INTRODUÇÃO: Apesar de ser a droga de primeira escolha na prevenção da hemorragia pós-parto, o uso da ocitocina em cesarianas permanece empírico. O objetivo deste estudo foi dosar a ocitocina sérica após a administração profilática de diferentes regimes de ocitocina em pacientes submetidas à cesariana eletiva. MÉTODOS: 30 pacientes que se apresentaram para cesariana eletiva foram randomizadas para receber ocitocina intravenosa, após o clampeamento do cordão umbilical, nos seguintes grupos: G1 (n=9): 10 UI de ocitocina infundidas em 30 minutos (0,33 UI/min), G2 (n=11): 10 UI de ocitocina infundidas em 3 minutos e 45 segundos (2,67 UI/min) e G3 (n=10): 80 UI de ocitocina infundidas em 30 minutos (2,67 UI/min). Este estudo foi encoberto para as pacientes e para os cirurgiões. A avaliação do tono uterino foi realizada pela equipe cirúrgica e a dosagem da concentração sérica de ocitocina foi feita pela técnica de enzimoimunoensaio (ELISA), antes da anestesia (T0) e nos tempos 5 (T5), 30 (T30) e 60 (T60) minutos após o início da infusão da ocitocina. RESULTADOS: Os níveis de ocitocina sérica (média ± erro padrão, ng/mL) foram semelhantes entre os grupos em T0 (0,062 ± 0,021; 0,039 ± 0,019 e 0,067 ± 0,041; respectivamente, p = 0,76) e em T60 (0,648 ± 0,257; 0,356 ± 0,257 e 0,683 ± 0,257; respectivamente, p = 0,58). G3 apresentou níveis maiores de ocitocina que G1 em T5 (3,651 ± 0,741 versus 0,709 ± 0,268; p = 0,01). Em T30, G3 apresentou níveis de ocitocina sérica maiores que G1 (6,190 ± 1,195 versus 1,174 ± 0,375; p < 0,01) e, também, que G2 (6,190 ± 1,195 versus 0,411 ± 0,206; p < 0,01). Os parâmetros hemodinâmicos foram semelhantes entre os grupos. O tono uterino foi considerado satisfatório em todos os intervalos estudados e não houve a necessidade de utilização de uterotônico complementar. CONCLUSÃO: Foram demonstradas dosagens séricas de ocitocina por ELISA em gestantes submetidas à cesariana eletiva. A administração de 80 UI de ocitocina em 30 minutos resulta em níveis séricos de ocitocina maiores que os outros dois métodos de administração aos 5 e 30 minutos, porém, estas concentrações não diferem aos 60 minutos / BACKGROUND: The use of oxytocin to prevent postpartum hemorrhage after elective cesarean delivery still remains empirical. The purpose of this study was to determine oxytocin serum levels following differents regimens of prophylactic oxytocin administration in pregnant women undergoing elective cesarean delivery. METHODS: 30 healthy pregnant patients were randomized to receive intravenous oxytocin, after clamping of the umbilical cord, into the following groups: G1 (n=9), 10 IU of oxytocin infused over 30 minutes (0.33 IU/min); G2 (n=11), 10 IU of oxytocin infused over 3 minutes and 45 seconds (2.67 IU/min) and G3 (n=10), 80 IU of oxytocin infused over 30 minutes (2.67 IU/min). Both patient and surgeon were blinded to the study group allocation. Uterine tone was assessed by palpation by the surgeon. Serum oxytocin concentration was determined by enzyme immunoassay (EIA) before anesthesia (T0) and at 5 (T5), 30 (T30) and 60 (T60) minutes following the start of oxytocin infusion. RESULTS: Serum oxytocin levels (mean ± standard error, ng/mL) were similar in the groups at T0 (0.062 ± 0.021, 0.039 ± 0.019 and 0.067 ± 0.041, respectively, P = 0.76), and T60 (0.648 ± 0.257, 0.356 ± 0.257 and 0.683 ± 0.257, respectively, P = 0.58). G3 presented higher serum oxytocin than G1 at T5 (3.651 ± 0.741 versus 0.709 ± 0.268, P = 0.01). At T30, serum oxytocin levels of G3 were higher than G1 (6.190 ± 1.195 versus 1.174 ± 0.375, P < 0.01) and also than G2 (6.190 ± 1.195 versus 0.411 ± 0.206, P < 0.01). Hemodynamic data were similar in all groups. Uterine tone was considered satisfactory in all intervals studied and no additional uterotonic agent was needed. CONCLUSION: We demonstrate serum oxytocin determinations by EIA in healthy pregnant women presented for elective cesarean delivery. Administering 80 IU in 30 min results in higher serum oxytocin levels at 5 and 30 min than the other two methods of oxytocin administration, but the concentrations did not differ at 60 min

Detection of miRNA by SMART technology

Aberrant expression of short non-coding micro RNAs (miRNA) in many human diseases, along with remarkable stability in physiological media, has made them attractive clinical biomarkers. In particular, miRNA-122 is substantially elevated in plasma of patients with established drug-induced liver injury and can also be used to identify early liver injury when current markers, such as alanine aminotransferase (ALT), still show normal levels. The development of a rapid test for miRNA-122 e.g. in drug poisoning would allow earlier and more sensitive clinical diagnosis of liver injury. Nucleic acids are traditionally analysed by polymerase chain reaction (PCR), which has a high degree of sensitivity but suffers from high cost and is prone to sample contamination. The aim of this project is to develop a PCR free method to directly detect miRNA- 122 in biological samples using SMART technology. The SMART technology takes advantage of dynamic chemistry for sequence specific recognition of nucleic acids using aldehyde-modified nucleobases (SMART nucleobases), and target-complementary peptide nucleic acid (PNA) probe containing an “abasic” position (so called modified PNA probe). In this study, this unique detection method was used in a fluorescent detection with the use of light up probes, which are probes with an environmental dye as nucleobase; a FRET system was also designed to allow the discrimination between perfect match target and mismatched one. The SMART technology was also transferred onto magnetic beads to develop an ELISA like assay allowing sensitive and rapid detection of single stranded DNA mimic of the miRNA-122. With its potential PCR free approach, this easily adapted platform promises to transform and expand routine clinical diagnostic testing and screening for circulating miRNAs.

Obligatory and optional resumption : case studies in the syntax of Romanian and Iraqi Arabic

Resumption has been the object of lively research (Doron 1982; Sells 1984; McClauskey 1990; 2002; Shlonsky 1992; Cann 1999; Sharvit 1999; Alexopoulou 2006; Guilliot 2006; Malkawi 2009; Rouveret 2011) and various analyses consider it a form of agreement, a last syntactic resort or a special kind of ellipsis. On the theoretical side, I survey the issues that are the background of the research, such as the nature of the pronouns that are involved in resumption. I develop a syntactic analysis of resumptive pronouns in which they are clitics (Cardinaletti and Starke 1999; Dechaine and Wiltschko 2002; Roberts 2010) and they form a complex determiner phrase together with the relative pronoun or interrogative pronoun. I argue that when resumption is obligatory, it follows from requirements in the syntax and only when it is optional it is a phenomenon at the syntax-pragmatics interface. On the empirical side, I contrast and compare the pronominal paradigms of Arabic and Romanian, an Eastern Romance language which strikingly shows a similar pattern of resumption as Arabic: (i) obligatory resumption in relativization, (ii) designated relative pronoun. Though for Romanian nobody has disputed the nature of the clitic pronouns as clitic in the sense that I am adopting (Cardinaletti and Starke 1999), it has not been thoroughly documented either. I then discuss the pragmatics of resumption in contexts in which it is optional, such as D-linked questions in Arabic. The difference between obligatory resumption and optional resumption is not found in the syntax, because this same syntactic derivation is associated with different interpretive effects depending on whether it occurs in obligatory or in optional contexts. I argue that the presence of the pronoun when it is not required by the syntax triggers a change in interpretation: the module pragmatics assigns it a pragmatic feature.

Template-Based Question Answering over Linked Data using Recursive Neural Networks

abstract: The Semantic Web contains large amounts of related information in the form of knowledge graphs such as DBpedia. These knowledge graphs are typically enormous and are not easily accessible for users as they need specialized knowledge in query languages (such as SPARQL) as well as deep familiarity of the ontologies used by these knowledge graphs. So, to make these knowledge graphs more accessible (even for non- experts) several question answering (QA) systems have been developed over the last decade. Due to the complexity of the task, several approaches have been undertaken that include techniques from natural language processing (NLP), information retrieval (IR), machine learning (ML) and the Semantic Web (SW). At a higher level, most question answering systems approach the question answering task as a conversion from the natural language question to its corresponding SPARQL query. These systems then utilize the query to retrieve the desired entities or literals. One approach to solve this problem, that is used by most systems today, is to apply deep syntactic and semantic analysis on the input question to derive the SPARQL query. This has resulted in the evolution of natural language processing pipelines that have common characteristics such as answer type detection, segmentation, phrase matching, part-of-speech-tagging, named entity recognition, named entity disambiguation, syntactic or dependency parsing, semantic role labeling, etc. This has lead to NLP pipeline architectures that integrate components that solve a specific aspect of the problem and pass on the results to subsequent components for further processing eg: DBpedia Spotlight for named entity recognition, RelMatch for relational mapping, etc. A major drawback in this approach is error propagation that is a common problem in NLP. This can occur due to mistakes early on in the pipeline that can adversely affect successive steps further down the pipeline. Another approach is to use query templates either manually generated or extracted from existing benchmark datasets such as Question Answering over Linked Data (QALD) to generate the SPARQL queries that is basically a set of predefined queries with various slots that need to be filled. This approach potentially shifts the question answering problem into a classification task where the system needs to match the input question to the appropriate template (class label). This thesis proposes a neural network approach to automatically learn and classify natural language questions into its corresponding template using recursive neural networks. An obvious advantage of using neural networks is the elimination for the need of laborious feature engineering that can be cumbersome and error prone. The input question would be encoded into a vector representation. The model will be trained and evaluated on the LC-QuAD Dataset (Large-scale Complex Question Answering Dataset). The dataset was created explicitly for machine learning based QA approaches for learning complex SPARQL queries. The dataset consists of 5000 questions along with their corresponding SPARQL queries over the DBpedia dataset spanning 5042 entities and 615 predicates. These queries were annotated based on 38 unique templates that the model will attempt to classify. The resulting model will be evaluated against both the LC-QuAD dataset and the Question Answering Over Linked Data (QALD-7) dataset. The recursive neural network achieves template classification accuracy of 0.828 on the LC-QuAD dataset and an accuracy of 0.618 on the QALD-7 dataset. When the top-2 most likely templates were considered the model achieves an accuracy of 0.945 on the LC-QuAD dataset and 0.786 on the QALD-7 dataset. After slot filling, the overall system achieves a macro F-score 0.419 on the LC- QuAD dataset and a macro F-score of 0.417 on the QALD-7 dataset. / Dissertation/Thesis / Masters Thesis Software Engineering 2018

Characterisation and strategic treatment of dystrophic muscle

The mdx mouse is widely used as a model for Duchenne Muscular Dystrophy, a fatal X-linked disease caused by a deficiency of the sub-sarcolemmal protein, dystrophin. This dissertation reports characterisation of the features of dystrophy in the mdx mouse, including parameters such as electrophysiological and contractile properties of dystrophic cardiac tissue, quantitative evaluation of kyphosis throughout the mdx lifespan, and contractile properties of respiratory and paraspinal muscles. Following these characterisation studies, the efficacy of antisense oligonucleotides (AOs) to induce alternative mRNA splicing in mdx skeletal muscles (diaphragm and paraspinal muscles) was evaluated. The left atria of younger (<6 weeks) and older (>15 months) mdx mice showed consistently lower basal forces and responsiveness to increased calcium, while action potential duration was significantly shorter in young mice (3 weeks) and older mice (9 and 12 months) (P<0.05). Cardiac fibrosis increased with age in mdx atria and ventricles and was elevated in young (6-8 weeks) and old (15 months) mdx compared to control mice (P<0.01). This study provided insights into DMD cardiomyopathy, and suggested that very young or old mdx mice provide the most useful models. Mdx mice show thoracolumbar kyphosis like boys with Duchenne Muscular Dystrophy. A novel radiographic index, the Kyphotic Index (KI), was developed and showed that mdx mice are significantly more kyphotic from 9 months of age, an effect maintained until 17 months (P<0.05). At 17 months, the paraspinal and respiratory muscles (latissimus dorsi, diaphragm and intercostal muscles) are significantly weaker and more fibrotic (P<0.05). Administration of AOs at four sites within the diaphragm at 4 and 5 months of age significantly increased twitch and tetanic forces compared to sham treated mdx (P<0.05). However, no difference in collagen was evident and dystrophin was not detected, possibly due to the low concentration of AO utilised. This study suggested that AOs can provide functional improvement in treated skeletal muscles. Monthly injections with AOs into the paraspinal muscles from 2 months to 18 months of age alleviated kyphosis, without significantly altering twitch and tetanic forces of latissimus dorsi, diaphragm and intercostal muscles. There was evidence of less fibrosis in diaphragm and latissimus dorsi muscles (P<0.05) and reduced central nucleation of the latissimus dorsi and intercostal muscles (P<0.05). Again, dystrophin was not detected by immunoblot. These studies indicate that very young and old mdx mice display previously uncharacterised dystrophic features, and are useful models for testing new therapies such as AOs. Low doses of AOs were shown to be safe and efficacious for long-term use, however there remains a need for testing higher concentrations and improved delivery strategies.

Strategic entrepreneurship in New Zealand's state-owned enterprises: underlying elements and financial implications

The concept of strategic entrepreneurship has received increased attention over the past ten years. Viewed as the intersection of entrepreneurship and strategy, this field of research is populated by conceptual studies which focus mainly on the nature and perceived benefits of strategic entrepreneurship. Similarly, the study of entrepreneurship in a public sector context has gained increasing support in recent years, but also remains underexplored. To address these gaps, this thesis considers: What are the underlying elements and financial implications of strategic entrepreneurship in New Zealand’s state-owned enterprises [SOEs]? New Zealand’s SOE sector, comprising 17 government-owned, commercially focused organisations, is considered to be a prime subject for this research. Well known for their implementation of new public management [NPM], many New Zealand SOEs have also been publicly recognised as both innovative and entrepreneurial. The research question is addressed by first developing a preliminary framework of strategic entrepreneurship from literature on entrepreneurship and strategy. This framework is then examined in the context of case studies on activity which is entrepreneurial and/or strategic within 12 of the 17 SOEs operating in New Zealand as at 2006. Transcripts from a series of interviews, and publicly available documents are analysed thematically. SOEs’ financial statements over a five year period are also analysed. The thesis contributes in two broad areas. First, much-needed empirical support is lent to the concept of strategic entrepreneurship. Key elements of strategic entrepreneurship identified include opportunity identification, innovation, acceptance of risk, flexibility, vision, growth, and leveraging from core skills and resources such that existing knowledge and skills are transferred and applied to create new products, services, and markets. Important supporting elements identified include an open, flexible, and progressive culture, operational excellence, and cost minimisation. The nature of each of these elements is also investigated. A detailed understanding of the relationship between strategic entrepreneurship and wealth creation reveals various internal and external factors which may influence the nature and strength of the relationship. These factors include changes within the organisation, as well as changes in the economic and political environment, and are important influences on the resulting returns realised. Second, this thesis offers valuable evidence in support of emerging change in the public sector towards the adoption of strategic entrepreneurship. Support for the value of NPM is provided, with clear evidence of financial returns from New Zealand’s SOE sector. Further, a key finding is the structured and systematic approach to entrepreneurial activity within the context of NPM in several New Zealand SOEs. Such behaviour is referred to in this thesis as new public entrepreneurship. This form of activity offers the potential for competitive advantage and financial gain traditionally associated with entrepreneurial activity, but also limits the respective risks through its structured, systematic approach.

The Pricing of Structured Products in Sweden : Empirical findings for Index-linked Notes issued by Swedbank in 2005

<p>Structured products are investment vehicles that combine basic financial instruments to provide private investors with packaged solutions to more advanced investment strategies in financial markets.</p><p>This paper investigates the pricing of 22 index-linked notes examined during their full life cycles between January 12, 2005 and January 17, 2007. The selected products constitute some 40% of the structured products issued by Swedbank in 2005, which at the time of the study is the second largest issuer of structured products to private investors in Sweden. Quoted prices on secondary markets are compared with duplication strategies using exchange traded options in order to calculate price differences.</p><p>The pricing results show that quoted prices deviate very little from their theoretical values in secondary markets. The price deviations are surprisingly low in an international comparison. Some indications have been found that the market maker is able to influence prices on secondary market by orienting the pricing towards the relative life cycle and moneyness of the structured products.</p><p>The importance of patterns in price deviations can, however, be questioned since the absolute level of pricing errors is low. There is little evidence to suspect that the issuer, Swedbank, systematically quotes prices that are not in line with their theoretical values. Sophisticated investors are thus likely to be able to judge the attractiveness of the structured product issue by comparing the transaction costs of the instruments in a duplication strategy with the transaction costs of the structured product. The author welcomes further research that includes multiple issuers to determine whether these findings apply for the Swedish market as a whole.</p>

Clinical and Genetic Studies of Hearing Impairment

<p>Monogenic disorders offer a possibility for studies of genetic disturbances in hearing impairment—a knowledge which could be essential for development of future treatment options. In this thesis, the underlying genetic disturbances in neurofibromatosis 2 (NF2) and familial Meniere’s disease (FMD) were evaluated, and familial X-linked hearing impairment was described from a clinical point of view. </p><p>In paper I, constitutional DNA from 116 individuals with NF2 of variable severity was studied using the array-CGH method focusing on a 7.6-Mb area surrounding the NF2 gene on chromosome 22q. Deletions were found in 20.7% of samples. In mild NF2, the deletions were small, but variable sizes of deletions were found in cases that were moderately or severely affected. Disease phenotype could not be predicted from the size of the deletions.</p><p>In papers II and III, a single five-generation family with autosomal dominant FMD was described. Anticipation concerning age of onset was observed. Genome scan revealed five candidate gene regions with a LOD score of > 1. Two additional families with autosomal dominant MD were analyzed for linkage to these five regions. A cumulative Zmax of 3.46 was obtained for a single 463-kb region on chromosome 12p12.3, containing only one known gene: PIK3C2G. This encodes a protein with a proposed role in hair cell regeneration in mammalian ears. No mutations were found in protein-coding sequences or exon-intron borders. In two of the three families, a shared haplotype, suggested common ancestry, was found to extend over 1.7 Mb, which could be a genomic region of importance for FMD.</p><p>In paper IV, a family in which five males displayed progressive low- and mid-frequency hearing impairment from the first or second decade was described. Female carriers were affected by a high-frequency hearing impairment from the fourth decade. The family could represent a novel X-linked dominant audiophenotype.</p>
