The Concept of Disability: A Philosophical Analysis

At the most general level, this project seeks to engage the question, "What is disability?" The conceptual exploration is undertaken against the background of the philosophical literature addressing the nature of disease, illness, and disability. This work contends that much of the literature bearing on the nature of disability fails to distinguish sufficiently between different domains of philosophical explanation and concern--ontological, non-moral normative, and moral normative, respectively. Specifically, this involves a failure to distinguish among (a) disputes regarding the proper ontological characterization of disability, particularly as expressed in medical-scientific explanations of the phenomenon; (b) disputes regarding the role of non-moral (aesthetic, epistemic, cultural) values or norms in the constitution of those explanations (i.e., non-moral normative concerns); and (c) disputes regarding moral and political considerations that shape the character of the social reality within which persons with disabilities live (i.e., moral normative concerns). This work advances the thesis that disabilities, like diseases, are "natural," in the sense that they are not mere social constructions, but that values of various sorts nevertheless do enter into the identification of states of affairs as disability, and that the "disability" designation has important socio-cultural implications that are inevitably the subject of ongoing political negotiation. Specifically, this work argues that "disability" involves a complex interplay of ontological realities, non-moral normative, and moral normative considerations or values. This interplay is captured well by a "biopsychosocial" (BPS) approach to disability, one which incorporates these various considerations into a single account, involving an integration of different levels of explanation (biological, psychological, social) of the disability phenomenon. This work first develops the theoretical underpinnings and rationale for a BPS approach to disability (Chs. 1-3), then explores in detail some of the relevant ontological (Ch. 4), non-moral and moral normative (Ch. 5), and sociological and political (Ch. 6) considerations that enter into the identification of states of affairs as "disability," concluding (in Ch. 7) with a brief consideration of some of the study's implications for understanding the nature of disability, the future of disability studies and the disability rights movement, and the relationship between the disabled and the broader society.

Local strategies in a global network: Disability rights in Jamaica

The declaration of the International Year of Disabled Persons in 1981 and the international activism of persons with disabilities resulted in the globalization of disability, which involved a new framework for the treatment of PWDs. Although disability activism was being played out globally, it has been about PWDs gaining equality and full participation in their local societies. Therefore, at the heart of an international movement was the strategic organization of PWDs, most of who live in the developing world. This dissertation is based on a case study of activism for disability rights in one particular developing society, Jamaica. The Jamaican movement is spearheaded by the Combined Disabilities Association (CDA), which has been at the forefront of disability rights activism locally and regionally. The primary research question: how does CDA organize strategically to meet the needs of the broader disabled population of Jamaica is broken down into the following four sub-questions: 1) how is "disability" constructed in the context of Jamaican culture, 2) how did CDA members negotiate their original involvement and level of participation with the organization, 3) what is the nature of the linkage between the CDA and Disabled People International, and how does this relationship affect CDA's policies and procedures, and 4) what strategies does CDA employ to mobilize PWDs in Jamaica to participate in advocacy activities? These questions are explored using participant observation, semi-structured interviews, survey, and document review. Findings reveal a complex construction of disability as Jamaicans negotiate between various beliefs and ideas to understand the meaning of disability. Furthermore, the negotiation of movement involvement by PWDs is based their connections have with fellow members as well as the personal benefits that they receive. Although the CDA has been influenced by the international disability movement, it has also contributed to the global activism, especially in the Caribbean. In a local environment overwhelmed by limited financial resources and social inequalities the CDA by default addresses some social welfare of PWDS, while pursuing its primary commitment to advocacy, resulting in numerous challenges.

A New Fuzzy-chaotic Modelling Proposal For Medical Diagnostic Processes

Main reason of this study is to set forth the internal paradox of the basic approach of the artificial intelligence in the medical field to by discussing on the theoretical and application levels and to suggest solutions in theory and practice against that. In order to rule out the internal paradox in the medical decision support systematic, a new medical model is suggested and based on this, concepts such as disease, health, etiology, diagnosis and treatment are questioned. Meanwhile, with the current scientific data, a simple application sample based on how a decision making system which was set up by fuzzy logic and which is based on the perception of human as a complex adaptive system has been explained. Finally, results of the research about accuracy and validity of this application, current improvements based on the current model and the location on the artificial intelligence theory is discussed.

Conceptual and contextual descriptions of the bipolar mood disorder spectrum: commentaries on the state of psychology as reflected through polarised epistemologies

Bipolar mood disorder has been traditionally researched, explored, and explained from a modernistic, psychiatric perspective. The purpose of this study is to explicate an alternative description for bipolar mood disorder, from a postmodern perspective. The widely accepted psychiatric knowledge focuses on the signs and symptoms of the disorder, pharmacological treatments, and manualised psychotherapies. This thesis shifts the focus from an intrapsychic, deficit perspective towards one which is inclusive of surrounding discourses and patterned relationships. The social constructionist research approach is followed, utilising vignette and thematic analyses for textual deconstruction and reconstruction. In addition to these data analyses, discourses were analysed using the actual text of the co-researchers. This allowed for a thorough explication of the ways in which discourses shape the construct bipolar mood disorder. From these analyses, emergent themes were then distilled and compared to the existing body of literature in the bipolar mood spectrum field of study. Process models were generated to depict the various pertinent aspects of the social construction of bipolar mood disorder. This research has value for the treating professional, allowing for a broader, more inclusive discourse perspective to add to the already established medical model view. Further, this research gives credence to the voice of the person who has been diagnosed with the illness. This research may also contribute to the epistemological debates within modernist and postmodernist paradigms. Employees’ perception of the factors that prevent disclosure of disability status to the employer: case of a selected higher education institution

Magister Commercii - MCom / There is an increase in the number of people with disabilities entering, and in the workplace. Industrialised countries are encountering a workforce that is ageing, which makes the prevalence of disability, due to chronic illness amongst employees, more evident. However, even with legislation and policies that support people in the workplace, such as the Employment Equity Act 55 of 1998, the Broad-Based Black Economic Empowerment Act 53 of 2003 and the Code of Good Practice on Disability in the Workplace, many people still choose not to disclose their disabilities. Therefore, the main purpose of the study is to identify the perceived factors that could possibly prevent the disclosure of disability in the workplace. The study was conducted at one of the universities in the Western Cape. The study was qualitative in nature and made use of semi-structured interviews. Ten participants took part in the study and comprised of two academic staff members from each of the faculties on the main campus. Content analysis was used to analyse qualitative data where various themes and subthemes emerged.

Good Kids, mad Schools

The western world has long viewed ‘mental illness’ from a biomedical perspective; treating the brain the same way it treats physical issues, through diagnosis, medication and clinical intervention. We however tend to forget that a person is interdependent on her or his environment, and resultantly we frame the person as ill or weak rather than the environment as sick, or ‘mad’. With this thesis I assess how mental health and ‘mental illness’ are being framed within secondary schools in the province of Ontario (Canada). I achieve this by analyzing mental health strategies using a theoretical lens developed from Critical Disability Theory and Mad Studies. Through use of a Critical Discourse Analysis (CDA) I analyzed a total of 4 mental health strategies from the federal government, the Ontario government and 2 Ontario school boards. My findings indicated that these mental health strategies generally subscribe to a medical or individualized understandings of mental health, and overlook the disabling influence that the school environment can have on the student. By minimizing the role of the social and physical environment on student mental health schools are reinforcing the dominant discourse, which is that distresses in mental health are the result of an individual deficit caused by a brain defect or personal weakness. This discourse has far reaching consequences that may contribute to many Ontario students not receiving the support they desire. I contend that social workers employed by school boards can be influential in challenging these dominant framings of mental health and carry forward the standpoint that the school environment and its social structures play a principal role in the mental health of students. / Thesis / Master of Social Work (MSW)

Domestic violence and health care: opening Pandora's Box - challenges and dilemmas

Yes / In this article we take a critical stance toward the rational progressive narrative surrounding the integration of domestic violence within health care. Whilst changes in recent UK policy and practice have resulted in several tangible benefits, it is argued that there may be hidden dilemmas and challenges. We suggest that the medical model of care and its discursive practices position women as individually accountable for domestic violence-related symptoms and injuries. This may not only be ineffective in terms of service provision but could also have the potential to reduce the political significance of domestic violence as an issue of concern for all women. Furthermore, it is argued that the use of specific metaphors enables practitioners to distance themselves from interactions that may prove to be less comfortable and provide less than certain outcomes. Our analysis explores the possibilities for change that might currently be available. This would appear to involve a consideration of alternative discourses and the reformulation of power relations and subject positions in health care.

Midwifery knowledge and the medical student experience : an exploration of the concept of midwifery knowledge and its use in medical students' construction of knowledge during a specialist obstetric rotation

The literature concerning what medical students learn from midwives during specialist obstetric rotations is scarce. In the UK, despite a long tradition of providing midwifery attachments for medical students, it is almost non-existent. Working with midwives is arguably the only opportunity medical students have to experience holistic or social models of maternity care, focusing on normality rather than on the medical concept of risk. This study sought to discover how medical students constructed their knowledge about childbirth during a six week specialist rotation in obstetrics in a Northern English teaching hospital (NETH), with particular emphasis on whether participants assimilated any concepts from midwifery knowledge (MK). A Delphi Study, done as the first phase of the research, focused on MK, utilizing an international sample of experienced midwives. Resulting themes were used to develop the data collection tool for the second phase of the research. The research employed a qualitative case study method with students from a single year cohort comprising the case. Data were collected using a tool consisting of three problem based learning (PBL) scenarios. These were presented to the students in consecutive interviews at the beginning, the middle and the end of their obstetric rotation. Following analysis, five main themes were identified which illuminated the medical students' construction of knowledge about maternity care. These were explored and discussed. The thesis concludes with recommendations for increasing opportunities for IPE in the medical and midwifery curricula.

Autonomy in the California Disability Services System

Legislation concerning California residents with intellectual and developmental disabilities (ID/DD) requires recipients of services to be treated as independent individuals while emphasizing self-determination. At the same time, under regulatory procedures, recipients are considered dependent on the delivered services and not self-determinant. Neither the California Department of Developmental Services nor the trade associations representing community service providers have established a unified, systematic practice to support self-determination. This phenomenological study explored the experience of adults with ID/DD working toward self-determination. Specifically, it explored how medical and social models contribute to shaping and actualizing the independence of this population. Interviews with eight adults with ID/DD explored the perceived barriers to, and opportunities for, achieving independence through self-determination. Under the current statutory regulations, the study viewed two conceptual lenses. The first lens, social role valorization, is based on the study of normalization. The second lens, social reaction, emphasizes a response to the disparities that acknowledge the political, cultural, and social beliefs associated with theories of deviance and social role valorization. The findings demonstrated that self-determination requires collaboration between coordinated services, primary social systems, and theoretical services supporting social role value. The discovery of these key elements may help California's disability service system fulfill legislative requirements to increase opportunities for personal choice.

Incapacity, disability and dismissal : the implications for South African labour jurisprudence

<p>Disability in South African labour law is reduced to incapacity. An evaluation of disability and incapacity was made to advocate a clear conceptual break between the two concepts. Also, that disability should be grounded in a social model paradigm of disability which was a materialist critique of how capitalism constructs disability. To enhance the analysis discourse analysis was employed to illustrate how language, ideology and power sustained the notion of disability in capitalist society. A comparative analysis was made drawing on American disability jurisprudence and Canadian disability jurisprudence to illustrate the difference in approach between the two legal systems with a suggestion that the Canadian approach was better suited to the development of a South African disability law. And the development of South African disability law it was argued would benefit if a legal construction of disability was crafted to deal with the obstacles that disabled people encounter in the work-place.</p>
