Estudos sugerem que portadores do vírus da hepatite C (HCV) com esta infecção exibem fibrose hepática mais extensa, pior resposta ao tratamento com interferon alfa e maior risco de desenvolvimento de hepatocarcinoma. Achados similares foram descritos em portadores de HCV com marcadores de infecção prévia pelo HBV (anti-HBc positivo, com ou sem anti-HBs), independentemente da detecção de HBV-DNA. Entretanto, tais estudos não exerceram controle adequado para variáveis associadas à progressão da fibrose na hepatite C, como tempo de infecção, etilismo e idade no momento da infecção. Objetivos: Determinar a prevalência e o impacto da infecção oculta e da infecção prévia pelo HBV nas características clínicas, bioquímicas, virológicas e histológicas de portadores de infecção crônica pelo HCV. Material e Métodos: Estudo caso-controle que avaliou pacientes não-etilistas com HBsAg e anti-HIV negativos e HCV-RNA positivo. Todos os casos possuíam exposição parenteral como provável modo de contaminação pelo HCV (antecedentes transfusionais ou drogadição). Amostras séricas coletadas em intervalo de até 6 meses da biópsia hepática foram submetidas à pesquisa de HBV-DNA por ensaio comercial baseado em PCR, com limite de detecção de 103 cópias/mL (Amplicor HBV MONITOR® Test, Roche). A classificação de METAVIR foi usada para análise histológica. Resultados: Cento e onze pacientes foram incluídos, sendo 46% do gênero masculino e 82% com transfusão prévia. As médias da idade na infecção e do tempo de infecção pelo HCV foram de 25,5+12,4 e 21,9+6,5 anos, respectivamente. O anti-HBc foi positivo em 31 indivíduos (28%) e nenhuma amostra foi positiva para o HBV-DNA. Pacientes com antiHBc positivo foram comparáveis aos com anti-HBc negativo com relação ao gênero, idade na infecção pelo HCV, tempo de infecção, raça, modo de aquisição do HCV e prevalência de descompensação hepática. A distribuição de genótipos do HCV foi semelhante entre os dois grupos. Entretanto, indivíduos com anti-HBc positivo mostraram níveis mais baixos de albumina (P = 0,001) e menor atividade de protrombina (P = 0,046) e maiores níveis de ALT (P = 0,052), AST (P = 0,004) e GGT (P = 0,010). Estes pacientes também demonstraram maior atividade histológica (P < 0,001), maior escore de fibrose hepática (P = 0,001) e taxa de progressão de fibrose (TPF) mais rápida (P = 0.002) do que aqueles com anti-HBc negativo. Na análise multivariada, ALT > 2 vezes o limite superior do normal (xLSN) (OR = 4,460; P = 0,002), GGT > 1,5 xLSN (OR = 7,582; P < 0,001) e anti-HBc positivo (OR = 4,009; P = 0,019) foram preditivos de atividade necroinflamatória moderada ou acentuada (A2/A3). A positividade do anti-HBc (OR = 3,364; P = 0,017) e a aquisição do HCV após 30 anos de idade (OR = 4,252; P = 0,002) foram independentemente associados à fibrose significativa (F2/F3/F4). Os fatores associados à rápida progressão da fibrose, definida como TPF > 0,133 unidade de fibrose/ano, foram idade na infecção > 30 anos (OR = 2,913; P = 0,033) e anti-HBc positivo (OR = 3,241; P = 0,015). O tempo esperado até o surgimento de cirrose hepática foi de 26 anos (20 a 44 anos) para indivíduos anti-HBcpositivo que se infectaram com o HCV com idade > 30 anos. Aqueles com anti-HBc negativo contaminados com menos de 30 anos de idade desenvolveriam cirrose após 80 anos de infecção (60 a 93 anos) (P < 0,001). Conclusões: A infecção prévia pelo HBV é encontrada em cerca de um terço dos portadores do HCV e pode exercer impacto negativo sobre a história natural da infecção crônica pelo HCV. Este efeito parece independer da presença da infecção oculta pelo HBV. / Background: Occult hepatitis B virus (HBV) infection is defined by the presence of HBV-DNA by PCR in serum or liver tissue samples from HBsAg-negative individuals. Recent reports suggest that hepatitis C virus (HCV) carriers who also harbor this silent infection have more advanced liver fibrosis, reduced response to interferon, and increased risk of developing hepatocellular carcinoma. Similar findings have been described among chronic hepatitis C patients with serological markers of prior HBV infection (anti-HBc positive, with or without anti-HBs), irrespective of HBV-DNA detection. However, these studies have failed to appropriately control for factors known to impact HCV-related fibrogenesis including duration of infection, alcohol abuse, and age at infection. Aims: To assess the prevalence and impact of occult and previous HBV infection on clinical, biochemical, virological and histological features in patients with chronic hepatitis C from a liver clinic cohort. Methods: This case-control study included non-alcoholic subjects whose sera tested negative for HBsAg and anti-HIV, and positive for HCV-RNA. All patients had prior parenteral exposure as the probable source of HCV infection (blood transfusions or IV drug use). Serum samples were collected within 6 months of liver biopsy and were tested for HBV-DNA using a commercial PCR assay with sensitivity of 103 copies/mL (Amplicor HBV MONITOR® Test, Roche). METAVIR scoring system was applied for grading necroinflammatory activity and staging fibrosis. Results: One hundred and eleven patients were evaluated. Forty six percent were male and 82% reported past transfusion of blood derivates. The mean age at infection and the estimated duration of infection were 25.5 + 12.4 and 21.9 + 6.5 years, respectively. Thirty-one out of 111 patients (28%) tested positive for antiHBc. HBV-DNA was not detected in any sample. There were no differences between anti-HBc-positive and -negative patients concerning gender, age at infection, estimated duration of infection, ethnicity, source of HCV infection, and prevalence of hepatic decompensation. In addition, HCV genotyping distribution were not influenced by antiHBc status. However, anti-HBc-positive patients showed lower albumin levels (P = 0.001), lower prothrombin activity (P = 0.046) and higher levels of ALT (P = 0.052), AST (P = 0.004), and GGT (P = 0.010). These patients also showed higher histological grading (P < 0.001) and staging scores (P = 0.001), and higher rate of fibrosis progression (RFP) (P = 0.002) as compared to those who tested negative for anti-HBc. By multivariate analysis, ALT > 2x upper limit of normal (ULN) (OR = 4.460; P = 0.002), GGT > 1.5x ULN (OR = 7.582; P < 0.001) and anti-HBc-positivity (OR = 4.009; P = 0.019) were predictive of moderate to severe necroinflammatory activity (A2/A3). AntiHBc-positivity (OR = 3.364; P = 0.017) and age at infection > 30 years (OR = 4.252; P = 0.002) were independently associated with significantly hepatic fibrosis (F2/F3/F4). Likewise, independent predictors of rapid fibrosis progression, defined as a RFP > 0,133 fibrosis unit/year, were age at infection > 30 years (OR = 2.913; P = 0.033) and anti-HBc-positivity (OR = 3.241; P = 0.015). The median duration from HCV infection to cirrhosis was 26 years (20 to 44) in anti-HBc-positive patients who were infected with HCV when older than 30 years. On the other hand, among anti-HBc-negative subjects who acquired HCV before the age of 30, the expected time to cirrhosis was 80 years (60 to 93) (P < 0,001). Conclusions: Previous HBV infection is common among HCVinfected individuals and may exert a negative impact on the natural history of hepatitis C virus infection. O presente trabalho objetivou estabelecer a prevalência dos marcadores sorológicos de imunidade e infecção pelo HBV e infecção pelo HCV em adolescentes com idade entre 10 a 15 anos, estudantes de Joinville, Santa Catarina. A pesquisa foi realizada com 400 voluntários, sendo que todos os participantes responderam a um questionário e as carteiras de vacinação foram verificadas. Os marcadores sorológicos HBsAg, anti-HBc, anti-HBs e anti-HCV foram analisados por meio da metodologia de quimioluminescência utilizando o equipamento automaticado ADVIA CENTAUR® e reagentes da marca SIEMENS®. Dentre os participantes, 60,5% eram do gênero feminino e 39,5% do masculino. A análise sorológica evidenciou ausência dos marcadores anti-HCV e HBsAg. A prevalência do anti-HBc reagente foi 1,25% e reatividade para anti-HBs de 39,5%. A maioria dos voluntários (71,75%) não souberam responder sobre o conceito de hepatite e 59,0% não sabiam responder sobre a forma de transmissão. Observou-se ainda que 56,25% dos voluntários demonstrou conhecimento sobre a vacina preventiva contra o HBV e que a cobertura vacinal foi de 96,5% Em conformidade com os dados obtidos, conclui-se que a circulação do HBV e HCV na cidade de Joinville entre adolescentes de 10 a 15 é baixa e que o nível de conhecimento sobre as hepatites B e C nessa população revelou a necessidade de implantação de estratégias educacionais voltadas ao tema.<br> / Abstract : Infection by the hepatitis B virus (HBV) and hepatitis C virus (HCV) is a serious global public health problem that affects millions of individuals every year. This study aimed to establish the prevalence of serological markers of HBV infection and immunity and HCV infection among adolescents aged 10 to 15 years, studying in Joinville, Santa Catarina. Four hundred volunteers answered a questionnaire and vaccination cards were verified. The serological markers HBsAg, anti-HBc, anti-HBs and anti-HCV were analysed via the chemiluminescence method using ADVIA CENTAUR ® automated equipment and SIEMENS ® reagents. Among the participants, 60.5% were female and 39.5% male. Serological analysis showed no markers of anti-HCV or HBsAg. The prevalence of anti-HBc reagent was 1.25% and reactivity to anti-HBs 39.5%. Most volunteers (71.75%) did not know what hepatitis is and 59.0% did not know how it is transmitted. It was also observed that 56.25% of volunteers knew about the preventive vaccine against HBV and vaccination coverage was 96.5%. It was concluded that the circulation of HBV and HCV among adolescents aged 10?15 in the city of Joinville is low and that the level of knowledge about hepatitis B and C in this population indicates the need to implement educational strategies focused on this topic.

Avaliação da resposta imunologica a vacina contra a hepatite B aplicada pelas vias intradermica ou intramuscular em profissionais da saude de hospital universitario : seguimento de cinco anos

Entretanto, o alto custo da vacina é um empecilho para que se possa garantir uma ampla cobertura vacinal. Vários estudos têm sido realizados no sentido de tentar-se viabilizar esquemas vacinais mais econômicos, mantendo a eficácia. A aplicação intradérmica reduz de sete a dez vezes o custo da vacinação e, em determinados grupos populacionais, apresenta eficácia semelhante à da intramuscular. Tentando buscar esquemas alternativos mais econômicos, para vacinação contra o VHB em P AS, selecionamos um grupo de funcionários da área da saúde da Unicamp para receberem a vacina contra o VHB. Estes profissionais foram divididos em dois grupos, conforme a via de aplicação: intradérmica (ID) ou intramuscular (1M). Todos os que foram alocados para aplicação ID e uma parcela dos que a receberam via 1M foram acompanhados para avaliação da resposta imunológica à vacinação, através da pesquisa qualitativa e quantitativa do anti-HBs. O esquema proposto para a vacinação foi de três doses, aplicadas nos intervalos de um e seis meses, em relação à primeira dose. A vacina utilizada foi a vacina recombinante belga ("Engerix B", SmithKline), na dose de 20 µ g (1 ml) para aplicação 1M e 0,2 µ g (0,1 ml) para aplicação ID (face ventral do antebraço esquerdo). De todos os funcionários que receberam a primeira dose da vacina fez-se a coleta de sangue, para determinação da prevalência do anti-HBc, como niarcador da infecção pregressa pelo VHB. Novas amostras foram colhidas no momento da aplicação das segundas e terceiras doses e seis meses após esta última para a pesquisa do anti HBs. Os profissionais que completaram todo este acompanhamento e permaneceram na instituição foram convocados, depois de cinco anos, para nova titulação do anti-HBs e pesquisa do anti-HBc. Nesta ocasião, aos que exibiram títulos de anti-HBs < 10 mUI/ml, após terem apresentado soro conversão com o esquema primário, foi administrada uma dose de reforço da vacina (20 µ g), via 1M. Após duas a quatro semanas, nova titulação do anti-HBs foi realizada. Compareceram para a primeira dose da vacina 1117 funcionários, sendo que 463 (41,5%) receberam a vacina via ID e, entre os 654 (58,5%) da via 1M, 228 foram alocados para constituírem o subgrupo de acompanhamento da soroconversão. A prevalência do anti-HBc entre os 1117 P AS foi de 10,3%. Determinadas categorias profissionais, ou especialidades, apresentaram prevalências mais altas, como laboratoristas anestesistas, anatomopatologistas e radiologistas. Observamos também maiores prevalências do anti-HBc nos indivíduos com idade superior a 35 anos e naqueles que atuam na área da saúde há mais de dez anos. Entre os 115 PAS reagentes para o anti-HBc, dois deles apresentaram HBsAg reagente e os demais eram positivos para o anti-HBs, revelando uma prevalência de portadores do VHB de 0,2%. A adesão entre a primeira e segunda doses foi de 81,8% e entre a segunda e terceira doses de 79,4%. No grupo intradérmico, entretanto, o absenteísmo foi de 29,3%. Em 77,4% dos vacinados contra o VHB, a aplicação foi segura e bem tolerada, não causando reações adversas. As reações foram mais freqüentes no grupo ID que no grupo 1M (31,4 e 15,7%; p=0,000). No grupo ID, predominaram as reações locais (hiperemia, prurido e edema no local de aplicação) e no grupo 1M, as reações sistêmicas (sonolência, mialgia, náusea)~ Houve um caso de reação anafilática após a aplicação via intradérmica da segunda dose da vacina. Após três doses da vacina, a soroconversão para o anti-HBs foi de 92,2% no grupo 1M e de 59,9% no grupo ID. No grupo 1M, obtivemos títulos de anti-HBs < 10 mUI/ml em 24,5%, títulos entre 10 e 100 mUI/ml em 13,7% e títulos acima de 100 mUI/ml em 61,8%. No grupo ID, estes títulos foram observados em 46,4%, 34,3% e 19,3%, respectivamente. Idade inferior a trinta anos e pertencer ao sexo feminino foram fatores associados a uma adequada resposta vacinaI. Após cinco anos do esquema vacinal, observamos títulos de anti-HBs >= 10 mUI/ml em 73,7 e 43,2% dos servidores dos grupos 1M e ID, respectivamente. Ao administrarmos uma dose de reforço em 36 funcionários com anti-HBs < 10 mUI/ml, 88,9% apresentaram títulos de anti-HBs protetores e 83,3% exibiram títulos superiores a 150 fi UI/mI / Abstract: Health care workers (HCW) present HBV risk of infection three to five times higher than the population in general. However, high vaccine cost is an obstacle to assure fulI vaccinal coverage. Several studies have been made in order to try to make the vaccinal scheme more economical but effective. Intradermic HBV vaccine reduces seven to ten times the cost of vaccination and in certain population groups its effectiveness is similar to intramuscular application. In order to find altemate and more economic schemes for vaccination against HBV in HCW, we have selected a group of health care workers at Unicamp to receive the HBV vaccine. These workers have been divided into two groups, according to the application route: intradermic (ID) or intramuscular (1M). All the workers who received ID application and part of those who received the vaccine via 1M were followed up for evaluation of the immunologic response to the vaccine through both quantitative and qualitative anti-HBs investigation. The proposed vaccination scheme was of three doses, at O, 1 and 6 months intervals. The vaccine used was the Belgium recombinant vaccine (Engerix B, SmithKline), with 20 µ g (1 ml) dose for 1M application and 0.2 µ g (0.1 ml) dose for ID application (ventral side of the left forearm). AlI the workers receiving the first vaccine dose were submitted to a blood test to determine the prevalence of anti-HBc as a marker of the previous HBV infection. New blood samples were collected at the time the 2nd and 3rd doses were applied and 6 months after the last one for anti-HBs survey. The workers who completed the program and who were stilI with the university, were called after 5 years for a new title of anti-HBs and survey on anti-HBc. At that time, the subjects who had anti-HBs titles < 10 mUI/ml after having shown seroconversion with the first scheme were administered a booster dose of the vaccine (20 µ g), via 1M. After two to four weeks a new anti-HBs title was made. Of the 1,117 subjects receiving the first dose, 463 (41.5%) received the vaccine via ID and from the remaining 654 (58.5%) receiving the vaccine via 1M, 228 were further divided into a seroconversion follow-up subgroup. The prevalence ofthe Anti-HBs among the 1,117 HCW was of 10.3%. Certain professional categories or specialties have shown higher prevalences, such as laboratorians (14.4%), surgeons (16.2%) and orthopedists (16.7%). There were no anti-HBc reagent cases among anesthetists, anatomopathologists and radiologists. We have also noted higher prevalence of anti-HBc on subjects over 35 years old and on those who have been working in the health care area for more than 10 years. Among the 115 anti-HBc reagent HCW, 2 have shown to be HBsAg reagent and the remaining were positive for anti-HBs, indicating a prevalence ofHBV ofO.2%. Attendance between the first and the second doses was of 81.8% and between the second and third 79.4%. ln the intradermic group, however, absenteism was of 29.3%. VBH vaccination was safe and well tolerated, with no adverse reactions in 77.4% of the subjects. Reactions were more frequent in the ID group than in the 1M group (31.4%, 15.7%; p=0,000). ln the ID group, local reactions prevailed (hyperemia, pruritus and edema) and in the 1M group, systemic reactions (drowsiness, myalgia, nausea). There was one case of anaphylactic reaction after intradermic application of the second dose of the vaccine. After three doses of the vaccine, the seroconversion for anti-HBs was of 92.2% in the 1M group and 59.9% in the ID group. lu the 1M group, we have achieved anti-HBs < 10 mUI/ml titles in 24.5%, titles between 10 and 100 mUI/ml in 13.7% and titles above 100 mUI/ml in 61.8%. ln the ID group, these titles were found in 46.4%; 34.3% and 19.3%, respectively. Age below 30 years and being female were the factors associated with an adequate vaccinal response. Five years after the vaccinal scheme, we found titles of anti-HBs >= 10 mUI/ml in 73.7% and 43.2% ofsubjects from groups 1M and ID, respectively. A combinação entre o interferon peguilado e a ribavirina tem sido considerado o padrão-ouro de tratamento para Hepatite C. A resposta ao tratamento vem sendo associada a fatores ambientais, do vírus e também do paciente, tais como polimorfismos genéticos dos antígenos leucocitários humanos (HLA), da interleucina-10 e do fator de necrose tumoral-a. Plaquetas possuem em suas membranas glicoproteínas que expressam segmentos protéicos polimórficos, os quais são chamados de antígenos plaquetários humanos (HPA). Os sistemas HPA-1, -3, -4 e -5 residem em integrinas, proteínas que possuem interações com interferon. O objetivo desse estudo foi avaliar a associação entre freqüência dos HPA-1, -3, -4 e -5 e a resposta ao tratamento, em 138 pacientes tratados para Hepatite C. A genotipagem dos HPA-1, -3 e -4 foi realizada pela técnica de PCR-SSP e do HPA-5 pela PCR-RFLP. A genotipagem do HCV foi realizada através do Kit comercial INNO-LiPA® v.1.0 (Innogenetics, Ghent, Belgium), segundo as instruções do fabricante. Os pacientes foram divididos em grupos e subgrupos de acordo com o esquema terapêutico, a resposta ao tratamento e o genótipo do HCV. Os pacientes que possuíam o genótipo do HCV não-1 e que foram tratados com IFN-a+RBV, com falha terapêutica, apresentaram uma diferença estatística significante (p<0.05) nas freqüências alélicas e genotípicas do sistema HPA-3, com aumento do alelo 3b. O sistema HPA-3 está localizado em uma integrina que se liga a fibronectina, um receptor de interferon. Nesse contexto, a alteração conformacional glicoprotéica decorrente da presença do alelo HPA-3b, poderia estar associada à falha ao tratamento com IFN-a+RBV em pacientes portadores de genótipo viral não-1. / Hepatic fibrosis leading cirrhosis in 20 to 30% of patients with chronic hepatitis C virus (HCV) infection. Rapid progression to fibrosis has been related to environmental, viral and host factors. However, genetic polymorphisms have recently been associated with this progression, including the expression of integrins. Platelet membrane glycoproteins express several polymorphic antigenic determinants on their surface, which are called human platelet antigens (HPA). HPA-1, -3, -4 and -5 reside in integrins. The association between HPA antigens and stage of fibrosis can determine if HPA is related to progression of fibrosis. Thus, the goal of this study was to determine the association between the HPA-1, -3, -4 and -5 and the liver fibrosis stage in 175 HCV-infected patients. HPA-1, -3 and -4 genotyping was performed by PCR-SSP and, HPA-5 by PCR-RFLP. Fibrosis progression was evaluated using the METAVIR scoring system. There were no significant differences (p>0.05) in allelic and genotypic frequency distribution of HPA-1, -3 and -5, residing in integrins.

É uma doença com importante morbidade e letalidade nos locais de ocorrência, podendo ser prevenível através da vacinação contra o vírus da Hepatite B (VHB). Os objetivos desse trabalho foram estudar clínica e epidemiologicamente os pacientes com essa infecção pelo VHD. Comparando as características clínicas e epidemiológicas das infecções pelo VHB e VHD e a monoinfecção pelo VHB na população estudada. O estudo em 2014 foi realizado na área urbana e rural do município de Sena Madureira – Acre, e foram entrevistados 310 moradores dessa localidade. Os voluntários assinaram o termo de consentimento livre e esclarecido, e responderam a um questionário e, posteriormente foram coletados as amostras de sangue para os resultados positivos de confirmação de Hepatite B e Delta, e consequentemente avaliação dos casos positivos com exame de carga viral, tanto para o VHB e VHD, após a assinatura do termo de consentimento livre esclarecido pós informado. O estudo permitiu concluir que, a prevalência de infecção pelo VHD, estimada através de biologia molecular é elevada (22,9%) nos que apresentam infecção prévia pelo VHB, na área do interior do estado do Acre. O resultado comprova a suspeição de uma elevada prevalência da Hepatite Delta, entre os moradores residentes na região que deu origem a pesquisa e a transmissão do VHD na região ocorre provavelmente na infância, por transmissão vertical e intrafamiliar, também associado a hábitos regionais. Os dados chamam a atenção por serem VHB e VHD reconhecidamente hiperendêmicos na região Amazônica, sendo o VHD especialmente prevalente em sua porção mais ocidental. Naqueles pacientes que foram avaliados, não foi possível estabelecer um valor de corte ideal de carga viral para discriminar o desenvolvimento de cirrose. Estudos apontam associação entre infecção crônica e certos genótipos de VHB, o que pode modificar a resposta ao tratamento. Se comparado com resultados obtidos de outros trabalhos já apresentados na literatura, mostram que o município de Sena Madureira - AC, apresenta importante prevalência do vírus da Hepatite Delta naqueles pacientes infectados pelo VHB. / Endemic to the tropical regions of South America, regions of Europe and Asia, Hepatitis Delta is an infectious disease caused by the hepatitis D virus (HDV). It is a disease with significant morbidity and mortality of its occurrence of localized, and which can be preventable through vaccination against hepatitis B virus (HBV). The objectives of this work was to study clinical and epidemiologically patients with this HDV infection. Comparing the clinical and epidemiological characteristics of infections with HBV and HDV and monoinfection HBV in this population. The study in 2014 was conducted in urban and rural area of the municipality of Sena Madureira - Acre, in which 310 residents were interviewed in this localition. The volunteers answered a questionnaire and blood samples were collected for examination detection of Hepatitis B and Delta, and consequently evaluation of the positive cases with viral load test for both HBV and HDV, after signing the consent form savvy post informed. The study concluded that the prevalence of HDV infection, estimated through molecular biology is high (22.9%) in those with a previous HBV infection in the area in the state of Acre. The result confirms the suspicion of a high prevalence of hepatitis Delta among residents living in the area that gave rise to research and the transmission of HDV in the region is probably in childhood, vertical and intra-family transmission, also associated with regional habits. The data are remarkable for being recognized hyperendemic HBV and HDV in the Amazon region, and the VHD especially prevalent in its western portion. In those patients who were evaluated, it was not possible to establish an optimal cut/off value of viral load to discriminate the development of cirrhosis. Studies suggest an association between chronic infection and certain genotypes of HBV, which can modify the treatment response. Compared with results from other studies already presented in the literature, show that the municipality of Sena Madureira - AC, has important prevalence of Hepatitis Delta virus in patients infected with HBV.

Orientador: Jussara Marcondes-Machado / Resumo: A hepatite viral do tipo B constitui um dos mais importantes problemas de saúde pública em todos os continentes. Estima-se que 300 milhões de indivíduos, em todo o mundo, sejam portadores crônicos do vírus e que 2 milhões morram anualmente dessa doença. O VHB se transmite por via parenteral e sobretudo por via sexual. O risco de infecção, que é de 5% na população geral, e pode ser de 15% a 20% em trabalhadores da área da saúde. Entre esses últimos, os que mantêm contato freqüente com sangue de pacientes apresentam risco duas a quatro vezes maior de adquirir o VHB. Sendo assim, atualmente, a hepatite B é considerada uma das mais prevalentes infecções ocupacionais contraídas no ambiente hospitalar. A medida mais eficaz no combate à infecção é a imunização ativa dos indivíduos suscetíveis, por meio de vacinas atualmente disponíveis, altamente eficientes e seguras. A soroconversão ocorre em proporção inversa à idade do imunizado, podendo a chegar a 90% nos adultos jovens. Os objetivos deste estudo foram: 1. avaliar a presença de infecção natural pelo VHB, anterior à vacinação, medida pelo anticorpo contra o AgHBc (anti-HBc) e sua relação com as condições epidemiológicas gerais, de vida pessoal e profissional e de risco de infecção pelo VHB; 2. os níveis de anticorpo contra o AgHBs (anti-HBs) e sua relação com as condições epidemiológicas gerais, de vida pessoal e profissional e de exposição ao risco de infecção pelo VHB. It is estimated that 300 million people are chronic carriers with 2 million deaths annually. HBV is transmitted by both parental and sexual routes. Infection risk, which is 5% in the general population, can be 15 - 20% in healthcare workers as they are in frequent contact with patient blood. Today Hepatitis B is considered one of the major occupational infections in hospitals. The most effective measure against infection is active immunization of susceptible individuals with available vaccines that are highly efficient and safe. Seroconversion occurs at an inverse proportion to immunized persons age, reaching up to 90% in young adults. The objectives of this study were to evaluate: 1.
