Patienters upplevelse av smärtlindring och livskvalitet efter botoxbehandling i smärtlindrande syfte : En intervjustudie

Syftet med studien var att undersöka patienters upplevelse av smärtlindring och livskvalitet efter botoxbehandling i smärtlindrande syfte samt patienternas inställning till Botox. Metod: En kvalitativ intervjustudie med en deskriptiv och explorativ design. Urvalet bestod av sju patienter, sex kvinnor och en man, vilka behandlas men botoxbehandling vid Smärtcentrum på Akademiska sjukhuset i Uppsala. Resultat: Materialet analyserades enligt syftet under tre kategorier: smärtlindring, oro/nedstämdhet samt inställning till Botox. Samtliga informanter har fått en minskad smärta till följd av botoxbehandlingen och upplever att livskvaliteten förbättrats. Trots biverkningar känner sig patienterna nöjda med behandlingen så länge fördelarna är fler än nackdelarna. Ingen av informanterna förutom en kände någon oro/nedstämdhet. Några informanter upplevde en viss oro inför eventuella biverkningar i framtiden. Slutsats: Botox kan vara ett bra behandlingsalternativ för patienter som lider av långvarig smärta. Samtliga studiedeltagare upplever smärtlindring även om inte smärtan försvinner helt. Behandlingen förbättrar patienternas livskvalitet vilken mäts genom upplevelse av rörlighet, hygien, aktivitet, oro/nedstämdhet och smärta. Hälso- och sjukvården har en viktig roll i att behandla sjukdom och främja hälsa och ge patienterna förutsättningar får att nå hälsorelaterad livskvalitet. / The aim of this study is to explore patients'experience of pain relief and quality of life after Botox treatment of pain-relieving purposes and patients' attitudes to Botox. Method: A qualitative interview study with a descriptive and explorative design. The sample consisted of seven patients, six women and one man, which is treated with the treatment of Botox at Center of pain at the University hospital in Uppsala. Results: The material was analyzed in three categories: pain, anxiety/depression, and attitude to Botox. All informants have experienced a reduction in pain as a result of Botox treatment and feel a higher quality of life. Despite the side effects patients feel satisfied with the treatment so long as the advantages outnumber the disadvantages. None of the respondents except one felt no anxiety/depression. Some respondents felt some concern for possible adverse events in the future. Conclusion: Botox may be a good treatment option for patients suffering from chronic pain. All study participants experienced pain relief even if the pain does not disappear completely. The treatment improves patients' quality of life which is measured by the experience of mobility, hygiene, activity, anxiety/depression and pain. The health service has an important role in treating disease and promoting health and providing patients with conditions, to achieve health-related quality of life.

Exploring the experience of pain in adults with sickle cell disease

Sickle Cell Disease (SCD) is England’s most common blood disorder whereby sickled shaped red blood cells block small blood vessels causing both acute and chronic pain. Currently there is poor understanding about the experience of SCD pain. Furthermore, there is a recognised difficult relationship between SCD patients and health care professionals (HCP) which has been found to prevent patients seeking medical assistance. Seven adults with SCD participated in semi-structured interviews regarding their experience of pain and receiving medical treatment. Interviews were transcribed and analysed using Interpretative Phenomenological Analysis (Smith, Flowers & Larkin, 2009). The analysis revealed three overarching themes: experiencing unimaginable pain, the dilemma of treatment and finding a life with pain. Findings suggest describing SCD pain is extremely difficult, participants favoured using analogies to attempt to communicate the constant, agonising, limitless, inescapable pain. Participants described that normal rules do not apply in that pain medication does not always relieve pain. These factors have caused misunderstanding with staff thinking SCD patients are drug seeking. Participants wanted HCPs to listen to their own expertise when considering treatment options. Study limitations, implications for clinical practice and current models of health and illness are considered, as well as possible for future research.

Bakgrund: Patienter med en demenssjukdom har en nedsättning av den kognitiva förmågan och svårigheter att kommunicera. Detta resulterar i att denna patientgrupp har svårigheter i att förmedla sin smärta till sjuksköterskan, vilket försvårar smärtbedömningen. Syfte: Syftet var att belysa sjuksköterskans möjligheter till att utföra individuell smärtbedömning av patienter med en demenssjukdom. Metod: Studien var en litteraturstudie. 12 artiklar inkluderades varav 3 artiklar var kvalitativa och 9 artiklar var kvantitativa. Alla artiklar granskades och analyserades och svarade mot studiens syfte. Resultat och konklusion: Det är en utmaning att bedöma smärta hos patienter med en demenssjukdom. Sjuksköterska bör observera patientens uttryck och beteenden för att finna tecken på smärta. Ett helhetsintryck av den enskilde individens smärtuttryck är avgörande för att sjuksköterskan ska kunna utföra en korrekt smärtbedömning. Med hjälp av olika smärtbedömningsinstrument kan smärtbedömningen underlättas. Sjuksköterskan bör avgöra vid varje enskilt patientfall, vilket bedömningsinstrument som lämpar sig bäst. Implikation: Sjuksköterskor bör enligt yrkesprofessionen uppdatera sin kunskap med aktuell forskning och sprida kunskapen vidare till arbetskollegor. Mer forskning om smärta och smärtbedömning är nödvändigt, samt att tydliga riktlinjer utformas så att sjuksköterskan kan utföra en korrekt smärtbedömning. / Background: Patients with dementia have a reduction in cognitive ability and difficulty communicating. The result is that this patient group has difficulty in communicating their pain to the nurse, which complicates pain assessment. Aim of the study: the aim of the study was to illuminate the nurse opportunities to perform an individual pain measurement of patients with dementia. Method: the study was a literature review. 12 articles were included, 3 articles were qualitative and 9 articles were quantitative. All articles were reviewed and analyzed and met the study’s purpose. Results and conclusions: It is a challenge to assess pain in patients with dementia. Nurses should observe the patient's expressions and behaviors in order to find signs of pain. An overall impression of the individual's pain expression is essential for the nurse to perform an accurate pain assessment. With the help of various pain measurement tools can pain-assessment be facilitated. The nurse should determine in each individual patient, which assessment instruments that are best suited Implication: Nurses should according to their profession, always update their knowledge of current research and disseminate knowledge on to colleagues. More research on pain and pain assessment is necessary, and that clear guidelines are designed so that the nurse can perform an accurate pain assessment.

Postoperativ smärta hos barn på en kirurgavdelning : En kvantitativ studie baserad på ett enkätunderlag

Bakgrund: Studier har visat sig att barns postoperativa smärta i många fall är otillräckligt behandlad och barnen upplever ofta smärta som något obehagligt och skrämmande. Barn kan påverkas negativt av smärta och de kan utveckla skadliga beteendeförändringar efter en operation på grund av otillräckligt behandlad postoperativ smärta. Syfte: Syftet med den här studien är att undersöka hur barnen, på en specifik kirurgavdelning, mår under det första dygnet efter en operation då de blivit sövda, avseende deras postoperativa smärta. Om möjligt ska grupper som har ökad risk för otillräcklig postoperativ smärtlindring försöka identifieras. Metod: En kvantitativ tvärsnittsstudie baserad på ett konsekutivt urval. Deltagarna består av barn på en kirurgavdelning i åldrarna 4-16 år som alla har genomgått en operation där de blivit sövda. De har fått besvara en enkät om sin smärta ett dygn efter operation. Resultat: I studien deltog 32 barn med en medelålder på 9,25 år. Medelvärdet av smärta just nu var 2,59. Medelvärdet av smärta som mest var 7,56. Medelvärdet av minsta smärtan var 1,03. De äldre barnen skattade sin smärta högre än de yngre. Pojkarna skattade sin smärta högre än flickorna. Allmän kirurgi skattade sin smärta högre än de andra kirurgityperna vid två av tillfällena. Urologi skattade sin smärta högst vid ett av tillfällena. Alla deltagande barn uppgav att den ordinerade smärtlindringen hjälpte. Inga signifikanta skillnader kunde ses mellan könen, åldrarna eller de olika kirurgityperna. Slutsats: En större studie, med fler tillfrågade barn, hade gett ett tydligare resultat. Mer forskning behövs inom området. Sjuksköterskorna måste bli bättre på att använda sig av smärtskattningsskalor. / Background: It has been shown that children's postoperative pain, in many cases, is inadequately treated. The children often experience pain as something unpleasant and frightening. Children might be affected in destructive ways by the pain and they can develop adverse behavioral changes because of inadequately treated postoperative pain. Aim: The aim of this study is to investigate how the children, at a specific surgical ward, are feeling during the first day after a surgery where they have been anesthetized, regarding their postoperative pain. Groups with increased risk of inadequate postoperative pain treatment are Method: A quantitative cross-sectional study based on a consecutive sample. The participants consist of children on a surgical ward aged 4-16 years who all had surgery where they were anesthetized. They have answered a questionnaire one day after the surgery. Results: 32 children participated in the study. Their mean age was 9.25 years. The mean of pain right now was 2.59. The mean of the worst pain was 7.56. The mean of the minimum pain was 1.03. The older kids rated their pain higher than the younger ones. The boys rated their pain higher than the girls. General surgery rated their pain higher than the other surgery types at two occasions. Urology rated their pain highest at one of the occasions. All participating children reported that the prescribed pain relief helped. No significant differences were seen between the sexes, ages or different surgery types. Conclusion: A larger study with more responding children, would have given a clearer result. More research is needed in this area. The nurses have to improve their usage of pain measurement instruments.

Most cases of LBP are self-limiting, however certain regional biomechanical deficits may be overlooked, such as core stability. As a result of this, LBP may become a chronic condition in the athletic and general population. This principle can be applied to road running, as the forces that pass through the muscles of the lower limbs and trunk cannot be properly absorbed if the trunk musculature is not properly trained. This may lead to lower back pain as a result of inadequate functioning and strength of stabilizing structures (Hedrick, 2000). The purpose of this exploratory cross-sectional study was to determine the relationship between core muscle strength and chronic lower back pain in amateur female road runners and non-runners. The focus was to determine the core stability values in mmHg between amateur female runners with and without chronic LBP, and female non-runners with and without chronic LBP; as well as to compare female runners and non-runners with regard to core muscle strength.
