Le condizioni di sovrappeso ed obesità anche in età evolutiva sono aumentate negli ultimi decenni al punto che, secondo l'Organizzazione Mondiale della Sanità, sono diventate un problema di salute pubblica in tutti i Paesi occidentali.
Questa tesi è nata nell’obiettivo di individuare caratteristiche odontostomatologiche in pazienti affetti da obesità; il gruppo campione(80 pazienti, età media 10,5 anni) è stato suddiviso in due sottogruppi: sottogruppo A comprendente 54 pazienti affetti da obesità primaria, sottogruppo B comprendente 26 pazienti obesi portatori o di sindromi genetiche in cui l’obesità risulta essere, in letteratura, parte del quadro sindromico o affetti da varie forme di disabilità. Al gruppo controllo, randomizzato, appartengono 54 pazienti normopeso con anamnesi medica negativa.
Sono state principalmente indagate la prevalenza del la patologia cariosa, l’ età dentale, il grado di maturazione ossea e la presenza di malocclusioni.
Dallo studio non emerge una correlazione tra obesità primaria e suscettibil ità al la patologia cariosa. Si rileva invece che tutti i pazienti affetti da obesità presentano un’anticipata età dentale, un’avanzata maturazione ossea e patologie ortopedico-ortodontiche.
Da qui la necessità di progettare piani di trattamento in relazione al l’anomala tipologia di accrescimento,valutando la maturità scheletrica sulla base dello stadio di maturazione delle vertebre cervicali.
Il trattamento del paziente af fetto da obesità va affrontato in un’ottica di interdisciplinarietà in cui all’odontoiatra e all’ortodontista si affiancano il pediatra anche per l’ individuazione delle fasce di pazienti a rischio di patologie del cavo orale e per la
sensibilizzazione del paziente e dei familiari nei confronti della prevenzione odontoiatrica, e figure specialistiche quali l’otorino, il fisiatra, l’auxologo e l’endocrinologo.

Relationships between oral health status in children with disability and their mothers’ depressive symptoms
Aim. The purpose of the present study was to evaluate the relationships between oral health status in children with chronic medical conditions and their mothers’ depressive symptoms. Methods. Fifty-one children (25 male and 26 female, ranging from 2 to 18 years) affected by chronic systemic diseases followed at the Sant’Orsola-Malpighi Hospital in Bologna, ,and, were referred with their mothers at the Dental Department of Bologna. Children were subclassified in 3 groups according to the ASA classification and orally examined for hygiene status, gingival condition and dental caries. The indexes used were O’Leary plaque Index (PI), bleeding on probing index (BOP), dmft/DMFT. Mothers were interviewed on knowledge about oral diseases prevention for their children and daily management (hygiene habits, sugared aliments consumption). Statistical analysis was performed through the use of linear regression. Results. The relationships between ASA and IP as well as between ASA and BOP are statistically significant (α = 0,01). Seventy percent of patients and their relatives in ASA groups 3 and 4 never received information on oral health and prevention of oral diseases by paediatricians and/or dentists. The 53% of mothers present depressive symptoms. The relationships between degree of depressive symptoms and dmft/DMFt as well as between degree of depressive symptoms and sugared aliments daily consumption are statistically significant (α = 0,05). Conclusion. Our results give support to the hypothesis of an association between degree of systemic disease and oral hygiene status. The psychological mothers condition seams to play a role on the oral conditions of their sons. Our analysis shows the needs for an interdisciplinar approach in order to promote the oral health of children with disability.

Il cavo orale del paziente obesoCostella, Giorgia <1979> 13 July 2009 (has links)
La frequenza delle condizioni di sovrappeso ed obesità è andata aumentando negli ultimi decenni al punto che, secondo l’Organizzazione Mondiale della Sanità, l’obesità è diventata un problema di salute pubblica di proporzioni epidemiche in tutti i Paesi occidentali.
Questa tesi nasce dall’obiettivo di individuare caratteristiche odontostomatologiche peculiari in pazienti affetti da obesità.
Lo studio è stato condotto su 160 pazienti. Il gruppo campione, composto da 93 pazienti di età compresa tra i 6 ed i 16 anni (età media 11 anni) ed affetti da obesità (percentile oltre il 95°), è stato suddiviso in due sottogruppi: sottogruppo A comprendente 67 pazienti con anamnesi medica negativa affetti da obesità primaria; sottogruppo B comprendente 26 pazienti obesi portatori di sindromi genetiche in cui l’obesità risulta essere dalla letteratura parte del quadro sindromico. Al gruppo controllo, randomizzato, appartengono 67 pazienti normopeso con anamnesi medica negativa.
Sono state indagate:
- prevalenza della patologia cariosa
- stato di salute parodontale
- età dentale
- grado di maturazione ossea
- presenza di malocclusioni.
Dallo studio non emerge una correlazione positiva tra obesità primaria e suscettibilità alla patologia cariosa. Si rileva invece una relazione statisticamente significativa tra obesità e infiammazione dei tessuti parodontali (CPI pari ad 1 nella quasi totalità dei pazienti del sottogruppo A) - età dentale e maturazione ossea anticipata e - patologie ortopedico-ortodontiche.
Da qui la necessità di progettare efficaci programmi di prevenzione e piani di trattamento specifici, in relazione alle problematiche relative ai rischi connessi al possibile sviluppo di malattia parodontale e all’anomala tipologia di accrescimento.
Il trattamento del paziente affetto da obesità va affrontato in un’ottica di interdisciplinarietà: all’odontoiatra e all’ortodontista si dovranno affiancare il pediatra, per l’individuazione delle fasce di pazienti a rischio e per la sensibilizzazione del paziente e dei familiari nei confronti della prevenzione odontoiatrica, e figure specialistiche quali l’otorino, il fisiatra, l’auxologo e l’endocrinologo.

Evaluation of fluid transport processes in dental enamel. Methods to assess the relevance of enamel permeability in caries prevention and etching treatments.Bertacci, Angelica <1977> 18 May 2009 (has links)
This thesis evaluated in vivo and in vitro enamel permeability in different physiological and clinical conditions by means of SEM inspection of replicas of enamel surface obtained from polyvinyl siloxane impressions subsequently later cast in polyether impression ma-terial. This technique, not invasive and risk-free, allows the evaluation of fluid outflow from enamel surface and is able to detect the presence of small quantities of fluid, visu-alized as droplets. Fluid outflow on enamel surface represents enamel permeability. This property has a paramount importance in enamel physiolgy and pathology although its ef-fective role in adhesion, caries pathogenesis and prevention today is still not fully under-stood.
The aim of the studies proposed was to evaluate enamel permeability changes in differ-ent conditions and to correlate the findings with the actual knowledge about enamel physiology, caries pathogenesis, fluoride and etchinhg treatments. To obtain confirmed data the replica technique has been supported by others specific techniques such as Ra-man and IR spectroscopy and EDX analysis.
The first study carried out visualized fluid movement through dental enamel in vivo con-firmed that enamel is a permeable substrate and demonstrated that age and enamel per-meability are closely related. Examined samples from subjects of different ages showed a decreasing number and size of droplets with increasing age: freshly erupted permanent teeth showed many droplets covering the entire enamel surface. Droplets in permanent teeth were prominent along enamel perikymata.
These results obtained through SEM inspection of replicas allowed innovative remarks in enamel physiology. An analogous testing has been developed for evaluation of enamel permeability in primary enamel. The results of this second study showed that primary enamel revealed a substantive permeability with droplets covering the entire enamel sur-face without any specific localization accordingly with histological features, without changes during aging signs of post-eruptive maturation. These results confirmed clinical data that showed a higher caries susceptibility for primary enamel and suggested a strong relationship between this one and enamel permeability.
Topical fluoride application represents the gold standard for caries prevention although the mechanism of cariostatic effect of fluoride still needs to be clarified. The effects of topical fluoride application on enamel permeability were evaluated. Particularly two dif-ferent treatments (NaF and APF), with different pH, were examined. The major product of topical fluoride application was the deposition of CaF2-like globules. Replicas inspec-tion before and after both treatments at different times intervals and after specific addi-tional clinical interventions showed that such globule formed in vivo could be removed by professional toothbrushing, sonically and chemically by KOH. The results obtained in relation to enamel permeability showed that fluoride treatments temporarily reduced enamel water permeability when CaF2-like globules were removed. The in vivo perma-nence of decreased enamel permeability after CaF2 globules removal has been demon-strated for 1 h for NaF treated teeth and for at least 7 days for APF treated teeth.
Important clinical consideration moved from these results. In fact the caries-preventing action of fluoride application may be due, in part, to its ability to decrease enamel water permeability and CaF2 like-globules seem to be indirectly involved in enamel protection over time maintaining low permeability.
Others results obtained by metallographic microscope and SEM/EDX analyses of or-thodontic resins fluoride releasing and not demonstrated the relevance of topical fluo-ride application in decreasing the demineralization marks and modifying the chemical composition of the enamel in the treated area.
These data obtained in both the experiments confirmed the efficacy of fluoride in caries prevention and contribute to clarify its mechanism of action.
Adhesive dentistry is the gold standard for caries treatment and tooth rehabilitation and is founded on important chemical and physical principles involving both enamel and dentine substrates.
Particularly acid etching of dental enamel enamel has usually employed in bonding pro-cedures increasing microscopic roughness. Different acids have been tested in the litera-ture suggesting several etching procedures. The acid-induced structural transformations in enamel after different etching treatments by means of Raman and IR spectroscopy analysis were evaluated and these findings were correlated with enamel permeability. Conventional etching with 37% phosphoric acid gel (H3PO4) for 30 s and etching with 15 % HCl for 120 s were investigated.
Raman and IR spectroscopy showed that the treatment with both hydrochloric and phosphoric acids induced a decrease in the carbonate content of the enamel apatite. At the same time, both acids induced the formation of HPO42- ions. After H3PO4 treatment the bands due to the organic component of enamel decreased in intensity, while in-creased after HCl treatment.
Replicas of H3PO4 treated enamel showed a strongly reduced permeability while replicas of HCl 15% treated samples showed a maintained permeability. A decrease of the enamel organic component, as resulted after H3PO4 treatment, involves a decrease in enamel permeability, while the increase of the organic matter (achieved by HCl treat-ment) still maintains enamel permeability. These results suggested a correlation between the amount of the organic matter, enamel permeability and caries.
The results of the different studies carried out in this thesis contributed to clarify and improve the knowledge about enamel properties with important rebounds in theoretical and clinical aspects of Dentistry.

Riabilitazione protesica ed implanto-protesica di pazienti in età evolutiva affetti da Displasia EctodermicaMontanari, Marco <1981> 13 July 2009 (has links)
Introduction. Ectodermal Dysplasias are a heterogeneous group of inherited disorders characterized by dysplasia of tissues of ectodermal origin (hair, nails, teeth, skins and
glands). Clinically, it may be divided into two broad categories: the X-linked hypoidrotic form and the hidrotic form. Hypohidrotic Ectodermal Dysplasia (H.E.D) is characterized by the triad oligo-anodontia, hypotricosis, hypo-anhydrosis (Christ-Siemens-Tourane syndrome). The incidence of HED is about 1/100,000. Mutation in the actodysplasin-A (EDA) and ectodysplasin-A receptor (EDAR) genes are responsible for X-linked and autosomal HED. The clinical features include sparse, fine hair, missing or conical-shaped teeth, decreased sweat and mucous glands,
hypoplastic skin, and heat intolerance with exercise or increased ambient temperature.
Complete or partial anodontia and malformation of teeth are the most frequent dental findings. Incisors and canines are often conical-shaped while primarily second molars, if present, are mostly affected by taurodontism. Treatment is supportive and includes protection from heat exposure, early prosthetic rehabilitation, skin, hair ear,
nose and nail care, and genetic counseling for family planning. The diagnosis of HED in the neonatal and early infancy period may be difficult since sparse hair and absent
teeth are normal finding at this age. In childhood the diagnosis is more easily made on the basis of history and clinical examination. Dental abnormalities are the most
common complaint. Prosthetic rehabilitation has been recommended as an essential part of the management of HED because is important from functional, esthetic, and
psychological standpoint. A team approach that includes input from a pediatric dentist, an orthodontist, a prosthodontist, and an oral and maxillofacial surgeon is
necessary for a successful outcome. Conventional prosthodontic rehabilitation in young patient is often difficult because of the anatomical abnormalities of existing teeth and alveolar ridges. The conical shaped teeth and “knife-edge” alveolar ridges result in poor retention and instability of dentures. Moreover, denture must permit jaws expansion and a correct pattern of growth.
Materials and Methods. Complete removable dentures were provided to allow for normal physiological development and a corrected masticatory function. Initial maxillary and mandibular impressions were made with smallest stock trays and irreversible hydrocolloid and then final impressions ware made with light-bodied polysulfide rubber base impression material. A base of autopolymerizing resin was
constructed and a wax rim was added to the base. The patient’s vertical dimension of occlusion was established by assessing phonetic and esthetic criteria. Preliminary
occlusal relations were recorded, and the mandibular cast was mounted on the articulator. Acrylic resin teeth specific for children dentures were selected and
mounted. The dentures were tried in and, after proper adjustments, were inserted. The patients were monitored clinically every month to fit prostheses. Cephalometric
radiographs were taken every 6 month with the prostheses in place in order to evaluate correct pattern of growth. Cephalometric measurements were realized and used to evaluate the effect of rehabilitation on craniofacial growth. Cephalometric measurements of sound patients were compared with ED patients. After two month expander screws (three-way screw in the upper denture and two-way the lower one)were inserted in each denture in order to permit the expansion of the denture and the jaws growth. Where conical teeth were present, composite crown were realized and
luted to improve the esthetic and phonesis. In order to improve retention the placement of endosseous implants was carried out. TC 3D Accuitomo was performed and a resin model of mandibular bone of the patient was realized. At the age of 11 years two implants were inserted into anterior mandible in a child with anodontia.
Despite a remarkable multi-dimensional atrophy of the mandibular alveolar process, the insertion of two tapered screw implants (SAMO Smiler, diameter 3.8, length 10
mm). After a submerged healing period of two-three month, the implants were exposed. Implants were connected with an expansion guide that permits mandibular growth and prosthetic retention. The amount of mandibular growth was also evaluate dusing the expansion guide.
Results. Early oral rehabilitation improve oral function, phonesis and esthetic, reducing social impairment. Treated patients showed normal cephalometric measurement. Early rehabilitation is able to prevent the prognatissm of the
mandibula . The number of teeth was significantly related to several changes in craniofacial morphology. Discussion. In the present study the 5,3% of ED patients showed hypodontia, the l’89,4% di oligodontia, and the 5,3% di anodontia. The cephalometric analysis supports that ED patients showed midface hypoplasia. ED groups showed an
increased pogonion to nasion measurement than sound patients, indicative of class III tendency. The present study demonstrated that number of teeth was significantly
correlated with deviation of cephalometric measurements from normality. Oligoanodontia is responsible for changing of cephalometric measuraments also on sagittal plane with a class III tendency. Maxillary jaw showed a retrused position related to the presence of hypodontia.

Manifestazioni Cranio-facciali delle NeurofibromatosiFelline, Carla <1979> 13 July 2009 (has links)
The term neurofibromatosis (NF) subsumes at least seven different genetic disorders associated by the presence of neurofibromas located in the skin, oral cavity, visceral and skeletal level. As NF1 (Von Recklinghausen disease), one of the most common genetic diseases, can have oral manifestations, dentists have to be aware about pathognomonic features. The thesis’ target is the literature’s review on the NF1 manifestations either systemic or cefalic area and these features’ research in a specimen of 30 patients NF1 affected.
NF1 is manifested in the cefalic area locating either in the jaws (isolated neurofibromas, ipoplasia or bone structures absence) or soft tissues (fibromas and neurofibromas located in: cheeck, lips, oral mucosa, tongue, mouth’s floor, gingiva and palate). Frequently, NF1 patients are affected by dental anomalies of position, number and eruption, that determinates the possibility of orthopaedic-orthodontic problems. An increased prevalence of the caries risk and a possible pulpar involvement of neurofibromas is reported.
Clinical and radiographical typical signs of the disease and specific indications for the differential diagnosis with other oral pathologies are described (cysts and odontogenic tumors, periapical lesions of endodontic origin and severe parodontitis).
The importance of screening programs and periodical follow-ups (biannual dental visits from the age of four years, annual X-ray checks from the age of six) is supported by the high frequency of manifestations at hard and soft tissues level of the cefalic area and by the documented risk of malignant transformation.

Ricerca clinico-epidemiologica sullo stato di salute del cavo orale nei pazienti tossicodipendentiBromo, Francesco <1979> 15 October 2010 (has links)
Obiettivo: Lo scopo della ricerca è stato quello di mettere in evidenza
la presenza di patologie del cavo orale in soggetti tossicodipendenti
Materiali e Metodi: Sono stati reclutati 71 pazienti in terapia di
recupero presso la Comunità Incontro di Amelia. I soggetti sono stati
sottoposti a visite clinico-anamnestiche adottando una cartella clinica
da noi stessi costruita.
Risultati: Dei 71 soggetti tossicodipendenti, 42 facevano uso di eroina
e 39 assumevano droghe per via parenterale, ma solo 25 risultavano
avere malattie infettive. Per quanto riguarda la patologia cariosa,
abbiamo riscontrato una media di carie complessive di 4,40 di cui solo
lo 0,52 era al colletto (non è stata evidenziata significativa differenza
tra soggetti con o senza patologie infettive). Dal punto di vista
parodontale solo 13 presentavano parodontite e 25 lesioni del cavo
orale non di carattere maligno.
Conclusioni: I dati ottenuti nella nostra ricerca preliminare ci hanno
portato a dire che non esiste una netta correlazione tra tipologia di
droga e patologia orale ma sembrerebbe piuttosto che le lesioni
cariose e parodontali riscontrate siano da associarsi allo stile di vita di
questi soggetti.
