Exploring nurses' implementation of postnatal care to HIV positive mothers in the Free State

Postnatal care provides the opportunity for protecting the health of women and their babies by optimising human immunodeficiency virus (HIV) treatment and management. This study intended to explore the implementation of postnatal care to HIV positive women by explicating nurses’ views regarding their practices with the aim of improving the programme. The study was conducted at three clinics in Mangaung. An evaluative case study design was used to provide a detailed account on implementation of postnatal care to HIV positive clients. The purposive non-probability sampling was used. Semi-structured methods using focus group interviews were used to collect data. Data were reviewed through thematic analysis. The study found that nurses understood guidelines, used them to direct their practice and challenges were highlighted. It was concluded that the system limitations needed to be addressed, skills of health care providers improved and linkages with community based services strengthened to improve effectiveness of care. / Health Studies / M.PH.

Adolescent sexual health has been identified as a significant health and development problem facing South Africa. Limited amounts of research on sexual interactions have been undertaken, with information on adolescents’ romantic relationships being particularly scarce. Qualitative research needs to foster an understanding of the dynamics of sexual interactions in specific settings, and with emphasis in the past on cognitive health psychology models, very little is thus known about how adolescents negotiate and make sense of their sexual experiences. This highlights the need to investigate the complexities of human sexuality in a contextual manner. In response, this study explores the lived experiences of four young black women as they negotiate their agency and sexuality in a local context. By way of in-depth qualitative interviews, which were analysed for recurrent themes using interpretative phenomenological analysis, this project examines the participants’ experiences regarding sex, relationships, communication, sexual health care, as well as HIV and pregnancy prevention. The results reveal that communication about sexuality in the participants’ homes was limited if not absent altogether. When seeking sexual health care, they found clinic nurses to be judgemental and rude. Regarding sexuality and HIV education, the participants stressed the need for outside educators to teach in more practical ways to increase efficacy. In their dating relationships, most participants revealed their boyfriends had a great deal of influence over their sexual initiation. Unwanted pregnancy surfaced as a greater fear than HIV in their accounts due to pressure to finish their education and attain well-paying jobs in the future. The participants felt unable to stop their boyfriends’ infidelity and had limited agency when facing sexual demands. Their accounts revealed that they negotiate their agency in an atmosphere of coercion and the threat of rape. However, areas of agency included their consistent condom use even when facing pressure to have unprotected sex, and their active accessing of sexual health services for hormonal contraception. These insights serve to better inform sexual and reproductive health education and intervention programmes for young women. Moreover, educators, researchers and programme developers alike may gain useful insights from the personalised accounts derived from this study.

Evalution of the knowledge and skills of the professional nurses regarding IMCI service delivery, including HIV/AIDS case management in primary health care facilities in Buffalo City Sub-District, Eastern Cape Province, South Africa

An increase in diarrhoea and upper respiratory tract infection has been noted for the past five years and this is associated with the prevalence of Human Immune-deficiency Virus and Acquired Immune Deficiency Syndrome. HIV infection is an increasingly common cause of childhood morbidity and mortality in South Africa. In some areas of the country, like the Eastern Cape, more than 30 percent of the women attending antenatal clinics are HIV infected. Without intervention, approximately one third of the babies born to these HIV infected mothers will get infection from their mothers (WHO, 2003). Aim and Objectives of the study: The study was meant to assess the specific knowledge and skills of professional nurses at the primary health facilities regarding management and integration of childhood illnesses, evaluate the quality of training of professional nurses on aspects of IMCI including integration of HIV within IMCI programme for children under five years in Primary Health Care facilities in Mdantsane Township. Methods: A quantitative descriptive method was used for this study. The questionnaire developed by the researcher was used for data collection. The items on the questionnaire were divided into four (4) subsections. The questionnaire was administered to all professional nurses on duty and those available at the time of data collection. A checklist for IMCI was used to collect data from Road to Health files of children who were HIV positive admitted in the pediatric ward at Cecilia Makhiwane Hospital. Results: The results of this study for all the variables examined showed that the highest score was 56 percent and the lowest was18 percent.These results indicated that the knowledge and skills of professional nurses were inadequate.

Factors contributing to low HIV Testing Services (HTS) uptake among Health Sciences students at the selected University in Limpopo Province

Despite undeniable benefits and increased availability of HIV Testing Services, uptake remains low among students. The aim of this study was to investigate factors contributing to low HIV Testing Services (HTS) among Health Sciences students. Quantitative research method was used to conduct this study. Descriptive quantitative research design was employed. 306 respondents determined by the sample formula were selected through Probability, Systematic sampling technique. Structured questionnaires were used to collect data from respondents. Validity was ensured through content and face validity. Reliability in the form of test-retest reliability were ensured through pre-testing the instrument using 31 respondents from the target population. Data collected was analysed using SPSS version 24. Ethical considerations were ensured throughout the study. The findings of the study showed that out of 306 respondents, 44,1% (n=135) of respondents, comprising 30,7% (n=94) of females and 13,4% (n=41) of males had gone for HIV testing. Respondents had adequate knowledge about HIV Testing Services (HTS) and their attitude towards HIV Testing Services (HTS) was favourable. HIV/AIDS-related stigma, fear of potential HIV positive diagnosis, low perception of HIV infection risk and negative attitude of healthcare service providers were attributed to low uptake of HIV Testing Services (HTS) among respondents. There were recommendations for HIV Testing Services (HTS) workshops, education and awareness campaigns emphasizing the benefits and importance of HIV Testing Services (HTS). The use of various forms of media including campus radio and newsletters to promote the uptake of HIV Testing Services (HTS) among university students was also recommended. / NRF

Strategies to facilitate the implementation of existing life orientation curriculum in secondary schools of Limpopo Province, South Africa

PhDPH / Department of Public Health / There is a growing concern over the rising cases of adolescent pregnancy, drug and substance abuse, poor academic performance, violence, high school dropout rate and HIV/AIDS prevalence among secondary school students. The grounds of these problems are considered to be related to inadequate Life Orientation Education which should equip the learners with psychosocial competencies, but the ability to make informed decision, solve problems, think creatively and critically, communicate effectively, build healthy interpersonal relationships is lacking among the youths (Dash, 2018). The purpose of the study was to develop strategies to facilitate the implementation of existing Life orientation curriculum in secondary schools of Limpopo province, South Africa. An exploratory sequential design was used in this study. This research design had three stages. In the first stage (1a) the researcher collected and analysed quantitative data. Based on the quantitative results, the researcher will then engage a second stage (1b), the qualitative phase, to test and make the general view of the initial findings. This was followed by phase 3, which was the development of the coping strategies and validation of the strategies as outlined. Purposive sampling was employed to select two districts and then four circuits. Phase 1a identified factors influencing implementation of existing Life Orientation curriculum. Eight schools were selected using stratified random sampling. Simple random sampling was used to select 521 grade 10, 11 and 12 adolescent girls. Data was collected through a self-designed and self-administered questionnaire. Data was analysed descriptively using statistical software Stata/IC version 15.0. of the computer program. Validity and reliability were ensured. Phase 1b identified the predisposing, enabling, and reinforcing factors that can affect the behaviours, attitudes, and environmental factors contributing to the implementation of Life skills programmes in schools. A qualitative approach, and explorative, descriptive and contextual designs were applied. The population were all guardians and school teachers. Purposive sampling was used to select guardians for the focus groups discussions and teachers for in-depth interviews. Data analysis employed Tesch open coding method. Phase 2 was the development strategies to facilitate the implementation of existing LO programme in Limpopo province. Results from Structured Interviews, Focused group, Questionnaires were fused together with the use of the Precede Proceed Model. Phase 3 was to validate the developed strategies. The aim of validation was to collect and evaluate data, from the process design stage, the consistency and quality of the product or outcome of the strategies. The objectives to validate the strategies were to: determine the credibility of the strategies. The researcher used the non–experimental, intervention validation design to validated the credibility of the developed strategies. The developed strategies were given to the learners, guardians and Life Orientation teachers Vhembe and Mopani district. To collect data, the researcher used a checklist with 6 questions as outlined by Chin and Kramer (refer to table 6.4). Simple descriptive statistics was use where the data was summarized using the frequency distribution. / NRF

An evaluation of the benefits of life skills training as a preventive strategy for HIV and AIDS for secondary school learners (Grade 9) in the Moretele district of Mpumalanga

This study aimed to evaluate the benefits of life skills training as a preventive strategy for HIV and AIDS, for secondary school learners in one secondary school in the Moretele District of Mpumalanga. The objectives of this study were to determine the levels of learners’ knowledge about HIV and AIDS, and levels of teacher training in life skills, to assess how teachers’ and learners’ attitudes affect life skills training, to explore the perceptions of teachers and learners regarding the usefulness of life skills training in the prevention of HIV transmission and to assess the usefulness of the learning and teaching support materials (LTSM) used in life skills training. The study also investigated the challenges encountered by learners and teachers in life skills training in order to make recommendations for improvements. This is a qualitative evaluation study that involved qualitative focus group interviews, qualitative semi-structured interviews and qualitative observation. Sampling was purposive and it entailed 30 grade 9 learners and 2 grade 9 Life Orientation teachers. The Health Belief Model and Social Cognitive Theory formed the theoretical framework for this study. The results of the study showed that the aspects which significantly benefited learners were: sufficiently acceptable levels of knowledge about HIV and AIDS, the use of other resources from the library, and development of positive attitudes of learners and teachers involved in life skills. Aspects of life skills training which demonstrated partial benefits for the learners were teacher training and involvement of parents and other health care professionals. However, aspects which were less developed were the learners’ and teachers’ knowledge and application regarding some skills needed for prevention of HIV infection / Social Work / M.A. (Social Behaviour Studies in HIV-AIDS)

However, the benefits of this therapy are limited by the difficulty of adherence to the treatment. This study sought to explore the role of professional nurses on anti-retroviral therapy adherence among children in Lejweleputswa district: Free State, South Africa. An exploratory descriptive qualitative research design was used to identify and describe role of professional nurses toward anti-retroviral therapy adherence among children. Population for this study included seventeen (17) professional nurses working in four purposively sampled Primary Health Care clinics invited to participate in the study. Four focus group discussions were conducted in which each group had 6 participants. The transcribed data was analysed using the framework approach of data analysis. Professional nurses in Lejweleputswa district report poor knowledge of parents/caregivers of children, perceived poverty, stigma and discrimination, inappropriate care approaches, and parental dynamics as factors influencing poor ART adherence. Recommendations for enhancing children ART adherence levels in Lejweleputswa district included: mainstreaming adherence counselling in children ART and adopting a comprehensive family centered care approach were identified as measures for improving children ART adherence. Other measures included integration of ART services into Primary Health Care (PHC) services, parental empowerment, development of a programme to reduce stigma and discrimination in the community.
