Analýza politiky duševního zdraví v České republice v kontextu Evropské unie / Mental health policy analysis in the Czech Republic in the context of European Union

Diploma thesis "Mental health policy analysis in the Czech Republic in the context of European Union" deals with the present state of health care of psychiatric patients in the Czech Republic. Main theories for the thesis are quality of life, human rights, health determinants and support of mental health. Analysis of mental health policy shows that health care for psychiatric patients is mainly concetrated in psychiatric institutions. Advanced trends and recommendations described in the political documents issued by the European Union urge to transformation of service framework stressing the importance of health care diferentiation and its move towards community care. At the end, health care framework for the Czech Republic fulfilling such requests and correspondig with the model of balanced care is described. Such a model is based on progressive health care movement from psychiatric institutions towards community care with regard on financial, personal and capacity resources of the state and region. The Czech Republic belongs to the states with average resource capacity thus such steps would lead to balancing institutional and community care and follows the line established by developed countries leading to larger and more diferentiated health care framework.

Akustická analýza vět složitých na artikulaci u pacientů s Parkinsonovou nemocí / Acoustic analysis of sentences complicated for articulation in patients with Parkinson's disease

This work deals with a design of hypokinetic dysarthria analysis system. Hypokinetic dysarthria is a speech motor dysfunction that is present in approx. 90 % of patients with Parkinson’s disease. Next there is described Parkinson's disease and change of the speech signal by this disability. The following describes the symptoms, which are used for the diagnosis of Parkinson's disease (FCR, VSA, VAI, etc.). The work is mainly focused on parameterization techniques that can be used to diagnose or monitor this disease as well as estimate its progress. A protocol of dysarthric speech acquisition is described in this work too. In combination with acoustic analysis it can be used to estimate a grade of hypokinetic dysarthria in fields of faciokinesis, phonorespiration and phonetics (correlation with 3F test). Regarding the parameterization, new features based on method RASTA. The analysis is based on parametrization sentences complicated for articulation. Experimental dataset consists of 101 PD patients with different disease progress and 53 healthy controls. For classification with feature selection have selected method mRMR.

61 Summary Wilms' tumor gene 1 (WT1) is highly expressed in acute leukemia and other hematological malignancies. It has been therefore suggested as a potential universal marker of minimal residual disease (MRD), particularly in patients with acute myeloid leukemia (AML). Due to controversial results of some of the studies, the role of WT1 in MRD follow-up and WT1 prognostic significance remain unclear. WT1 protein is produced in more than 36 different isoforms. These variants have distinct, partially overlapping functions and their ratio is supposed to influence the final effect of WT1. However, despite the increasing number of studies, the clinical impact of WT1 and its isoforms in acute leukemia have not yet been elucidated. We established a unique qPCR method to assess the expression pattern of the main 4 WT1 isoforms. Using this method, we determined the ratio of WT1 variants in the samples of patients with AML, myelodysplastic syndrome (MDS) and healthy controls. Our data showed that this pattern can distinguish among particular hematological malignancies, but lacks a prognostic significance. Lungs transplantation is a solution for the end stage of pulmonary disease after other therapeutic possibilities that the modern medicine has to offer have been exhausted. The main objective of this thesis is to show postoperative patient care after lung transplantation in the department anesthesiology and resuscitation. A highly specialized complex care and cooperation of various fields is needed. Nurses who take care of these patients must be experienced and skillful. They also need to manage well the nursing care for patients in sedation as well as fully conscious patents. It is also needed that the nurses are acquainted with specialized methods of hemodynamics measurements, administration of nitrous oxide and operation of extracorporeal membrane oxygenation machine. They need to know well the drugs that are being administered. The thesis summarizes anatomy and physiology of respiratory system, it deals with lung disease and their symptoms which most often lead to lung transplantation. This thesis concentrates on the case of Russian economy and assessment of its dependence on oil. Russia is often cited as an example of country suffering from resource curse as its natural wealth forms significant share of country's exports and revenues. Thesis will first concentrate on factors determining current state of Russian economy. Presence of the symptoms of Dutch disease in the Russian economy will be studied using the Vector error correction model (VECM) applied on the real effective exchange rate of country (REER). Thesis will also contain an assessment of Russian institutional environment to check for other symptoms of resource curse theory. Analysis of latest federal budget will be used to evaluate the sustainability of Russian federal finances. The thesis is concluded by discussion of results and possible paths of future development of Russian economy.

Transmissive spongiform encephalopathies (TSEs) are neurodegenerative diseases characterized by depositions of abnormally folded prion protein (PrPTSE ) in brain. PrPTSE is at present the only specific biochemical marker of human and animal TSEs. Diagnostic tests are based on the detection of PrPres after proteinase K digestion of brain homogenate using Western blot or on the immunohistochemistry of fixed brain tissue, which are both difficult and time consuming. In this work we focused on development of a new type of tests based on PrP detection without need of proteinase K digestion. As deposits of PrPTSE remain in the body for a long time, there is a substantial chance of them being nonenzymatically modified by glycation. The detection of glycated PrPTSE may have a potential to serve as a diagnostic marker. We prepared monoclonal antibodies specific for carboxymethyl lysine/arginine modified prion protein. Bacterially expressed and purified recombinant human prion protein (rhPrP) was modified by glyoxylic acid that introduces carboxymethyl groups on lysine and arginine residues present within the molecule of the protein. Klinické a genetické prediktory lékové závislosti u idiopatických střevních zánětů / Clinical and genetic predictors of drug dependency in inflammatory bowel disease

IN ENGLISH Drug dependency in inflammatory bowel disease (IBD), Crohn's disease (CD) and ulcerative colitis (UC), is a specific disease phenotype which determines disease prognosis and hence may be used as a prognostic marker for treatment management. Drug dependency in IBD has been well described in corticosteroid treatment and recently also in infliximab (IFX) therapy. The aims of this thesis were: 1) to assess the occurrence of IFX dependency in paediatric and adult patients with CD; further to search for clinical and genetic predictors of IFX outcome and to evaluate the impact of IFX dependency on surgical rate; 2) to assess in CD patients the outcome of the first course of 5-ASA monotherapy with emphasis on 5-ASA dependency and to define clinical predictors of 5-ASA treatment outcome. We found that 66% of children and 29% of adults with CD became IFX dependent. The high frequency in paediatrics is in agreement with previously published studies, while the finding in adult patients indicates a lower rate of IFX dependency in the only study to date. Perianal disease and no bowel surgery prior to IFX start were predicative of IFX dependency in paediatric patients. This thesis is inspired by the theory of social construction of mental illness. The work is a case study that uses the metaphor of the story and narrative analysis of interviews examining the individual human experience with mental illness. My goal was to find out what makes us understand the disease and how this understanding can be used in a daily struggle with the disease. In the theoretical part, I drew from documented knowledge about mental illness, particularly for depressive disorder and schizophrenia simplex. Then I presented the narrative approach as a legitimate way of exploring understanding of lived experience. The practical part is implemented as a case study and through narrative analysis provides interesting insight into the way of understanding and coping with mental illness a person who has the expertise and also suffer from mental illness. This thesis presents results of analysis of internet forum, which appeared under article from internet server aktualne.cz in 2006. The article is dealing with abusing of cage beds in context of on people with mental handicap and people with mental illness focused social care system. For qualitative research was used the method of coding inspired by methodological approach of "grounded theory". In theoretical part of this thesis I analyzed relevant theoretical concepts from area of disability studies and gender studies with special focus on critic of concept of liberal citizenship and social contract theory. The key theoretical concept of the thesis is dichotomy of private and public sphere. In next part of the thesis is I am describing context of causes of cage beds in Czech Republic and I am delineating the basic traits of debate here. In empirical part I am describing five key themes, axial codes. In framework of these themes I am defining the differences of people with mental handicap or people with mental illness. The themes are: space and isolation, efficiency, responsibility, stigmatization and care work.
