satisfaction provided by the use hearing protectors in pop-rock musicians.
Background: Studies have shown evidences of the risk of pop-rock musicians
to develop hearing loss and other symptoms related to amplified music, as well
as to adopt preventive measurements to protect their hearing. Method: It was
evaluated 24 pop-rock musicians, male gender and ages between 25 to 45
years old, with professional experience between 3 to 21 years. It was
accomplished: anamnesis, audiometric evaluation, transient otoacoustic
emission (TEOAE), distortion product otoacoustic emission (DPOAE) and also
was applied an ear protective devices satisfaction assessment questionnaire.
Results: The prevalence of noise-induced hearing loss was 20.8%. In normal
audiograms, there was a notch in high-frequencies in 58.4%, in at least one ear.
The responses of TEOAEs were absent in 45.8% of the musicians and
DPOAEs were absent in 58.4%. Concerning to satisfaction with the ear
protective devices usage, 75.0% of the musicians evaluated them above grade
7.0. Conclusions: There was a high prevalence of hearing losses as well as
alterations in TEOAEs and DPOAEs responses. Therefore, it is fundamental to
develop and implement hearing loss preventive programs for musicians. There
was a positive tendency towards the use of the ear protective devices / Objetivo: Avaliar a audição e o uso de protetores auditivos em músicos de
bandas de pop-rock. Justificativa: Estudos demonstram evidências dos
músicos de pop-rock apresentarem risco para o desenvolvimento de perda
auditiva e outros efeitos associados à exposição à música amplificada, bem
como a necessidade de adotar medidas de proteção à audição desses
profissionais. Método: Foram avaliados 24 músicos de pop-rock, do sexo
masculino, com idades entre 25 e 45 anos e com tempo de profissão entre três
e 21 anos. Os sujeitos responderam a uma anamnese, realizaram avaliação
audiométrica, realizaram os testes de emissões otoacústicas evocadas por
estímulo transiente (EOAET) e emissões otoacústicas produto de distorção
(EOAPD) e preencheram um questionário sobre a satisfação após o uso do
protetor auditivo. Resultados: A prevalência de perdas auditivas sugestivas de
PAIR foi de 20,8%. Dentre os audiogramas classificados como normais 58,4%
apresentaram entalhe audiométrico em pelo menos uma orelha. No teste de
emissões otoacústicas, 45,8% dos músicos tiveram respostas ausentes nas
EOAET e 58,4% nas EOAPD. 75,0% dos músicos forneceram nota maior do
que 7,0 para refletir sua satisfação no uso do protetor auditivo. Conclusões:
Tanto a prevalência de perdas auditivas, quanto o número de ausências nos
testes de EOAET e EOAPD foram elevados. Assim sendo, é necessária a
implementação de um programa de prevenção de perdas auditivas com
músicos. Houve uma tendência favorável à aceitação do protetor auditivo

OBJECTIVE: to study the register of OAE among two different groups of workers of a graphical industry: exposed and non-exposed to occupational noise and with regular audiometric thresholds. METHOD: Transient evoked otoacustic emissions (TEOAE) and distortion product otoacustic emissions (DPOAE) were researched in 158 ears of 79 subjects, all male, matched by two groups: exposed (42) and non-exposed to occupational noise (37). RESULTS: Among the non-exposed group was observed 54.1% of present response in both ears in TEOAE, and 24.3% have absent responses in both ears in the DPOAE test. Among the exposed group, 14.3% has presence responses in TEOAE in both ears, and 61.9% have absence in two ears. The statistical analysis indicated noise exposure effect at the percentage of presence of EOAPD in f2(s) of 2002, 5042 and 6348 Hz and in the general result both at TEOAE and at DPOAE. The present responses in non-exposed group was significantly higher than in exposed group in both tests. CONCLUSIONS: The test of otoacustic emissions was an important procedure in the early identification of auditory alterations, differentiating exposed and non-exposed subjects to occupational noise / INTRODUÇÃO: O teste de emissões otoacústicas representa um importante instrumento de vigilância epidemiológica de alterações auditivas decorrentes da exposição a ruído. OBJETIVO: estudar os registro de EOA em dois grupos de trabalhadores de uma indústria gráfica: expostos e não expostos a ruído ocupacional e com audiometrias dentro dos padrões de normalidade. MÉTODO: Foram realizadas as emissões otoacústicas por estímulo transiente (EOAET) e por produto de distorção (EOAPD) em 158 orelhas de 79 sujeitos, sendo todos do sexo masculino, divididos em dois grupos: com exposição a ruído ocupacional (42) e outro sem exposição (37). RESULTADOS: No grupo não exposto observou-se que 54,1% dos indivíduos têm EOAET presentes nas duas orelhas e 24,3% têm EOAET ausente nas duas orelhas. Já no grupo exposto, 14,3% têm EOAET presente nas duas orelhas, e 61,9% têm ausência em ambas às orelhas. As análises estatísticas indicaram efeito de exposição a ruído na porcentagem de presença de EOAPD nas f2(s) de 2002, 5042 e 6348 Hz e no resultado geral tanto nas EOAET quanto nas EOAPD. A ocorrência de respostas presentes nos não expostos foi significativamente maior que nos expostos em ambos os testes. Com o objetivo de verificar a necessidade de aplicação dos testes de Emissões Otoacústicas Evocadas por Estímulos Transientes (EOAT) e Potencial Evocado auditivo de Tronco Encefálico (PEATEA) combinados e comparar a ocorrência de resultados " Passa" entre os grupos de berçário comum e de UTI-Neonatal, foram avaliados 150 recém-nascidos de berçário comum e 70 recém-nascidos de UTI-Neonatal. Os testes de EOAT e PEATEA apresentaram ocorrência de resposta semelhante quando os grupos foram analisados separadamente, porém na comparação entre os grupos, o grupo de berçário comum apresentou uma ocorrência maior de resposta " Passa" tanto nas EOAT quanto no PEATEA quando comparado com o grupo de UTI-Neonatal / The objective of the study was verify the necessity of the Transient evoked Otoacustic Emission (TEOAE) and Automatic Auditory Brainstem Response (AABR) combined and to compare the "Pass" results between the well baby nursery group and Newborn Intensive Care Unit (NICU). Time-related Aspects of Otoprotection : Experimental Studies in RatLidian, Adnan January 2013 (has links)
Intratympanic injection of various otoprotectants through the round window membrane (RWM) might become available in the near future as an alternative to the currently available medical and surgical methods used to treat several inner ear diseases. The most common outcome of such diseases is sensorineural hearing loss (SNHL). Two examples of these otoprotectants are Edaravone and Brain-Derived Neurotrophic Factor (BDNF), both of which have already proved effective against noise-induced hair cell loss, barotrauma and ototoxicity caused by cisplatin. In four different studies we used two electrophysiological methods, auditory brainstem response (ABR) and distortion product otoacoustic emission (DPOAE), to study the effects of tobramycin and Pseudomonas aeruginosa exotoxin A (PaExoA) on the inner ears of 129 male Sprague-Dawley rats. In two investigations, not only the otoprotective effects of Edaravone on tobramycin-induced ABR threshold shifts and PaExoA-induced DPOAE threshold changes, were studied but even different application times, in order to establish in which interval it was still possible to achieve effective otoprotection.We found that Edaravone gave otoprotection from tobramycin when injected simultaneously or within 7 days, but it had only a limited effect on the changes in DPOAE thresholds caused by PaExoA when injected 1, 2, or 4 hours after the exotoxin. The effect of BDNF on PaExoA-induced ABR threshold shifts was investigated in two studies, where different doses of intratympanically injected PaExoA were used and where BDNF was applied simultaneously, 12 or 72 hours efter exotoxin instillation. We found that BDNF had an otoprotective effect on SNHL induced by different doses PaExoA when injected simultaneously or with no more than 12 hours delay.

The study investigated the feasibility of including DPOAEs in the annual medical surveillance test battery for the identification of NIHL in a group of employees in a manufacturing industry in KwaZulu-Natal. Feasibility was investigated by exploring the sensitivity, specificity and predictive efficiency of DPOAEs, the ability of DPOAEs to detect subtle noise-induced cochlea
changes, the test-retest reliability of DPOAEs and lastly, the duration of time taken to conduct the DPOAE test bilaterally. A cross-sectional and repeated measures within-in participant design was utilized in the study. A purposive convenience sampling technique was used, as well as a
stratified sampling approach in order to realize objective two of the study. The study consisted of 60 participants, which were further stratified into four test groups, i.e. Group A: 0-3 years, Group B: 3.1-6 years and Group C: 6.1-9 years and Group D: 9.1-13 years of working within the beverage manufacturing industry. A high sensitivity and negative predictive value was reported in the current study, suggesting that DPOAEs may be able to identify those who present with subtle cochlea changes as a result of exposure to occupational noise. The sensitivity of DPOAEs was 100% at 1, 2, 4, 6 and 8kHz in the right ear and at 4 and 6kHz in the left ear. The specificity of DPOAEs in the current study ranged between 55%-97% across the frequency range in the right ear and 49%-88% in the left ear. A negative predictive value of 100% was obtained bilaterally across the frequency range, except at 8kHz in the left ear. Visual inspection of the DPgram in the current study revealed a bilateral reduction in DPOAE amplitudes for all test groups in the high frequency region of the DP-Gram, namely, 5477Hz and 7303Hz, in the absence of a statistically significant difference (p>0.05). A greater frequency range appears to be affected in
this group of workers, indicating that the type of noise, namely, impulse noise, may result in cochlea changes. Corresponding changes on the pure tone audiogram were not observed, however, noise notch configurations were observed for the groups with a longer history of noise exposure. This was not seen bilaterally as is typically expected with NIHL. Good test-retest
reliability across the frequency range obtained in the current study further indicates the feasibility of including DPOAEs in the annual medical surveillance test battery. Additionally, the current study calculated an average of 86 seconds (1 minute 26 seconds) to conduct the DPOAE
test bilaterally, confirming that DPOAEs are a quick test to administer. The findings of this study suggest that DPOAEs may be used to monitor early subtle noise-induced cochlea changes for workers exposed to noise in the beverage manufacturing industry as part of the annual medical

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / The organophosphate pesticides are widely used in agriculture, and the widespread application of these agents, without the appropriate use of bio-security measures, has contributed to environmental degradation and the increased incidence of occupational poisoning, becoming one of the main public health problems in rural areas. Studies show that the ototoxic agents, in addition to the peripheral vestibular and auditory systems compromise, also impair central auditory pathways. Among the major toxic agents that can lead to hearing loss are the solvents, metals, suffocating and organophosphate pesticides. This study examined the acute ototoxic action of a pesticide on the peripheral auditory system. This is a
prospective experimental study. We used male albino guinea pigs, divided into three groups, which was administered distilled water (group 1 - control), pesticide - 0.3 mg/Kg/day (group 2), pesticide - 3 mg/Kg/day (group 3), for seven consecutive days. The pesticide used was
Tamaron® (methamidophos). The auditory functional evaluation was performed using the Distortion Product Otoacoustic Emissions (DPOAE) and Auditory Brainstem Response (ABR), both performed before and immediately after the application of the pesticide. The anatomical assessment was performed with scanning electron microscopy. The guinea pigs subjected to pesticide had cochlear morphological changes, with lesions in three turns
examined in electron microscopy, which was increased according to the dosage received from the agent. In all animals the DPOAE was present, but it was verified that the signal/noise ratio of the frequencies of 1.500 and 6.000 Hz in DPOAE of groups 2 and 3 showed statistically
significant difference when compared to control group, indicating possible cell impairment. There were no statistically significant changes in functional assessment of VIII nerve when evaluated by wave I of ABR. It is concluded that the organophosphorous can be considered as
a harmful acute agent of outer hair cells, seen the correlation between the dose applied and the amount of changes observed by electron microscopy, however, the acute exposure to Tamaron® did not cause functional alteration of the peripheral auditory system. / Os agrotóxicos organofosforados são amplamente utilizados na agricultura, e a elevada aplicação desses agentes, sem o emprego das devidas medidas de biossegurança, vem
contribuindo para a degradação ambiental e para o aumento da incidência de intoxicação ocupacional, tornando-se um dos principais problemas de saúde pública no meio rural.
Pesquisas demonstram que os agentes ototóxicos, além de comprometer os sistemas auditivo e vestibular periféricos, provocam ainda alterações nas vias auditivas centrais. Dentre os principais agentes químicos que podem levar à perda auditiva incluem-se os solventes, os metais, os asfixiantes e os agrotóxicos organofosforados. O objetivo deste estudo foi analisar a ação ototóxica aguda de um agrotóxico do grupo dos organofosforados no sistema auditivo periférico. Trata-se de um estudo experimental prospectivo, realizado em cobaias albinas machos, divididas em três grupos, nos quais se administrou água destilada (grupo 1 - controle), agrotóxico - 0,3mg/Kg/dia (grupo 2), agrotóxico 3 mg/Kg/dia (grupo 3), durante sete dias consecutivos. O agrotóxico utilizado foi Tamaron® (metamidofós). A avaliação auditiva funcional foi realizada utilizando-se Emissões Otoacústicas Produto de Distorção (EOAPD) e Potencial Evocado Auditivo de Tronco Encefálico (PEATE), ambos realizados antes e imediatamente após o período de aplicação do agrotóxico. A avaliação anatômica foi realizada com Microscopia Eletrônica de Varredura. As cobaias submetidas ao agrotóxico apresentaram alterações morfológicas cocleares, com lesões nas três espiras analisadas na microscopia eletrônica, intensificadas de acordo com a dosagem recebida do agente. Na avaliação auditiva funcional, todas as cobaias apresentaram EOAPD presentes, no entanto verificou-se significância estatística nos valores da relação sinal/ruído das frequências de
1.500 e 6.000 Hz das EOAPD das cobaias dos grupos 2 e 3 quando confrontados com os achados dos animais do grupo controle, indicando um possível sofrimento celular. Não foram
encontradas alterações estatisticamente significantes na avaliação do VIII par craniano por meio da análise da onda I do PEATE. Concluiu-se que o organofosforado pode ser
considerado um agente lesivo agudo das células ciliadas externas visto a correlação entre a dosagem aplicada e a quantidade de alterações observadas à microscopia eletrônica,
entretanto a exposição aguda ao Tamaron® não causou alteração funcional do sistema auditivo periférico.

INTRODUÇÃO: Uma possibilidade de pesquisa em audição é a supressão das emissões otoacústicas, que verifica a diminuição de sua magnitude a partir de estimulação acústica simultânea. Acredita-se que esta diminuição ocorra devido ao papel da via eferente auditiva. OBJETIVO: verificar a atividade do sistema auditivo eferente, por meio da supressão contralateral das emissões otoacústicas, em neonatos com e sem indicadores de risco para perda auditiva. MÉTODO: foram avaliados 21 neonatos com indicadores de risco para perda auditiva (grupo estudo) e 30 neonatos sem indicadores (grupo controle) por meio das emissões otoacústicas evocadas por estímulo de freqüência específica \"tone burst\" em 2000 e em 4000 hertz, na ausência e na presença de ruído branco contralateral. RESULTADOS: a orelha direita e o grupo controle apresentaram vantagem na amplitude das EOA TB. Os valores médios de \"Response\" para 2000 hertz foram 17,73 dB no grupo controle e 16,55 dB no grupo estudo, para o gênero feminino e 16,63 dB no grupo controle e 16,12 dB no grupo estudo, para o gênero masculino. Para 4000 hertz, os valores médios de \"Response\" foram 14,63 dB no grupo controle e 15,09 dB no grupo estudo, para o gênero feminino e 18,57 dB no grupo controle e 15,06 dB no grupo estudo, para o gênero masculino. Na supressão, houve vantagem da orelha direita, do grupo controle para o gênero feminino e do grupo estudo para o gênero masculino, embora sem plena significância estatística. Os valores médios de supressão para a variável \"Response\" em 2000 hertz foram 1,66 dB no grupo controle e 0,84 dB no grupo estudo, para o gênero feminino e 1,41 dB no grupo controle e 2,07 dB no grupo estudo, para o gênero masculino. Para 4000 hertz, os valores médios da supressão em \"Response\" foram 1,29 dB no grupo controle e 0,98 dB no grupo estudo, para o gênero feminino e 1,75 dB no grupo controle e 2,14 dB no grupo estudo, para o gênero masculino. CONCLUSÃO: A partir de estímulos de freqüência específica, em 2000 e em 4000 hertz, foi possível verificar o efeito inibitório da via eferente auditiva sobre a amplitude das EOA, em neonatos. A utilização de estímulos de freqüência específica pode contribuir para a avaliação da função auditiva periférica e central nesta população. / INTRODUCTION: A possible hearing research is the suppression of otoacoustic emissions which verifies a decrease of its magnitude from a simultaneous acoustic stimulation. It is assumed that this decrease occurs because of the role of the auditory efferent pathway. AIM: to verify the activity of the auditory efferent pathway by the contralateral suppression of OAE in neonates with and without risk factors for hearing loss. MEHOD: 21 neonates at risk for hearing loss (study group), and 30 neonates without risk factors for hearing loss (control group) were assessed through otoacoustic emissions evoked by a frequency specific stimulus, \"tone burst\", at 2000 and at 4000 hertz, with and without contralateral white noise. RESULTS: the right ear and the control group presented advantage in the amplitude of OAE TB. The mean values of \"Response\" at 2000 hertz were 17,73 dB in the control group and 16,55 dB in the study group, for female subjects, and 16,63 dB in the control group and 16,12 dB in the study group, for male subjects. At 4000 hertz, the mean values of \"Response\" were 14,63 dB in the control group and 15,09 dB in the study group, for female subjects and 18,57 dB in the control group and 15,06 dB in the study group, for male subjects. In the suppression, there was an advantage of the right ear, of the control group for female subjects, and of the study group for male subjects, although without statistical significance. The mean values of suppression for the variable \"Response\" at 2000 hertz were 1,66 dB in the control group and 0,84 dB in the study group, for female subjects, and 1,41 dB in the control group and 2,07 dB in the study group, for male subjects. At 4000 hertz, the mean values of suppression for \"Response\" were 1,29 dB in the control group and 0,98 dB in the study group, for female subjects, and 1,75 dB in the control group and 2,14 dB in the study group, for male subjects. CONCLUSION: From frequency-specific stimuli at 2000 and at 4000 hertz, it was possible to verify the inhibitory effect of the auditory efferent pathway upon the amplitude of otoacoustic emissions in neonates. The use of frequencyspecific stimuli may contribute for the assessment of the central auditory function in this population.

Data collected during the implementation of the Neonatal Hearing Screening (NHS) need to be recorded, evaluated and disseminated in the scientific environment in order
to ensure its quality and to encourage the creation and improvement of other services. Risk indicators (RI) for the hearing loss can influence in the occurrence of transients evoked otoacoustic emissions (TEOAE) and distortion product (DPOAE), as well as in the result of the NHS. This study aimed to analyze the NHS at the University Hospital of Santa Maria during one year; to verify the influence of the presence of RI for the
hearing loss on the TEOAE and DPOAE as well as on the result of NHS with TEOAE. The sample consisted of 1198 neonates and infants users of Unified Health System.
The criterion for passing was the presence of Cochlea Palpebral Reflex (CPR) and TEOAE bilaterally. RI were investigated through medical history and records, and were
related to pass/fail variables in the NHS and to the occurrence of TEOAE and DPOAE for each frequency measured. Children who failed were retested after two weeks, using
the same procedures. The evaluation of auditory evoked potentials in brainstem response (ABR) was carried out when the absence of Cochlea Palpebral Reflex and/or
TEOAE persisted. When alterations in the examination were observed, children were referred to the use of prostheses. The NHS could not be considered universal, since the rate of children affected was 66.56%. The first hearing screening occurred up to 28 days of life in 78.80% (n = 944) The rate of children who passed the first test was
91.49% (n = 1096). Among the ones who failed, 17.65% (18) did not attend the retest. Of the 84 children who returned, 17 failed again and were referred for ABR. Eight
children presented deafness, and the rate of hearing loss was 0.67%. The referral for diagnosis rate was 1.41% and the false-positive result was 0.17%. The result of the
NHS is dependent on the occurrence of RI for the hearing impairment. The absence of TEOAE and DPOAE is dependent on the presence of RI for hearing impairment. Besides, the presence of TEOAE and DPOAE is dependent on the absence of
indicator(s) in some ears and frequencies. The rates of referral for diagnosis and falsepositive results of this study suggest that the program is implemented effectively and demonstrate the adequate experience professionals who work with it have. However, the universal and mandatory character of the NHS is still to be achieved. The presence
of RI may influence in the occurrence of EOAE and in the result of the NHS. The RI that offered the greatest risk of failure in the NHS was jaundice. Some key aspects in
the planning, effectiveness, maintenance and improvement of the program have been outlined, and some of them are already being used. / Os dados coletados durante a aplicação dos programas de Triagem Auditiva Neonatal (TAN) precisam ser registrados, avaliados e divulgados no meio científico, a fim de assegurar a qualidade e incentivar a criação e aprimoramento de outros serviços. Os indicadores de risco (IR) para a deficiência auditiva podem influenciar na ocorrência das Emissões Otoacústicas Evocadas Transientes (EOAET) e produto de distorção (EOAEPD), e no resultado da TAN. Os objetivos desta investigação foram analisar o programa de TAN no Hospital Universitário de Santa Maria (HUSM) durante um ano;
verificar a influência da presença de IR para a deficiência auditiva sobre as EOAET e EOAEPD e sobre o resultado da TAN com EOAET. A amostra constituiu-se de 1198 neonatos e lactentes usuários do Sistema Único de Saúde. O critério de passa foi a presença de RCP e EOAET bilateralmente. Os IR foram investigados através de anamnese e pesquisa de prontuários, e foram relacionados às variáveis passa/falha
na TAN e ocorrência de EOAET e EOAEPD para cada freqüência avaliada. As crianças que falharam foram re-testadas em quinze dias, com os mesmos procedimentos. Ao persistir a ausência de RCP e/ou EOAET realizou-se avaliação de Potenciais Evocados Auditivos de Tronco Encefálico (PEATE). Ao apresentar alteração neste exame, foram encaminhadas para protetização. A TAN não pôde ser
considerada universal, já que o índice de crianças atingidas foi de 66,56%. A primeira triagem auditiva ocorreu até os 28 dias de vida em 78,80% (n=944) delas. Passaram na primeira testagem 91,49% (n=1096). Dentre as que falharam, 17,65% (18) não compareceram para re-teste. Das 84 crianças que retornaram, 17 mantiveram a falha e foram encaminhadas para PEATE. Em oito delas a surdez foi confirmada, sendo o
índice de surdez de 0,67%. O índice de encaminhamento para diagnóstico foi de 1,41% e o de resultado falso-positivo foi de 0,17%. O resultado da TAN é dependente da ocorrência de IR para a deficiência auditiva. A ausência de EOAET e de EOAEPD é dependente da presença de IR para a deficiência auditiva, assim como a presença de EOAET e de EOAEPD é dependente da ausência de indicador(es) em algumas
orelhas e freqüências. Os índices de encaminhamento para diagnóstico e de falsopositivo deste estudo sugerem que o programa é executado com eficácia, e demonstram que os profissionais que neles atuam possuem experiência adequada
para tal. Entretanto, o caráter universal e obrigatório da TAN ainda deve ser alcançado. A presença de IR pode influenciar na ocorrência das EOAE e no resultado da TAN. O IR que ofereceu maior risco de falha na TAN foi a icterícia neonatal. Alguns aspectos determinantes no planejamento, efetividade, manutenção e aprimoramento do programa foram delineados, e alguns deles já estão em execução.
