Near-Infrared Spectroscopy neurofeedback (NIRS neurofeedback) em crianças com Transtorno de Déficit de Atenção/Hiperatividade (TDAH)

As intervenções com neurofeedback têm tido grande difusão na área como uma alternativa possível para tratamento do Transtorno de Déficit de Atenção/Hiperatividade (TDAH). Este estudo tem o objetivo de avaliar a viabilidade da utilização de um sistema de neurofeedback de baixo custo chamado Near Infrared Spectroscopy/Hemoencephalography Neurofeedback (NIR/HEG-NF) para tratar crianças com TDAH. Método: trata-se de um estudo clínico piloto aberto, que incluiu 5 crianças (3 meninos e 2 meninas - idade 10,4 ± 0,89 anos) com TDAH-C (tipo combinado). Os indivíduos completaram um programa de 24 sessões de NIR/HEG-NF, duas ou três vezes por semana. Os seguintes desfechos foram avaliados pré e pós-tratamento: 1) sintomas de desatenção e hiperatividade; 2) desempenho neuropsicológico; 3) imagens de SPECT cerebral; 4) qualidade de vida; e 5) efeitos adversos. Resultados: houve maior resistência do que o esperado para a participação dos indivíduos devido a vários fatores, tais como logísticos para a participação nas sessões e requerimento de não uso de medicação durante o protocolo. Detectamos uma diferença significativa na comparação pré e pós-intervenção nos escores atribuídos pelos pais na dimensão de hiperatividade e impulsividade do Swanson, Nolan and Pelham Questionnaire (SNAP-IV) (p = .042; tamanho de efeito [ES] =.38) e uma tendência de melhora nos escores atribuídos pelos pais e professores na dimensão desatenção (p = .066; ES = .47 / p = .068; ES = .60) naquelas crianças que finalizaram o tratamento. Detectamos uma tendência de aumento perfusional em diferentes partes do córtex cerebral em três indivíduos. As comparações pré e pós-intervenção nas outras medidas não indicaram diferenças estatisticamente significativas. Não foram relatados efeitos adversos significativos. Discussão: Os achados iniciais sugerem que o NIR/HEG-NF como tratamento para sintomas de TDAH pode ser promissor e deve ser melhor investigado. O protocolo lança luz sobre novas abordagens para avaliar a eficácia da intervenção. Pode-se, por exemplo, implementar o programa de intervenção no ambiente escolar e com uso concomitante de medicamentos para superar as resistências na alocação de pacientes e para uma avaliação com maior validade externa. / Background and objectives: Neurofeedback interventions have been quick and extensively introduced for clinicians to treat Attention-Deficit/Hyperactivity Disorder (ADHD). This study aims to assess the feasibility of using a low cost neurofeedback system called Near Infrared Spectroscopy/ Hemoencephalography Neurofeedback (NIR/HEG-NF) for treating children with ADHD. Method: This open pilot clinical study included 5 children (3 boys and 2 girls - age 10.4 ± 0.89 years) with ADHD-C (combined type). The subjects completed a program of 24 sessions of NIR/HEG-NF, two or three times a week. The following outcomes were assessed pre and post-treatment: 1) clinical symptoms of inattention and hyperactivity; 2) neuropsychological performance; 3) brain SPECT images; 4) quality of life and; 5) adverse effects. Results: Higher resistance than expected was experienced for patient allocation due to logistic reasons to attend the sessions in the hospital and the requirement of not using medication during the protocol. We detected a significant decrease in hyperactivity/impulsivity comparing pre and post-intervention scores in the parent’s Swanson, Nolan and Pelham Questionnaire - SNAP-IV (p = .042; Effect Size [ES] r = .38) and a trend of improvement in scores attributed by parents and teachers in inattention (p = .066; r = .47 / p = .068; r = .68) . We detected a trend for increased perfusion in different parts of the entire cortex in 3 subjects. Comparisons between pre and post-intervention scores in other measures did not suggest meaningful differences. No significant adverse effects were reported. Discussion: Our initial findings suggest that NIR/HEG-NF technique might be promising and should be further investigated. The protocol shed light on new approaches to assess the effectiveness of the intervention such as the need to both implement the intervention program in a school environment and to pursue the investigation of the treatment effects with concomitant use of medications to surpass resistances to enroll patients and to have an assessment with more external validity.

Die Beeinflussung der cerebralen Oxygenierung bei partieller Flüssigkeitsbeatmung gesunder Ferkel

Die intratracheale Applikation von Perfluorcarbonen (PFC) in ungeschädigte Lungen, z.B. als Röntgenkontrastmittel und zur PFC-induzierten cerebralen Kühlung, wird derzeit diskutiert. Ob es aufgrund der Dichte und Sauerstofflöslichkeit der PFC zu einer Beeinflussung der cerebralen Oxygenierung und Hämodynamik durch die pulmonale PFC Applikation kommt, ist bisher unbekannt. Änderungen der Konzentration von cerebralem oxygeniertem und totalem Hb koennen mittels Near-infrared Spectroscopy gemessen werden. I.) Effekt durch die PFC-Applikation in gesunde Lungen neugeborener Ferkel: Es wurden zwei verschiedene Applikationsgeschwindigkeiten (30 ml PFC pro kg Körpergewicht als Bolusgabe versus 1,5 ml/min pro kg) und zwei Füllvolumen (30 versus 10 ml/kg) verglichen. Die Bolusgabe bewirkt einen sofortigen Abfall des PaO2 und der cerebralen Oxygenierung, bei langsamer Gabe des gleichen Volumens ist dies weniger ausgeprägt. Mit 10 ml/kg PFC fand sich nahezu keine Beeinflussung der Parameter. II.) Effekte der Änderung der FiO2 in PFC-gefüllten Lungen: Hierfür wurden Änderungen der FiO2 unter konventioneller Druckbeatmung vor PFC-Applikation und unter PFC-Füllung (30 oder 10 ml/kg PFC) verglichen. Beide PFC-Volumen (mit FiO2 1,0) bedingten PaO2-Werte wie unter CMV mit FiO2 von 0,5. Mit 30 ml/kg PFC kam es unter FiO2 von 0,5 zur Abnahme des cerebralen oxygenierten Hb. Zusammenfassend ergibt sich, dass die Applikation von 10 ml PFC/kg bevorzugt werden sollte. Bei kompletter Füllung der Lungen werden durch langsame Applikation cerebrale Nebenwirkungen minimiert. In PFC-gefüllten gesunden Lungen ist zum Erhalt der systemischen und cerebralen Oxygenierung die FiO2 zu erhöhen. / Intratracheal administration of perfluorochemicals (PFC) has been suggested for reasons other than respiratory insufficiency, such as pulmonary imaging and PFC-associated brain cooling. Due to their high density and oxygen solubility, PFC application has been described to affect systemic hemodynamics and oxygenation during liquid ventilation. Whether the PFC application into healthy lungs or changes in inspired oxygen fraction (FiO2) in PFC-filled healthy lungs affects cerebral hemodynamics is not known. Changes in the concentration of cerebral oxygenated and total Hb can be measured by near-infrared spectroscopy (NIRS). I.) Initial effects of PFC application into healthy lungs of newborn piglets: Two different filling modes (rapid versus slow) and two different filling volumes (slow filling of 30 versus 10 ml PFC/kg body weight) were compared. Rapid filling caused an immediate drop of systemic and cerebrale oxygenation, which was less prominent by filling 30 ml/kg PFC slowly. Almost no changes for all parameters were found with 10 ml kg/PFC. II.) Effects of variations in FiO2 in the PFC filled lungs of healthy piglets: Changes in FiO2 during conventional mechanical ventilation (CMV) prior to PFC-application and in the PFC-filled lungs (30 ml/kg or 10 ml/kg PFC) were compared. Both PFC-volumes (at a FiO2 of 1.0) caused PaO2-values that were similar to CMV with FiO2 of 0.5. In the 30 ml/kg PFC group a reduction of cerebral oxygenated Hb was found at FiO2 of 0.5. According to the data application of 10 ml/kg PFC should be preferred. If complete filling of the lung is needed, the slow administration of PFC minimizes cerebral side effects. In PFC- filled healthy lungs an increase in FiO2 is necessary to maintain systemic and cerebral oxygenation.

Linking remotely-sensed UAS imagery to forage quality in an experimental grazing system

Forage quality is a principal factor in managing both herbivores and the landscapes they use. Nutrition varies across the landscape, and in turn, so do the distributions of these populations. With the rise of remote sensing technologies (i.e. satellites, unmanned aerial vehicles, and multi/hyperspectral sensors), comes the ability to index forage health and nutrition swiftly. However, no methodology has been developed which allows managers to use unmanned aerial systems to the fullest capacity. The following methodologies produce compelling evidence for predicting forage quality metrics (such as fiber, carbohydrates, and digestibility) using 5 measured bands of reflectance (Blue, Green, Red, Red Edge, and NIR), 3 derived vegetation indices (NDVI, EVI and VARI), and a variety of environmental factors (i.e. time and sun angles) in a LASSO framework. Fiber content, carbohydrates, and digestibility showed promising model performance in terms of goodness-of-fit (R2= 0.624, 0.637, and 0.639 respectively).

NONINVASIVE NEAR-INFRARED DIFFUSE OPTICAL MONITORING OF CEREBRAL HEMODYNAMICS AND AUTOREGULATION

Many cerebral diseases are associated with abnormal cerebral hemodynamics and impaired cerebral autoregulation (CA). CA is a mechanism to maintain cerebral blood flow (CBF) stable when mean arterial pressure (MAP) fluctuates. Evaluating these abnormalities requires direct measurements of cerebral hemodynamics and MAP. Several near-infrared diffuse optical instruments have been developed in our laboratory for hemodynamic measurements including near-infrared spectroscopy (NIRS), diffuse correlation spectroscopy (DCS), hybrid NIRS/DCS, and dual-wavelength DCS flow-oximeter. We utilized these noninvasive technologies to quantify CBF and cerebral oxygenation in different populations under different physiological conditions/manipulations. A commercial finger plethysmograph was used to continuously monitor MAP. For investigating the impact of obstructive sleep apnea (OSA) on cerebral hemodynamics and CA, a portable DCS device was used to monitor relative changes of CBF (rCBF) during bilateral thigh cuff occlusion. Compared to healthy controls, smaller reductions in rCBF and MAP following cuff deflation were observed in patients with OSA, which might result from the impaired vasodilation. However, dynamic CAs quantified in time-domain (defined by rCBF drop/MAP drop) were not significantly different between the two groups. We also evaluated dynamic CA in frequency-domain, i.e., to quantify the phase shifts of low frequency oscillations (LFOs) at 0.1 Hz between cerebral hemodynamics and MAP under 3 different physiological conditions (i.e., supine resting, head-up tilt (HUT), paced breathing). To capture dynamic LFOs, a hybrid NIRS/DCS device was upgraded to achieve faster sampling rate and better signal-to-noise. We determined the best hemodynamic parameters (i.e., CBF, oxygenated and total hemoglobin concentrations) among the measured variables and optimal physiological condition (HUT) for detecting LFOs in healthy subjects. Finally, a novel dual-wavelength DCS flow-oximeter was developed to monitor cerebral hemodynamics during HUT-induced vasovagal presyncope (VVS) in healthy subjects. rCBF was found to have the best sensitivity for the assessment of VVS among the measured variables and was likely the final trigger of VVS. A threshold of ~50% rCBF decline was observed which can completely separate subjects with or without presyncope, suggesting its potential role for predicting VVS. With further development and applications, NIRS/DCS techniques are expected to have significant impacts on the evaluation of cerebral hemodynamics and autoregulation.

Chernozem became the crucial soil for the beginnings of soil science through the work of Dokuchaev from 1883. Since then the genesis of chernozems in Central Europe has raised many questions among soil scientists, botanists and paleo-environmentalists. While in Eastern Europe chernozems have been described as zonal soils, that are typical for continental steppe and forest-steppe areas, there are areas in Central Europe which are predisposed by their climatic characteristics to the presence of woodlands. The goal of this dissertation is to enrich the discussion about the genesis of chernozems by restoring the environmental conditions that were prevalent during the formation of chernozem soils in Central Europe. Chernozems are usually developed on loess with a very thick and dark organic surface which passes directly to a calcareous horizon. The organic matter underwent a polymerization in dependence on climate contrasts. However, in Central Europe, the climatic characteristics of the areas of chernozems are a bit different. These chernozems are supposed to have been formed under the climatic conditions that dominated Central Europe during the Late Glacial and the Early Holocene.
