Comparação dos efeitos hemodinâmicos da efedrina ou da fenilefrina em eqüínos anestesiados com isoflurano / Comparison of hemodynamics effects of ephedrine or phenylephrine in horses anesthetised with isoflurane

A depressão do sistema cardiocirculatório é um fator associado ao uso dos agentes halogenados na anestesia inalatória eqüina. Como conseqüência direta é observada a queda na perfusão tecidual, causadora de complicações como a miopatia pós-anestésica, o íleo paralítico ou a insuficiência renal. O objetivo do presente estudo foi avaliar os efeitos hemodinâmicos de dois fármacos simpatomiméticos, efedrina e fenilefrina, em eqüinos anestesiados com isoflurano que apresentaram episódio de hipotensão durante o procedimento anestésico. Foram utilizados 10 eqüinos sadios, em decúbito lateral, distribuídos aleatoriamente em dois grupos de cinco animais cada. O grupo I recebeu a infusão de efedrina (0.02mg/kg/min), e o grupo II de fenilefrina (0,002mg/kg/min), sendo que o avaliador não conhecia a natureza dos fármacos. Foram analisados os parâmetros hemodinâmicos, de oxigenação e de ventilação. Os dados foram submetidos à comparação estatística que constou de análise de variância seguida dos testes de Tukey Kramer e T de Student. Quando comparou-se os diferentes momentos do mesmo grupo, foi observada melhora ou manutenção dos parâmetros hemodinâmicos avaliados após o uso dos agentes simpatomiméticos, nos dois grupos. Porém, quando os momentos correspondentes dos grupos foram confrontados, o índice cardíaco, o conteúdo de oxigênio venoso misto, o índice de transporte do oxigênio, a pressão de oxigênio venoso misto e a saturação de oxigênio venoso misto foram superiores no grupo I. Foi concluído que os dois agentes proporcionaram os efeitos hemodinâmicos desejáveis, entretanto, a efedrina apresentou melhor resposta cardiovascular e de oxigenação que a fenilefrina. Assim, pode-se pressupor que a infusão com efedrina provavelmente proporciona melhor perfusão sangüínea, sendo portanto mais indicada para o tratamento da hipotensão nos eqüinos submetidos à anestesia geral. / Depression of the cardiovascular system is a feature of the use of halogenated anesthetics in equine anesthesia. A decrease in tissue perfusion is the main result and can be the cause of complications such as post-anesthetic myopathy, ileus and renal insufficiency. The aim of then present study was to evaluate the hemodynamic effects of two sympathomimetic agents, ephedrine and phenylephrine in horses anesthetized with isoflurane, which presented hypotension during the anesthetic procedure. Ten healthy horses, in lateral recumbence, were randomly distributed in two groups of five animals each. Group I received an infusion of ephedrine (0.02mg/kg/min) and group II an infusion of phenylephrine (0.002mg/kg/min) and the surveyor did not know the agent administered. Hemodynamic, oxygenation and ventilation parameters were evaluated and the results submitted to statistical analysis performed by means of analysis of variance followed by the Tukey-Kramer and Student-T test. When the different moments of the same group were compared, an increment or maintenance of the hemodynamic parameters evaluated after the infusion of both agents was noticed. However, when the correspondent moments of each group were compared, the cardiac index, the mixed venous oxygen content, oxygen transport index and both mixed venous oxygen saturation and partial pressure were superior in group I. It was concluded that the two agents promoted the desired hemodynamic effects, although ephedrine caused the best cardiovascular and oxygenation responses. For this reason, we can presume that ephedrine infusion probably promotes superior tissue perfusion, and therefore is more indicated for the treatment of hypotension in horses submitted to general anesthesia.

Índice bispectral, efeitos hemodinâmicos e respiratórios da infusão contínua de diferentes taxas de propofol em bezerros /

Resumo: O estudo teve por objetivo avaliar o índice bispectral, efeitos hemodinâmicos e respiratórios da infusão contínua de diferentes taxas de propofol em bezerros. Foram utilizados 8 animais de seis a 12 meses de idade, holandeses, pesando de 84 a 124 kg. Os animais foram induzidos à anestesia com propofol na dose de 5 mg/kg IV e posicionados em decúbito lateral direito, onde permaneceram respirando espontaneamente ar ambiente. Ato contínuo, a manutenção anestésica foi realizada pela infusão contínua de propofol, administrado por meio de bomba de infusão em duas diferentes taxas 0,6 mg/kg/minuto IV (G06) e 0,8 mg/kg/minuto IV, (G08) durante 60 minutos. Os 8 animais foram anestesiados duas vezes, com uma semana de intervalo entre uma anestesia e outra. As variáveis hemodinâmicas (FC, PAS, PAD, PAM, DC, PVC, PAPm, PAPOm, IC, IS, IRVS e IRVP), respiratórias (ETCO, f, SpO, 2 2 V, V, e PFI), hemogasométricas (PaO, PaCO, SaO, DB, pH e HCO -) e TE ME 2 2 2 3 índice bispectral (BIS, EMG e IQS) foram avaliadas antes da indução anestésica (M ), e 15, 30, 45 e 60 minutos após o início da infusão contínua do B propofol (M15, M30, M45 e M60, respectivamente). Já a colheita das amostras para a dosagem plasmática do propofol foi realizada aos M15, M30, M45 e M60 minutos durante a infusão contínua do propofol e as demais colheitas foram realizadas após o termino da infusão com intervalos de 10 minutos por um período de 50 minutos (M70, M80, M90, M100 e M110 respectivamente). A infusão contínua de diferentes taxas de propofol não alterou as variáveis do BIS ao longo do tempo durante a infusão, também não causou alterações hemodinâmicas importantes, porém o G08 apresentou acidose respiratória indicando uso de ventilação controlada / Abstract: The aim of this study was to evaluate the bispectral index, hemodynamic and respiratory effects of continuous infusion of different rates of propofol in calves. Eight Holstein calves aging between six and 12 months and weighing 84-124 kg were used. They were induced to anesthesia with propofol at a dose of 5 mg / kg IV and positioned in right lateral recumbency, where they remained spontaneously breathing ambient air. Immediately thereafter, the anesthesia was maintained by continuous infusion of propofol administered with the aid of an infusion pump in two different rates 0.6 mg / kg / minute, IV (G06) and 0.8 mg / kg / minute, IV, (G08), during 60 minutes. The 8 animals were anesthetized twice, with one-week interval between anesthesia. The measurements of hemodynamic (HR, SAP, DAP, MAP, CO, CVP, mPAP, mPAPO, CI, SI, SVRI and PVRI), and respiratory parameters (ETCO2, f, SpO2, VTE, VME, and PFI), arterial blood gases (PaO2, PaCO2, SaO2, BE, pH and HCO3-) and bispectral index (BIS, EMG and SQI) were assessed before induction of anesthesia (MB), and 15, 30, 45 and 60 minutes after the start of continuous infusion of propofol (M15 , M30, M45 and M60, respectively). Sampling to measure plasma propofol was performed M15, M30, M45 and M60 minutes during continuous infusion of propofol and other samples were harvested after the end of infusion with 10-minute intervals during a period of 50 minutes (M70, M80, M90, M100 and M110 respectively). Functional magnetic resonance imaging : an intermediary between behavior and neural activity

Blood oxygen level dependent (BOLD) functional magnetic resonance imaging is a non-invasive technique used to trace changes in neural dynamics in reaction to mental activity caused by perceptual, motor or cognitive tasks. The BOLD response is a complex signal, a consequence of a series of physiological events regulated by increased neural activity. A method to infer from the BOLD signal onto underlying neuronal activity (hemodynamic inverse problem) is proposed in Chapter 2 under the assumption of a previously proposed mathematical model on the transduction of neural activity to the BOLD signal. Also, in this chapter we clarify the meaning of the neural activity function used as the input for an intrinsic dynamic system which can be viewed as an advanced substitute for the impulse response function. Chapter 3 describes an approach for recovering neural timing information (mental chronometry) in an object interaction decision task via solving the hemodynamic inverse problem. In contrast to the hemodynamic level, at the neural level, we were able to determine statistically significant latencies in activation between functional units in the model used. In Chapter 4, two approaches for regularization parameter tuning in a regularized-regression analysis are compared in an attempt to find the optimal amount of smoothing to be imposed on fMRI data in determining an empirical hemodynamic response function. We found that the noise autocorrelation structure can be improved by tuning the regularization parameter but the whitening-based criterion provides too much smoothing when compared to cross-validation. Chapter~5 illustrates that the smoothing techniques proposed in Chapter 4 can be useful in the issue of correlating behavioral and hemodynamic characteristics. Specifically, Chapter 5, based on the smoothing techniques from Chapter 4, seeks to correlate several parameters characterizing the hemodynamic response in Broca's area to behavioral measures in a naming task. In particular, a condition for independence between two routes of converting print to speech in a dual route cognitive model was verified in terms of hemodynamic parameters.

O objetivo desta dissertação é rever de maneira sistemática, os estudos clínicos aleatórios controlados que utilizaram algum tipo de protocolo de otimização hemodinâmica da perfusão tissular em pacientes cirúrgicos com alto risco para complicações no pós-operatório. Foi realizada uma busca computadorizada nas bases de dados da MEDLINE, EMBASE, LILACS, COCHRANE, utilizando o cruzamento de termos médicos, e recuperado estudos clínicos aleatórios controlados, de pacientes cirúrgicos submetidos de maneira controlada, a algum tipo de protocolo de otimização hemodinâmica peroperatória com objetivo de reduzir taxa de mortalidade, número de disfunções orgânicas e dias de internação hospitalar, sendo realizada uma metanálise dos resultados obtidos. Foram recuperados 31 estudos clínico, cuja metanálise global demonstrou uma redução estatisticamente significativa no odds ratio para taxa de mortalidade (odds ratio de 0,71 com 95% de intervalo de confiança entre 0,58-0,87), no número de pacientes com disfunções orgânicas no pósoperatório (odds ratio de 0,64 com 95% de intervalo de confiança entre 0,57- 0,73) e no tempo de internação hospitalar (diferença de médias de -1,06 com 95% de intervalo de confiança entre -1,19/-0,93). Ficando mais evidente nos estudos com taxa de mortalidade maior que 20%. Houve um benefício estatisticamente significativo na redução da taxa de mortalidade com a utilização do cateter da artéria pulmonar para aferição do débito cardíaco (odds ratio de 0,71 com 95% de intervalo de confiança entre 0,57-0,89) e utilização das... / Surgical patient with limited organic reserve are considered of high risk and possess a rate of larger mortality than the expected, needing, therefore, of a protocol of control hemodynamic more rigorous , in the sense of impeding situations of hypoperfusion tissue in the peroperative period. The objective of this dissertation is to review in a systematic way, the random clinical studies controlled that used some type of protocol of hemodynamic optimization of the perfusion tissue in patient surgical with high risk for complications in the postoperative one. A computerized search was accomplished in the bases of data of MEDLINE, BASE, LILACS, COCHRANE, using the crossing of medical terms, and recovered random clinical studies controlled of patient surgical submitted in the some type of protocol of hemodynamic optimization in the preoperative period with objective of reducing mortality rate, number of organic dysfunctions and length of hospital stay, being accomplished a meta-analysis of the obtained results. 31 clinical studies were recovered, whose global meta-analyses demonstrated a reduction significant in the odds ratio for mortality rate (odds ratio of 0,71 with 95% of confidence interval among 0,58-087), in the number of patients with organic dysfunctions in the postoperative (odds ratio of 0,64 with 95% of confidence interval among 0,57-073) and in the length of hospital stay (it differentiates of averages of -1,06 with 95% of confidence interval among - 1,19 /-093), being more evident in the studies with rate of larger mortality than 20%. There was a benefit significant in the reduction of the mortality rate with the use of the catheter of the lung artery for gauging of the heart debit (odds ratio of 0,71 with 95% of trust interval among 0,57-089) and use of the derived variables of the transport and consumption of oxygen (odds ratio of 0,75 with 95% oconfidencef interval among - ... Estudo comparativo entre a atropina e a hioscina na reversão da bradicardia induzida pela detomidina em equinos /

Orientador: Francisco José Teixeira Neto / Banca: Denise Tabacchi Fantoni / Banca: Carlos Augusto Araújo Valasco / Resumo: O anticolinérgico N-butilbrometo de hioscina (hioscina), por resultar em ação depressora da motilidade intestinal de curta duração, pode representrar uma melhor alternativa ao sulfato de atropina (atropina) no tratamento da bradicardia de origem vagal em eqüinos. Os efeitos hemodinâmicos e gastrointestinais da hioscina e da atropina foram comparados em eqüinos através de um modelo de bradicardia induzida pela detomidina. Seis cavalos adultos (391±47 kg) receberam detomidina (0,02 mg/kg IV) em 3 ocasiões distintas com intervalo mínimo de 1 semana entre os tratamentos. Dez minutos após a administração da detomidina foram administradas pela via intravenosa solução salina de NaCl a 0,9% (controle), atropina (0,02mg/kg) ou hioscina (0,2mg/kg). Escores de auscultação intestinal [0 (ausência de sons) a 16 (sons intestinais normais)], determinados por 2 avaliadores que desconheciam o tratamento e as variáveis cardiorrespiratórios foram avaliadas por 24 horas e 90 minutos após a administração da detomidina, respectivamente. O trânsito gastrointestinal foi monitorado por 96 horas através da detecção de cromo em matéria fecal seca. Para análise dos dados foi utilizado delineamento em parcelas subdivididas com teste ajustado para Tukey ou teste Kruskall-Wallis seguido de Dunn (p < 0.05). No tratamento controle a detomidina diminuiu a frequência cardíaca (FC) e o índice cardíaco (IC) em relação aos valores basais por 30 e 60 minutos, respectivamente. A FC (bat/min) aumentou acima dos valores basais aos 5 minutos após a administração da atropina (79±5) e da hioscina (75±8), permanecendo reduzida após a administração de salina (29±8). A partir deste momento a FC foi significativamente maior no grupo atropina em comparação aos demais tratamentos; enquanto a hioscina resultou em valores intermediários de FC (menores que no tratamento atropina e maiores... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: The anticholinergic hyoscine-N-butylbromide (hyoscine), because of its shorter acting gastrointestinal motility depressant effects, may represent a better alternative to the use of atropine sulphate (atropine) in the treatment of vagally mediated bradycardia in horses. The hemodynamic and gastrointestinal effects of hyoscine and atropine were compared in model of bradycardia induced by detomidine. Six adult horses (391±47 kg) received detomidine (0.02 mg kg-1, intravenously) on 3 occasions (1-week washout intervals among experiments). Ten minutes after detomidine, physiological saline (control), atropine (0.02 mg kg-1), or hyoscine (0.2 mg kg-1) were administered intravenously. Intestinal auscultation scores [range: 0 (absent sounds) to 16 (normal intestinal sounds)], determined by 2 blinded evaluators, and cardiopulmonary data were monitored for 24 hours and 90 minutes after detomidine, respectively. Gastrointestinal transit was assessed for 96 hours via detection of chromium in dry feces. A split-plot design and a Tukey-Kramer test, or a Kruskall-Wallis and a Dunn's test analyzed data were appropriate (p < 0.05). In the control treatment, detomidine decreased heart rate (HR) and cardiac index (CI) from baseline for 30 and 60 minutes, respectively. Heart rate (beats minute-1) increased above baseline 5 minutes after atropine (79±5) and hyoscine (75±8) but not after saline (29±8). After this time, HR after atropine was higher in comparison to the other treatments; while hyoscine resulted in intermediate HR values (lower than atropine but higher than controls). Hyoscine and atropine resulted in higher CI than controls for 5 and 20 minutes, respectively; but this effect coincided with a significant hypertensive response (mean arterial pressure > 200 mmHg). Foram utilizados 8 animais de seis a 12 meses de idade, holandeses, pesando de 84 a 124 kg. Os animais foram induzidos à anestesia com propofol na dose de 5 mg/kg IV e posicionados em decúbito lateral direito, onde permaneceram respirando espontaneamente ar ambiente. Ato contínuo, a manutenção anestésica foi realizada pela infusão contínua de propofol, administrado por meio de bomba de infusão em duas diferentes taxas 0,6 mg/kg/minuto IV (G06) e 0,8 mg/kg/minuto IV, (G08) durante 60 minutos. Os 8 animais foram anestesiados duas vezes, com uma semana de intervalo entre uma anestesia e outra. As variáveis hemodinâmicas (FC, PAS, PAD, PAM, DC, PVC, PAPm, PAPOm, IC, IS, IRVS e IRVP), respiratórias (ETCO, f, SpO, 2 2 V, V, e PFI), hemogasométricas (PaO, PaCO, SaO, DB, pH e HCO -) e TE ME 2 2 2 3 índice bispectral (BIS, EMG e IQS) foram avaliadas antes da indução anestésica (M ), e 15, 30, 45 e 60 minutos após o início da infusão contínua do B propofol (M15, M30, M45 e M60, respectivamente). Já a colheita das amostras para a dosagem plasmática do propofol foi realizada aos M15, M30, M45 e M60 minutos durante a infusão contínua do propofol e as demais colheitas foram realizadas após o termino da infusão com intervalos de 10 minutos por um período de 50 minutos (M70, M80, M90, M100 e M110 respectivamente). A infusão contínua de diferentes taxas de propofol não alterou as variáveis do BIS ao longo do tempo durante a infusão, também não causou alterações hemodinâmicas importantes, porém o G08 apresentou acidose respiratória indicando uso de ventilação controlada / The aim of this study was to evaluate the bispectral index, hemodynamic and respiratory effects of continuous infusion of different rates of propofol in calves. Eight Holstein calves aging between six and 12 months and weighing 84-124 kg were used. They were induced to anesthesia with propofol at a dose of 5 mg / kg IV and positioned in right lateral recumbency, where they remained spontaneously breathing ambient air.
