Reflexology and massage in the treatment of Type II diabetic neuropathy

This study examined Neuro-Reflex Therapy, a type of reflexology, and effleurage massage for the treatment of diabetic neuropathy in men and women 40-73 years old with Type II diabetes. Participants were physician-referred to an alternative medicine clinic and were randomly assigned to Neuro-Reflex Therapy, effleurage massage, or a wait-list control group. The treatment groups received 12 individual 30 min sessions scheduled once a week for 12 weeks, and the control group received treatment after the study was completed. Scores on the Neuropathic Pain Scale, Section III of the Multidimensional Pain Inventory, the Profile of Mood States, the Short-Form McGill Pain Questionnaire, changes in foot temperature, and changes in scores on a Visual Analogue Scale (VAS) were analyzed. As hypothesized, pain for both treatment groups significantly decreased from the control group; however, mood states, participation in daily activities, and temperature for the treatment groups did not significantly increase, compared to the control group, as expected.

Dissertation submitted in partial compliance with the requirements for the Master's Degree in Technology: Chiropractic, Durban Institute of Technology, 2006. / Lower back pain and lower back injuries have been documented as one of the most common musculoskeletal problems in both amateur and professional tennis players. It has also been documented that the serve, which may be considered one of the most important strokes of the game, is also the most likely stroke to cause back pain. A good tennis serve requires considerable trunk rotation. The serve is the highest stress strain action during tennis. In a two set game the minimum number of serves a player may hit is 24 with a maximum excluding deuces and advantages of 96. The “Topspin serve” in particular requires the player to arch their back and this puts the lumbar spine into hyperextension. These movements thus put considerable pressure on the facet joints and multifidi muscles. It stands to reason that any joint related clinical entity can change biomechanics and affect the serve. In research done on golfers with mechanical lower back pain, it was found that club head velocity as well as pain decreased in symptomatic golfers with mechanical lower back pain after manipulation (Jermyn, 2004). No research has yet been done on manipulation of tennis players with lower back pain. The aim of this investigation was to determine the immediate effect of a lumbar manipulation on the clinical and performance measures of amateur tennis players suffering from lower back discomfort associated with playing tennis. / M

Learners' and educators' perceptions of the Massage Therapy Institute's combined practicum and community service programme

M.Phil. / Therapeutic massage therapy (TMT) as a profession has undergone major transformations on two levels simultaneously in the past five years. The first is the statutory recognition of TMT as a health profession regulated by the Department of Health and the Allied Health Professions Council of South Africa (AHPCSA) that has propelled the therapy from the services sector into the country's primary health care system. The second is the transformation of the South African education structures requiring TMT to be formalised and accredited by the South African Qualifications Authority's (SAQA) and the accommodation of the qualification on the National Qualifications Framework (NQF) in accordance with the Higher Education Act of 1997. These impetuses have led to a shift in the profession's approach to how it defines itself and how it trains future therapists. The focus of this study is the Massage Therapy Institute's Community Service Programme (MTI CSP), the focal point of the institution's second year programme. The CSP entails the combining of practical sessions and community service in order to expose learners to a wide variety of conditions to encourage theory-practice integration. It also fosters a social awareness through exposure to patients from varied cultural and socio-economic backgrounds in a clinical setting. The purpose of this study was to investigate the perceptions of learners and educators regarding the efficacy of the programme to integrate theory and practice and to prepare TMT therapists for a health profession in the primary health care domain. From the analysis it was apparent that despite a number of shortcomings, learners and educators agreed that the CSP encouraged theory practice integration and personal development. They were also able to hone their professional skills. All participants indicated however, that learners failed to effectively transfer the knowledge and skills gained during the combined practical and community service sessions to written learning tasks that formed part of and tasks outside the programme. This was confirmed by text analysis of treatment and case study reports. The implications for teaching and learning are discussed in the context of language learning and learning through language in order to move beyond the what and the how to the why. It was also clear from the research that participation in the programme led to perspective transformation in the majority of learners' perception of TMT as a health profession located in the PHCS. The analysis showed that the reasons for the exception to this trend, is related more to macro policies rather than the efficacy of the CSP. Finally, a number of recommendations are made to improve the transferability of the knowledge, skills and attitudes gained in the CSP to other learning tasks and beyond that to learners' personal and public lives. The recommendations emphasise metacognitive skills and critical reflection to equip learner-therapists to become fully participating members of a responsible and relevant health profession.

The Effects of Instrument-Assisted Cross Fiber Massage on Ligament Healing

Indiana University-Purdue University Indianapolis (IUPUI) / Ligament injury is one of the most prevalent musculoskeletal disorders that may lead to disability or disease, such as osteoarthritis. Conservative interventions which accelerate or augment ligament healing are needed to enhance therapeutic outcomes. The purpose of this research agenda was to investigate the tissue level effects of a type of manual therapy, cross fiber massage (CFM), in particular instrument-assisted CFM (IACFM), on ligament healing. Bilateral knee medial collateral ligament (MCL) injuries were created using an established rodent model where one MCL received IACFM treatment and the other untreated MCL served as a within subjects control. The short and long term effects of IACFM on the biomechanical and histological properties of repairing ligaments were investigated. Tensile mechanical testing was performed to determine ligament mechanical properties. Ligament histology was examined under light microscopy and scanning electron microscopy. IACFM was found to accelerate early ligament healing (4 weeks post-injury), possibly via favorable effects on collagen formation and organization, but minimal improvement was demonstrated in later healing (12 weeks post-injury). Regional blood flow and angiogenesis were investigated as possible mechanisms underlying the accelerated healing found in IACFM-treated ligaments. Laser Doppler perfusion imaging was used to investigate vascular function. Micro-computed tomography was used to determine vascular structural parameters. Compared to untreated contralateral injured controls, IACFM-treated injured knees demonstrated a delayed increase in blood flow and altered microvascular structure, possibly suggesting angiogenesis. Mechanotransduction is discussed as a mechanism for the beneficial effects of CFM in that application of a mechanical force was found to enhance biomechanical and histological properties as well as vascular function and structure acutely in healing ligaments. Although this thesis focused on IACFM treatment of injured knee ligaments, it is plausible for concepts to apply to other manual modalities that offer conservative alternatives to invasive procedures or pharmaceuticals in the treatment of soft tissue injuries.

O Brasil, dos 90 para os 210 milhões; consequentemente o número de idosos avançou, os cuidados melhoraram, as questões sociais, especialmente as gerontológicas, tornaram-se mais presentes. Esta pesquisa constitui um estudo descritivo-exploratório, com coleta e análise dos dados, qualitativa e quantitativa. Diante do tema proposto, questões surgiram que se subsumiram nesta: - A massagem pode trazer reais benefícios aos idosos? Objetivou-se: (i) Verificar a importância do “toque” na vida do idoso; (ii) Analisar e discutir os efeitos proporcionados pela técnica de Massagem Gerontológica Epicrânia (na cabeça) e Dorsal (nas costas), quanto ao bem-estar à pessoa idosa, independentemente de sexo, mas com autonomia de movimentos. A aplicação foi feita em sessão única, em duas partes, uma com 30 minutos de cada massagem, em um total de 5 sessões. Levadas em consideração: a pressão do “toque”, a área a ser aplicada e as posições da pessoa idosa. Os idosos entrevistados responderam a questionários (Ficha de Anamnese, Stress, Satisfação, Dor) antes e após a realização dos procedimentos, a fim de atestar os resultados da massagem. A pesquisa foi realizada com 10 idosos voluntários, de ambos os sexos, com idades entre 60 e 74 anos, convidados para os Programas de Massagens da Universidade Anhembi Morumbi-SP. Coleta de dados realizada por meio da aplicação de ficha de Anamnese com informações gerais sobre:- o estado de saúde física e informações sociais, - a importância da massagem na vida destes idosos e – de como se sentiram antes e depois da prática; - suas prioridades na busca desta atividade; - aferições dos sinais vitais antes e após a massagem. Aplicado o questionário de Escala Visual Analógica de Dor (EVA) que registra a intensidade da dor, onde 10 é o máximo e 0 o mínimo, adaptado para buscar informações sobre as principais alterações ocorridas no comportamento desses idosos, após iniciarem esta atividade com massagem, relacionadas a sentimentos, emoções e relacionamentos interpessoais, levantando-se o impacto sobre auto-estima e bem-estar. Em relação ao teste de Perfil de Estado de Humor (POMS), para avaliar estados emocionais e de humor cujos adjetivos da tabela referem-se à tensão, depressão, vigor, hostilidade, fadiga e confusão, com graduação em 5 pontos, sendo: 0=nunca,1= pouco, 2= moderado,3=bastante,4=muitíssimo. Os resultados foram relevantes nesses indivíduos, em que foi possível perceber a diminuição da pressão arterial; após a massagem: 90% confirmaram se sentir bem, mais motivados, mais dispostos; enfim, houve significante resultado positivo nos idosos em relação à queda gradual no nível de stress fisiológico e psicológico pós-massagem gerontológica Epicrânia e Dorsal, podendo ser este um dos melhores tratamentos na redução dos níveis de estresse nos idosos, contribuindo, assim, a uma saúde de qualidade

Syftet med denna studie är att undersöka hur upplevd stressnivå påverkas av hand mot hudkontakt vid och efter massage hos massageterapeuter. Studien avgränsas till att endast undersöka massage där massageterapeutens händer har kontakt mot klientens hud, av diplomerade massageterapeuter inom svensk klassisk massage. En enkät utformades specifikt för att undersöka massageterapeuternas upplevelse av stressnivå i samband med och efter massagebehandling. Enkäten besvarades av 78 diplomerade massageterapeuter inom svensk klassisk massage från hela Sverige. Resultatet visar att skattning av upplevd stressnivå var lägre i samband med massagebehandling och direkt efter massagebehandling än 12 och 24 timmar efter utförd massagebehandling. En majoritet (73 procent) angav att de upplevde stressreducering och känsla av lugn under och efter utförd massagebehandling. Ett en-vägs ANOVA test visade att desto längre tid som passerade efter massagebehandlingen, desto högre upplevelse av stressnivå rapporterade respondenterna. Skattningen direkt efter massage jämfört med 24 timmar efter massage visade på en signifikant skillnad. Medan skattning under massage och direkt efter massage visade på en mindre signifikant skillnad. Ett tvåsvansat T-test visade att det fanns en statistiskt signifikant skillnad direkt efter massage och 24 timmar efter. Alltså är det mindre än en chans på 1000 att den upplevda stressnivån (upplevt lugn) beror på slumpen. / The aim of this study is to examine how perceived levels of stress are affected by skin on skin contact during and after giving massage. The study is limited to licensed massage therapists within Swedish massage, where the therapists’ hands are in contact with the clients’ skin during the entire massage. A questionnaire was specifically designed to investigate with the objective to measure the experienced levels of stress during and after giving massage. The questionnaire was answered by 78 certified massage therapists across Sweden. The results show that the perceived level of stress was lower during and shortly after having given massage, than after 12 and 24 hours respectively. With more time passing since the massage was administered, the level of stress increased. A majority (73 percent) of the massage therapists stated that they experienced reduced levels of stress and increased level of calmness during and after giving the massage treatment. A one-way ANOVA test showed that the longer time that passed after giving the massage, the higher levels of stress was reported. The reported level of stress directly after giving massage compared to 24 hours later showed significant difference. While the reported level of stress during and directly after giving massage did not show a significant difference. An independent two-tailed T-test demonstrates that there was a statistically significant difference immediately after giving massage and 24 hours after. Meaning that there is a 0,1 percent risk that the perceived levels of stress (sense of calmness) is a result of random factors.
