Arbetet med lidande patienter i palliativ vård - Sjuksköterskors upplevelser

Bakgrund: I Sverige beräknas ålderskategorin 65 år och äldre att öka vilket resulterar i ett ökat behov av palliativ vård. I den palliativa vården möter sjuksköterskor patienter med olika former av lidande. Lidandet är komplext och likaså är arbetet med lidande patienter i palliativ vård. Tidigare forskning visar att sjuksköterskan kan påverkas av sitt arbete i den palliativ vården.Syfte: Syftet är att i litteraturen undersöka sjuksköterskors upplevelser av att arbeta med lidande patienter i palliativ vård. Metod: Studiens syfte besvarades med en litteraturstudie och baserades på 10 stycken vetenskapliga artiklar med kvalitativ ansats. Tillvägagångssättet baserades på rekommendationer från SBU (2014b) och Polit & Beck (2014). Materialet genomgick en tematisk analys.Resultat: I litteraturstudiens resultat framkommer att sjuksköterskor upplevde arbetet med lidande patienter som meningsfullt. Arbetet innebar även utmanande möten med emotionella påfrestningar, krav från patienter och anhöriga samt upplevelsen av att inte räcka till. Kompetensens betydelse i relation till arbetet belystes också. Beträffande hantering av arbetet använde sig sjuksköterskorna av olika copingstrategier.Slutsats: Upplevelsen av arbetet i den palliativa vårdkontexten varierar. Arbetet innebär både positiva och negativa upplevelser. Olika känslor och tankar väcks i mötet med patienten och deras anhöriga. Meningsfullheten med arbetet balanseras med arbetet som utmanande. Saknas kompetens eller strategier för att hantera arbetet finns risken att de utmanande faktorerna tar över och arbetet blir överväldigande. / Background: The need for palliative care in Sweden is estimated to increase in line with an estimated increased survival rate for the demographic group of inhabitants 65 years and older. Nurses employed within palliative care are frequently encountering patients that experiences pain in various ways. Given that pain is a quite complex psychical and physical reaction this could potentially affect the work environment for nurses dealing with patients within palliative care that experience such feelings. This notion, that nurses can be affected by the complex working environment that is palliative care, has been supported by previous research on the subject.Aim: The aim of this study is to critically research how nurses experience working with suffering patients within the context of palliative care.Method: The aim of this study will be examined by using the method of literature review and thematic analysis, examining ten qualitative studies published on the subject on how nurses experience working with suffering patients within the context of palliative care. The motivation to carry out this study using the method of literature review and thematic analysis is based in recommendations from SBU (2014b) and Polit & Beck (2014). Results: The results of this literature review reveals that nurses experienced the work of the suffering patients as meaningful. The work also means challenging meetings with emotional stress, demands from patients and relatives as well as the experience of feeling inadequate. The competence in relation to the work was highlighted. Regarding the handling of the work nurses used various coping strategies.Conclusion: The experience of working within the context of palliative care varies. The work involves both positive and negative experiences. Different feelings and thoughts raised in the meeting with the patient and their relatives. The meaningfulness of the work is balanced with the work as challenging. A lack of competence or strategies to manage work may result in challenging factors taking over and the work becomes overwhelming.
