Mining patient journeys from healthcare narratives

The aim of the thesis is to investigate the feasibility of using text mining methods to reconstruct patient journeys from unstructured clinical narratives. A novel method to extract and represent patient journeys is proposed and evaluated in this thesis. A composition of methods were designed, developed and evaluated to this end; which included health-related concept extraction, temporal information extraction, and concept clustering and automated work-flow generation. A suite of methods to extract clinical information from healthcare narratives were proposed and evaluated in order to enable chronological ordering of clinical concepts. Specifically, we proposed and evaluated a data-driven method to identify key clinical events (i.e., medical problems, treatments, and tests) using a sequence labelling algorithm, CRF, with a combination of lexical and syntactic features, and a rule-based post-processing method including label correction, boundary adjustment and false positive filter. The method was evaluated as part of the 2012 i2b2 challengeand achieved a state-of-the-art performance with a strict and lenient micro F1-measure of 83.45% and 91.13% respectively. A method to extract temporal expressions using a hybrid knowledge- (dictionary and rules) and data-driven (CRF) has been proposed and evaluated. The method demonstrated the state-of-the-art performance at the 2012 i2b2 challenge: F1-measure of 90.48% and accuracy of 70.44% for identification and normalisation respectively. For temporal ordering of events we proposed and evaluated a knowledge-driven method, with a F1-measure of 62.96% (considering the reduced temporal graph) or 70.22% for extraction of temporal links. The method developed consisted of initial rule-based identification and classification components which utilised contextual lexico-syntactic cues for inter-sentence links, string similarity for co-reference links, and subsequently a temporal closure component to calculate transitive relations of the extracted links. In a case study of survivors of childhood central nervous system tumours (medulloblastoma), qualitative evaluation showed that we were able to capture specific trends part of patient journeys. An overall quantitative evaluation score (average precision and recall) of 94-100% for individual and 97% for aggregated patient journeys were also achieved. Hence, indicating that text mining methods can be used to identify, extract and temporally organise key clinical concepts that make up a patient’s journey. We also presented an analyses of healthcare narratives, specifically exploring the content of clinical and patient narratives by using methods developed to extract patient journeys. We found that health-related quality of life concepts are more common in patient narrative, while clinical concepts (e.g., medical problems, treatments, tests) are more prevalent in clinical narratives. In addition, while both aggregated sets of narratives contain all investigated concepts; clinical narratives contain, proportionally, more health-related quality of life concepts than clinical concepts found in patient narratives. These results demonstrate that automated concept extraction, in particular health-related quality of life, as part of standard clinical practice is feasible. The proposed method presented herein demonstrated that text mining methods can be efficiently used to identify, extract and temporally organise key clinical concepts that make up a patient’s journey in a healthcare system. Automated reconstruction of patient journeys can potentially be of value for clinical practitioners and researchers, to aid large scale analyses of implemented care pathways, and subsequently help monitor, compare, develop and adjust clinical guidelines both in the areas of chronic diseases where there is plenty of data and rare conditions where potentially there are no established guidelines.

The twentieth-century brought a rapidly increasing global mobility and is causing a de-territorial effect on the world. The borders of countries are becoming more fluid. The bounds of cultures that were based on nationality, have become blurred, contested, open-ended, and unstable. These frequently modified cultural boundaries have created multiple cultural diasporic groups. A diaspora is a large group of people with a similar heritage or homeland who have since moved out to places all over the world. Globalism, with its elusive cultural identity, leads to a seemingly unified world culture and the once static sense of nation-state-based cultural identity is now mobile. This mobility and replacement of our locations destabilize our traditional sense of identity that traditionally has been deeply rooted in a sense of nation-state. As a reaction and resistance to the global forces, “localism,” or “nativism,” have simultaneously increased. Thus, the cultural diaspora can be understood as a journey through multiple magnitudes of cultural boundaries. This thesis reviews cultural identity with an emphasis on cultures that are undergoing a diasporic condition. I specifically emphasize notions of the nationality of a selected diasporic group of Bangladeshi people living in New York City over the past few decades. The vehicle of the research is the study of their current cultural identification, considering the varied struggles of this group in their new host land. After assembling and acquiring a holistic understanding of the current condition (economic, social, and political) of this group, a set of appropriate programs will be proposed to be incorporated into the design for a cultural center. The primary goals of this project are to encourage the socio-cultural, economic, and educational enhancement of Bangladeshi people living in New York City. The project will also raise a sense of unity among the diasporic group and enable a better understanding of cultural interchange.

Rosa Montero's novels Lágrimas en la lluvia and Peso del corazón focus on Bruna Husky, an android private detective living in Madrid in 2109. Husky's career leads her into investigating various sinister plots, both of which she falls victim to. These quests lead her to make discoveries about herself that challenge her beliefs about herself. All the cases that Bruna investigates take her on journeys that mirror Campbell's structure of the monomythic quest. My analysis of the novels focuses on how Bruna follows the model of the Campbellian cycle while at the same time questioning the quest, keeping true to archetypes of the feminine quest as established by Anis Pratt and Dana Heller. My article focuses on three distinctive steps in the quest, the crossing of the threshold, initiation, and the return, as well as the most important characteristic of the feminine quest, the heroine's negation of the patriarchal society.

This diploma thesis is devoted to a central motive of a journey home in the sense of returning to the roots in two books by Czech authors writing in German, Jaroslav Rudiš and Libuše Moníková. These are the novels Winterbergs letzte Reise and Zjasněná noc. The first part of the thesis is theoretical and deals with space as a narratological category. The findings are drawn mainly from the publication Narratology: strukturální analýza vyprávění. Within the theory there is also an introduction to topology listed as a literary discipline, drawn notably from two publications of the literary theorist Daniela Hodrová, Místa s tajemstvím and Poetika míst: kapitoly z literární tematologie. In addition to the history of this science, more details are given about the four important toposos, which are often part of the narratives. Specifically these are the topos of the city, room, road and Czech pub. The first part of the diploma thesis includes factual information about Moníková and Rudiš, including a brief outline of content of both analyzed novels. The second part of the thesis is called analytical, because it analyzes space in both novels. Is it time for a Social Media Detox? Understanding the journey of intermittent discontinuance of Instagram among Gen Y

Background: Even though the use of social media is one of the most popular activities of our time a decline in the number of active users can be observed at present. Users discontinue using social media platforms temporarily or stop the usage permanently. Thereby, the trend to do a social media detox has emerged, meaning users consciously take a break from social media platforms like Instagram, often for a set period of time. Instagram is one of the most widely used social media platforms of the younger generation, especially Generation Y, and is also noted for its addictive potential. Purpose: Previous studies and scholars have primarily concentrated on the permanent discontinuance of social media, whereby users permanently stop using social media platforms without actively planning to return. Thereby, the focus of the research was often on the social media network Facebook. By studying the journey of intermittent discontinuance of Instagram, the purpose of this research study is to explore Generation Y’s motivation before, experiences and feelings during and post-behavior after a 14-day detox by holistically representing the Instagram Detox Journey. Method: To fulfil the purpose of this thesis being of exploratory nature, a qualitative research design was applied. Empirical data were collected over 4 weeks, beginning with a focus group discussion before, followed by diary research during and 15 in-depth semi- structured interviews after the detox, and concluding with another focus group discussion at a later time. Using an inductive approach, the data was decoded, analysed, and interpreted using thematic analysis. Conclusion: The findings of the study show two main motivations before starting the detox: gaining back control and self-reformation. During the detox, five final themes were identified: mood &amp; satisfaction, technological craving, mental well-being, physical well- being, and social relationships. Lastly, three different post-behaviors after completion were noted: the boomerang effect, conscious consumption, and an attitude change. The findings led to the development of the conceptual framework “Instagram Detox Journey”.

Embrace the Experience : A multiple case study of customer experience strategies within the Swedish retail market

The concept of customer experience has an increasing interest among organizations today since customers have become picky and demands for an experience are an existing fact. In the early 2000s, a paradigm shift was found, where focus from a goods-dominant logic was moved to a service-dominant logic which came to have a great impact on how to manage customer experiences today. Digitalization in society has increased which has created both opportunities and difficulties for organizations. A new norm has emerged where organizations implement an omnichannel strategy. In turn, omnichannel marketing can improve the customer experience. However, omnichannel marketing makes the customer journey highly individual and more complex to understand. Therefore the purpose of this thesis is to explore how Swedish retailers on a strategic level manage the customer experience and if there are any differences in the strategies between price segments. The empirical findings were conducted using a qualitative multiple case study of five cases in the Swedish retail sector. The data was gathered from semi-structured interviews and was analyzed and compared to existing theory.  The conclusion of this thesis shows customer insights as a common theme that emerges frequently throughout the semi-structured interviews. Through customer insight, retail organizations have a chance to meet their customers’ demands and face the complexity that occurs with technology and new channels. In the conclusion, there are also found differences in how to manage customer experiences between price segments. A common factor among the cases within customer experience is quality. Although, quality has a different meaning depending on which price segment. Further, the quality can be staged into three levels connected to each price segment.

Getting Fit From The Living Room : A qualitative study on how the digitalization of fitness services have impacted the customer experience

Purpose The purpose of this study is to get insights in how the digitalization of fitness services has affected the customer experience, this with a focus on fitness services such as dance classes and PT-sessions online.  Theoretical background The theory has its foundation in both the customer journey where different touchpoints are perceived and the factors affecting the customer experience. Many studies have been done in the subject of customer experience and five factors are categorized. First, the cognitive factors then the affective factors, sensory factors, social factors and last the conative factors.  Methodology The study employs a qualitative research type with a cross-sectional research design. Primary data are collected through semi-structured interviews with four customers and one gym. Empirical findings and analysis Multiple differences and similarities between the digital and physical experience are presented. For example, physical gyms have more equipment than all of the respondents have in their home. At the same time, it can be a disadvantage that the internet connection and digital tools are required at the digital classes. However, the flexibility is higher when the classes are online, both because of the accessibility and the higher ability to choose time and place for the workout.  Conclusion The digitalization of fitness services has affected the customer experience, both positive and negative. To what extent the change is positive or negative are based on personal preferences. For instance, some customers enjoy the lack of social interaction while some miss is, some may like the smell of the gym while some prefer the smell of their own home. Therefore, the ability to choose in the future is recommended and the gyms do need to work on the optimalization of the experience both for their digital and physical customers.
