Multiple system atrophy : a translational approach Characterization of the insulin/IGF-1 signaling pathway / L'atrophie multisystématisée : une approche translationnelle

Ce travail porte sur des approches translationnelles dans les synucléinopathies notamment l’atrophie multisystématisée (AMS). Au-delà de leur rôle dans la régulation du glucose, l’insulin et l’insulin like growth factor-1 (IGF-1) ont des propriétés neurotrophiques. Des études ont montrées que la signalisation de l’insuline/IGF-1 est altérée dans la maladie d'Alzheimer et des données suggèrent l’altération de l’insuline/IGF-1 dans la maladie de Parkinson (MP) et l’AMS. Nous avons mis en évidence une résistance à l’insuline dans les neurones des patients MP et AMS ainsi que dans les oligodendrocytes chez les patients AMS.Mon travail a également consisté à cibler la troncation de l’α-synuclein (α-syn) comme cible thérapeutique. Nous avons démontré dans un modèle murin d’AMS que la diminution de l’α-syn tronquée permettait de réduire l’agrégation d’α-syn et la dégénérescence des neurones dopaminergiques.Enfin, nous avons étudié l’implication dans l’AMS des métalloprotéinases matricielles (MMP), des enzymes impliquées dans remodelage de la matrice, la démyélinisation, la troncation de l’α-syn et la perméabilité de la barrière hémato-encéphalique. Ce travail nous a permis de montrer une augmentation de l’expression et de l’activité de MMPs chez les patients AMS. Nous avons également montré que les cellules gliales sont la source de cette augmentation et que la MMP-2 est retrouvée dans les agrégats des patients AMS.Nous montrons ici de caractéristiques distinctes de l’AMS comme des altérations qui se produisent dans les oligodendrocytes. Nous présentons aussi VX-765 comme un candidat prometteur pour ralentir la progression de la pathologie dans un contexte de synucléinopathie. / This work focused on translational approaches in synucleinopathies and more specifically in multiple system atrophy (MSA). Beyond their role in glucose homeostasis, insulin/IGF-1 are neurotrophic factors in the brain. Studies have shown altered insulin/IGF-1 signalling in Alzheimer’s disease and data suggest impaired insulin signaling/IGF-1 in Parkinson's disease (PD) and MSA. The aim of my work was to characterize insulin/IGF-1 signalling in MSA and PD brain tissue. Both groups showed neuronal insulin resistance. Oligodendrocytes in MSA patients were also insulin resistant.In line with the translational approach, we also targeted α-synuclein (α-syn) truncation pharmacologically in MSA transgenic mice, which led to reduced α-syn aggregation and the protection of dopaminergic neurons.We also assessed the activity and distribution of matrix metalloproteinases (MMPs) in the brain of MSA patients compared to healthy controls. MMPs are involved in the remodelling of the extracellular matrix, demyelination, α-syn truncation and blood brain barrier permeability. We showed altered expression and activity of MMPs in two distinct structures in MSA brains. We were also able to show that glial cells were the source of increased MMPs and show a unique expression of MMPs in α-syn aggregates of MSA patients compared to PD, evidence that might hint at a mechanism that is differently altered between PD and MSA.We here show distinct pathological features of MSA such as key alterations occurring in oligodendrocytes, further supporting MSA as a primary oligodendrogliopathy. We also present VX-765 as a candidate drug for disease modification in synucleinopathies.

Sabe-se que algumas alterações nutricionais maternas durante o período perinatal estão associadas com doenças metabólicas na vida adulta das proles, tais como diabetes melito tipo 2, resistência à insulina, obesidade e hipertensão arterial. O período da gestação em que estas alterações nutricionais influenciam a prole na idade adulta ainda não está elucidado. Modificações epigenéticas têm sido propostas como mecanismos responsáveis por estas desordens metabólicas. Ratas Wistar de doze semanas de idade foram alimentadas com dieta com conteúdo baixo (HO - 0,15% NaCl) ou normal (NR - 1,3% NaCl) de sódio desde o primeiro dia de gestação até o nascimento da prole ou HO durante a primeira (HO10) ou segunda (HO20) metade da gestação. O peso corpóreo e a ingestão de água e ração foram avaliados semanalmente durante a gestação. Teste de tolerância à insulina (ITT) e à glicose (GTT) e HOMA-IR foram realizados nas proles adultas. Expressão gênica por qRT-PCR e metilação do DNA na região promotora dos genes foram mapeadas utilizando tratamento com bissulfito de sódio e avaliadas por pirosequenciamento. O ganho de peso materno foi menor no HO e HO20 na terceira semana de gestação em comparação com NR e HO10. O peso ao nascimento da prole foi menor em machos e fêmeas dos grupos HO e HO20 em relação ao NR e HO10. O HOMA-IR foi maior nos machos com 12 semanas de idade do grupo HO em comparação com NR e com 20 semanas de idade do grupo HO10 em comparação com NR e HO20. Nas fêmeas com 12 semanas de idade o HOMA-IR foi maior no HO10 comparado com HO. Os níveis de insulina no soro foram maiores tanto nos machos com 20 semanas de idade do grupo HO10 comparado com NR quanto nas fêmeas com 12 semanas de idade do grupo HO10 comparado com HO. A área sob a curva do GTT indicou intolerância à glicose nos machos do grupo HO. A porcentagem de metilação das ilhas CpG no promotor dos genes de Igf1, Igf1r, Ins1, Ins2 e Insr no fígado de machos e fêmeas neonatais e no fígado, tecido adiposo branco e músculo em machos com 20 semanas de idade foi influenciada pela baixa ingestão de sal durante a gestação. Nenhuma destas alterações foi identificada nas fêmeas com 20 semanas de idade. Em conclusão, a baixa ingestão de sal na segunda metade da gestação é responsável pelo baixo peso ao nascimento em ambos os sexos. A intolerância à glicose observada na prole adulta ocorreu somente se a dieta hipossódica é dada durante a gestação inteira. Por outro lado, a resistência à insulina em resposta ao consumo de dieta hipossódica durante a gestação está relacionada com o momento em que ocorre este insulto e com o envelhecimento da prole. Também foi observado que alterações na metilação do promotor do gene Igf1 está correlacionado com o baixo peso ao nascimento em resposta a ingestão de dieta hipossódica durante a gestação / It is known that some maternal nutritional alterations during pregnancy are associated with metabolic disorders in adult offspring, such as insulin resistance, type 2 diabetes mellitus, obesity and arterial hypertension. The period of pregnancy in which these nutritional alterations influence adult offspring remains uncertain. Epigenetic changes are proposed to underlie these metabolic disorders. Twelve-week-old female Wistar rats were fed a low-salt (LS - 0.15% NaCl) or normal-salt (NS - 1.3% NaCl) diet since the first day of gestation until delivery or LS during the first (LS10) or second (LS20) half of gestation. Body weight, food and water intake were weekly evaluated during gestation. Blood glucose, insulin (ITT) and glucose (GTT) tolerance tests, HOMA-IR were performed in adult offspring. Gene expression and DNA methylation were mapped using bisulfite treatment evaluated by pyrosequencing in the male and female neonates and adult offspring. Weight gain was lower in LS and LS20 dams than in NS and LS10 dams in the third week of pregnancy. Birth weights were lower in male and female LS20 and LS rats compared with NS and LS10 neonates. HOMA-IR was higher in 12-week-old LS males compared with NS and in 20-week-old male LS10 rats compared with NS and LS20 rats. In 12-week-old LS10 females, HOMA-IR was higher than in LS. Serum insulin levels were higher in 20 week-old LS10 male compared with NS rats and in 12-week-old LS10 female compared to LS rats. The area under the curve of GTT indicated glucose intolerance in 12- and 20-week-old LS male. Methylation of CpG islands of the Insr, Igf1, Igf1r, Ins1 and Ins2 genes in liver in neonates male and female offspring and liver, white adipose tissue and muscle in 20-week-old male offspring were influenced by low-salt intake during pregnancy. None of these alterations was identified in 20-week-old females. In conclusion, low-salt diet consumption in the second half of pregnancy can result in low birth weights in the males and females offspring. Glucose intolerance observed in adult offspring occurred only if low salt intake was given throughout pregnancy. However, insulin resistance in response to low salt intake during pregnancy is related to the time at which this insult occurs and to the age of the offspring. Alterations in the DNA methylation of Igf1 were observed to be correlated with low birth weight in response to low salt feeding during pregnancy

Genetic predisposition to spontaneous preterm birth:approaches to identify susceptibility genes

Abstract Approximately 5.5% of all infants are born preterm (before 37 completed weeks of gestation) in Finland. Preterm birth is the cause of several life-threatening neonatal diseases and long-term morbidity. The most important risk factor for preterm birth is intrauterine infection and inflammation. Approximately 70% of preterm births have a spontaneous onset. Evidence suggests that genetic factors are involved in spontaneous preterm birth (SPTB), but knowledge about the actual genes conferring genetic predisposition is limited. The major aim of this work was to identify genetic factors that predispose to SPTB. Genome-wide linkage analysis was performed to identify genomic regions associating with SPTB in large northern Finnish families recurrently affected by SPTB. Genes near regions with linkage signals were subsequently analyzed in a Finnish case-control population of mothers and infants. Due to their roles in innate immunity, the genes encoding surfactant protein A (SP-A), SP-C, SP-D and mannose-binding lectin (MBL) were also investigated as candidates for SPTB in this population. In addition, expression of SP-C in human and mouse gestational tissues was examined. Linkage signals were detected on chromosome loci 15q26.3, Xq13.1 and Xq21.1 with the phenotype of being born preterm. In subsequent association analyses, the genes encoding the insulin-like growth factor 1 receptor (IGF1R) located within locus 15q26.3 and the androgen receptor (AR) located near locus Xq13.1 were identified as potential novel fetal SPTB susceptibility genes. These genes did not associate with SPTB in the mothers. An association was found between the Met31Thr polymorphism of the SFTPD gene encoding SP-D and SPTB in the infants. There was no association in the mothers. Polymorphisms of the genes encoding SP-A or MBL did not associate with SPTB. The Thr138Asn polymorphism of the SFTPC gene encoding SP-C did not associate with SPTB. However, this polymorphism associated strongly with the interval between preterm premature rupture of membranes and SPTB in the fetuses. Expression of SP-C was detected in human and mouse fetal membranes and placenta, and in mouse pregnant uterus. Currently, there is no effective method to prevent SPTB. The results of this study may help to clarify some of the biological mechanisms underlying SPTB and finally allow the development of specific treatment strategies for its prevention. / Tiivistelmä Suomessa syntyy noin 5,5 % lapsista ennenaikaisina eli raskauden kestettyä vähemmän kuin 37 täyttä viikkoa. Näillä lapsilla on alttius hengenvaarallisiin sairauksiin, ja osalle heistä jää pysyvä kehitysvamma. Noin 70 % ennenaikaisista syntymistä käynnistyy spontaanisti. Tärkein ennenaikaisen syntymän riskitekijä on kohdunsisäinen tulehdusreaktio. Myös perinnöllisten tekijöiden tiedetään vaikuttavan spontaanin ennenaikaisen syntymän (SEAS) käynnistymiseen, mutta alttiusgeenejä tunnetaan huonosti. Työssä pyrittiin tunnistamaan SEAS:lle altistavia perinnöllisiä tekijöitä. Perimänlaajuista kytkentäanalyysiä käyttäen etsittiin SEAS:ään liittyviä perimän kohtia tutkimalla toistuvasti ennenaikaisia syntymiä kokeneita isoja pohjoissuomalaisia perheitä. Kytkentäsignaalien lähellä olevia geenejä tutkittiin tutkimusaineistossa, joka koostui suomalaisista äideistä ja lapsista. Surfaktanttiproteiini A:ta (SP-A), SP-C:tä, SP-D:tä ja mannoosia sitovaa lektiiniä (MBL) koodaavia geenejä tutkittiin ehdokasgeeneinä SEAS:lle tässä populaatiossa, koska nämä proteiinit osallistuvat elimistön puolustukseen ja voivat siten vaikuttaa SEAS:ään liittyvään tulehdusreaktioon. Lisäksi tutkittiin SP-C:n ilmentymistä ihmisen ja hiiren sikiökalvoilla, istukassa ja kohdussa. Kytkentäsignaaleja havaittiin kromosomikohdissa 15q26.3, Xq13.1 ja Xq21.1, kun tutkittavana ilmiasuna oli ennenaikaisena syntyminen. Lisätutkimukset osoittivat, että sikiön insuliininkaltaisen kasvutekijän 1 reseptoria koodaava IGF1R-geeni (kohta 15q26.3) ja androgeenireseptorigeeni AR (lähellä kohtaa Xq13.1) ovat mahdollisia uusia SEAS:n alttiusgeenejä. Nämä geenit eivät selittäneet SEAS:ää äideissä. Sikiön SP-D:tä koodaavan geenin Met31Thr-polymorfismi tunnistettiin mahdolliseksi riskitekijäksi, mutta tämä polymorfismi ei selittänyt SEAS:ää äideissä. SP-A:ta ja MBL:ää koodaavat geenit eivät liittyneet SEAS:ään. SP-C:tä koodaavan geenin Thr138Asn-polymorfismi ei ollut yhteydessä SEAS:ään. Sikiön Thr138Asn-polymorfismi liittyi kuitenkin vahvasti sikiökalvojen puhkeamisen ja SEAS:n väliseen kestoon. SP-C:n havaittiin ilmentyvän ihmisen ja hiiren sikiökalvoilla ja istukassa sekä raskaana olevan hiiren kohdussa. Tulokset antavat uutta tietoa SEAS:n perinnöllisestä taustasta. Tämä tieto voi auttaa selvittämään sen käynnistymiseen johtavia biologisia mekanismeja ja johtaa lopulta uusiin hoitokeinoihin, joilla pystytään estämään spontaaneja ennenaikaisia syntymiä.

Insulin-like growth factors 1 and 2 (IGF-1/2) are single-chain peptides exerting homology (in both amino-acid sequence and tertiary structure) to insulin. The main function of these peptides is promoting celular growth, proliferation and differentiation. Both insulin and insulin-like growth factors mediate their function through membrane receptors - insulin receptor (isoforms A and B) and IGF-1 receptor. All these receptors are members of the tyrosinkinase family of receptors and they exert the same subunit and domain composition. The activation of insulin and IGF-1 receptors is tightly associated with activation of two intracellular signaling pathways. The PI3-K/Akt pathway is involved in the glucose transport to the cell, induction of proliferation or inhibition of apoptosis, while the Ras/MAPK pathway is involved mainly in the induction of cell growth and differentiation. Due to the structure similarity in both the ligands and receptors, every ligand can activate different receptors (with different potency) and the signaling pathways associated with these receptors. Thus, the functions of IGFs and insulin, the same as their receptors, are overlapping.
