Reasons of early sexual debut of “female” adolescents attending Town-Two Clinic in Khayelitsha, South Africa

Thesis (MPhil)--Stellenbosch University, 2015. / ENGLISH ABSTRACT: Introduction: There is an increase in the prevalence of HIV among young females compared to their male counterparts in South Africa (Shisana, 2012). Previous studies have shown early sexual debut is one of the contributing factors to acquiring HIV. Early sexual debut being defined as first sexual intercourse below the age of 15 years as illustrated in the literature (Aji et al., 2013). The aim of the study is to determine the contributing factors to early sexual debut by female adolescents in the ages between 13-19 attending Town-Two Clinic. To establish the knowledge of adolescent females about HIV & STI transmission, to evaluate what sexual and reproductive health education and management is required by adolescents attending the clinic, and to provide guidelines to deal with the concerns and needs of female adolescents attending Town-Two Clinic as well as reproductive health education. Methods: This is an exploratory qualitative study. Twenty female adolescents between the ages 13-19 attending Town Two Clinic for family planning were voluntarily recruited. Data collection was done by means of a 20 minute interview; two participants were interviewed a day. The interview was semi-structured using open-ended questions. Socio-demographic data was obtained by means of a questionnaire; linked to the interviewee by means of a study code to maintain anonymity. Data analysis was done using inductive analysis and creative synthesis, which analyses the details of the information collected to discover important patterns, themes and interrelationships (Larry, 2014). Results: The average age of the participants was 16 years. The youngest and eldest was 13 and 19 years old respectively; average grade was grade10. Majority of participants resided with their mothers (35%), 20% lived with both parents, 20% with their elder siblings, 15% with grandparents and 5% with other relatives. Participants (45%) reported they were sexually active; average age at first sexual intercourse was 14 years. The participants’ acceptable age for sexual debut was 18yrs. Reasons contributing to early sexual debut were identified as: peer pressure, easy access to alcohol, transaction and cross-generational sex, media and social networks, crowded living conditions, sexual abuse and lack of parental supervision and communication with children. The participants (95%) said they received sex education at school;. (65%) reported they were taught about safe sex practices and using a condom every time they have sex. Knowledge gaps; (60%) of the participants reporting they would like to receive more information and teaching about STI’s, as they knew very little about them. Conclusion: The study demonstrates the contributing factors to early sexual debut amongst female adolescents as: Peer pressure, easy access to alcohol, transaction and cross-generational sex, media and social networks, crowded living conditions, sexual abuse and lack of parental supervision and communication with children, and its risk to HIV infection. Appropriate and current sex education with regards HIV, STI prevention, testing and treatment is crucial. Parents play a vital role in educating their own children and need to overcome their socio-cultural views about sex. Interdisciplinary collaboration is needed between the health sector, education, law enforcement, church youth groups, media and social networks in developing effective programs for educating youth on HIV prevention, reproductive health and dangers of substance abuse. / AFRIKAANSE OPSOMMING: Nie beskikbaar

Nurturing democratic virtues: a case study of a primary school in Khayelitsha

The study sought to explore the priorities and practices of educators with regard to the mediation of moral qualities/democratic virtues related to education for citizenship by means of a case study in one school. In South Africa educators are regarded as among the stakeholders responsible for the mediation of values and morals in children and youth so that they will be effective citizens of democracy. Hence the school is seen as an important context in which this could take place. Citizenship education usually contains a moral element. Practices of moral development generally draw on the work of Kohlberg, which is linked to a Piagetian understanding of development. Vygotsky&rsquo / s approach focuses on the contribution that the context has on cognitive<br /> development and suggests that this may also be important for moral development. However there is little evidence available concerning the effectiveness of specific interventions. Two groups of educators and one group of learners were interviewed.<br /> Educators were asked what moral qualities/democratic virtues they regarded as important to nurture in the learners at this school and what they did to nurture these virtues. Learners were also asked the same question and what they saw their educators doing in order to mediate these moral qualities/democratic virtues. Any constraints and successes were explored together with what educators thought could be done to improve the situation. The theoretical understanding of the research was constructivist and it followed a qualitative case study approach. Data were collected using semistructured interviews and were analysed using a thematic analysis approach. Before the research was undertaken permission was sought from the Western Cape Education Department and the study was conducted following the guidelines for ethical research set by the American Psychological Association. The main priorities that emerged as moral qualities/ democratic virtues that educators thought were important to nurture, were accountability, respect, and commitment to &ldquo / being good.&rdquo / Socially desirable behaviours that were highlighted were the following: economic initiative, citizenship, and politeness and consideration. Cognitive qualities that they thought were important to nurture were the following: open mindedness, literacy, and numeracy. There were also skills that the educators thought were important to nurture in their learners which did not seem to have a moral dimension. The main practices mentioned were: modelling, active instruction and discussion, and discipline. Educators highlighted the importance of consistency throughout the school. Constraints mentioned were the following: lack of discipline, socio-economic factors, and lack of parental involvement. Supportive factors mentioned were parental support, and consistency and collaboration.

A qualitative exploration of the complexities in agenda-setting and participation processes in sanitation services in Site C, Khayelitsha: 2010-2013

Masters in Public Administration - MPA / "Sanitation is dignity" as the state has proclaimed while water is life. Yet to date, there are families and communities that are still dreaming that one day their dignity will be restored and they will have access to proper toilets, clean water that are within close proximity including proper houses. In the 24th year of democracy, people in South Africa are still protesting and challenging government to address the inequalities of the past and reset the agenda of change. These persistent protests are about basic needs and service delivery, but increasingly protestors are invoking the concept of relative needs, dignity and human rights and taking protests to the powerful and wealthy. In fact, the idea of the state as sacrosanct has been deflated since protestors throw poo at state officials and vandalise state infrastructure. The “poo wars” that broke out in 2012 with poo dumped at the airport and government buildings continued with the dumping of excrement on the Rhodes statue at UCT shows that the poor can sometimes set the agenda of change and force politicians to listen. Among the defensive responses raised by authorities is that people put their shacks on private land or pieces of land that are not suitable for housing (wetlands). Politically, there are complex issues in the Western Cape, both the Province and the City of Cape Town Metro are Democratic Alliance (DA) run whilst national government is ruled by the African National Congress (ANC). The majority of townships residents are ANC supporters with a few DA Proportional Representative (PR) councillors. This study looks at a qualitative exploration of the complexities in agenda-setting and participation processes in sanitation services in Site C, Khayelitsha between 2010-2013.Residents see agenda setting and engagements as unilateral, as this study found. It is designed into six chapters. The study was designed in a manner that it would reflect the knowledge and understanding the notion of consultation, community participation in decision-making, agenda-setting and implementation of projects or programmes by the people of Khayelitsha-Site C, Councillors, Shopstewards and officials of the City of Cape Town. Over 20 interviews were completed. A key finding is that by taking poo out of its usual place, taking it out of the private into the public domain and to the rich and by invading their space, the issues of the poor are no longer confined to ghetto townships. Boundaries between state and civil society have become porous. Cape Town’s poor residents using portable toilets commonly known as "pota-pota", and also the temporary toilets commonly known as ‘Mshengu’ have argued that these interim services are not only poorly maintained and dirty but are vastly inferior compared to white areas.

Urban agriculture a livelihood strategy for food security in the Cape Flats: A case study of community-based and home food gardens in Khayelitsha, Cape Town

Magister Artium (Development Studies) - MA(DVS) / Growing urban food insecurity has prompted many researchers, NGOs, international agencies and governments to advocate for urban agriculture as a livelihood strategy to improve the household food security of the urban poor. Urban agriculture is an instrument for ensuring greater food security and a livelihood strategy for urban households. In South Africa increased attention on urban agriculture is triggered by current trends of urbanization, economic instability, high unemployment rates among the urban poor, and high food prices. Unemployment and urban food insecurity are high in low-income areas. In the Cape Flats, households with no or little disposable income, are food insecure and vulnerable to food insecurity. Income and wage employment are the main determinants of food security in urban areas. However, urban agriculture projects by two NGOs assist communities to be resourceful. Abalimi Bezekhaya, an NGO assists individuals and communities to start and maintain their own community gardens while Soil for Life promotes home food gardens. Abalimi Bezekhaya and Soil for Life seek to address the urban challenge by promoting self-sustained agriculture for food security and livelihoods. There has, however, been little empirical evidence suggesting that urban agriculture projects improve the food security and livelihoods of participants. This study assessed the potential of urban agriculture to address food security, examined the ways in which agriculture is used as a livelihood strategy for household food security, determined other livelihood strategies and coping mechanisms assumed by gardeners to become food secure, and demonstrates the contribution of NGOs in promoting agriculture in poor urban areas. Furthermore, this study addressed the following research questions: do community and household gardens provide a way of improving food and nutrition security and in what way are these impacts observable within participating households. The research followed a mixed-method methodology. The literature is mapped out using international and local papers and empirical evidence collected on the subject. This study used the sustainable livelihoods approach as the theoretical lens through which to analyse the ways in which urban agriculture can be used as a viable livelihood strategy by urban gardeners. It also classified the constraints and opportunities, assets accessible, policies and institutions that exist, livelihood strategies and outcomes of the urban gardeners. The findings of the study reveal that community and home gardens contribute moderately to livelihoods and food security in Khayelitsha. The results also reveal that 85% of the gardeners were either moderately or severely food insecure. Furthermore, 76.67% of gardeners purchased their food from supermarkets and local shops. Urban agriculture therefore plays a supplementary role in addressing household food security in Khayelitsha. The potential of community and home gardens to contribute to urban household food security and livelihoods is limited access to land and government assistance. There is a need for the City of Cape Town and the Department of Agriculture to assist and strengthen the practice of community and home gardens in Khayelitsha.

Assessing the role of youth civic engagement in promoting social change : a critical investigation of ILISO in site C, Khayelitsha

Magister Artium (Development Studies) - MA(DVS) / Youth Civic Engagement is not a new phenomenon in South Africa. Throughout the history of country, spanning from the early resistance to colonialism in 1652 to the formation of the African National Congress in 1912 and its Youth League in 1944, the Soweto uprising in 1976, right up to the 1994 independence struggle, the youth has always played a pivotal role in social transformation. Unlike in the past where there was a common enemy in Apartheid, today the country faces a more complex set of socio-economic challenges. Despite being a middle income country, South Africa grapples with extreme poverty and income inequality, which impacts on educational opportunities and ultimately civic awareness and involvement. Approximately 42% of young people under the age of 30 are unemployed. The country currently has a youth population (14-35 years of age) which is about 41% of its entire population of almost 54 million. This youth population growth in itself implies that youth development should be a major priority area if growth and development are to be realised. The research is based in Khayelitsha. The social-economic problems faced among Khayelitsha township youth are complex and multidimensional in nature. The research question is: how is youth civic engagement able to initiate and foster collective action among community members of Site C in Khayelitsha, in order to promote social change? ILISO Care Society, a Community Based Organisation based in Site C was used as a case study for the research. In line with the theory and conceptual framework of social capital, the study demonstrates how reciprocal relations, trust and strong bonds, act as seedbeds for collective action. The Integrated Model of Communication for Social Change is incorporated into the framework to narrow down the social capital theory to an operational level. It is also used to illustrate how novel methods of dialogical communication adopted by ILISO Care Society reinforce social learning and promote democratic practices among young people. Both the quantitative and qualitative approaches were used for the study, with much of the analysis being grounded in qualitative methods. Data collection was done by means of the following utilities: a survey questionnaire which was administered among 52 respondents, semi- structured interviews, focus group discussions and participant observation by the principle researcher. The study revealed how the legacy of apartheid’s segregation policies have had enduring effects particularly on the education system, in turn, negatively impacting on youth civic participation, as well as other interlinked spheres of society. Most importantly, the findings revealed that the ILISO youth civic engagement projects have contributed in increasing the level of confidence (efficacy) to solve community problems of not only the ILISO project members, but also the wider Site C community. This was evidenced in the research participants‟ own belief in their ability to produce change (self-efficacy) and the ILISO youth members‟ shared belief as a group, in their ability (collective self-efficacy) to bring about social transformation. This has led to Site C youth acting collectively (collective action)when faced with challenges, thereby promoting social change.

Local solutions from local people : community participation in crime prevention in Khayelitsha

Magister Artium (Development Studies) - MA(DVS) / The involvement of local communities in crime prevention programmes emerged as an alternative strategy for fighting and preventing crime after the failure of the criminal justice system to control and deter criminal activities effectively. Governments across the globe regard local communities as key actors in fighting and preventing crime. Community participation in crime prevention has become a key strategy to improve safety and security. The main aim of this study is to explore the extent to which residents of Khayelitsha contribute to the maintenance of security and order in their area; and to investigate the extent to which residents are empowered to solve crime problems on their own. The framework of this study is grounded on theories of crime namely: occupational choice, social learning, and social disorganisation; and concepts such as crime, crime prevention, and community participation. The literature review of this study focuses crime situation in South Africa with emphasis on crime trends, costs of crime, determinants of crime, and attempts made by the South African government to fight and prevent crime at national and local government levels. The study used key informant in-depth interviews with representatives of anti-crime community-based organisations in Site B and as well as ordinary residents of Site B. Data of this study is largely qualitative although it is supplemented with quantitative data relating to crime statistics which was collected as secondary data. In this study, both narratives and crime statistics reveal that robbery and theft-related crimes, drug abuse, and assaults are among the most predominant crimes. All informants perceived poverty and the use of drugs and alcohol as root causes of crime in Site B. The residents contribute in preventing and fighting crime in Site B by engaging in the following activities: patrolling streets as volunteers in Community Policing Forums (CPF); and providing crimerelated information to anti-crime organisations such as South African Police Service (SAPS), South African National Civic Organisation (SANCO), and Khayelitsha Development Forum (KDF). The residents also get involved in crime prevention informally by exercising informal social sanctions. However, challenges such as ineffectiveness of the law enforcement and lack of financial support hinder the residents’ participation in crime prevention.

The pattern, frequency and suitability of informal day care provision for pre-school children in Khayelitsha

Bibliography: leaves 265-271. / The study was designed to investigate the structure and function of informal day care provision for pre-school children in Khayelitsha, but the inquiry succeeded in generating information beyond the original assignment. Data was gathered from interviews with active in the day care field and from local agencies meetings with residents in Khayelitsha, as well as from a field study carried out in Khayelitsha. A review of international and local literature was also undertaken. A brief history of black settlement in the Western Cape and an examination of their socio-cultural environment provided the necessary backdrop for the study. The field study involved systematic selection of 200 houses in Khayelitsha. Respondents completed a questionnaire administered by the investigator. The questionnaire furnished information on the use of day care and produced a profile of day carers and the services they offered. A similar questionnaire was used to collect information from the pre-school centre. Analysis of the data revealed that child rearing practices differed from those of technologically advanced societies, but they were not deficient! The findings demonstrated that parents preferred day care in the home setting to that of the school setting. The pattern of day care services accentuates the importance of mutual aid and kinship and social networks in the community. Day care arrangements tended to be stable and were provided predominantly by relatives, friends and neighbours. The frequency with which day care services were required, corresponded to the work commitments of the parents, and usually involved overnight care. The pre-school centre operated during the weekdays from 7.30 am to 4.30 pm, with after-school care frequently undertaken by older siblings. The findings reveal that day carers function as surrogate mothers and incorporate the children into their families. The data collected with regard to suitability of the service offered, relates to physical needs, emotional needs, discipline and promoting readiness for the future, and emphasizes the universal poverty that abounds in the area, but simultaneously highlights the importance of traditions and the resourcefulness of the people in transcending the culture of poverty and providing an enriching environment for the children. The recommendations draw attention to the need for finance, training and supportive services, but recognises the justified resistance of the community to initiatives from the State.
