Sjuksköterskors erfarenhet av att ge omvårdnad när en patient har PICC-line. : En intervjustudie

Bakgrund: Perifert insatt central venkateter (PICC-line) är en medicinteknisk produkt som används till bland annat provtagning, nutrition och infusioner. Det blir allt vanligare att patienter får PICC-line som central infart och att dessa patienter får sin omvårdnad på vanliga vårdavdelningar. Det saknas studier om hur sjuksköterskor på vårdavdelningar upplever förändringen av ett ökat antal patienter med PICC-line och hur väl de anser sig vara förberedda och utbildade för att kunna ge säker vård. Syfte: Syftet med studien är att beskriva sjuksköterskors erfarenhet av att ge omvårdnad när en patient har PICC-line. Metod: Empirisk intervjustudie med kvalitativ ansats som analyserades med konventionell innehållsanalys beskriven av Hsieh och Shannon. Resultat: Resultatet visade att sjuksköterskorna hade olika erfarenhet av att ge omvårdnad till patienter när de har PICC-line, vilket de beskrev ur flera olika synvinklar. I analysen framkom tre av dessa speciellt tydligt, vilka utgjorde kategorierna. De tre kategorierna benämndes som ”påverkar arbetsbelastningen”, ”påverkar säkert handhavande” och ”påverkar omvårdnaden”. Slutsats: Enligt sjuksköterskornas erfarenhet påverkades arbetsbelastningen när en patient hade PICC-line. Den beskrevs främst kunna vara tidssparande och förenklande men också stressande då problem uppstod. Sjuksköterskorna kunde uppleva osäkerhet vid handhavandet av PICC-line relaterat till bristande utbildning och förberedelse, men upplevde samtidigt teamarbetet och stödet från kollegor som betryggande. PICC-line kunde resultera i minskat lidande för patienten, men beskrevs också som begränsande för patienten.

Analysing the effect of DSM projects at South African cement factories / Johannes Paulus Spangenberg

In any developing country an increasingly higher demand for electricity supply exists. South Africa experienced load shedding during late 2007 and early 2008 and again in 2014 due to a supply shortfall. New power stations are being built to increase the capacity of the national power grid. However this is a lengthy process. Demand Side Management (DSM) was adopted by Eskom’s Integrated Demand Management (IDM) division. DSM is a short-term solution to stabilise the national grid in South Africa by managing the electricity demand on the consumer’s or client’s side. DSM aims to reduce the electricity consumption with immediate results in the short-term. DSM projects were successfully implemented at nine South African cement factories since 2012. Cement factories are ideal for the implementation of DSM projects for the following reasons: cement factories are energy intensive; have adequate reserve production capacity; sufficient storage capacity and interruptible production schedules. The aim of this study is to analyse the effect of DSM projects at South African cement factories. A detailed understanding of the cement production process is a prerequisite. Therefore a critical review of energy utilisation in the cement industry was conducted. Previous work done in the cement production field is evaluated to identify the possible literature shortfall on DSM projects. A set of five distinctive parameters was derived from the literature survey to quantify the possible effects of DSM projects at cement factories. The parameters are demand reduction and electricity cost; production targets; infrastructure; product quality and sustainability. One cement factory, Factory #1, was selected as a primary case study for the analysis model. Factory #1 was used to determine and quantify the effects of DSM projects at cement factories. A simulation was developed to verify the analysis model outcome. DSM projects were implemented at various factories in South Africa and the results from nine sites were used to validate the aim of this study. The study concluded that most DSM projects at South African cement factories were sustainable. Both the electricity supplier and the factories benefitted from the projects. The funding received from Eskom to implement DSM projects is a short-term initiative. The long-term effects of methamphetamine on depressive-like behaviour and neuroplasticity in stress-sensitive rats / Moné Mouton

Methamphetamine (METH) abuse has become a fast growing drug problem that has developed into a global epidemic. In fact, METH is one of the most commonly abused substances with an estimated 35 million abusers worldwide and is said to be the second most popular illicit drug. The Western Province of South Africa has seen a dramatic increase in drug abuse in recent years where METH is the primary or secondary drug of abuse. Interestingly, more than 50% of these individuals are under the age of 20 years. The longer duration of euphoric effects of METH has attracted many users away from cocaine in favour of METH. In addition to the rapid euphoric effect of METH, the direct short-term effects include arousal, reduced fatigue, an increase in blood pressure, reduced appetite as well as sustained attention. Chronic METH abuse may result in debilitating and long-lasting effects that includes mood disorders such as depression. Studies suggest a strong relationship between exposure to adverse environmental factors early in life and the later development of a neuropsychiatric disorder, such as depression. However, these severe consequences do not seem to invoke cessation of the drug. The euphoric and addictive properties of METH causes users to abuse the drug with an increase in frequency and dose, even though it might not have been their original intention. The primary objective of this study was to investigate the effect of early-life administration of METH to stress-sensitive (Flinders Sensitive Line - FSL) and control (Flinders Resistant Line - FRL) rats on depressive-like behaviour and regional brain monoamine levels later in life. The study implemented a sixteen-day period for administration of METH or a vehicle control from postnatal day 19 (PnD19) to postnatal day 34 (PnD34). The latter developmental stage corresponds to pre-adolescence in the rat when neurological development are similar to that seen in human adolescents, and represents the stage when drug abuse is most common in humans. Chronic dosing of METH and saline was performed twice daily at 09:00 and at 15:00. The animals received a sub-cutaneous (SC) escalating dose regimen of METH during the 16 day period (mimicking binging behaviour in humans), with every dose escalating in increments of 0.2 mg/kg from 0.2 mg/kg to 6.0 mg/kg. The study then investigated whether early-life administration of METH would cause depressive-like behaviours directly after the injection period (immediate drug effects before withdrawal on PnD35) or later in life (after the withdrawal period in early adulthood on PnD60). The behavioural effects were assessed in a battery of tests and thereafter the rats were sacrificed and the frontal cortex removed and snap frozen for later analyses of altered neurochemistry. The study demonstrated that chronic METH treatment during pre-adolescence induces significant behavioural changes related to depression in humans directly after the injection period (PnD35) and later in life (PnD60). The animals displayed antidepressant-like behaviour in the forced swim test (FST) before withdrawal, yet a depressogenic effect was observed 25 days post-withdrawal. This effect also seems to be additive to the congenital depressive-like phenotype of FSL rats, suggesting a role for genetic susceptibility. This observation would be in line with the two-hit hypothesis of depression, suggesting that the manifestation of depression will result when a genetic predisposition is followed by an environmental stressor (i.e. METH) later in life. The data suggests a working hypothesis that individuals that already have a predisposition to depression may be more susceptible to developing depression when abusing METH. The fact that the FSL control rats were more immobile than FRL control rats also confirmed the face validity of the FSL genetic rat model of depression. Locomotor activity assessment indicated that METH treatment decreased locomotor activity in FSL and FRL rats compared to their vehicle controls on PnD35 but not on PnD60. It is important to note that the effects observed in locomotor activity could not have contributed to the immobility observed in the FST, confirming that the immobility in the FST indeed reflects psychomotor and not locomotor effects. The study also demonstrated that METH significantly lowers social interaction behaviour in both FRL and FSL rats, both immediately following drug treatment (PnD35) and after withdrawal (PnD60). It is therefore clear that this effect of METH is long-lasting, putatively related to neurodevelopmental effects. In addition, the rats investigated the familiar object for a greater amount of time in the novel object recognition test (nORT) on PnD35 and PnD60 and may be the result of loss of recognition memory for the familiar object. This data confirms that METH results in cognitive memory deficits probably due to sustained adverse neurodevelopmental effects. Neurochemical analyses of the frontal cortex indicated decreased serotonin (5-HT) and norepinephrine (NE) levels on PnD35. METH is widely recognised for its pro-inflammatory effects, while the reduced 5-HT levels observed may have been the result of an increase in circulating pro-inflammatory cytokines. Neurochemical analyses provided thought-provoking data concerning the role of the permissive hypotheses of depression, indicating that dopamine (DA) is most likely not responsible for the behavioural effects observed, at least under the current study conditions, whereas 5-HT is decidedly more involved than expected. The data also suggest that depletion in NE plays a role in the development of depressive-like behaviours following METH exposure. Based on these findings, we propose that disturbances in 5-HT and NE are a crucial mechanism in how METH abuse may precipitate or worsen depressive-like symptoms in individuals who abuse METH. It should be noted that this study does not discard the role of DA in the development of depression after METH exposure, although under the current study conditions it appears that DA does not play a central role. The current study demonstrated that pre-adolescent exposure to METH can reproduce most of the behavioural changes seen in depressed individuals, and that these behavioural data can be used to identify causal neurochemical factors. Environmental stressors such as METH abuse should be regarded as an additional diagnostic criterion and is relevant to an accumulative risk factor hypothesis. Evaluation of different safety aspects of centerline rumble strips

Based on the literature review center line rumble strips (CLRS) are considered to be effective safety countermeasure for reducing crossover accidents on two-lane, two-way roadways. CLRS are indentations milled into the center line of undivided two-lane, two-way roadways to warn driver of drifting into upcoming traffic. Researchers at Kansas State University (KSU) have conducted studies on CLRS and retroreflective pavement markings (RRPM) installed over them. Based on the literature review and the survey conducted on motorcycle riders it can be concluded that majority of riders believe in the effectiveness of CLRS and they recommend the KDOT to implement CLRS in more locations. From the survey conducted on residents of US 40 it can be concluded that RRPM help them in providing visual guidance. They also noticed that there is considerable deterioration of RRPM over CLRS on US 40. From the studies conducted on US 24, US 50 and US 40 it can be concluded that wet retroreflectivity of pavement markings installed over CLRS is considerably lower than dry retroreflectivity. In locations without CLRS wet retroreflectivity of RRPM is higher than dry retroreflectivity. Also, the analysis performed on retroreflectivity measurements from US 24, US 50 and US 40 show that retroreflectivity follows a linear reduction in performance over time. In addition, a new methodology was developed for evaluating RRPM over CLRS. Various tests and analysis were performed and the new method seems effective.

Simulation and design methodology for hybrid SET-CMOS logic at room temperature operation / Simulation et méthodologie de conception de circuits logiques hybrides SET-CMOS opérants à température ambiante

The purpose of this thesis is to research the possibility of realizing hardware support for hybrid single electron transistor (SET)-CMOS circuits by a systematic approach of design, analysis and simulation. The metallic SET transistors considered in this work are fabricated within the chip interconnect layers using CMOS back-end-of-line (BEOL)-compatible processing. The CMOS process integration can be divided into front-end-of-line (FEOL) and BEOL processes. The FEOL includes processes required to form isolated CMOS transistors whereas BEOL is the second portion of the IC fabrication where the devices get interconnected through the wiring using multiple layers of dielectrics and metals. Therefore, metallic SET circuits can be easily stacked above the CMOS platform presenting a low cost, low thermal budget, improving the overall yield at high-volume production of highly integrated systems. This considerably decreases the interconnect parasitics and increases the density of functions while maintaining the overall acceptable performance. Many problems such as low current drivability, delay and small voltage gain that hinder SET technology for its implementation in integrated circuits can be alleviated by intelligent circuit design. Although a complete replacement of CMOS by SETs is unlikely in the near future, an augmentation of CMOS with SETs is desirable if interfacing from and to CMOS works well. Interfacing from CMOS to SET circuitry is simple as the current and voltage levels are small and in accessible range. But interfacing CMOS from SET circuits is delicate due to SET logic's low current driving capability for CMOS and its interconnect. There is no concrete research on the interface issue wherein a SET-only circuitry drives a CMOS and its interconnects. For such hybridization to become possible, it is necessary to demonstrate the SET logic driving capability for CMOS with sufficient current drive and output voltage. The core SET logic can be designed to operate at low voltage, but at the interface the output of the SET logic must be in a voltage range that can be fed to a CMOS input for proper logic functionality. It is hence necessary to develop and adopt a systematic design methodology for such hybrid circuits at a specific technology node for room temperature operation. In this thesis we will look at a generalized design methodology that can be applied to (a) develop a fabrication model with parasitic effect of a hybrid SET-CMOS and SET-only circuits, (b) design and analyze the SET based fundamental building block in hybrid SET-CMOS or SET-only circuit and (c) simulate such a circuitry to assess its merits. More specifically, we will address the interfacing issue of such hybrid circuits in which we exploit the maximum capability of a SET logic in terms of driving capability, voltage response and power for a room temperature operation. The result of this research motivates the application of SET logic in 2 stages realizing some properties beyond those of CMOS devices. The first stage is the heterogeneous integration at chip level around a CMOS core. In such a circuitry, the SET introduces new functionalities such as reconfigurable logic, random number-based circuits, and multiband filtering circuits that can be combined with CMOS based general purpose processors or I/O signal restoration. The second stage of application is to use a new information processing technology focussed on a "new switch" exploiting a new state variable to provide functional scaling substantially beyond that attainable solely with ultimately scaled CMOS.

Leveraging PLC Technology to Interface Network Sensors and Subsystems on Legacy Platforms

Adding new aircraft functionality or installing special purpose instrumentation often requires significant engineering and aircraft down time to complete. PLC-based networks can reduce project cost and schedule by enabling localized aircraft modifications and leveraging existing aircraft wiring for the Ethernet medium. PLC standards continue to evolve and achieve greater throughput rates and noise mitigation. Ethernet communications have been tested over AC and DC power busses, data busses, and discrete wiring. PLC networks have been successfully demonstrated in avionics test beds and aircraft (including live video transfers) without causing interference to the basic systems or the underlying wiring functionality. PLC transceivers provide a cost effective solution to the adding Ethernet capabilities or Ethernet-based subsystems to existing aircraft.
