Spánek a snění pacientů v intenzivní péči / Sleep and dreaming of a patients in intensive care

With the progress of the intense medicine, over the past ten years there has been documented more and more patients/clients who have survived a critical illness. Intensive care unit admission is a great source of somatopsychological burden to the patient/client. One of the main aspects of the negative experience is a quality of sleep which is subjectively perceived as low. This master thesis summarizes the knowledge of a current research which is dealing with the causes of poor sleep quality on the intensive care department. It offers a detailed overview of environmental and physiological factors which are related to the poor quality of sleep. It contains summary information of the description and recalling dreams. The research of the master thesis is handling searching for factors which predict the poor quality of sleep, and also the factors which predict if a patient/client will recall the dream which they were having. Within our research we executed an exploratory pilot study. In conclusion of the analysis there was a discovery of a deteriorative sleep quality by perceiving heat discomfort. We were also observing the trend of a deteriorative sleep quality by the connection to the mechanical ventilation, but only when not provided sedation. The thesis "Shared Custody - Advantages and Disadvantages" focuses on joint custody as one of post-divorce arrangements. The theoretical part summarizes important position of a family in child's life, the position of parents and children in the time of divorce, and covers shared custody after divorce. It points out pros and cons of shared custody in more details and summarizes presumptions for its successful implementation. The survey represents opinions of professionals on shared custody, its pros and cons as well as important criteria such as suitable time interval and suitable child's age for joint custody. In addition, personal experience case studies are added for better illustration. The aim of the thesis is to point out aspects that are necessary to focus on while choosing this type of post-divorce arrangement.

The graduation theses is about urinary tract infections and the preventive care that we are able to provide within nursing care of patients with urinary catheter. The theoretical part of graduation theses includes the chapters about general introduction to nosocomial infections, as well as epidemiology, pathogenesis, diagnosis and treatment of urinary tract infections. Furthermore, it relates to their antibiotic therapy and resistence to antibiotics. The main task is the prevention of urinary tract infections. Generally I have focused on the methods of urinary catheterization and their correct indications, the materials and design of urinary catheters, as well as on nursing care of urinary catheters. The thesis includes research aimed at providing preventive measures that are implemented in practice, comparing them to each other and comparison with the current studies and recommendations.. This research provides both positive and negative results. Some of the recommendations for preventive care of urinary tract infections are being followed, other are being omitted. There are also differences between respondents from different departments as well as among the departments. Alzheimerova choroba a prostředí intenzivní péče / Alzheimer's disease and department of intensive care

Introduction: The thesis "Alzheimer's disease and department of intensive care" deals with the specifics and difficulties of nursing care for patients with this chronic and degenerative disease in intensive care units (ICUs). The number of patients with Alzheimer's disease is constantly growing and caring for them in such specific wards is extremely demanding. The purpose of this thesis was to map the most problematic areas in the care provided and propose their solution. Methods and results: The empirical part presents the results of a conducted qualitative research which took place in the form of interviews with 10 general nurses who have experience with such patients. Following a word-for-word transcription of the obtained interviews, the results are summarized in a final discussion. They show that when caring for a patient with dementia, general nurses see the main problems in his decreased ability to cooperate, behavioral changes, which often appear as a result of environment change, and difficult communication. They consider the patient's confusion, his negativism, and the inability to find the right words to be communication barriers. This results in enormous demands on the caring staff's patience. This diploma thesis explores the area of sexuality of older people, summarizes previously published theoretical knowledge in this area and is complemented by empirical research. It deals with aging and outlines the changes that may occur in humans during this process, especially in terms of development and transformation of sexuality. This thesis also pursues the perception of senior sexuality by his surroundings and the general public. This thesis also partially covers the issue of institutional care for the elderly. Objective: Mapping of senior population's behaviors and attitudes towards sex and their comparison across genders. Methods: Research conducted by a quantitative survey. Processing and interpretation of data regarding to the relevant age groups, which are based on a survey of sexual behavior carried out in 2013 and guaranteed by Prof. PhDr. Petr Weiss, Ph.D. a Doc. MUDr. Jaroslav Zvěřina, CSc. from the Institute of Sexology, 1st Faculty of Medicine at Charles University, Results: More than a half of the senior population in the Czech Republic does not feel the need to sexually realize themselves. This master thesis deals with educational needs of foster parents. The main assumption of this thesis is that foster parents are despite all specifics among all parents. Therefore it firstly mentions competences, that are essential for good parenthood and relationship between parents as partners. Than it focuses on specific competences that are essential for foster parents. The main question then is how to encourage these competences during educational activities. In the next part this thesis describes the current state of education of foster parents, examples of good practice and present researches. The empirical part of the thesis consists of a qualitative research, which aims to describe the educational needs of foster parents, current deficiencies and suggestions on what to improve. The research is based on semi-structured interviews with foster parents and professionals in the field of alternative family care. This dissertation is concerned with the issues surrounding substitute family care in the Czech Republic and presents an analysis of its legal regulation as set out in both private law and public law regulations. The term substitute family care encompasses several independent legal institutions, which together form a system serving to protect children at risk who cannot grow up in their own family. The system of substitute care is divided into family-type individual care on the one hand and collective care provided in institutional facilities on the other. This dissertation examines the historical development of forms of substitute care, analyses their current legal regulation, including procedural regulation, and evaluates the interconnectedness and mutual harmoniousness of individual legal regulations. It analyses the inner workings of the substitute care system and delineates the rights and obligations of entities which enter into it in various capacities, with an emphasis on how the fulfilment of the rights of a minor is enforced and how their best interest is considered. FOSTER CARE FOR A TEMPORARY PERIOD This thesis describes and critically judges the legal regulation, practice and purpose of foster care for a temporary period in the Czech Republic. Children temporarily or permanently deprived of their family environment or children, who cannot be left in such environment, are entitled to a special protection and assistance granted by the state in one of alternative care forms. The Czech Republic is criticized for a long time by the international organizations because of numbers of children placed in any type of institutional care. Consequently, the amendment of the Act N. 359/1999 Coll., on social and legal protection of children, has been approved in June 2006. This amendment has established a new legal institute called "the foster care for a temporary period". The function of this institute is to avoid a placement of children to an institutional care in the case, when the children can not stay in the original family because of whatever reason however there is a presumption of re-entry to that original family. The objectives of the work The objective of this work has been set as the mapping of degree of patient's contentment/ rate of patient's satisfaction in view of the nurse's approach within the scope of the post-thoracic operation care and the evaluation/ assessment whether the needs of the aforementioned patients have been satisfied. Another objective of the research enquiry has been set as finding out the level of the nurse's awareness regarding the ways and possibilities of the subsequent care. Methodology For the purpose of the research enquiry the form of qualitative research has been chosen and adopted, which has been applied and performed by means of using a method of interviewing i.e. putting questions which have been prepared beforehand, well in advance. Research array of patients consisted of 10 patients from one hospital (which is not going to be specified in this work) as well as 8 nurses whose job was to take care of the post-operative patients/ the post-thoracic operation patients. The results of the research have been processed by applying the method of open coding, pencil-paper method. Assets and Benefits of this Work The research enquiry has showed certain level of absence of desirable knowledge as well as ignorance of the staff taking care of the patients regarding the ways of subsequent care. Nurses should be educated about the possibilities of home care, hospice care as an integral part of their profession. The results gained by research enquiry process could be presented at seminars, conferences aimed at the subsequent care.

Breathing is one of the basic life functions and for its proper functioning it is necessary to have the airways free and clear. One way to maintain free and open airways is the tracheostomy. Tracheostomy may be performed either by surgical or puncture method. Nursing care and its quality in patients with tracheostomy are important factors that can significantly affect postoperative complications, their course and overall clinical outcome of treatment of patients. Sufficiently qualified, knowledgeable and experienced nurse applies her knowledge in practice with a holistic view of the patient and his needs. The theoretical part of the thesis introduces us to the anatomy of the respiratory tract, the history of tracheostomy, individual methods of tracheostomy - surgical versus puncture and nursing care for adult patients with tracheostomy for intensive care. Methods: In the empirical part, the data obtained on the basis of a quantitative survey in the form of a questionnaire will be evaluated.
