Child Abuse Prevention By Home Visitors: A Study of Outstanding Home Visitors Using Mixed Methods

Indiana University-Purdue University Indianapolis (IUPUI) / Child abuse remains a serious health problem in the U.S. Yet, there are preventative programs that can significantly improve the parenting experienced by very young children and result in lower incidence of maltreatment. Home visitation is the most prevalent and successful form of primary prevention. These programs are staffed by home visitors who empower parents and are the lynchpin of home visitation programs. What makes some home visitors excel at this work is the focus of this research. A small, non-random, purposive sample of excellent home visitors and their administrators was used to learn about the personal characteristics of outstanding home visitors. The mixed methods design of this research included qualitative interviews, home visiting situational vignettes, and quantitative tests of personality attributes. The results indicated that this group of outstanding home visitors possessed important similarities. First, the home visitors were effective at forming and maintaining empathic relationships. The variables that facilitate the formation of the therapeutic relationships between home visitor and client include: (a) “good enough empathy” (need not be extraordinarily empathic but at least averagely so), (b) positive regard (showing respect to the families and recognizing that the parent is the expert on their child), and (c) congruence. Secondly, the home visitors possessed self-awareness allowing for reflective practice and forming and maintaining better client relationships. Thirdly, the excellent home visitors possessed an attitude of lifelong learning. Fourthly, the home visitors strongly believed in the ability of their clients to change. This was accomplished by focusing on client strengths. Finally, in order for the home visitors to assist parents in therapeutic change, they had a belief in and understanding of systems theory and the impact that the environment can or has had on those clients. Without an ecological approach, home visitors are likely to blame the families with which they work for their unfortunate situations such as poverty. Each of the five characteristics can be taught and improved, but entrenched beliefs are less easily manipulated. The five characteristics are ordered with the first (effective at forming relationships) the most amenable to change and the last (an ecological approach) the least.

Many people in South Africa are aware of the need for intervention and rehabilitation of disabled children. The lack of resources in the state healthcare system has necessitated a move towards capacity building and empowerment of community members to reduce the burden of responsibility on the healthcare system. Such engagement of community members in healthcare initiatives also has the potential of re-integrating disabled people back into society. The implementation of these strategies is undertaken largely by non-governmental organisations. One such NGO, the Child Development and Resource Centre (CORC) provides a service to children with multiple disabilities and their caregivers for whom no other resources are available. In attempting to increase the range of its impact on disabled people in Kwazulu-Natal, a training programme was developed to deal with the needs of the caregivers. The purpose of this study is to describe, evaluate and recommend changes of this nine module training course facilitated by the CDRC. The target population was the primary caregivers of disabled children living in peri-urban and rural areas and educare workers from non-profit organisations who were involved in service delivery to such families. The purpose of the training programme was to raise community awareness around disability, to sensitise and educate individuals to the needs of the disabled, and to empower caregivers in caring for disabled children in their communities. The CDRC training programme was subjected to on-going evaluation and monitoring throughout the entire process. Based on an action research model, programme evaluation was implemented and data was gathered from three sources: observation notes on each of the nine modules, evaluation forms completed by the participants, and focus group discussions with the participants. / Thesis (M.Ed.)-University of Natal, Pietermaritzburg, 2001.

A tribal journey : canoes, traditions, and cultural continuity

In addressing the necessity of cultural transmission from one generation to the next, this ethnographic study examines ways that Indigenous canoe journeys enable communication of ancestral teachings and traditions, particularly to Kw‟umut Lelum youth. The objective is to identify how experiences and interactions within Indigenous canoe journeys, specifically Tribal Journeys, can connect youth to traditions, environments, Elders, other individuals, and each other. Drawing on interviews with adults and participant observation, I consider relational themes of self and identity to explore the cultural impact on the young people as they participate in Tribal Journeys 2010 and symbolic ceremonies within it. Through qualitative inquiry and inductive reasoning, this interpretive epistemological approach includes concepts specific to the Indigenous research paradigm and uses a performative narrative to present results. Kw‟umut Lelum Child and Family Services is a society committed to the well-being of Indigenous children residing within nine Coast Salish communities on Vancouver Island. The agency focuses on family, community, and sacredness of culture as guided by the Snuw‟uy‟ulh model, which uses the teachings of the present to unite the past and future. Tribal Journeys is a significant cultural event that upholds the Snuw‟uy‟ulh principles while facilitating the communication of ancestral teachings and traditions. Nesse estudo foram realizadas 124 entrevistas a usuários cadastrados nas Unidades Básicas de Saúde ou Unidades de Saúde da Família da Secretaria Municipal de Saúde do município de Botucatu – SP, que participaram das visitas domiciliares em 2011, e aplicado um questionário semi estruturado, identificando-se suas características socioeconômicas e demográficas. Para a análise estatística foi utilizado o software SPSS/WindowsÒ (versão 10.5), calculamos as porcentagens das variáveis estudadas e utilizamos o teste de qui-quadrado, com correção de Pearson quando necessário, considerando-se nível de significância de 5%. A análise dos resultados possibilitou conhecer as características dos moradores do domicílio, das condições de vida familiar, bem como o meio em que vivem e como vivem e identificamos que não há diferença significativa entre as famílias selecionadas em dois anos diferentes e consecutivos. Assim, entendemos que nossos resultados podem contribuir para o planejamento das atividades de visita domiciliar realizada pelos estudantes da Disciplina IUSC / The establishment of strategies that articulate the university, the services and the community are teaching-learning practices which have increasingly gained space in the health area, stimulating higher education institutions to adequate the teaching policies directed to the formation of a generalist professional, qualified to understand the health-disease process at the different care levels. The present study aimed to learn the profile of families that received home visits by 1st and 2nd-year undergraduate students of Medicine and Nursing during the discipline Interação Universidade Serviços Comunidade da Faculdade de Medicina de Botucatu - UNESP, in 2011. In this study, 124 interviews were conducted to users registered at the Basic Health Units or Family Health Units of the Secretariat of Health of Botucatu Municipality – São Paulo State, which participated in the home visiting in 2011, and a semi-structured questionnaire was applied to identify socioeconomic and demographic characteristics. For statistical analysis, the software SPSS/WindowsÒ (version 10.5) was employed; we calculated the percentages of the studied variables and used the Chi-square test, with Pearson's correction when necessary, considering a significance level of 5%. Analysis of the results allowed us to learn the characteristics of the residents of the home, the living conditions of family, as well as the environment where they live, and we identified that there is no significant difference between the families selected in two different and consecutive years. The utility of Weingarten's witness positions in the understanding of compassion fatigue in people who care for their own family members with AIDS

This paper explores the utility of Weingarten's (2003) witness positions in the understanding of compassion fatigue in people who care for their own family members with AIDS. The research is embedded in Weingarten's theory of witnessing and narrative theory and practice. The literature review explores the shift from hospital-based care to community/home-based care which has led to family members assuming the role of caring for their family members with AIDS, an overview of the effects of caring for AIDS patients on caregivers and an overview of Weingarten's (2003) theory of witnessing with special emphasis on the witnessing positions and their consequences. Interviews, based on narrative theory and practice in which Weingarten's theory is rooted, gave access to the participants' experiences, which were then analysed and interpreted through a framework developed from the witnessing theory. This article demonstrates the utility of Weingarten's (2003) theory of witnessing to people who are caregivers to their own family members with AIDS. I argue that witness positions occupied by caregivers during witnessing determine whether the caregivers will experience compassion fatigue. The negative consequences related to compassion fatigue that will be reviewed could probably be prevented through active, intentional, compassionate witnessing.

Vulnerabilidade social juvenil: uma análise a partir de jovens do Projovem Adolescente no município de Pato Branco - PR / Youth social vulnerability: an analysis from young people of the Adolescent Projovem in the municipality of Pato Branco - PR

A vulnerabilidade social abarca uma diversidade de fenômenos que não se limita apenas à insuficiência de renda, mas perpassa questões de ciclos de vida, gênero, etnia, orientação sexual e outros e está intimamente associada ao processo de desenvolvimento de uma sociedade. Em especial os jovens têm sido um segmento da população que tem se apresentado como emergente no cenário da vulnerabilidade social e têm sido considerados, por diversas vezes, frágeis para enfrentar adversidades cotidianas. Esta pesquisa teve como objetivo analisar a percepção de jovens participantes e não participantes do Projovem Adolescente do Centro de Referência de Assistência Social Paulina Bonalume Andreatta no município de Pato Branco - PR, sobre vulnerabilidade social relacionada ao próprio ciclo de vida, família, trabalho, educação e política pública. Metodologicamente, foi uma pesquisa de cunho social e exploratório. Realizou-se uma pesquisa de cunho quantitativo e qualitativo com análise de conteúdo. Inicialmente, foi realizada a análise de agrupamento de dados sobre o perfil socioeconômico de famílias com a presença de jovens entre 15 e 29 anos, residentes no município de Pato Branco – PR, que estão inseridas no Cadastro Único para Programas Sociais – CadÚnico, com o objetivo de caracterizar o cenário da pesquisa. Depois, utilizou-se a entrevista semiestruturada para compreender a percepção de jovens participantes e não participantes do Projovem adolescente sobre aspectos de vulnerabilidade social relacionada ao seu próprio ciclo de vida. As famílias analisadas apresentaram características de escassos recursos físicos e humanos, o que sugere a exposição de seus membros, sejam eles, crianças, jovens, adultos ou idosos, a situações de vulnerabilidade social. A percepção dos jovens indica que a condição social e econômica de suas famílias, o local onde residem, as relações familiares e locais, a insuficiência de atividades para lazer e os casos de gravidez na adolescência apresentam fatores que fragilizam e dificultam as suas vidas quanto a conseguir um trabalho, ter um rendimento adequado na escola e ter uma boa convivência familiar e comunitária. / Social vulnerability encompasses a diversity of phenomena that is not limited to income insufficiency but pervades issues of life cycles, gender, ethnicity, sexual orientation and others and is closely associated with the development process of a society. In particular, young people have been a segment of the population that has emerged as an emerging social vulnerability scenario and have been considered, on several occasions, fragile to face daily adversities. This research had the objective of analyzing the perception of young participants and nonparticipants of the Projovem Adolescent of the Paulina Bonalume Andreatta Social Assistance Reference Center in the city of Pato Branco - PR, about social vulnerability related to the life cycle, family, work, education and public policy. Methodologically, it was a research of social and exploratory nature. Quantitative and qualitative research was carried out with content analysis. Initially, the analysis of data grouping on the socioeconomic profile of families with the presence of 15 to 29 year olds living in the municipality of Pato Branco - PR, which is part of the Cadastro Único para Programas Sociales - CadÚnico, was carried out. objective of characterizing the research scenario. Then, the semi-structured interview was used to understand the perception of young participants and nonparticipants of the Projovem adolescent on aspects of social vulnerability related to their own life cycle. The analyzed families presented characteristics of scarce physical and human resources, which suggests the exposure of their members, be they children, young people, adults or elderly, to situations of social vulnerability. Young people's perception indicates that the social and economic condition of their families, their place of residence, family and local relations, lack of leisure activities and teenage pregnancy present factors that weaken and hinder their lives in terms of get a job, have an adequate income in school and have a good family and community coexistence.
