Charles University in Prague Faculty of Pharmacy in Hradec Králové Department of Biological and Medical sciences Student: Veronika Krčmářová Supervisor of master thesis: PharmDr. Miroslav Kovařík Ph.D. Title of master thesis: Resting energy expenditure in patients with chronic obstructive pulmonary disease II The aim of this study was to compare the resting energy expenditure (REE) and nutritional substrate utilization among patients with chronic obstructive pulmonary disease (COPD) and control group. Previous studies have confirmed increased values of REE in COPD patients. On the basis of the increase of this parameter there was a loss of lean and muscle mass. The utilization of nutritional substrates in patients with COPD is not yet known. Our study by a total of 20 patients includes 11 patients with COPD, and 9 from the control group . The age of patients ranged from 55 to 71 years. Each patient was examined once. We determined the value of REE by indirect calorimetry and utilization of nutritional substrates. According to our results, there was an increase in REE in 82 % of patients with COPD, while only 33 % of patients from the control group was hypermetabolic. Besides the respiratory symptoms there are often developed alterations in metabolism and body composition in COPD patients. Clinically important are mainly malnutrition and skeletal muscle protein loss. Especially if the respiratory muscles are affected, the lung function is negatively influenced. The main aim of this study was to compare the body composition between 15 patients with COPD 3rd and 4th stage and 9 patients of control group without respiratory impairment and comparable anthropometric characteristics (age, body height and weight). By means of bioelectrical impedance analysis we determined the composition of main body compartments. Title: Integration of pupil with epidermolysis bullosa dystrophica disease with special focus on physical education into the primary school education. Objectives: The aim of this thesis is to point out possibilities of pupil's integration with corporal defect, concretely epidermolysis bullosa dystrophica into primary school education with special focus on Physical education through case study. Methods: We have used case study, questionnaire and interview methods because of uniqueness of occurrence this disease in primary school. The main case study is focused on pupil with EBD disease. The partial research is focused on classmates, teachers, teacher's assistant and girl's family. Results: Results have shown us the only partly possibility to integrate pupil with EBD disease into PE. The most suitable period of time was 1 - 3 school year. The research has shown girl's fitting in class and also her good feelings there. In generally we can say, that pupil is well perceived by classmates and teachers. There is very important teacher's and assistant's activity. The class teacher promoted positive, peaceful and friendly atmosphere since first year a thanks to her the class functions as a whole now. Název práce v anglickém jazyce: Liability for damage regarding occupational accidents and diseases The aim of this thesis is to analyze liability for damages regarding accupational accidents and diseases as a part of employer's liability and the evaluation of the current regulation in this area. This thesis is devided to eight chapters. First part of this thesis is a theoretical introduction to the matter of occupational accidents and diseases and the liability for damages caused by them. The target of the first part is to define the institutes of the labor law, which are crucial for the regulation of material and non-material damages in labor law. The theoretical first part consists of Chapter One, Chapter Two and Chapter Three. Chapter One is introductory and defines basic terminology used in the thesis, with the main term being liability in labour law. Chapter Two enumerates various types of liability, such as material liability, non-material liability and other types of liability. This diploma thesis deals with the employer's liability for damages caused by work-related accidents or occupational diseases of his employees. The reason for choosing this topic was partly because of the author's experience with the assessment of work-related accidents in legal proceedings, partly due to the high- frequency of work-related accidents and also the fact, the results of work-related accidents and occupational diseases not rarely cause a long-term adverse repercussion in real life of an employee. The main point of this thesis is to analyse elements of stated employer's liability for damages. Using the case law, detailed analysis of the concept of work- related accidents is performed in the same chapter. There are analysed specific situations with conclusions if the injury can be evaluated as an work-related accident or not. Besides work-related accidents, occupational diseases and other elements of liability, this chapter deals with the possibility of an employer to be absolved from any liability as well. Case law concerning the absolution from liability is also included in this chapter. This thesis describes coping in pacients with chronic musculoskeletal pain. It focuses especially on the cognitive evaluation of pain and coping strategies. Furthermore it deals with the emotional changes in chronic pain. The theoretical part consists of the development of access to pain and the overview of some current knowledge of biological and psychosocial determinant of pain. Emphasis is placed on a holistic, biopsychosocial approach to the issue. Empirical part consists of research aimed at assesing the important factors that contribute to cope life with pain, as in terms of personal dispositions of an individual, so in terms of environmental influence. 1 Abstract Charles University in Prague Faculty of Pharmacy in Hradec Králové Department of Biological and Medical Sciences Student: Martin Jenšík Supervisor of master thesis: PharmDr. Miroslav Kovařík Ph.D. Title of master thesis: Resting energy expenditure in patients with chronic obstructive pulmonary disease Chronic obstructive pulmonary disease (COPD) is step by step moving ahead in public and scientific interest due to its fast rising in mortality rate. Contemporary studies observed a phenomena that patients with COPD have increased resting energy expenditure (REE) that leads to loss of fat free mass and total weight. Our goal was to confirm or disprove this hypothesis. Our research ran in Department of research and development at University hospital Hradec Králové. In our study participated 30 patients, 26 men and 4 women, mean age 67 ± 8 let. Every patient was once examined after twelve-hour fasting. We determined REE and substrate utilization by method of indirect calorimetry. We confirmed increased REE from the average value of REE - 122 ± 14 % predicted by Harris - Benedict equation. None patient was hypometabolic, 7 were normometabolic and 23 hypermetabolic. Substrate utilization did not differ contrary to physiological values. Besides the lung function impairment, there are often described changes in body composition in COPD patients e.g. the skeletal muscle protein loss. The main aim of the study was determining the body composition by DEXA in patients diagnosed with COPD and comparing them with physiological values or studies dealing with this issue. On each patient was performed one examination, patients were coming after twelve-hour fasting. Average age was 66 ± 8 years. The amount of fat-free mass was not reduced. There was not therefore a loss of muscle tissue due to COPD. When evaluating total fat, we find that men have more fat than normal levels. Title: The Evaluation of Managers and Employees' Awareness of the Ergonomics in Office Environment Objectives: The aim of the thesis was a quantitative research to assess the awareness of ergonomics, working environment, office equipment, motor behavior and the appearance of professionally conditioned diseases among office workers, recruiters and management of companies in the Czech Republic. In conclusion outlined recommendations to improve the situation. Methods: The thesis using electronic polling using non-standardized questionnaire, which provides primary data collection. The method is used to analyze the awareness of ergonomic environments, motor behavior in the workplace, the occurrence professionally conditioned diseases and office equipment. The questionnaire was distributed to companies throughout the Czech Republic, a total of 678 employees were contacted, data was compiled from 442 respondents with an average age of 32,6 years. For data analysis was used software Microsoft ® Excel for Mac version 15.20. Results: It was found that ergonomics training and OSH passed only 25% of employees of Czech companies in the research sample. Title: Brain Activity Changes During Walking Affected by Cuieng in Parkinson's Disease Patients Viewed by sLORETA Imaging. Objectives: The aim of this diploma thesis is to acertain changes in electrical activity of the brain, evaluated by sLORETA program, between gait affected by visual and auditory cueing and normal comfortable gait in patients with Parkinson's disease. Methods: This experiment was conducted on 11 patients with Parkinson's Disease aged between 60-78 years. The electrical activity of the brain was registred by scalp EEG during the whole experiment. The experiment consisted of 4 parts. In the beginning we measured resting EEG with closed and then opened eyes (5 + 5 minutes), after that we measured EEG during normal comfortable gait on 3 meters long track (3 minutes). Subsequently we measured EEG during walking affected by cueing, again on the 3 meters long track (3 + 3 minutes). The order of visual and auditory cueing was randomized. Visual cueing was mediated by white lines (tapes) stuck to the floor within 50 centimetres of each other. Auditory cueing was mediated by a metronome. The basic rhythm of the metronome was set on the 70 BPM.
