Subarachnoid haemorrhage : clinical and epidemiological studies

Background: Subarachnoid haemorrhage (SAH) is a severe stroke that in 85% of all cases is caused by the rupture of a cerebral aneurysm. The median age at onset is 50-55 years and the overall mortality is approximately 45%.Sufficient cortisol levels are important for survival. After SAH hypothalamic/pituitary blood flow may be hampered this could result in inadequate secretion of cortisol. SAH is also associated with a substantial inflammatory response. Asymmetric dimethyl arginine (ADMA), an endogenous inhibitor of nitric oxide synthase, mediates vasoconstriction and increased ADMA levels may be involved in inflammation and endothelial dysfunction. Continuous electroencephalogram (EEG) monitoring can be used to detect non-convulsive seizures, leading to ischemic insults in sedated SAH patients. Elevated ADMA levels are risk factors for vascular diseases. Vascular disease has been linked to stress, inflammation and endothelial dysfunction. SAH possesses all those clinical features and theoretically SAH could thus induce vascular disease. Aims: 1. Assess cortisol levels after SAH, and evaluate associations between cortisol and clinical parameters. 2. Assess ADMA levels and arginine/ADMA ratios after SAH and evaluate associations between ADMA levels and arginine/ADMA ratios with severity of disease, co-morbidities, sex, age and clinical parameters. 3. Investigate occurrence of subclinical seizures in sedated SAH patients. 4. Evaluate if patients that survive a SAH ≥ one year have an increased risk of vascular causes of death compared to a normal population. Results: Continuous infusion of sedative drugs was the strongest predictor for a low (&lt;200 nmol/L) serum cortisol. The odds ratio for a sedated patient to have a serum cortisol &lt; 200 nmol/L was 18.0 times higher compared to an un-sedated patient (p &lt; 0.001). Compared to admission values, 0-48 hours after SAH, CRP increased significantly already in the time-interval 49-72 hours (p&lt;0.05), peaked in the time-interval 97-120 hours after SAH and thereafter decreased. ADMA started to increase in the time-interval 97-120 hours (p&lt;0.05). ADMA and CRP levels were significantly higher, and arginine/ADMA ratios were significantly lower in patients with a more severe condition (p&lt;0.05). Epileptic seizure activity, in sedated SAH patients, was recorded in 2/28 (7.1%) patients during 5/5468 (0.09%) hours of continuous EEG monitoring. Cerebrovascular disease was significantly more common as a cause of death in patients that had survived a SAH ≥ one year, compared to the population from the same area (p&lt;0.0001). Conclusions: Continuous infusion of sedative drugs was associated with low (&lt;200 nmol/L) cortisol levels. ADMA increased significantly after SAH, after CRP had peaked, indicating that endothelial dysfunction, with ADMA as a marker, is induced by a systemic inflammation. Patients with a more severe condition had significantly higher ADMA and CRP levels, and significantly lower arginine/ADMA ratio. Continuous sedation in sedated SAH patients seems to be beneficial in protecting from subclinical seizures. Conscious by Default : The Role of the Default Mode Network in Internal Awareness

Abstract In the 1990s researchers discovered task-deactivated regions in the human brain. Together, these areas make up the default mode network (DMN). It was originally proposed to act as a balancing mechanism between different brain systems, explaining the deactivations, but is now mostly studied with regards to internal awareness, such as daydreaming and mental imagery. The purpose of this thesis is to present a summary of DMN research, focusing on the network’s suggested role in internal awareness. This will be done by reviewing a wide variety of research that either explicitly or indirectly correlate default mode network features with aspects of consciousness and internal awareness. The subjective experience of being conscious have been a source of argument primarily among philosophers, but the qualities we feel are intimately linked to cognitive functions that are supported by the regions found in the DMN. Cognitive neuroscience may therefore be able to contribute to the concept of internal awareness and consciousness.

The Temporal Binding Window in Cross-Modal Sensory Perception : A Systematic Review

Previous research shows that integration of the senses is interchangeably dependent by  temporal neural mechanisms. One unsolved problem is how sensory timing differences in the brain is processed. In this systematic review (K = 18), audio-visual behavioral task paradigms are investigated with a focus on temporal binding window estimates. The results showed among other things that temporal integration is an adaptive neural process and that temporal acuity increases with age. Measurements between studies were sometimes incompatible which limited conclusions. Future studies should focus on standardizing operational parameters and compare within and between group designs.

Bakgrund  Fysisk aktivitet under skoltid är av stor vikt då barn och ungdomar rör sig allt mindre. Skolans elevhälsa, däribland skolsköterskor, arbetar med främjandet av fysisk aktivitet i skolan. Studier kring hur en fysioterapeut kan verka inom elevhälsan är idag begränsade. Hur skolsköterskor uppfattar att en fysioterapeut kan bidra i skolhälsoarbetet var därför av intresse att undersöka. Syfte  Att undersöka skolsköterskors uppfattning om behovet av en fysioterapeut inom skolverksamheten, samt undersöka vad de anser har betydelse för att främja fysisk aktivitet i skolan.  Metod Studien var av kvalitativ design. Fem skolsköterskor intervjuades genom enskilda semistrukturerade intervjuer. Kvalitativ innehållsanalys användes för bearbetning av data.   Resultat  Utifrån insamlade data identifierades nio kategorier med sammanlagt trettiotvå underkategorier. Kategorier om behovet av en fysioterapeut inom skolverksamheten var ”Efterfrågad kompetens”, ”Hänvisar till extern kompetens” och ”Osäkerhet kring fysioterapeutens roll”. Kategorier om främjandet av fysisk aktivitet var ”Individanpassat”, ”Hemsituationen påverkar”, ”Gemensamma organiserade aktiviteter”, ”Aktivitetsfrämjande miljöer”, ”Gemensamt förhållningssätt” och ”Utmaningar att förhålla sig till”.  Konklusion  Samtliga skolsköterskor ger uttryck för att det finns ett behov av en fysioterapeut, samtidigt framkommer det att de är osäkra kring hur rollen skulle se ut inom skolverksamheten. Främjandet av fysisk aktivitet i skolan ansågs påverkas av skolans organisation, personal, föräldrar och elever. Utmaningar i arbetet med fysisk aktivitet beskrivs bero på tidsbrist och bristande kunskaper inom området. / Background  Physical activity during school hours is of great importance since children and adolescents move less and less. The school health services, including school nurses, is working to promote physical activity in school. Studies who aim to examine how a physiotherapist can contribute to the school health services are limited. To examine how school nurses perceive the need for a physiotherapist in the school health services was therefore of interest to investigate.  Purpose To examine how school nurses perceive the need for a physiotherapist in the school health services, as well as examining what they consider to play a significant role in promoting physical activity in school. Method  The study was of qualitative design. Five school nurses were interviewed through individual semi-structured interviews. Qualitative content analysis was used to analyzing the data.   Result A total of nine categories and thirty-two subcategories was identified from the collected data. Categories about the need for a physiotherapist were “Requested expertise”, “Refers to external competence”, “Uncertainty about the role of the physiotherapist”. Categories about the promotion of physical activity were “Individualized”, “Home situation affects”, “Organized activities”, “The environment affects physical activity”, “Common approach” and “Challenges to relate to”.  Conclusion  The school nurses perceive that there is a need for a physiotherapist within the school health services. At the same time, it appears that they are a bit unsure about the physiotherapists’ role. The promotion of physical activity in school is considered to be affected by the schools’ organization, school staff, pupils and parents. The work with physical activity in school has certain challenges which is described to depend on shortage of time and lack of knowledge in the area.

Procrastination as a form of Self-regulation Failure : A review of the cognitive and neural underpinnings

The action of postponing an intended plan is often referred to as procrastination. Research on procrastination generally views the phenomenon as a form of self-regulation failure. Self-regulation refers to the conscious and non-conscious processes that enable individuals to guide their thoughts, feelings, and behaviors purposefully. Research indicates correlations between self-regulation and executive functions providing a fruitful integration. From a neuroscientific perspective, this integration generally associates the prefrontal cortex with top-down control whenever successful self-regulation is achieved. On the contrary, self-regulation failure appears to involve a bottom-up control, in which subcortical regions have greater influence on behavioral outcomes. Subcortical regions involved in emotional and rewarding processes, such as the amygdala and nucleus accumbens appears to lie at the coreof self-regulation failure, whereas cortical executive functions of regulating emotion and impulsive behaviors may contribute to successful self-regulation, thus overcoming procrastination. This thesis aims to obtain a deeper understanding of the mechanisms of procrastination, specifically investigating self-regulation failure and its relationship with executive functions and the neural underpinnings of self-regulation.

Rubber hand illusion and affective touch : A systematic review

The feeling of owning a body part is often investigated by conducting and manipulating the rubber hand illusion, a three-way integration of vision, touch, and proprioception. In the last decade, more research on the role of interoception, the sense of the body's’ internal state, in the illusion has been made. One of the studied factors has been the affective touch, a caress-like, gentle, touch that is performed at a slow specific speed (1-10 cm/sec). Affective touch activates the C tactile afferents which send interoceptive signals to the brain, specifically the insula. The present systematic review investigated the role affective touch has on the strength of the rubber hand illusion. A range of electronic databases was searched for papers reporting research findings published in English before March 20, 2020. Twelve different articles were identified, but only five papers met the inclusion criteria. This thesis looked at the results from these five different studies and compared the effect of affective touch and discriminative, regular, touch have on the rubber hand illusion to see whether there is a significant difference. The results could not show a main effect of stroking velocity, site of stimulation, or social touch, which are components of affective touch. The results was based on four different measurements, the subjective experience of the illusion, pleasantness ratings, proprioceptive drift, and temperature difference in the skin. Opposed what was hypothesized, it could not be demonstrated that affective touch would induce a stronger rubber hand illusion than discriminative touch.

Well-Being and Self-Compassion : A study on the relationship between well-being and self-compassion in a Cuban sample

Well-being is a human goal globally and has been considered so since the early days of philosophical thought. As the science of well-being has grown the last decades, well-being has widely been agreed to consist of two core components: Hedonic well-being and Eudaimonic well-being. These components have been studied to a large extent and further explored, conceptualized, and divided. How components of well-being interrelate is still being explored, as well of what drivers there are of experiencing well-being.Self-compassion is a relatively new field within psychology and cognitive neuroscience research which has been linked to components of well-being. Thus, the first aim of this study was to provide a theoretical overview of previous studies of the relationship between well-being and self-compassion. As research of self-compassion is still in its infancy, there are yet very few studies on self-compassion and its relationship to well-being in different cultures. The concept stems from East-Asian culture, and if self-compassion is important for well-being globally, or if it is depending on difference in culture, has been discussed. Hence, the second aim was to explore if there would be a potential relationship between self-compassion and well-being in a Cuban sample, as there is a lack of research in that region. 200 university students in Havana participated and filled in a questionnaire regarding subjective well-being, psychological well-being, and self-compassion. Results revealed that there was a significant relationship between well-being and self-compassion, and especially between psychological well-being and self-compassion. Components of well-being correlated with each other as well. These results go in line with previous research. Hence the present study suggests that the patterns of correlations between self-compassion and well-being is similar in Cuban university students as in other cultures studied so far.

The Therapeutic Potential of Psilocybin and 3,4-Methylenedioxymethamphetamine in the Treatment of Depression and Post-Traumatic Stress Disorder

The psychedelic psilocybin and the entactogen 3,4-methylenedioxymethamphetamine (MDMA) are being scientifically studied again after a long hiatus, and especially for their potential in the treatment of psychiatric disorders. Their profound effect on cognitive, perceptual, and affective processes have led to several clinical studies during the last decade that have forced the reconsideration of the utility of these substances. The research includes clinical trials with psilocybin-assisted psychotherapy for depressive and anxiety symptoms, and MDMA-assisted psychotherapy for the treatment of post-traumatic stress disorder (PTSD). The results have shown a significant reduction in depressive and anxiety symptoms in psilocybin-assisted psychotherapy, and in PTSD symptoms in MDMA-assisted psychotherapy, with acceptable adverse effects. Moreover, the reductions in symptoms have been shown to be sustained several years later. Given the results indicate short- and long-term safety and efficacy, even for treatment resistant conditions, this suggest that these substances administered with psychotherapy are promising and deserve to be taken seriously as a therapeutic tool. The present thesis provides an overview of the latest clinical studies on the treatment of depression, anxiety, and PTSD with psilocybin and MDMA, respectively, as well as reviews the history, mechanisms of action, the therapeutic process used with psilocybin and MDMA, and any adverse physiological and psychological effects of both substances.

Moral machines : The neural correlates of moral judgment and its importance for the implementation of artificial moral agency

Society and technology are advancing, in which morality is being artificially implemented into machines, often known as artificial moral agency. Along with this implementation, knowledge about the underpinnings of morality, such as the neurocognitive and ethical basis are an important matter. Human moral judgment has been speculated to be a function for survival, as it favors altruism and prosocial behavior. The neural correlates of moral judgment stem from several structures of the human brain that control cognitive and affective functions such as decision making, cognitive control, theory of mind (ToM) and empathy. In relation to these, the ventromedial prefrontal cortex (vmPFC) has been widely linked to moral behavior such as ToM and moral judgment. The anterior cingulate cortex (ACC) has been linked to regulation of conflict and the dorsolateral prefrontal cortex (dlPFC) to remain cognitive control, which both have strong correlations to moral behavior. Damage to areas such as the vmPFC and ACC have demonstrated abnormal response to guilt, ToM, empathy, risky behavior as well as sociopathic tendencies, which emphasizes the importance of these structures for human morality. By investigating research in the fields of cognitive neuroscience, moral psychology and machine ethics, this thesis aims at seeking out the importance of cognitive neuroscience for the development of artificial moral agency and to furthermore discuss the necessity of emotions in artificial moral agents, which naturally lack the affective part of moral judgment. Lastly, this thesis will cover some of the main dilemmas with this integration and some future implications.

Covert visual attention : An event-related potential study of the N2pc and PD components

In the study of covert visual attention, two event-related potential (ERP) components have been identified by earlier research. The N2 posterior contralateral (N2pc) component has been suggested to index the enhancement of attention to a specific lateralized target item. The distractor positivity (PD) component has been suggested to index the suppression of distractors appearing in the same search array. Earlier studies have reported different latencies for the PD component depending on the task and experiment. Furthermore, the N2pc and the PD component are not always elicited in the same experiment. Relative target-to-difficult-distractor placement have been shown to affect the mean amplitude of the N2pc. Less is known about how different relative placements affect the PD component. The aim of the present study was to try and elicit both an N2pc and a PD component in the same visual search paradigm. The PD was recorded later time-window which previous studies have suggested to indicate the ending of attention to a previously attended target. Three relative placements were analysed, horizontally opposite, vertically opposite and diagonally opposite. When combining all three relative placements an N2pc component was elicited contralateral to the target. No PD component was found when combining all relative placements. A larger mean amplitude N2pc was measured for the vertically opposite condition. The results are not in line with previous research, that have found the N2pc to be smaller in conditions where both target and distractor are on the same side of the visual field. However, when comparing upper and lower visual field targets the N2pc was found to be larger for lower visual field stimuli, which is in line with previous research. A larger mean amplitude for the PD was found in the diagonally opposite condition. Earlier research has suggested that when difficult distractor and target are located on separate sides of the visual field, this leads to successful inhibition, indexed by the PD component. In contrast to earlier research a larger PD component was not found for upper visual field stimuli. The present study differs from previous studies in the way the target and difficult distractor were placed and analysed. By separating what has previously been called “opposite side” condition into two separate conditions diagonally opposite and horizontally opposite the results from the present study seem to suggest that these two conditions are not synonymous. However, the results should be regarded with caution due to the small sample size. Furthermore, the horizontally opposite side condition also differs from previous studies with regards to relative target and distractor distances, which could have had an effect on the results.
