Evaluation of Oral Neutrophil Levels as a Quantitative Measure of Periodontal Inflammatory Load in Patients with Special Needs

Purpose: To validate and assess the feasibility of using an assay of oral neutrophils to measure periodontal inflammation in uncooperative patients with special needs. Methods: Periodontal examination and neutrophil counts derived from oral swabs were performed on patients with special needs having comprehensive dental treatment under general anaesthesia (GA). The conventional periodontal measurements were compared to neutrophil levels while patients were under GA, and later at their recall examination. Results: Forty-nine patients were assessed under GA and 30 (61%) returned for recall examination. Spearman’s correlation allowed for comparisons between periodontal parameters and oral neutrophil counts. Despite limited cooperation, it was possible to acquire neutrophils (using swabs) for all patients that presented for recall examination in the ambulatory dental clinic. Conclusions: Oral neutrophil levels correlated significantly with conventional parameters of gingival inflammation and may serve as a standardized method for clinical assessment of periodontal diseases in the special needs population.

CORRELAÇÕES ENTRE BIOFILME E CONDIÇÃO GENGIVAL EM DIFERENTES FREQUÊNCIAS DE HIGIENE BUCAL / CORRELATIONS BETWEEN BIOFILM AND GINGIVAL STATUS AT DIFFERENT ORAL HYGIENE FREQUENCIES

Evidence has shown the importance of gingivitis as a precursor of periodontal attachment loss and tooth loss. Despite its importance, often the diagnosis of gingival inflammation is overlooked. For maintaining gingival health, biofilm mechanical removal procedures should be perfomed effectively and periodically. Studies have shown oral hygiene frequencies of 12 and 24h are related to the gingival health maintenance. As important as frequency, it is the quality in which the procedure is performed. Studies assessing the association between toothbrushing frequency, effective biofilm removal and gingival clinical changes have an important role in the prevention and treatment of gingivitis. The aim of this study was to evaluate correlation between plaque and gingival inflammation in individuals underwent effective biofilm control at different frequencies. Data from this study were collected from a randomized clinical trial conducted in 2012 in Santa Maria, RS, Brazil. The sample consisted of 52 UFSM students, of courses not related to health, initially randomized into 4 oral hygiene frequencies and grouped into 2: Group G12/24 (12 and 24h) and group G48/72 (48 and 72h). In G12/24, even with an increase in plaque levels during 30 days, gingival inflammation levels were not statistically different from baseline. In G48/72, increase in plaque levels were accompanied by increasing levels of gingival inflammation. Positive correlation was observed in both groups, however, correlation is reduced in G12/24, demonstrating that increase in plaque levels were not sufficient to develop inflammatory response. In subjects who perform effective biofilm control, the correlation between plaque and gingival inflammation is affected by oral hygiene frequency. / A gengivite é considerada a forma mais prevalente dentre as doenças periodontais. Evidências têm mostrado a importância da gengivite como precursora da perda de inserção periodontal e perda dental. A despeito de sua importância, muitas vezes o diagnóstico da inflamação gengival é negligenciado. Para a manutenção de saúde gengival, procedimentos de desorganização mecânica do biofilme devem ser realizados de forma efetiva e periódica. Estudos observaram que frequências de higiene bucal (HB) de 12 e 24 horas estão relacionadas a manutenção de saúde gengival. Tão importante quanto a frequência, é a qualidade em que esse procedimento é realizado. Estudos que avaliam a associação entre frequência de escovação, desorganização efetiva do biofilme e alterações clínicas gengivais têm um papel importante na prevenção e tratamento das gengivites. Portanto, o objetivo deste estudo foi avaliar a existência de correlação entre biofilme e inflamação gengival em indivíduos que realizaram efetivo controle do biofilme em diferentes frequências. Os dados deste estudo foram coletados de um ensaio clínico randomizado realizado no ano de 2012 na cidade de Santa Maria, RS, Brasil. A amostra foi composta por 52 estudantes de cursos não relacionados a área da saúde da UFSM inicialmente randomizados em 4 frequências de higiene bucal e agrupados em 2: grupo G12/24 (12 e 24h) e grupo G48/72 (48 e 72h). No G12/24, mesmo havendo aumento nos níveis de Índice de Placa (IPl) durante os 30 dias, os níveis de inflamação gengival não diferiram estatisticamente do baseline. Já no G48/72, aumento nos níveis de IPl foi acompanhado por aumento nos níveis de inflamação gengival. Houve correlação positiva em ambos os grupos, no entanto, a correlação diminuiu no G12/24, demonstrando que o aumento nos níveis de biofilme não foi suficiente para desenvolver resposta inflamatória gengival. Em indivíduos que realizam controle efetivo do biofilme, a correlação entre biofilme e inflamação gengival é afetada pela frequência de higiene bucal.

Alterações periodontais em diabéticos tipo II e controles não diabéticos

O diabetes mellitus (DM) é caracterizado por uma deficiência no transporte de glicose da corrente sanguínea para o interior da célula, elevando os níveis de glicose no sangue. Isso ocorre pela deficiência e até mesmo resistência à insulina, e é controlada por agentes hipoglicêmicos orais e/ou dieta, associada a exercícios físicos. É classificada em tipo I e II, sendo que o segundo é a forma mais comum da doença e tem a idade como um dos fatores predisponentes. O periodonto é formado por estruturas de suporte e proteção ao elemento dentário e seu aspecto de saúde tem uma importância significante na cavidade bucal. A doença periodontal (DP) é um processo inflamatório crônico caracterizado por inflamação do tecido gengival e/ou perda de estruturas que compõem o periodonto, causando danos à estrutura dentária ou até perda do dente. É reconhecida como a sexta maior complicação do diabetes. Esse estudo teve como objetivo comparar as alterações periodontais em indivíduos diabéticos tipo II e não diabéticos residentes no município de Araguaína Tocantins (TO). Para isso, foram examinados 34 pacientes divididos em dois grupos: diabéticos e não diabéticos, que foram submetidos a exames periodontais para determinar a Profundidade de Sondagem (PS), Perda de Inserção Clínica (PIC), Índice Gengival (IG), Índice de Placa (IP), Índice de Higiene Oral (IHO) e exames laboratoriais para os diabéticos tipo II. Para a análise estatística foi usado o teste t no intuito de obter as correlações dos índices entre os diabéticos e o teste U para analisar as alterações periodontais entre os grupos. Foram encontrados PIC e IHO maiores em diabéticos, sendo os demais índices maiores no grupo dos não diabéticos. Só IP não apresentou diferença estatística significante (p>0,05). Também foi verificada uma correlação entre IP e IG, não ocorrendo entre os demais índices. Concluiu-se que os diabéticos têm doença periodontal mais severa que os não diabéticos. / Diabetes Melitus (DM) is characterized by a deficiency in the glicose transport from the blood stream to the inside of cells, raising the glicose leves in the blood. This occurs due to the deficiency or even resistance to insulin, and it is controlled by oral hypoglicemic agents and/or diet, associated with physical exercises. It is classified in types I and II, in such a way the second type in the most common and its predisponent factor is age. The periodonto is made of structures that support and protect the dental element and its health aspect has a significant importance to the bucal cavity. The periodontal disease (PD) is a chronic inflammatory process characterized by inflammation of the gingival tissue and /or loss of structures that compose the periodonto, causing damage to the dental structure or even loss of teeth. Its known as the sixth biggest complication derived from diabetes. This work had the purpose of comparing periodontal modifications in diabetic type II and non diabetic patients who lived in Araguaína Tocantins (TO). Therefore, 34 patients were examined and separated in two groups with PD: diabetics and non diabetics. They were submitted to periodontal examination in order to measure the Depth Proof (PPD), Clinical Attachment Loss (CAL), Gingival Index (GI), Plaque Index (PII), Oral Hygiene Index (OHI) indices. Also, laboratorial tests were made for type II - diabetic patients. The statistical analysis was made using the t-test to obtain the indices of correlation in diabetic group and the u-test to evaluate periodontal modifications between the groups. The CAL and the indices were higher in diabetic group and the other indices were higher in non diabetic group. Only the PII index show no significant statistic difference (p>0,05). It was also verified a correlation between PII and GI indices, yet it was not true in other indices. It was possible the conclude that the diabetics have more severe periodontal disease than the non diabetics.
