Prevalence užívání návykových látek u pracovníků nízkoprahových a ambulantních adiktologických služeb v ČR / Prevalence of substance abuse among employees of low-threshold and outpatient addictology services in the Czech Republic

Background: The prevalence of substance use in the general population is commonly mapped, as is the issue of substance prevalence among drug users themselves. There is no official research in the Czech Republic that deals with the prevalence of substance use among workers in addictology services. Objectives: The aim of the research is to map the prevalence of the use of legal and illegal addictive substances in a selected research population. The results of the mapping will show which addictive substances are most frequently used by members of the research population, which substances have the highest lifetime prevalence and which substances are not very popular in this population. Methods: The basic orientation of the empirical survey was quantitative. Data were obtained through an online questionnaire survey. Out of the total number of 589 employees, 41.9% of employees of outpatient or low-threshold addictology services participated in the research, ie 247 respondents. This diploma thesis deals with one of the most frequently used addictive stuffs - alcohol. The theoretical part contains the characteristics of the addiction and the syndrom of risky behaviour in the adolescent age, it deals with the topic of drugs and drug addiction. Later, it investigates alcohol - its characteristics, metabolism and intake, its effects on single organ systems, the levels and types of alcohol addiction. It focuses on the prevention of using addictive stuffs and alcohol and on the treatment of alcohol addiction. The practical part of the thesis settles the targets and the questions of the research. The main goal of the practical part, eventually of the whole thesis, was to analyze the basic school students' (sixth grade and higher) awareness of alcohol, its effects and the risk of it being consumed. As well as this, the aim was to find out whether the pupils have any experience with preventive programs and where they get in touch with these. The method of quantity research was used to hit these targets - semi-structured questionnaires for the pupils of basic schools (sixth grade and higher), followingly, school prevention methodologists were asked written questions. Alkaloids are important natural substances, which are not directly mentioned within the Chemistry Curriculum in the Framework Education Programme for Secondary General Education (Grammar Schools). However, they are a topic which unites more disciplines and allows educating in wider context and across different subjects (Chemistry, Biology and Health Education). They are certainly important topic in terms of drug abuse prevention and Health Education. The objective of this diploma thesis is creation of new educational materials for this topic for grammar school teachers and students. The theoretical part contains a literary research consisting of an analysis of curricular documents, a selection of secondary school chemistry textbooks, a selection of electronic teaching materials, and a proposal for including the topic in grammar school education. Furthermore, it contains a chapter dealing with the issues of drug abuse and its prevention. Research has been conducted by a questionnaire survey on the teaching practice of the topic at grammar schools in the Czech Republic in the practical part. The main output of this diploma thesis is a collection of educational materials on the given topic (PowerPoint presentations, textbook, projects, worksheet, instruction for laboratory exercise, didactic test).

This diploma work Addicted Substances - project for the lower secondary school is divided into two parts, theoretical and practical. The aim of this work is giving information about addicted drugs and method of teaching by form of projects, proposing and realization of project with this topic, which could be used for other teachers. Theoretical part is intent on summary of question of addicted substances, where the terms addicted substances, abuse of addicted drugs - its symptoms or risks, causes of drugs addiction and some terms, what are connected with this question (e.g. habit, tolerance, addiction, syndrome of abstinence), are clarified. My diploma thesis deals with the addictive substances in road transport, especially with alcohol and other narcotic drugs used by drivers of motorized and non-motorized vehicles. The thesis is divided into three main parts. First chapter provides the definition and clarification of the terms. Second chapter, Substantive law, contains two subchapters, Criminal law and Administrative law. The subchapter Criminal law analyzes crimes that can be committed in conjunction with the addictive substances in road transport, namely endangerment under influence of an addictive substance according to § 274 TZ, general endangerment and general endangerment alleging negligence according to § 272 TZ and § 273 TZ and inebriation according to § 360 TZ. The most attention is paid to the offence of endangerment under the influence of an addictive substance, which is, as the most frequently committed crime in connection with the addictive substances, analyzed in terms of elements of crime, complicity and participation, stages of the commission of the crime, qualified elements of crime and sanctions. Background - The thesis deals with the topic of committing misdemeanors related to addictive substances. It focuses on primary and secondary misdemeanors, in particular in the areas of public order, cohabitation, property, health protection from harmful substances and others, sometimes also called "general misdemeanors". A systematic collection of information on this issue is only carried out for some primary misdemeanors, and systematic data collection is not available for secondary misdemeanors. Objectives - The aim of this work is to provide information on the rate of committing misdemeanors in relation to addictive substances, i.e. primary and secondary, and knowledge of the occurence and frequency of specific misdemeanors of this issue. Methods - The research part was conceived as a pilot study. For the purposes of the research, the data were obtained through a content analysis of the files kept at the misdemeanor department of the City of Liberec. The thesis anatomizes issues of habit-forming substances using among secondary school students who have tendencies to behave dangerous in relation to drugs and content of their leisure time. The thesis is divided into practical and theoretical part. The aim of the theoretical part is to describe the views on the sense and role of leisure time, the forms of spending leisure time and the risk behaviour at this age group. The attention is also paid to the ways of prevention. The practical part contains the description and analysis of spending leisure time among secondary school pupils and the risk behaviour of this age group in the relation to the habit-forming substances using. The author tries to prove potential connection between spending leisure time and the usage of habit-forming substances. On the basis of gained experience the author can recommend possible ways of organization of free time activities and related prevention to secondary schools where the research was provided.

The diploma thesis deals with neonatal abstinence syndrome which occur to newborns whose mothers were on addictive illegal drugs in the time of pregnancy. The most frequently taken drugs are canabinoids, stimulancias, opiods and opiates, halucinogens, and drugs taken during substitutional therapy. Neonatal abstinence syndrome is a multisystem disorder that frequently affects central nervous system, gastrointestinal system, respiratory system and autonomic system. To diagnose neonatal abstinence system is the most important careful monitoring of baby in the first days of its life. There are different scoring systems which have been developed for assessing the severity of neonatal abstinence syndrome, we use the Finnegan scoring system. Treatment of neonatal abstinence syndrome is mainly supportive and pharmacological therapy. There are also social serious consequences of this problems; mothers are usually not able to take care of babies and children live in children's homes. The aims of this study were to find out special needs of newborns born to mothers using addictive illegal drugs during pregnancy, to find care interventions which help newborns with neonatal abstinence syndrome in their first days of life and to discover interventions helping mothers stop using addictive illegal drugs. The theoretical part of this study is based on research of already presented scientific studies. The practical part of the study problems of neonates of mothers using addictive illegal drugs in first days after delivery. We have used Virginia Henderson's need theory to evaluate neonate's need. We have chosen only the most important elementary components out of 14 components in Virginia Henderson's need theory. I was also trying to find out if nurses know how to help neonates with neonatal abstinence syndrome and what nursing interventions have positive influence to neonates. Another goal was to know how nurses communicate with mothers and if they have positive influence on mothers. The method of communication with mother using addictive illegal drugs was semistructured interview. I wanted to know what kind of addictive drugs mothers were using during pregnancy and if mothers tried to solve their problem in pregnancy, before delivery. Results are sorted into categories and subcategories and showed in diagrams and tables. Neonates of mothers using addictive illegal drugs during pregnancy have problems in all investigated needs such as breathe, eat and drink adequately, sleep and rest, thermoneutral zone and social need. I also found out that some nursing intervences have positive influence on neonates with neonatal abstinence syndrome. Very important intervenes were individual contact with each neonate and quick reaction to every change in behavior. It is also necessary to respect their sleep and rest time, take nursing and medical care together in blocks, use non-pharmacological treatment of pain, feed them individually according to their needs and also try different techniques of feeding, put babies into the right position and eventually nestle, rock or touch them. In last part of the study we discuss mothers who use addictive illegal drugs. We found out that nurses did not speak with them very actively, did not offer them to breast-feed in special conditions, did not tell them about possibility to be with neonate together in the hospital until a discharge. On the other hand, mothers were not interested in the breast-feeding or staying with babies. The goal of the study was to create brochure with information of negative consequences of taking addictive drugs on babies. This brochure is convenient for education of mother and students as well.

EN: The misdemeanor of endangering under the influence of addictive substance is one of the most frequently occurring criminal offenses that the Czech society encounters. I have divided my thesis into nine chapters. The first chapter deals with elements of the criminal offense under the influence of addictive substance as divided by legal theory. The second chapter describes the issue of the unlawfulness of the deed. The third chapter deals with an analysis of qualifying circumstances that legislature provided in the second paragraph of the misdemeanor of endangerment under the influence of drugs. The fourth chapter describes the issue and the conditions of participation of persons other than the principal perpetrator of the deed. The sixth chapter deals with the most common criminal offenses that occur along with the abovementioned misdemeanor. The seventh chapter is devoted to the new phenomenon of proving a condition that excludes competence in cases of influencing other addictive substance. In the eighth chapter I focus on the issue of showing of various types of sentences with regard to current practice in relation to sentencing for the deed in question under Section 274 of the Criminal Code.
