Ukončení náhradní rodinné péče před 18. rokem věku dítěte / Termination of substitute family care before 18th year of child´s age

The objective of my work has been to monitor the opinions of social workers operating within the departments of social and legal protection of children at village municipalities with an extended scope of authority in the South Bohemian region as related to the problems of child care and education in substitute family care from the perspective of a social worker. The second objective has been to map the situation of children registered by the departments of social and legal protection of children of South Bohemia, the substitute family care of which was terminated before the 18th year of child´s age during the years 2003 {--} 2009. Due to significant reasons substitute family care, with the exception of adoption, can be terminated earlier. This presents a difficult situation both for the foster parents and for the foster children put into substitute family care. The results of my work have shown that most social workers do not lack the assistance of professional facilities that would support foster families in their regions. The most common reason for the termination of substitute family care before the 18th year of child´s age have been educational problems involving a whole range of issues. For most children, the termination of substitute family care resulted in institutional care {--} youth educational centers.

The diploma work processes the present issues if it is important or even necessary keeping the institutional care for the children under 3 years of age or if it can be replaced by short term foster care. The research was done by quantitative strategy and with the method of forms. The forms were addressed to fosters [short term and long term fosters] and to the child care institutions for children under 3 years of age under the direction of Ministry of Health. The data from ÚZIS also served the research these data confirm that about 85% of children return to their own or substitute families. The return to biological families did not go under 50% between 2011 and 2015. The inquiry also revealed the different view on the issue of short term foster care where fosters [long and short time] prefer the short time foster care method, while the child care institutions for children under 3 years of age under the direction of Ministry of Health mention the risks connected with the foster [short term] deficiency. The work also shows the problem that short time foster care is not convenient for disabled children and children from different ethnic groups, the institutions see a big problem in this too. The ways how the children are distributed into substitute family care are also connected with above mention. The children come to short time foster care generally from social issues and to institutional care mostly from health-social issues. The fosters themselves mention the situations when they would fear doing the care. These situations mostly correspond with the cases of children placed to the institutions with special care. Finally it is obvious the short time foster care and the institutional care play their own irreplaceable role in the substitute family care system. Together they make the necessary balanced system for children at risk and they complement each other. Well, not every child is suitable for institutional care and otherwise. For example the children hardly disabled or children from repetitively non-functional families would be in high risk of failing again in the foster care. That is the reason, why the planned closing down of the special facilities should be discussed and reconsider again.

The topic of my thesis is called Current Questions about Foster Care in the Czech Republic. European Union organizations and UNICEF organizations, due to the information from experts, have been drawing attention to the fact, that there is a high percentage of children under the age of three years living in various institutions. It is therefore necessary for the state to increase the support for substitutive family care. The theoretical part describes the function and defects of a family, psychic deprivation of children, the history of foster care, the institution of foster care and its new forms and other. The target of the research and hypothesis is stated in the practical part. The target of the thesis is an attempt to clarify all the reasons why children live in institutions and why finding a substitutive family for them is difficult. The targeted sample was represented by 110 children from all regions of the Czech Republic except the South Bohemia region. These children{\crq} s collection of documents was forwarded in the second half of the year 2007 to the Regional Council of South Bohemia Region. In accordance with the target of the work following hypothesis were determined: Hypothesis no. 1: Most of the targeted children have serious disabilities. Hypothesis no. 2: The targeted sample were mainly children between 7 {--} 10 years old. Hypothesis no.3: The most frequent reason why children stay in institutions is the fact that their parents are not able to look after them. Hypotheses have been refuted. 1 Abstract I wrote my thesis on "The New Trends in Foster Care". My work aimed to find out the opinions of foster parents on the chase of the alternative family care system which was formed by the amendment of the Act on social and legal protection of children in 2013. In the theoretical part, I was dealing with the changes that occurred in the amendment, I characterized variol forms of alternative family care and I described the preparation of foster parents and applicants. In the practical part, I described a survey which, after the evaluation, pointed out to the issue of foster care after its amendment in 2013, from the point of view of foster parents. By this survey the information on satisfaction with changes to the current alternative family care system, on the need for foster care for a transitional period, and also information on co-operation with social workers was received.

This diploma thesis deals with the possibilities of further education in the agencies of domiciliary care. This is a very current topic. Like any other profession, the nursing profession requires a particular education. The development of the new methods in the medical treatment and home care demands more thorough and specialised education of the nurses employed by the domiciliary-care agencies. The aim of the thesis was to find out the present situation of the nurses´ education in the domiciliary-care agencies, the professional specialisation of nurses, the possibilities of education, the nurses´ need of further education and the employers´ willingness to enable the nurses of the agencies to take part in the further education in their field. It can be said that the aims of the research have been fulfilled.

The main factors which influence these social and health facilities during the planning of social and health services are a demographic prognosis, the social structure of its inhabitants, the informal support and solidarity within the family, and also the geographic profile of the region, urbanization, human settlement and the extent of municipality. The planning of social and health services has to follow modern principles in social services which are widely used in Western Europe. The goal of this thesis is to undertake research in the capacity and quality of health and social services and the use of these services by seniors in the region. The next goal is to see if modern European principles are applied in the social and health services in the region. This research was carried out though the analysis of documents and methods of questioning. This research has shown that there is a lack of institutional social and health services in the region. Also modern European principles have not been applied in the social and health services.

The thesis deals with the topic of palliative care of child patients/clients. The theoretical part explains the terms palliative, respite and hospice care of both, adult and particularly child clients and their families. The work shows the development of palliative care in our country as well as in the world and highlights the present modern elements of the palliative care approach to a child and his/her family. The practical part is based on a qualitative research that followed two goals and tried to answer two research questions. A research method of content analysis of a document containing data obtained from the Czech Statistical Office was applied on the first goal, aimed at mapping the possible need of palliative care of children in the CR. The number of children that might require palliative care in the CR was estimated upon the data. The second goal was to map the possibilities of palliative care services for children at the territory of the CR. A quantitative method of deep interview was used for implementation of the goal. The research sample consisted of 6 respondents from 6 workplaces. Only general and paediatric nurses from wards and facilities chosen upon the fact that nurses may more often meet an incurably ill child and his/her family were asked.This thesis and its conclusions are mainly aimed at professionals, but also to the general public. The research results inform on the possibilities of providing palliative care to children and their families at the specific departments. The work may be used for study purposes as well as for nurses working at hospices, children oncology wards, home care agencies etc. Paediatric nurses may make use of it as well.

For most people, family represents a place where they feel safe and loved. But not everyone is lucky enough to have a place like this, to have a functional family. If the biological family does not work as it should and a baby is threatened in some way, the state has several options to help the child. If there are solvable problems in the family, the state can try to help it by various interventions to improve the situation. But if these actions do not help and the child is forced to leave this family, it is necessary to ensure alternative environment for baby. The child can either be placed in institutional care, or to substitutional family care. Considering the shortcomings of institutional care, various forms of substitutional care are mainly supported. Probably the best known form of substitutional care is adoption. In the case of adoption, baby becomes the member of new family and relationships between child and his biological family vanishes. This form is only possible for a small percentage of abandoned children, because most of them still has some kind of relationship with biological family.
