uso inadequado de métodos contraceptivos e o recurso aos métodos de baixa eficácia podem levar a uma gestação não planejada. 77% do total de entrevistados afirmou que gostaria de ter até dois filhos e a idade ideal para ter o primeiro filho seria próxima aos 30 anos. Somente 4% dos entrevistados e entrevistadas referiram ter passado por uma gestação. Apesar do pequeno número de gestações referidas, o aborto provocado foi a forma de finalização de uma alta proporção dessas gestações. Os resultados da pesquisa indicam que uma complexa rede de representações simbólicas subsidia as condutas diante da contracepção e da saúde reprodutiva. Essas representações constróem o sentido da sexualidade, classificando-a, definindo regras e obrigações segundo cada situação e orientando práticas. Questões de gênero pontuam os discursos sobre os temas tratados. As diferentes concepções sobre o "ficar" e o namorar, a opção pelo método contraceptivo, as representações sobre as responsabilidades do homem e da mulher frente à contracepção e, especialmente, os discursos sobre o aborto revelam a importância que os diferenciais de gênero assumem perante a construção do sentido da sexualidade e da vida reprodutiva. Questões de gênero também marcam a busca de orientação médica, no âmbito da saúde reprodutiva. A consulta médica é restrita ao grupo das mulheres e o método mais indicado é a pílula, embora trate-se de jovens mulheres solteiras que estão iniciando a sua vida sexual. Observa-se, por sua vez, que os homens jovens estão totalmente alijados do sistema de saúde, ainda que se trate de um grupo com alta escolaridade que recorre à medicina privada. O estudo indica que os diferenciais de gênero estão presentes desde o processo de rotulações e significações que dão sentido a vivência da sexualidade e da regulação da fecundidade até o âmbito das políticas públicas voltadas para a saúde reprodutiva dos jovens, revelando uma importante lacuna nos serviços de saúde que necessita ser contemplada por políticas públicas capazes de promover a eqüidade de gênero na atenção à saúde reprodutiva e de incluir os jovens do sexo masculino nos serviços de saúde. / The present study aimed at investigating the practices and representations linked to the reproductive life of university youths from the Universidade São Paulo (USP) especially those geared at contraception and prevention of STD’s in the scope of heterosexual relations. The project was divided into two phases. In the first a representative sample of 952 male and female students between 17 and 24 years old enrolled in under-graduate courses at USP in the city of São Paulo in 2000 was elaborated. In the second phase in-depth-interviews with 33 students that volunteered to continue take part of the research were recorded. The use of condom is frequent among university students mainly at the first sexual intercourse. However, this method presents discontinuity. Beyond the first sexual intercourse it is used especially in the casual relationships and in the beginning of new sexual relationships with a new partner. Youth neglect the condom in the dating context tending to replace it for the pill. Contraception is surrounded by carelessness, mistakes and neglect and the students mention the use of low efficacy methods such as interrupted copulation and periodic abstinence for fertility control. The students revealed strong will to delay fertility but the inadequate use of contraceptive methods and use of low efficacy methods may lead to an unplanned pregnancy. 77% of the interviewees state that they would like to have up to two children and that the ideal age for the first child would around 30. Only 4% of the interviewees referred having had a gestation. Despite the small number of pregnancies referred the provoked abortion represented a high proportion of pregnancy termination. The results of the survey indicate that a complex network of symbolic representations accounts for the conducts in terms of contraception and reproductive health. These representations construct the sense of sexuality, classifying it, defining rules and obligations according to each situation and guiding practices. Gender issues mark the discourses on the themes discussed. The different conceptions about noncommitted dating and dating proper, the option for the contraceptive method, representations on the male and female responsibilities with reference to contraception and especially the discourses on abortion reveal the importance the gender differentials possess in face of the construction of meaning of sexuality and reproductive life. Gender issues influence the search for medical guidance in the context of reproductive health. Medical consultation is restricted to the female group and the most prescribed contraceptive method is the pill in spite of the fact that they are young single women in the beginning of their sexual life. On the other hand it was observed that young men are totally cast out of the health system notwithstanding the fact that this is a group with high level education with access to private medicine. The study indicates that the gender differentials are present from the process of labeling and signification that attribute meaning to the experiencing of sexuality and the regulation of fertility to the scope of public policies capable of promoting gender equity in the attention to reproductive health of youth. That reveals an important gap in the health services that must be addressed by gender equity promoting public policies that also inscribe male youth in the health services.
