Great progress has been achieved in the diagnostics and therapy of childhood acute lymphoblastic leukemia (ALL) during the last few decades and the permanent cure rate for children and adolescents has risen to nearly 90%. The basic principle of ALL treatment is to split patients into several groups receiving treatment of different intensity according to exactly defined prognostic features. This is aimed at reducing both the risk of relapse and toxic complications of treatment. The development of new diagnostic methods, especially in the field of molecular genetics and flow cytometry, allowed further improvements in the risk stratification - the minimal residual disease (MRD) has become a crucial prognostic factor in modern treatment protocols for pediatric ALL as a sensitive marker of both response to therapy and subclinical leukemic involvement of various tissues of the organism. Nevertheless, there is still an intensive search for new markers that would enable even more precise characterization of the leukemic clone, and treatment strategies reflecting the biology of leukemic cells are being optimized. The thesis is based on three months of participant observation in a long-term care facility in 2010 and interviews conducted during next three years. It draws from a perspectives of medical anthropology and anthropology of institutions. The focus is on an ethnography of institution for clients with chronic mental disease, dementia and substance abuse. The thesis is mapping institution's components and actors, their social life and relationships in the institution. The analyses is based upon the concept of total institution by Erving Goffman, and draws from thoughts of Josef Pieper, Martin Buber, Emanuel Lévinas and Michel Foucault. A long-term facility is an institution, which shares clients and some of problems with classical examples of total institutions, but recent reforms aimed to minimalize features of total institutions. Still it does not seem to be a vital institution and new aspects of social situation of staff and clients are recognized. Among new problems is non-existence of therapy for the inhabitants, their marginalization through poor financial situation in an institution, where lot of services are paid and little continuity to other forms of care of more community and ambulant character. Methodology This thesis is literature review. The selected studies were according to inclusion and exclusion criteria. The selections of recent studies were published from the year of 2000 until 2017 in English language. The following research databases were selected to identify the relevant topic: PubMed, MEDLINE, Embase, Cochrane, PEDro, and CINAHL. The parameters' results from the articles have been also selected by evaluation the statistical differences according to the P-values. Hypokinetická dysartrie (HD) je častým symptomem vyskytujícím se až u 90% pacientů trpících idiopatickou Parkinsonovou nemocí (PN), která výrazně přispívá k nepřirozenosti a nesrozumitelnosti řeči těchto pacientů. Hlavním cílem této disertační práce je prozkoumat možnosti použití kvantitativní paraklinické analýzy HD, s použitím parametrizace řeči, statistického zpracování a strojového učení, za účelem diagnózy a objektivního hodnocení PN. Tato práce dokazuje, že počítačová akustická analýza je schopná dostatečně popsat HD, speciálně tzv. dysprozodii, která se projevuje nedokonalou intonací a nepřirozeným tempem řeči. Navíc také dokazuje, že použití klinicky interpretovatelných akustických parametrů kvantifikujících různé aspekty HD, jako jsou fonace, artikulace a prozodie, může být použito k objektivnímu posouzení závažnosti motorických a nemotorických symptomů vyskytujících se u pacientů s PN. Dále tato práce prezentuje výzkum společných patofyziologických mechanizmů stojících za HD a zárazy v chůzi při PN. Nakonec tato práce dokazuje, že akustická analýza HD může být použita pro odhad progrese zárazů v chůzi v horizontu dvou let.

This work deals with a design of hypokinetic dysarthria analysis system. Hypokinetic dysarthria is a speech motor dysfunction that is present in approx. 90 % of patients with Parkinson’s disease. The work is mainly focused on parameterization techniques that can be used to diagnose or monitor this disease as well as estimate its progress. Next, features that significantly correlate with subjective tests are found. These features can be used to estimate scores of different scales like Unified Parkinson’s Disease Rating Scale (UPDRS) or Mini–Mental State Examination (MMSE). A protocol of dysarthric speech acquisition is introduced in this work too. In combination with acoustic analysis it can be used to estimate a grade of hypokinetic dysarthria in fields of faciokinesis, phonorespiration and phonetics (correlation with 3F test). Regarding the parameterization, features based on modulation spectrum, inferior colliculus coefficients, bicepstrum, approximate and sample entropy, empirical mode decomposition and singular points are originally introduced in this work. All the designed techniques are integrated into the system concept in way that it can be implemented in a hospital and used for a research on Parkinson’s disease or its evaluation.

This thesis deals with speech analysis of people who suffer from Parkinson’s disease. Purpose of this thesis is to obtain statistical sample of speech parameters which helps to determine if examined person is suffering from Parkinson’s disease. Statistical sample is based on hypokinetic dysarthria detection. For speech signal pre-processing DC-offset removal and pre-emphasis are used. The next step is to divide signal into frames. Phonation parameters, MFCC and PLP coefficients are used for characterization of framed speech signal. After parametrization the speech signal can be analyzed by statistical methods. For statistical analysis in this thesis Spearman’s and Pearson’s correlation coefficients, mutual information, Mann-Whitney U test and Student’s t-test are used. The thesis results are the groups of speech parameters for individual long czech vowels which are the best indicator of the difference between healthy person and patient suffering from Parkinson’s disease. These result can be helpful in medical diagnosis of a patient.

The theoretical part deals with health and social challenges confronted by PAP focusing on social and individual rehabilitation in conducting daily life. It is divided into five parts. Part One describes health issues faced by PAP with emphasis on particular issues which affect this group. Part Two defines the basic terminology of disability and autonomy.Through work experience it was possible to provide first-hand observation of particular issues which affect this group, e.g. physical disability as a factor limiting autonomy. The aim of the study was to analyze and show how the conception of cellulite, thanks to which a normal physiological fact changed into a defect to the beauty and to an illness, was developed and medialized, by using a brief quantitative analysis of Czech press as a whole, a brief quantitative analysis of the research sample and consequently a qualitative analysis of this research sample. Hereafter the aim of the diploma thesis was also seeking of an answer to a question to what extent cellulite is an construct created by advertising industry and manufacturers of cosmetic devices and products, which is abusing and supporting normative imaginings of ideal female body and beauty.
