Legitimation för hälso-och sjukvårdskuratorer - Kompetenshöjande eller kontrollerande funktion? : En tematiskm analys av offentliga dokument bakom införandet av hälso-och sjukvårdkuratorslegitimation ur ett professionsteoretiskt perspektiv

Denna examensuppsats ämnade att teoretiskt undersöka offentliga dokument bakom införandet av hälso-och sjukvårdskuratorslegitimationen. För att besvara syftet utgick studien från frågeställningarna: Hur motiveras införandet av hälso-och sjukvårdskuratorslegitimation? Hur skulle motiveringarna kunna inverka på kuratorer inom hälso-och sjukvården som profession? Datamaterialet bestod av Socialstyrelsens rapport från 2014, samt propositionen 2017/18:138, vilka låg till grund för riksdagsbeslut om införande av hälso-och sjukvårdskuratorslegitimation. Dokumenten analyserades med tematisk analys för att synliggöra mönster i motiveringarna bakom införandet. Resultatet analyserades utifrån de professionsteoretiska begreppen professionalism och legitimation. Analysen av resultatet visar att legitimation är ett sätt att stärka kuratorers auktoritet i en kontext som domineras av ett biomedicinskt perspektiv. Däremot är legitimationens främsta syfte att stärka patientsäkerheten genom statlig kontroll i form av specialiserad vidareutbildning och möjligheter till tillsyn av professionen. Eftersom legitimationen inte skyddar legitimerade hälso-och sjukvårdskuratorers arbetsuppgifter sker ingen stängning av yrket. Legitimationen får snarare en funktion som en eftersträvansvärd yrkesidentitet för professionen, denna strävan bidrar till att yrkesverksamma och arbetsgivare agerar utefter legitimationens riktlinjer. Därmed sker en indirekt styrning av professionen genom införande av legitimation, där rädslan för att förlora legitimationen kan ha en negativ inverkan på interaktionen mellan patienter och hälso-och sjukvårskuratorer. / The aim of this study was to do a theoretical analysis of official documents behind the introduction of licensure for healthcare counselors. To answer the purpose, the study was based on the following questions: How was the introduction of health care counselor’s licensure justified? How might the motivations affect counselors in health care as a profession?The selection of documents that were analysed consisted of the National Board of Health and Welfare's investigation into the introduction of identification from 2014, as well as the bill 2017/18:138 which was the basis for the actual decision the Swedish parliament made on the introduction of licensure. The documents were analysed using a thematic analysis to map the arguments for the inclusion of licensure for healthcare counselors. The empirical evidence was then analysed based on the professional theoretical concepts of professionalism and legitimation.The result was analysed based on the theoretical concepts of professionalism and legitimation. The analysis of the results shows that licensure is a way to strengthen healthcare counselors’ authority in a context that is dominated by a biomedical perspective. On the other hand, the main purpose of the licensure is to strengthen patient safety through governmental control by specialized tertiary education and supervision of the profession. Since the license does not protect the work tasks of healthcare counselors, no occupational closure of the profession has been made. Instead, the licensure functions as an aspirational professional identity for the profession, this aspiration contributes to professionals and employers acting according to the guidelines of the licensure. Thereby, an indirect control of the profession takes place through the introduction of licensure, where the fear of losing the licensure might have a negative impact on the interaction between patients and healthcare counselors.

Medication errors cause harm, yet most of them are preventable (Institute of Medicine, 2006). Nurses spend 40% of their time administering medications; therefore they play a key role in the reduction of medication errors. Little empirical evidence has been collected about the effectiveness of nursing education in reducing medication errors committed by nursing students. Traditional educational interventions focus on the five rights of medication administration; however, the literature shows that interventions focused on instilling a culture of safety have a greater impact on reducing medication errors. The purpose of this article is to review educational strategies that have been implemented and tested in pre-licensure nursing programs to reduce medication errors committed by nursing students.

Considering Stress in a Nursing Student Context: Pre-Admission to Pre-Graduation

In moderation, stress is a normal response to a perceived challenge which can motivate an individual to perform at their best. Nursing students consistently report a significant amount of stress which has been found to be greater than their non-nursing peers. The purpose of this study is to assess the level of stress reported by second-semester freshman and sophomore students who have declared nursing as a major, compared with the level of stress experienced by junior and senior students who are enrolled in the university's nursing program. Through this comparison, a conclusion can be drawn as to the level of stress experienced while enrolled in a nursing program, with the level of stress prior to being exposed to the challenges presented by the program. It is intended that the outcomes from this study can be utilized to address stress as it relates to a nursing student from pre-admission through graduation.

The Competency Pipeline: Examining the Association of Doctoral Training with Early Career Outcomes

Participants from earlier nationwide studies on predictors of internship match were contacted 7-10 years after obtaining their doctoral degree to gather additional data concerning their attained early career competencies and benchmarks (e.g., scores on the national licensing exam). In this sample (N = 190), licensure exam scores were significantly positively associated with scores obtained on the Graduate Record Examination (GRE), family of origin socioeconomic status, and student debt load. However, obtained licensure scores were not found to be significantly associated with any pre-doctoral training variables (e.g., intervention/ assessment hours, number of integrated reports, number of publications, rank order of matched internship site) or most post-doctoral early career activities. Weak positive associations between licensures scores and engaging in teaching / supervision / consultation services were observed. Few self-reported early career competencies were found to be weakly positively associated with scores on the national licensing exam (i.e., integrity, seeking supervision, scientific mindedness, evidence-based practice). Significantly inverse associations were found between national licensing exam scores and self-reported competencies in the areas of management and systems change. Findings are discussed and implications for the national licensing exam considered.

Variation in the Willingness of Superintendents to Recommend Hiring Alternatively Licensed Principals

In many parts of the country principal candidates are being licensed through alternative pathways. Some view this movement as a plausible solution to the shortage of principals and inadequacy of principal preparation programs (Hess, 2003; Southern Regional Education Board, 2006; Thomas B. Fordham Institute, 2003). Others are insulted and threatened by the prospect of a person from a non-traditional background leading a school (Fenwick & Pierce, 2001). The debate as to whether or not these candidates possess the prerequisite skills and knowledge to effectively lead a school continues. But, will superintendents, as gatekeepers to school districts, afford these individuals the opportunity to prove their worth as principals? In this study, superintendents’ attitudes toward alternative licensure of school principals; past behaviors related to hiring alternatively licensed school personnel; attitudes toward specific alternatively licensed personnel; perceptions of the conditions in their school districts; anticipated concerns about hiring alternatively licensed principals; and the presence or absence of a clearly articulated induction program for new principals are used as predictors of superintendents' willingness to recommend hiring alternatively licensed principals to their school boards. The composite model of attitude-behavior consistency and data from a qualitative study of 18 superintendents were the bases for the development of a theory. To test the theory, an on-line questionnaire, using Likert and Thurstone scaled items, was administered to 1200 randomly selected superintendents who were members of the American Association of School Administrators (AASA) in 2005. Multiple regression analysis was used for the analysis of the quantitative data. Five of the 19 predictor variables were significant predictors of superintendents’ willingness to recommend hiring alternatively licensed principals. The strongest relationship existed between superintendents’ willingness to recommend hiring alternatively licensed principals and their perceptions of the instructional leadership ability of alternatively licensed principals. Other significant predictors were superintendents’ past experiences hiring alternatively licensed principals, perceptions of the community acceptance of alternatively licensed principals, general attitude toward alternative licensure, and willingness to hire under the given definition. Superintendents displayed a low neutral (more unfavorable) score on the Thurstone scale, which means they view the employment of alternatively licensed principals slightly unfavorably. / Ph. D.

Depuis 2000, l’examen professionnel de l’Ordre des infirmières et infirmiers du Québec (OIIQ) est constitué d’une partie écrite et d’une partie pratique sous la forme d’un examen clinique objectif structuré (ECOS). Des programmes de préparation à l’examen professionnel ont été mis en place par différents milieux afin d’aider les candidates à l’exercice de la profession infirmière (CEPI) à se préparer pour cet examen, toutefois aucune évaluation de ces programmes n’a encore été effectuée. Cette étude avait pour but d’évaluer qualitativement un programme de préparation à l’examen professionnel. Des groupes de discussion ont été organisés avec des CEPI ayant participé au programme de préparation afin d’obtenir leur perception quant à l’impact de ce programme sur leur expérience de l’examen professionnel. La méthode d’analyse par questionnement analytique de Paillé et Mucchielli (2008) a été utilisée pour réaliser l’analyse des données. L’approche par compétences de deuxième génération développée par Goudreau, Pepin, Dubois, Boyer, Larue et Legault (2009) fut utilisée comme cadre de référence pour cette étude. L’analyse des données a permis d’identifier les attentes des CEPI à l’égard du programme de préparation et les impacts de celui-ci sur leur expérience à l’examen professionnel. Ainsi, le programme permettrait aux CEPI d’évaluer leurs forces et leurs faiblesses afin d’orienter l’étude préparatoire, tout en diminuant le stress relié à l’inconnu et en augmentant leur motivation et leur confiance en elles. Le parcours ECOS et la rétroaction sont les activités du programme jugées les plus pertinentes dans une perspective d’approche par compétences. L’examen écrit serait toutefois moins représentatif de l’examen professionnel et moins pertinent pour le développement des compétences. Person and professional program determinants of health provider student attitudes toward inter-professional teamwork

Health provider student characteristics and professional program were evaluated as predictors of attitudes toward inter-professional (IP) teams. Sixteen months after completing a self-report battery of demographic and non-ability trait measures, participants completed a second survey (N = 213), assessing components of attitudes toward IP teams. Non-ability traits showed comparable within-program predictive validities for affective reactions toward IP behavior. Additionally, results indicated the incremental predictive validity of trait Dominance and Motivational Inter-professional Team Intelligence, over professional program, for IP attitudes and affective reactions toward IP behavior, respectively. The independent, relative, and joint roles of non-ability individual differences and professional program as determinants of IP training outcomes are discussed.
