Bronchodilating and Bronchoprotective Effects of Deep Inspirations in Asthma, Cough Variant Asthma, and Methacholine-induced Cough but Normal Airway Sensitivity

Rationale: The pathophysiologic differences between classic asthma (CA), cough variant asthma (CVA), and methacholine (MCh)-induced cough but normal airway sensitivity (COUGH) are poorly understood and may relate to differences in small airway function. We hypothesized that the bronchodilating and bronchoprotective effects of DIs are: (a) absent or impaired in individuals with CA; (b) impaired in individuals with CVA; and (c) preserved in those with COUGH. Purpose: To compare the effect of DIs in individuals with CA, CVA and COUGH using high-dose methacholine (MCh). Methods: Individuals aged 18-65 years with CA, or suspected CVA attended 3 visits. On visit 1, subjects performed body plethysmography and high-dose MCh testing to a maximum change (Δ) in FEV1 of 50% from baseline (MAX). On visits 2 and 3, subjects performed one of two modified single-dose MCh challenge tests with or without DIs prior to MCh administration. Partial and maximal-flow volume curves (used to calculated a DI index), impulse oscillometry (IOS) measurements and breathlessness (Borg scale) were recorded at baseline and at each dose of MCh. Lung volumes were measured by body plethysmography at baseline and MAX. Responses from baseline to a 20% decline in FEV1 (PC20) or MAX, and between visits with or without DIs were compared using paired t-tests. Between group differences were assessed by analysis of variance. Results: 19 subjects (14 female; 45.1±13.1 years (MEAN±SD)) completed the protocol (n=8 CA, n=7 CVA, and n=4 COUGH). At the dose nearest to PC20, the DI index was positive in all 3 groups, indicating preserved bronchodilation (CA: 0.88±0.67; CVA: 0.33±0.39; COUGH: 0.016±0.46; P=.0.076). There were no significant differences in: the Δs in spirometry, lung volumes, airway resistance, IOS measurements, closing indices, the DI index, or dyspnea when the single dose of MCh was preceded either by five DIs or by DI avoidance in CVA and COUGH. DIs in CA resulted in greater Δs in mid-to-late flows. Conclusion: The bronchodilating effect of DIs is preserved in CA, CVA and COUGH. The bronchoprotective effect may fall along a continuum. Vi har gjort en metodkombination av kvalitativ samt kvantitativ studie där vi undersökte om ledarskapsstilar har någon påverkan på anställdas motivation och i så fall vilken ledarskapsstil som höjde motivationen bäst. Studien genomfördes med hjälp av intervjuer samt enkäter som skickades ut via mejl. Resultatet av studien visar på att transformativt ledarskap har en påverkan på anställdas motivation och att skillnader kunde utläsas mellan huvudkomponenterna inom transformativt ledarskap. Den huvudkomponent som har den högst positiva påverkan på motivation är inspirations motivation.

Parmi toutes les contrées lointaines, c’est en Chine qu’elle conçoit son rêve le plus charmant et son monde intime le plus merveilleux. La Chine dans son ensemble la séduit, répondant à la fois à sa curiosité envers l’Antiquité et à son désir de décrypter des documents indéchiffrables. C’est de là qu’elle a tiré ses meilleures inspirations pour ses créations littéraires. What Would The Doctor Do (WWTDD)? : Dialogues with Feminist Whovians: Exploring Feminist Inspirational Potential in Doctor Who

What Would The Doctor Do?  This is a question at the heart of the famous BBC science-fiction show Doctor Who led by the eponym character: the Doctor. As a fan of the show myself, a Whovian, this question matters to me as I look up to the Doctor, this hero who is described as an alien ethical guide for their human companions. Inviting other Feminist Whovians on this journey of reflection about the Whoniverse, we have attempted to unveil if this inspirational motto is shared by the feminist fandom. Concretely, we wondered in what ways can Doctor Who inspire feminist fans. Together, we have explored the feminist inspirational potential of Doctor Who, investigating how we make sense of the show as feminists. We have focused on the reboot and the last evolutions of the show, introducing a woman in the leading role after 60 years of white males. Juggling with fan theory, feminist methodology and phenomenological frameworks analyzing emotions, this study therefore explores the agency of the feminist fandom regarding the political and inspirational potential of Doctor Who. To collect the voices of other Feminist Whovians, I have experimented with the method of chat-discussions, providing an intimist space to share reflections and interpretations of the show. Collectively, we painted a three-dimensional feminist inspirational potential, connecting feminist emotions, feminist values and feminist role models. Doctor Who seems to trigger strong emotions in feminist fans which opens a bridge where values can be shared, and heroes can be admired.    This study shows that reading feminist inspirational potential into the show is rendered possible by the agency held by Feminist Whovians. The feminist fandom indeed adapts the messages of the show towards feminist interpretations, while having a critical understanding of the issues displayed on screen. Choosing specific moments and assigning them powerful meanings, Feminist Whovians are therefore both receivers and modelers of the feminist inspirational potential of Doctor Who.    Navigating this emotional journey uncovering inspirational values and relatable heroes, I have attempted to grasp the concrete influence of the show on the lives of feminist fans, immerging myself with them, with us, into our common love for Doctor Who. Together, we shaped a critical vision of what Doctor Who is and what it can mean for Feminist Whovians.   Fancy a trip?

This study examines the oral histories of midwives who practiced in Ontario without legal status in the two decades prior to the enactment of midwifery legislation on December 31, 1993. The following questions are answered: Who were Ontario’s pre-legislation midwives? What inspired and motivated them to take up practice on the margins of official health care? Current scholarship on late twentieth century Ontario midwifery focuses on a social scientific analysis of midwifery’s transition from a grassroots movement to a regulated profession. Pre-legislation midwives are commonly portrayed as a homogenous group of white, educated, middle class women practicing a “pure” midwifery unmediated by medicine and the law. Analysis of the oral history narratives of twenty-one “alegal” Ontario midwives reveals more complex and nuanced understandings of midwives and why they practiced during this period. The midwives’ oral histories make an important contribution to the growing historiography on modern Canadian midwifery.

Bohuslav Martinů - Voják a tanečnice / Bohuslav Martinů - The Soldier and the Dancer

The first opera by Bohuslav Martinů (from the total of 16) named Voják a tanečnice (The Soldier and the Dancer), H. 162 was created in the initial years of Martinů's stay in Paris (1926-1927), when the effort to reflect different contemporary music trends and influences can be traced in his compositions. Voják a tanečnice is an original example of absorbing such influences on the background of the three-act comic opera. Together with the librettist, Jan Löwenbach, they tried to create a "new opera buffa", which was however not understood by the contemporary reviewers. The opera was first performed in Brno in 1928. The frequency of the first performances of Martinů's stage works in the National Theatre in Brno indicates that the first performance of his first opera was not a coincidence but a part of targeted dramaturgy at the Brno stage. The opera follows both the tradition of an opera and tendencies in art of the 1920's, e. g. playing with humour in music, the epic theatre inspirations or using ancient theme.
