It is characterized by chronic hyperglycemia that in a long term can cause retinopathy, nephropathy, peripheral and autonomic neuropathy. The present interventional, prospective and open study was designed to evaluate therapeutic efficacy of sitagliptin in metabolic control of diabetes mellitus type 2 recently diagnosed (less than 6 months) and in pathways of visual and somatosensory nerve conduction. Blood pressure and anthropometric data were colected before treatment. Measures of fasting triglycerides, total cholesterol, high density lipoprotein cholesterol gamma glutamyl transpeptidase, alanine aminotransferase, aspartate aminotransferase, microalbumin, nitric oxide, glucose, glycated hemoglobin (A1C), C-peptide, insulin and glucagon were also collected. Also during the pre-study, after a feeding stimulus of 566 Kcal, biochemical evaluations were conducted of the metabolism in fasting and during 3 hours, in standard intervals of 5, 15, 30, 45, 60, 90, 120, 150 and 180 minutes for glucose, insulin and glucagon for design of the area under curve (AUC) of these analytes. In a subgroup of 20 patients were performed dosages in pre-fixed times of incretins (active Glucagon Like Peptide -1, GLP-1, and total Glucose-dependent Insulinotropic polypeptide, GIP) after feeding stimulus and electroneurophysiological tests (somatosensory evoked potentials - SEP, and visual - VEP). After these evaluations, the patients received sitagliptin 100 mg (1 tablet a day) for 3 months and then returned for clinical evaluation and to repeat all the biochemical dosages, SEP and VEP exams. From 41 patients evaluated, 28 were female and 13 male, the average age was 53.2 ± 9.43 years, in average obese and 50% were hypertensive. After treatment, improvement was observed in all anthropometric and laboratory parameters, specially fasting glucose (pre: 174,43 ± 67,18 mg/dL and post: 129,07 ± 29,19 mg/dL), A1c (pre: 8,76 ± 2,68 and post: 6,64 ± 1,11%), and nitric oxide (pre: 85,21 ± 44,02 and post: 39,91 ± 20,99 µM), all with P<0.0001, and also increase in the concentration of the AUC of active GLP-1 (P=0.003), decrease of the AUC of total GIP (P=0.001), and decrease of the N9D (P<0.0001), N13E (P=0.036 ), N20D (P=0.028) latencies and of the central conduction time N13-N9E (P=0.045), N20-N13D (P=0.044). There were also statistically significant correlations between biochemical and electrophysiological parameters. This study concluded that there was clinical, metabolic and somatosensory and visual nerve impulse conduction pathways improvement in patients treated with sitagliptin. / Diabetes mellitus (DM) é uma síndrome associada a secreção de insulina deficiente, resistência à ação da insulina ou uma associação de ambas. Caracteriza-se por hiperglicemia crônica que a longo prazo poderá causar retinopatia, nefropatia, neuropatia periférica e autonômica. Este estudo do tipo intervencionista, prospectivo e aberto teve como objetivo avaliar a eficácia terapêutica da sitagliptina no controle metabólico do diabetes mellitus do tipo 2 recentemente diagnosticado (menos de seis meses) e nas vias de condução nervosa somato-sensitiva e visual. Antes de iniciar o tratamento, foram avaliados os índices antropométricos e a pressão arterial. Também foram realizadas as dosagens em jejum de triglicerídeos, colesterol total, colesterol de alta densidade (HDL), gama glutamil transferase, aspartato amino transferase, alanina amino transferase, proteína C reativa, microalbuminúria, óxido nítrico, glicose, hemoglobina glicada (A1C), peptídeo C, insulina e glucagon. Após um estímulo alimentar de 566 Kcal foram feitas coletas de sangue seriadas durante 3 horas nos tempos de 5, 15, 30, 45, 60, 90, 120, 150 e 180 minutos para a dosagem de glicose, insulina e glucagon com a construção da área sob a curva (AUC) destes analitos. Um subgrupo de 20 pacientes realizou ainda, análises seriadas das incretinas (GLP-1 ativo e GIP total) após estímulo alimentar e, também, exames eletroneurofisiológicos (potenciais evocados somato-sensitivos – PESS, e visuais - PEV). Após estas avaliações, os pacientes receberam sitagliptina 100 mg (1 comprimido/dia) por um período de 3 meses, após o qual, eles retornaram para avaliação clínica e para realizar todas as dosagens bioquímicas e os exames de PESS e PEV que haviam realizado no início do estudo. Dos 41 pacientes avaliados, 28 eram do sexo feminino e 13 do sexo masculino, idade média de 53,2 ± 9,43 anos, em média obesos e 50% deles eram hipertensos. Após o tratamento, observou-se a melhora dos índices antropométricos, de todos os parâmetros laboratoriais com ênfase na glicose em jejum (pré: 174,43 ± 67,18 e pós: 129,07 ± 29,19 mg/dL), A1C (pré: 8,76 ± 2,68 e pós: 6,64 ± 1,11%) e óxido nítrico (pré: 85,21 ± 44,02 e pós: 39,91 ± 20,99 µM), todos com P<0,0001 e ainda, aumento da concentração da AUC de GLP-1 ativo (P=0,003) e diminuição da AUC de GIP total (P=0,001), bem como diminuição das latências N9D (P<0,0001), N13E (P=0,036), N20D (P=0,028) e dos tempos de condução central N13-N9E (P=0,045), N20-N13D (P=0,044), além de correlações com significância estatística entre parâmetros bioquímicos e eletroneurofisiológicos. Este estudo concluiu que houve melhora clínica, metabólica e nas vias de condução do impulso nervoso somato-sensitivo e visual dos pacientes tratados com sitagliptina.

Métodos: Um grupo de pacientes com não-remissão do DM2 (NR) foi formado (n=13) baseado na revisão de prontuários de pacientes obesos mórbidos diagnosticados com DM2 submetidos ao RYGB. Um grupo remissão (R) foi composto por pacientes que tiveram normalização da glicemia e da A1c, sem drogas antidiabéticas e pareados para as mesmas características (i.e., duração da doença, IMC prévio e final, distribuição de gordura, e idade; n=15). Um grupo controle foi formado por pacientes magros (n=41). Resultados: O grupo NR apresentou maiores níveis de ácido úrico (5.1 vs. 3.9 mg/dL), número de leucócitos (6866.9 vs. 5423.6), PCR-us (0.27 vs. 0.12 mg/dL), MCP-1 (118.4 vs. 64.4 ng/mL), HOMA-IR e AUCglicose, mas níveis menores de adiponectina (9.4 vs. 15.4 ng/mL), leptina (12.7 vs. 20.7 ng/mL) e AUCGLP-1 em comparação com o grupo R; o grupo NR também teve menor leptina e maior adiponectina, HOMA-IR, AUCglicose, AUCpeptídeo-C, AUCglucagon e AUCGLP-1 que o grupo controle. O grupo R apresentou menor nível de MCP-1 e maior de adiponectina em relação ao grupo controle. A sensibilidade à insulina foi significantemente menor no grupo NR que nos grupos R e controle. Os índices de secreção de insulina foram menores no grupo NR que nos grupos R e controle. Conclusão: Este estudo demonstrou que pacientes sem remissão do DM2 após RYGB apresentam maior resistência à insulina, menor secreção de insulina, adiposopatia e inflamação subclínica crônica persistentes apesar de perda de peso semelhante. Mecanismos fisiopatológicos persistentemente alterados podem explicar a não remissão do DM2 após RYGB / Abstract: Objective: The aim of this study was to evaluate the pathophysiological mechanisms underlying the remission of type 2 diabetes in Roux-en-Y gastric bypass (RYGB) patients. Research Design and Methods: A group of patients not in remission (NR) was formed (n=13). A remission group (R) was composed of patients who had undergone normalization of fasting glycemia and A1c, without anti-diabetic drugs and matched for selected baseline characteristics (i.e., duration of disease, previous BMI, final BMI, fat distribution, and age; n=15). A control group of lean subjects (n=41) was formed. Results: The NR group had higher uric acid (5.1 vs. 3.9 mg/dL), number of leukocytes (6866.9 vs. 5423.6), hs-CRP (0.27 vs. 0.12 mg/dL), MCP-1 (118.4 vs. 64.4 ng/mL), HOMA-IR and AUCglucose but lower adiponectin (9.4 vs. 15.4 ng/mL), leptin (12.7 vs. 20.7 ng/mL) and AUCGLP-1 in comparison to R group; NR group also had lower leptin and higher adiponectin, HOMA-IR, AUCglucose, AUCC-peptide, AUCglucagon and AUCGLP-1 than controls. The R group had lower MCP-1 and higher adiponectin compared to controls. Insulin sensitivity was significantly lower in the NR group than in the R and control groups.The insulin secretion index values were lower in the NR group than in the R and control groups. Conclusions: This study found greater insulin resistance, lower insulin secretion, persistent adiposopathy and chronic subclinical inflammation and less robust incretin response in NR group despite a similar level of weight loss. Efectos de la incretina glp-1 sobre el acoplamiento retículo endoplásmico-mitocondria en células de músculo liso vascular : rol de pka y mitofusina-2

Magíster en Bioquímica, área de especialización Bioquímica de Proteínas Recombinantes / Memoria de Título de Bioquímico / La hormona GLP-1 es una reguladora importante de la homeostasis de la glucosa, favoreciendo su metabolismo en varios tejidos a través de la activación de la vía del AMP cíclico-proteína kinasa A. En las células de músculo liso vascular (VSMC), el control del metabolismo de la glucosa es esencial para la mantención de la función y el fenotipo celular, ya que al favorecer su oxidación en las mitocondrias, se previene la aparición de un fenotipo desdiferenciado, característico de patologías cardiovasculares. Un mecanismo que promueve la actividad mitocondrial es su acoplamiento con el retículo endoplásmico (RE), pues se favorece el traspaso de metabolitos y Ca2+ desde el RE a la mitocondria. Estructuralmente, el acoplamiento entre ambos organelos depende de Mitofusina-2 (Mfn-2). Existe evidencia de que una disminución en el acoplamiento entre RE y mitocondrias en VSMC conduce a la aparición de un fenotipo proliferativo, mientras que el uso de GLP-1 previene el desarrollo de este fenotipo. Además, datos de nuestro grupo de trabajo sugieren que esta hormona potencia la actividad mitocondrial. Basados en estos antecedentes, quisimos estudiar si GLP-1 era capaz de promover el acoplamiento entre RE y mitocondrias en la línea celular vascular A7r5, y evaluar el posible rol de PKA y de Mfn-2 en el fenómeno. A través de inmunofluorescencia indirecta con marcación de RE y mitocondrias, se observó un aumento en la colocalización entre ambos organelos en células estimuladas con GLP-1 (100 nM, 3 h). Además, mediante el uso de la sonda fluorescente Rhod-FF, sensible a Ca2+ mitocondrial, se determinó que la pre-incubación de estas células con GLP-1 favorecía la entrada de Ca2+ reticular a la mitocondria. En forma paralela, se determinó que el tratamiento por 3 h con GLP-1 aumentó los niveles de Mfn-2, efecto que se perdió cuando las células se pre-incubaron con el inhibidor de PKA, H-89. Finalmente, se determinó que el incremento en la colocalización entre RE y mitocondrias y el aumento en la entrada de Ca2+ reticular a las mitocondrias en respuesta al tratamiento con GLP-1, se inhibían en células pre-tratadas con H-89. Así, concluimos que la incretina GLP-1 promueve el acoplamiento funcional entre RE y mitocondrias en células de la musculatura lisa vascular, a través de un mecanismo que requiere de la activación de PKA, y presumiblemente, del aumento de la proteína Mfn-2. / The hormone GLP-1 is an important regulator of glucose homeostasis that promotes its metabolism on several tissues through a cyclic AMP-protein kinase A pathway. In vascular smooth muscle cells (VSMC) glucose metabolism is involved in the control of both cellular function and phenotype, given that promoting its oxidation in the mitochondria prevents the appearance of VSMC undifferentiated phenotype, a hallmark of cardiovascular pathologies. Mitochondrial activity is activated by their coupling with the endoplasmic reticulum (ER), which enhances metabolite and Ca2+ transfer from the ER to the mitochondria. Structurally, the coupling between these organelles depends on Mitofusin-2 (Mfn-2). A decrease on ER-mitochondria coupling in VSMC leads to a proliferative phenotype, while GLP-1 treatment prevents its appearance. Besides, data from our work group suggests that this hormone enhances mitochondrial activity. Based on this background, we evaluated whether GLP-1 modulated functional coupling between ER and mitochondria in the vascular cell line A7r5, and the involvement of PKA and Mfn-2 on this phenomenon. Inmunofluorescence analysis of ER and mitochondria stained cells revealed that GLP-1 (3 h, 100 nM) treatment increased colocalization of these two organelles. Furthermore, by using the mitochondrial Ca2+ sensitive fluorescence probe, Rhod-FF, we determined that pre-incubation of these cells with GLP-1 enhanced reticular Ca2+ entry into the mitochondria. Moreover, the treatment with GLP-1 by 3 h increased Mfn-2 levels, an effect that was prevented by the pre-incubation with the PKA inhibitor, H-89. Finally, the increment of ER and mitochondria colocalization and the increase on reticular Ca2+ entry into the mitochondria in response to GLP-1 stimulation were both abolished in H-89 pre-treated cells. Therefore, we concluded that the hormone GLP-1 promotes ER and mitochondria coupling in VSMC, through a mechanism that requires PKA activation, and presumably, an increment of Mfn-2 levels. / Fondecyt

Regulación de la dinámica y metabolismo mitocondrial por incretina GLP-1 en células de la línea A7r5

Su acción clásica es regular la respuesta secretoria de insulina por el páncreas. Sin embargo, recientemente se ha descrito que GLP-1 posee acciones extrapancreáticas muy similares a la insulina, con mecanismos de señalización tejido-específicos. Por sus muchas acciones fisiológicas, y la mayoría relacionadas con los efectos insulinotrópicos de esta incretina, derivados peptidomiméticos de GLP-1 se utilizan actualmente para controlar la hiperglicemia en individuos con diabetes mellitus tipo 2. Debido a esto se hace muy interesante investigar su acción específica sobre distintos tipos celulares que sufren alteraciones en patologías relacionadas con alteraciones metabólicas asociadas a la diabetes mellitus. En particular, se hace muy interesante estudiar el efecto de GLP-1 sobre las células vasculares musculares lisas (VSMC). Estas células responden a variadas señales ambientales fisiopatológicas o bioquímicas, regulando su función y su fenotipo. Sin embargo, aún no se comprende la totalidad de mecanismos involucrados en los cambios fenotípicos que ocurren en estas células en patologías como la diabetes. Evidencias recientes han involucrado a las alteraciones metabólicas que sufren las VSMC con la mayor incidencia de aterosclerosis en pacientes diabéticos. Las mitocondrias son los principales organelos asociados con el control del metabolismo energético dentro de una célula. Actualmente, se sabe que procesos subcelulares tales como la interacción mitocondria-retículo endoplásmico y los fenómenos de fusión y fisión mitocondrial, conocido como dinámica mitocondrial, son capaces de regular la función y el metabolismo mitocondrial. La dinámica mitocondrial está finamente regulada por las proteínas Mitofusinas 1/2 y OPA-1 para el caso del proceso de fusión, y Drp-1 y Fis-1 para el caso del proceso de fisión mitocondrial. Se ha descrito un papel trascendental para la proteína Drp-1 en la regulación del proceso de fisión, ya que puede sufrir modificaciones postraduccionales que modulan su interacción con la mitocondria generando cambios en el proceso de fisión. Dentro de las modificaciones postraduccionales de Drp-1 se destaca una fosforilación inhibitoria por PKA. Debido a que el receptor clásico de GLP-1 es una proteína de siete dominios de transmembrana acoplada a proteína Gs, la activación por GLP-1 de este receptor aumentaría los niveles intracelulares de cAMP activando PKA, pudiendo regular potencialmente el proceso de fisión mitocondrial vía Drp-1. Esta regulación podría ser un nuevo mecanismo por el cual GLP-1 podría regular, vía el control de la dinámica, el metabolismo mitocondrial y por ende el metabolismo celular. Con todos estos antecedentes se propuso como hipótesis que GLP 1 inhibe el proceso de fisión mitocondrial, incrementando el metabolismo mitocondrial a través de la fosforilación de Drp 1, en células de la línea A7r5. Para probar esta hipótesis, células A7r5 correspondientes a células embrionarias de VSMC de aorta de rata se trataron con GLP-1 (100 nM) por 0 – 24 h y se incubaron con la sonda Mitotracker Orange en los últimos 30 min de exposición a la hormona. Posteriormente, las células se visualizaron en un microscopio confocal, la red mitocondrial se reconstruyó tridimensionalmente a partir de las imágenes obtenidas, determinándose el número, volumen promedio y área promedio de las mitocondrias. Los resultados muestran que GLP-1 inhibió el proceso de fisión de la red mitocondrial a las 3 h de tratamiento, lo cual se evidenció por un aumento del volumen y el área mitocondrial promedio. Además, en estas condiciones, los niveles de la proteína constitutiva mitocondrial mtHsp70 no se modificaron en relación a los controles, descartando que GLP-1 induzca biogénesis mitocondrial. El metabolismo mitocondrial se evaluó mediante la determinación de potencial de membrana mitocondrial, usando la sonda JC1; producción de especies reactivas de oxígeno (ROS), usando la sonda dihidrocloro fluoresceína; niveles intracelulares de ATP, usando lucifenina/luciferasa; y tasa de respiración, medido por oxigrafía. GLP-1 (100 nM) aumentó el potencial de la membrana mitocondrial, los niveles de ROS, los niveles intracelulares de ATP y el consumo de oxígeno. Se demostró que en células A7r5, GLP-1 activa a la vía transduccional dependiente de PKA. Además, mediante inmunoprecipitación de Drp-1 y posterior detección de la fosforilación de Drp-1 por Western blot se determinó que GLP-1 induce la fosforilación de Drp-1 de una manera similar a la inducida por forskolina. Además, mediante inmunofluorecencia se observó que GLP-1 previene la translocación de Drp-1 a la mitocondria. Utilizando H-89, un inhibidor específico de PKA, se demostró que la inhibición del proceso de fisión mitocondrial es dependiente de la activación de PKA. Posteriormente, se analizó la dependencia de la vía PKA para la acción de GLP-1 sobre el metabolismo mitocondrial. H-89 revertió el efecto de GLP-1 sobre el potencial de membrana mitocondrial y el incremento de los niveles de ROS. En resumen, los datos obtenidos permite sugerir que GLP-1 aumenta el metabolismo mitocondrial en las células A7r5 por un mecanismo que involucra la inhibición del proceso de fisión mitocondrial y la fosforilación de Drp-1 a través de una vía transduccional dependiente de PKA / Glucagon like peptide-1 (GLP-1) is an incretin hormone secreted by intestinal cells. Its classical action is to regulate insulin secretion in the pancreas. However, recently extrapancreatic insulinotropic actions of GLP-1 were described involving tissue-specific signaling pathways. Because GLP-1 have a variety of physiological actions mainly related with its insulinotropic actions, currently GLP-1 peptide mimetics are used to treat hyperglycemia in diabetes mellitus type 2 patients. For this reason, GLP-1 actions on different cell types that undergo pathological metabolic alterations related with diabetes mellitus are very interesting to explore. Particularly, we have focused on GLP-1 effects on vascular smooth muscle cells (VSMC). These cells can regulate both function and phenotype depending on biochemical and physiopathological environmental signals. However, mechanisms involved in VSMC phenotypic change in diseases such as diabetes are just beginning to be understood. Recent evidences have associated the existence of metabolic alterations in VSMC with an increase of atherosclerosis in diabetic patients. Mitochondria are the main organelle that controls energetic metabolism within cells. Specifically, subcellular events such as the interaction between mitochondria and endoplasmic reticulum and mitochondrial fusion and fission (i.e. mitochondrial dynamics) regulate mitochondrial function and metabolism. Mitochondrial dynamics is regulated by proteins, Mitofusins 1/2 and OPA-1 control fusion, and Drp-1 and Fis-1 control fission. Drp-1 has an important role in fission control, because it can be post-translationally modified, changing its interaction with mitochondria and affecting the mitochondrial fission rate. The most important post-translational modification is an inhibitory phosphorylation by PKA. The classical GLP-1 receptor (GLP-1R) is a seven transmembrane Gs-protein coupled receptor, and its activation causes an increase of cAMP levels activating PKA. Because this PKA activations GLP-1R could potentially regulates mitochondrial fission by Drp-1 phosphorylation. This regulation could be a novel GLP-1-dependent mechanism to control mitochondrial dynamics, mitochondrial metabolism and cellular metabolism. Consequently, we hypothesized that GLP-1 decreases mitochondrial fission and increases metabolism through Drp-1 phosphorylation in the A7r5 cell line. To test this hypothesis, A7r5 cells (rat embryonary aortic VSMC) were exposed to GLP-1 (100 nM) for 0 – 24 h and incubated with Mitotracker Orange dye in the last 30 min of “the hormone treatment”. Later, cells were observed under confocal microscope, mitochondrial network was reconstructed from images obtained, and number, average volume and area were determined. Our findings showed that GLP-1 decreased mitochondrial fission at 3 h, evidenced by the increase in mitochondrial volume average and area. These events did not affect mitochondrial constitutive mtHsp70 protein levels, discarding mitochondrial biogenesis induced by GLP-1. Mitochondrial metabolism was evaluated by measuring mitochondrial membrane potential, using JC1 dye; reactive oxygen species (ROS), by dihydrochloro fluorescein dye; intracellular ATP level, using luciferin/luciferase; and respiration rate, by oxygraphy. GLP-1 (100 nM) increased mitochondrial membrane potential, ROS levels, ATP content and oxygen consumption. We demonstrated that GLP-1 activates PKA signaling pathway in A7r5 cells. Moreover, through Drp-1 immunoprecipitation and phospho-Drp-1 detection by Western blot, we showed that GLP-1, as well as forskolin, induced Drp-1 phosphorylation. Consequently, Drp-1 translocation to the mitochondria was prevented by GLP-1, as determined by immunofluorescence. Using the PKA specific inhibitor, H-89, it was demonstrated that GLP-1 decreased mitochondrial fission depends on PKA activation. Subsequently, PKA dependence on GLP-1-induced mitochondrial metabolism was explored. We determined that H-89 prevented GLP-1 effects on both mitochondrial membrane potential and ROS production. A obesidade caracterizada pelo excesso de tecido adiposo está associada a comorbidades como diabetes mellitus tipo 2 (DMT2), hipertensão arterial, doenças cardiovasculares e alguns tipos de câncer. Para pacientes considerados obesos mórbidos (OBM) (IMC>40Kg/m²) a melhor opção terapêutica é a cirurgia de bypass gástrico em Y de Roux (RYGB). Estudos clínicos mostram que após esse procedimento os indivíduos OBM com DMT2 melhoram seu estado. Uma explicação para esse fenômeno é a mudança anatômica que ocorre no trato gastrintestinal, alterando assim a produção das incretinas, GIP e GLP-1, que promovem a liberação de insulina. Essas incretinas são liberadas pela mucosa do intestino delgado na forma de próhormônios e necessitam da ativação feita pela enzima PC 1/3. Carboidratos e ácidos graxos estimulam sua liberação. Após exercerem sua ação são inativadas pela enzima DPP-IV. Logo o objetivo desse trabalho foi a quantificação de mRNA de pró-glucagon, GIP, PC1/3 e DPP-IV em jejuno proximal de OBM DMT2 e não diabetes mellitus tipo 2 (NDM2) , bem como avaliar o efeito, in vitro, de ácidos graxos monoinsaturados cis, trans e glicose sobre esse tecido. O jejuno foi obtido de indivíduos OBM submetidos RYGB. Para a quantificação basal do mRNA dos genes de interesse, o tecido (n=25) retirado na cirurgia foi diretamente imerso em TRIzol para a extração do RNA total. O mRNA foi quantificado pela reação em cadeia da polimerase (qRTPCR). Para avaliar o efeito dos ácidos graxos in vitro, o jejuno (n=5) foi fatiado (400 µm) e incubado em meio DMEM suplementado com 50 µM de acido oléico, elaidico, vaccênico ou 11 mM de glicose por 4 horas a 37°C em atmosfera de 5% de CO2 Entre OBM DMT2 e NDM2 não houve diferença estatística na expressão do mRNA de GIP, GLP-1 e DPP-IV, apenas observamos que indivíduos NDM2 tem tendência a uma maior expressão do mRNA de PC1/3 (p=0,065). No entanto, alguns pacientes não expressaram mRNA de PC1/3. A expressão dos genes estudados foi heterogênea, permitindo a divisão dos pacientes em alta e baixa expressão de GIP, sendo este utilizado como gene calibrador. Constatamos que a expressão de mRNA dos demais genes acompanham o perfil de expressão de GIP. Os testes in vitro revelaram que o ácido oléico diminuiu a expressão do mRNA de DPP-IV; o ácido vaccênico, aumentou significante a expressão do mRNA de próglucagon; e o ácido elaídico, não modulou a expressão dos genes estudados nesse trabalho. O tratamento com alta concentração de glicose aumentou o mRNA da enzimas PC1/3 e DPP-IV. Em conclusão, OBM NDM2 e DMT2 apresentaram expressão heterogênea desses genes não permitindo verificar diferença significativa entre os grupos. One explanation for this phenomenon is the anatomical change that occurs in the gastrointestinal tract, thereby altering the production of incretins, GIP and GLP-1, those promote the release of insulin. These incretins are released in the prohormones form by mucosa of small intestine and require the activation by the enzyme PC 1/3. Carbohydrates and fatty acids stimulate its release. After act they are inactivated by enzyme DPP-IV. Once the objective of this study was to quantify the mRNA of proglucagon, GIP, PC1/3 and DPP-IV in the proximal jejunum of OBM without type 2 diabetes mellitus (NDM2) and with T2DM and to evaluate the effect in vitro of fatty acids cis and trans monounsaturated and high glucose on this tissue. The jejunum was obtained from individuals OBM undergo RYGB. For the quantification of basal mRNA of these genes, the tissue removed at surgery was directly immersed in TRIzol for extraction of total RNA. The mRNA expression was quantified by polymerase chain reaction (qRT-PCR). To evaluate the effect of fatty acids in vitro, the jejunum was sliced (400 µm) and incubated in DMEM supplemented with 50 µM of oleic, elaidic, vaccênic acid or 11 mM of glucose for 4 hours at 37°C in an atmosphere of 5% CO2. Between OBM T2DM and NDM2 no statistical difference was observed in the expression of mRNA of GIP, GLP-1 and DPP-IV, only NDM2 tend to have greater expression of mRNA of PC1/3 (p=0.065). Furthermore, some patients did not express mRNA of PC1/3. The expression of genes studied was heterogeneous, allowing the division of patients into high and low expression of GIP, which was used as a calibrator gene. So, we found that the expression of mRNA of other genes accompany this profile. The in vitro tests showed that oleic acid decreased the DPP-IV mRNA expression, the vaccenic acid, significantly increased the proglucagon mRNA expression and elaidic acid not modulated the expression of genes studied in this work. Treatment with high concentration of glucose increased the mRNA of enzymes PC1/3 and DPP-IV. In conclusion, OBM NDM2 and T2DM showed heterogeneous expression of these genes consequently did not permit verifying significant differences between groups. Some patients do not express the mRNA of PC1/3 and NDM2 transcribe major amount of mRNA of this enzyme. We suggest that in the proximal jejunum, the oleic and vaccenic acid of diet, act directly on cells and modulate incretins levels, reducing the expression of DPP-IV mRNA and increasing the proglucagon, respectively. Glucose at high concentrations stimulates the transcription of enzymes involved in synthesis and degradation of incretins and may interfere with the homeostasis of glucose metabolism. Our research shows that this intestine region that directly receives the food after the surgery, can be modulated by foods that contains especially fat and carbohydrate. Greater attention should be given to these patients about nutritional view.

nÃo hà / Diabetes mellitus (DM) à uma sÃndrome associada a secreÃÃo de insulina deficiente, resistÃncia à aÃÃo da insulina ou uma associaÃÃo de ambas. Caracteriza-se por hiperglicemia crÃnica que a longo prazo poderà causar retinopatia, nefropatia, neuropatia perifÃrica e autonÃmica. Este estudo do tipo intervencionista, prospectivo e aberto teve como objetivo avaliar a eficÃcia terapÃutica da sitagliptina no controle metabÃlico do diabetes mellitus do tipo 2 recentemente diagnosticado (menos de seis meses) e nas vias de conduÃÃo nervosa somato-sensitiva e visual. Antes de iniciar o tratamento, foram avaliados os Ãndices antropomÃtricos e a pressÃo arterial. TambÃm foram realizadas as dosagens em jejum de triglicerÃdeos, colesterol total, colesterol de alta densidade (HDL), gama glutamil transferase, aspartato amino transferase, alanina amino transferase, proteÃna C reativa, microalbuminÃria, Ãxido nÃtrico, glicose, hemoglobina glicada (A1C), peptÃdeo C, insulina e glucagon. ApÃs um estÃmulo alimentar de 566 Kcal foram feitas coletas de sangue seriadas durante 3 horas nos tempos de 5, 15, 30, 45, 60, 90, 120, 150 e 180 minutos para a dosagem de glicose, insulina e glucagon com a construÃÃo da Ãrea sob a curva (AUC) destes analitos. Um subgrupo de 20 pacientes realizou ainda, anÃlises seriadas das incretinas (GLP-1 ativo e GIP total) apÃs estÃmulo alimentar e, tambÃm, exames eletroneurofisiolÃgicos (potenciais evocados somato-sensitivos â PESS, e visuais - PEV). ApÃs estas avaliaÃÃes, os pacientes receberam sitagliptina 100 mg (1 comprimido/dia) por um perÃodo de 3 meses, apÃs o qual, eles retornaram para avaliaÃÃo clÃnica e para realizar todas as dosagens bioquÃmicas e os exames de PESS e PEV que haviam realizado no inÃcio do estudo. Dos 41 pacientes avaliados, 28 eram do sexo feminino e 13 do sexo masculino, idade mÃdia de 53,2  9,43 anos, em mÃdia obesos e 50% deles eram hipertensos. ApÃs o tratamento, observou-se a melhora dos Ãndices antropomÃtricos, de todos os parÃmetros laboratoriais com Ãnfase na glicose em jejum (prÃ: 174,43  67,18 e pÃs: 129,07  29,19 mg/dL), A1C (prÃ: 8,76  2,68 e pÃs: 6,64  1,11%) e Ãxido nÃtrico (prÃ: 85,21  44,02 e pÃs: 39,91  20,99 ÂM), todos com P<0,0001 e ainda, aumento da concentraÃÃo da AUC de GLP-1 ativo (P=0,003) e diminuiÃÃo da AUC de GIP total (P=0,001), bem como diminuiÃÃo das latÃncias N9D (P<0,0001), N13E (P=0,036), N20D (P=0,028) e dos tempos de conduÃÃo central N13-N9E (P=0,045), N20-N13D (P=0,044), alÃm de correlaÃÃes com significÃncia estatÃstica entre parÃmetros bioquÃmicos e eletroneurofisiolÃgicos. Este estudo concluiu que houve melhora clÃnica, metabÃlica e nas vias de conduÃÃo do impulso nervoso somato-sensitivo e visual dos pacientes tratados com sitagliptina. / Diabetes mellitus (DM) is a syndrome associated with deficient insulin secretion, resistance to insulin action or the combination of both. It is characterized by chronic hyperglycemia that in a long term can cause retinopathy, nephropathy, peripheral and autonomic neuropathy. The present interventional, prospective and open study was designed to evaluate therapeutic efficacy of sitagliptin in metabolic control of diabetes mellitus type 2 recently diagnosed (less than 6 months) and in pathways of visual and somatosensory nerve conduction. Blood pressure and anthropometric data were colected before treatment. Measures of fasting triglycerides, total cholesterol, high density lipoprotein cholesterol gamma glutamyl transpeptidase, alanine aminotransferase, aspartate aminotransferase, microalbumin, nitric oxide, glucose, glycated hemoglobin (A1C), C-peptide, insulin and glucagon were also collected. Also during the pre-study, after a feeding stimulus of 566 Kcal, biochemical evaluations were conducted of the metabolism in fasting and during 3 hours, in standard intervals of 5, 15, 30, 45, 60, 90, 120, 150 and 180 minutes for glucose, insulin and glucagon for design of the area under curve (AUC) of these analytes. In a subgroup of 20 patients were performed dosages in pre-fixed times of incretins (active Glucagon Like Peptide -1, GLP-1, and total Glucose-dependent Insulinotropic polypeptide, GIP) after feeding stimulus and electroneurophysiological tests (somatosensory evoked potentials - SEP, and visual - VEP). After these evaluations, the patients received sitagliptin 100 mg (1 tablet a day) for 3 months and then returned for clinical evaluation and to repeat all the biochemical dosages, SEP and VEP exams. From 41 patients evaluated, 28 were female and 13 male, the average age was 53.2  9.43 years, in average obese and 50% were hypertensive. After treatment, improvement was observed in all anthropometric and laboratory parameters, specially fasting glucose (pre: 174,43  67,18 mg/dL and post: 129,07  29,19 mg/dL), A1c (pre: 8,76  2,68 and post: 6,64  1,11%), and nitric oxide (pre: 85,21  44,02 and post: 39,91  20,99 ÂM), all with P<0.0001, and also increase in the concentration of the AUC of active GLP-1 (P=0.003), decrease of the AUC of total GIP (P=0.001), and decrease of the N9D (P<0.0001), N13E (P=0.036 ), N20D (P=0.028) latencies and of the central conduction time N13-N9E (P=0.045), N20-N13D (P=0.044). There were also statistically significant correlations between biochemical and electrophysiological parameters. This study concluded that there was clinical, metabolic and somatosensory and visual nerve impulse conduction pathways improvement in patients treated with sitagliptin.

O aumento nos casos de obesidade em todo o mundo tem gerado grande preocupação e estimulado pesquisas na prevenção e tratamento dessa condição patológica. A combinação de diabetes tipo 2 ou resistência insulínica com obesidade agrava o potencial evolutivo da enfermidade. Mesmo pacientes submetidos com sucesso à cirurgia bariátrica ou metabólica, podem não se curar do diabetes, pois a melhora das taxas circulantes de glicose e insulina nem sempre corresponde à recuperação da massa beta pancreática. Até o momento, não há consenso sobre como medir a massa de células beta in vivo. As ferramentas disponíveis padecem de baixa sensibilidade e especificidade, muitas vezes revelando-se também complexas e dispendiosas. Incretinas radiomarcadas ,como os análogos do peptídeo glucagon-like-peptide-1 / GLP-1, têm-se revelado promissoras para avaliação de células beta pancreáticas, em diabetes e insulinoma. O objetivo do presente trabalho foi o desenvolvimento de dois conjugados de análogo de incretina GLP-1, marcados com tecnécio-99m, a fim de propor um método não invasivo de imagem, para monitoração da massa de células beta pancreáticas, em organismos afetados por obesidade. O estudo foi conduzido em diferentes modelos animais, incluindo obesidade induzida por dieta hiperlipídica, estado pós-obesidade em que o distúrbio inicialmente gerado foi parcialmente corrigido, e como controle, diabetes induzido com aloxana. Nos resultados, os radiotraçadores alcançaram um rendimento radioquímico superior a 97%. O melhor radiomarcador, dentre os dois análogos ensaiados, foi o 99mTc-HYNIC-&beta;Ala-Exendin-4. Animais com obesidade induzida por dieta revelaram captação reduzida nas células beta pancreáticas. A restrição dietética (estado pós-obesidade) não se seguiu de recuperação completa, embora notável melhora de glicemia haja sido observada. There is no consensus about how to estimate beta cell mass in vivo. Available tools suffer from low sensitivity and specificity, often being as well cumbersome and expensive. Radiolabeled incretins, such as glucagon-like-peptide 1 (GLP-1) analogs, seem to be promising options for the measurement of beta cell mass in diabetes and insulinoma. The objective of this study was the development of two conjugates of GLP-1 analog, radiolabeled with 99m Technetium, as a noninvasive imaging method for the estimation of pancreatic beta cell mass, in the presence of obesity. Animal models were selected, including hyperlipidic diet-induced obesity, diet restricted obesity, and as controls, alloxan diabetes. Results indicated that both radiotracers achieved over 97% radiochemical yield. The most successful product was 99mTc-HYNIC-&beta;Ala-Exendin-4. Low beta cell mass uptake occurred in diet-induced obesity. Diet-restricted obesity, with substantial shedding of excess body weight, was followed by remarkable decrease of fasting blood glucose, however beta cell mass uptake was only mildly improved. Future studies are recommended in obesity, type 2 diabetes, and dieting, including bariatric and metabolic operations.
