Public-private partnerships : a qualitative approach to prospects for pharmacy in the South African health care environment / Johan Christiaan Lamprecht

BACKGROUND: Powerful public-private partnerships (PPPs) can only be established if the partners are able to deal with complexity. Such partnerships may serve to stimulate local community and economic development. Thus, it may maximise the effectiveness of local groups and resources in meeting the needs for rebuilding a community through a partnership representative of the public and private sectors. A problem that exists in South Africa, is the uneven distribution of population ratios dependent on public and private sector health care service delivery, in relation to the proportion of pharmaceutical service providers in the different sectors. OBJECTIVE: The main objectives of this qualitative research investigation were to examine the prospects for PPP development in the pharmaceutical sector of South Africa as well as to explore the possibilities of a proposition for a proposed generic public-private partnership model to be managed and used in the pharmaceutical sector of South Africa. METHOD: The study comprises of the exploration of the research questions by means of a qualitative research design. The study design implicated a balance between the in-depth literature study and a qualitative research process. The researcher employed a grounded theory approach to collect and analyse the data. Data collection represented the identifiable role players and opinion formulators in the South African health care sector. By following a combination of the various qualitative sampling methods and techniques, a total of 38 (n=38) interviews were conducted. The data collected from the interviewees and from the literature study were integrated and analysed by making use of computer assisted data analysis. SETTING: The researcher selected interviewees from the South African health care sector. The interviews included role players in the pharmaceutical sector in both the public and private sectors. The interviewees further represented eight different spheres of the pharmaceutical setting in South Africa. KEY FINDINGS: The investigation identified a range of prospects for PPP development in South Africa and these were reported in terms of views, expectations and scope for success. The management elements for developing and sustaining joint ventures between the public and private sectors were identified and a proposition was formulated in theory to serve as a proposed generic PPP model (PGM) in the pharmaceutical sector for the South African health care milieu. CONCLUSION The exploratory qualitative investigation surfaced the various facets of the complexity of PPPs. The investigation concluded that several barriers, such as competition and market entry disparities between the macro and micro level pharmaceutical entities, which impede PPP development, affected the prospects for PPP development in South Africa. The South African legislation, South African Treasury guidelines, regulations and the views of the SA Competition Commission need transformation to accommodate both the micro and macro level pharmaceutical service providers in the formation of PPPs. Capacity building within the sphere of pharmaceutical service delivery to the bigger population of South Africa may become sustainable on removal of these barriers. Protecting forests through partnerships

This thesis addresses the potential of private-public partnerships (PPPs) to involve private forest owners in formal forest protection. These partnerships have been widely advocated as means to engage actors from diverse sectors in collaborative new relationships, formed in a step-wise manner, to improve management of resources that combine public and private goods. Nature Conservation Agreements (NCAs) are the first kind of PPPs to be used in Swedish forest protection. NCAs were introduced in 1993 and are agreements based in civil law between a private forest owner and the Swedish Forest Agency or County Administrative Board. Although NCAs were introduced to promote interest in nature conservation among forest owners, the response has been rather weak. Thus, in 2010 the government launched a pilot project called the Komet program, in which private forest owners in selected pilot areas initiated protective measures. Although criticized by environmental non-governmental organizations, the government decided after the pilot project terminated in 2014 to implement the Komet program’s working methods nationwide. In this thesis, PPPs’ potential to contribute to forest protection is analyzed by applying the “Ladder of Partnership Activity” framework, developed to study global PPPs, with appropriate modifications for a national context. The framework incorporates, in a stepwise manner, context, the actors’ motives relating to trust-building, the creation of collaborative advantages in the partnering process and the institutionalization of PPPs. The thesis contributes to an empirical understanding of top-down and bottom-up PPP processes. It is based on studies in which qualitative research methods were applied to examine selected cases presented in four papers, designated Papers I-IV. The main sources of information are qualitative interviews with involved forest actors and policy documents they have produced. Papers I and II focus particularly on trust-building and the partnering process as perceived by involved forest actors, while Papers III and IV address the institutionalization of PPPs and their requirements to change the political order of forest protection in accordance with governmental objectives. The results show that willingness to adopt PPPs is dependent on past experience of collaborative efforts. They also show there is substantial discretion in involved actors’ interpretation of prescribed guidelines, and their motives may vary substantially. However, as long as they share the same ultimate objective, i.e. to protect forests, PPPs may still be successfully established. A major potential problem is that public officials tend to prioritize protection of biodiversity, while forest owners want to protect social values and unproductive (“useless”) forests. Thus, shared motives are essential to establish trust and initiate collaborative efforts. The voluntary element of initiatives supported by the Komet program appears to be essential for deliberation. PPPs need to be implemented nationwide to be institutionalized. However, the Swedish government has not provided sufficient resources and leadership capacity to enable PPPs to play their envisaged role in its forest governance system. If the government wants to adopt bottom-up approaches, it needs to provide sufficient resources so that the partnerships does not compete with other formal instruments and protection arrangements. Furthermore, coordination within and between sectors needs to be improved to clarify the purpose of the policy recommendations.

Understanding inter-organisational relationships in public-private partnerships : a study of educational PPPs in Pakistan

Given the increasing proliferation of public-private partnerships (PPPs) in both developed and developing countries, and the huge challenges that are often associated with establishing and managing them, and ensuring that they achieve their objectives, it is important to understand multiple aspects of their operation. Whilst the structural and economic aspects of PPPs have long been recognised and researched, the relational aspects of PPPs remain under-researched. This thesis is a contribution to addressing this gap in the literature. It uses a dimensional approach to understand the nature of inter-organisations relationships (IORs) in PPPs and considers the factors that shape these relationships. It also investigates whether a particular pattern of relationships is needed for PPPs to deliver more than could have been achieved by each partner working alone (synergistic benefits). These issues are studied empirically in three educational PPP programmes in Pakistan. In two of these, not-for-profit organisations ‘adopt’ state schools. In the third, the state funds private sector schools on the condition that they offer free education to students and achieve threshold quality standards. A case study methodology is used and an integrative conceptual framework, derived from a wide-ranging literature review, is used to guide both data collection and analysis. The research finds that partners’ motives for entering into a PPP play a dominant role in shaping inter-organisational relationships. These motives are, in turn, influenced by a range of contextual and organisational factors. Inter-organisational relationships can be broadly characterised as collaborative, contractual, cooperative or conflictual. Whereas much of the existing literature emphasises that collaborative relationships are a prerequisite for PPPs to deliver synergistic outcomes, this research finds that these outcomes are also present in PPPs characterised by cooperative relationships. However, inter-organisational relationships in PPPs are not static; they develop and change over time. These changes result from a dynamic interplay between contextual factors, organisational factors, partner motives and the perceived outcomes of the partnership. The research reported in the thesis makes a number of contributions to knowledge. It sheds new light on the relational aspects of PPPs and offers a new conceptual framework for explaining and investigating inter-organisational relationships, which integrates insights from the largely separate literatures on PPPs and inter-organisational relations. It counters an apparent pro-collaboration emphasis in the existing PPP literature by documenting and explaining the benefits associated with cooperative relationships. It also offers new empirical evidence on the operation of PPPs in a developing country context, which contributes to redressing the predominance of evidence from developed countries in the existing literature. The insights from the research have theoretical and practical implications for the development and management of PPPs and future research in this area.

Transferability of Policies and Organisational Practices across Public and Private Health Service Delivery Systems: A Case Study of Selected Hospitals in the Eastern Cape: Exploring Lessons, Ambiguities and Contradictions

Philosophiae Doctor - PhD / Since the advent of South Africa‘s democracy in 1994 there have been several changes in the policy and legislative arena specifically promoting public-private-partnerships in the health sector. These initiatives have given rise to opportunities for inter-sectoral policy transfer under the rubric of ―best practices‖. This exploratory study examines the character, obstacles and contested nature of a selection of policy transfers between private and public health institutions in a single province of South Africa. The study looks at the dynamics at play around envisaged, current and past transfers of policies and organisational practices in relation to administrative systems and technologies used in four different hospital settings – two public and two private hospitals in the Eastern Cape Province of South Africa. This thesis explores the views of managers and labour organisations about policy transfer focusing on local contexts, and how various parties construct policy transfer, hence providing a perspective of policy at the ―plant‖ level. In this research, special focus is placed on different agents‘ role and understandings of their contexts and how and why policies move and contradictions of these developments. In-depth interviews were conducted at four major Eastern Cape hospitals. The thesis argues that in practice, policy transfer is messy, politicized and traversed by power and vested interests and that organised labour plays a key role in policy transfer process. The thesis focuses on the different philosophical/ideological underpinnings, socio-political values and operational environments in each sector. This study is designed to contribute to existing knowledge on practices particularly between the public and private sectors in order to widen the understanding of the complexity of transferability.

A presente pesquisa teve como objetivo investigar o Programa de Atendimento Especial à Educação Infantil (PAEEI) instituído no município de Campinas-SP a partir de 2007, com a criação dos CEIs Naves-Mães. Essas unidades educacionais pertencem à Secretaria Municipal de Educação, mas a sua gestão é realizada por instituições privadas sem fins lucrativos a partir de parcerias instituídas com o poder público municipal. Desde que o Programa foi lançado, a utilização do convênio como instrumento jurídico utilizado para a transferência de recursos às organizações da sociedade civil sem fins lucrativos tem sido a principal opção dos dirigentes locais para a ampliação do atendimento às crianças de zero a 5 anos. Procuramos compreender as características e as consequências do Programa para a oferta da educação infantil no município, à luz das recentes transformações no papel do Estado num contexto de ajustes neoliberais e utilizando-se da Terceira Via. Para subsidiar essa análise, além do levantamento teórico concernente ao tema da pesquisa, realizamos análise documental e entrevistas semi-estruturadas. Concluímos, entre outras coisas, que, intrínseco ao processo de privatização da educação infantil em Campinas, estão os baixos salários pagos aos profissionais que atuam nos CEIs Naves-Mães, as salas superlotadas e, principalmente, o interesse de algumas instituições em firmarem parceria com a PMC. / This study is an attempt to investigate the Special Childhood Education Program (Programa de Atendimento Especial à Educação Infantil (PAEEI)) implemented in the municipality of Campinas-SP, Brazil, starting in 2007, through the creation of CEIs Motherships (CEIs Naves-mães). These educational units belong to the local educational authority (SME), but are managed by non-profit private institutions through partnerships developed in conjunction with the municipality. Since its inception, its utilization as a legal instrument used towards the transfer of resources to non-profit entities of civil society has been the main option available to local managers to amplify service to children ages 0 to 5. We seek to understand the characteristics and consequences of the Program to the offering of childhood education in the municipality, in light of the recent transformations in the role of the state in a context of neoliberal adjustments and reliance on the Third Sector. To substantiate our analysis, in addition to doing a literature review on the subject at hand, we have conducted documental analyses and semi-structured interviews. We find that the low wages paid to the professionals who work at the CEIs Motherships, the overcrowded classrooms, and, mainly, the interest of certain institutions in developing partnerships with the Campinas City Hall, among other things, are intrinsically related to the process of privatization of childhood education in the municipality of Campinas.

Courting corporate sports partners in education: Ethnographic case study of corporate philanthropy in urban public schools

Thesis advisor: Andy Hargreaves / This dissertation examines the nature of the longstanding cross-sector relationship between an urban public school district and a corporate-owned team franchise in the National Basketball Association (NBA). The study found that while this collaboration is often talked about as a partnership, in practice, it advances a corporate philanthropic and promotional relationship that is characterized by mutual affinities but not mutually agreed upon goals. This philanthropic connection to a powerful national sporting institution provides benefits to local public schools through incentives for perfect student attendance, motivational assemblies with professional athletes, and periodic, one-time donations in much needed technology. However, this relationship also raises key questions related to the mechanisms for social accountability in leadership decision-making, the effective and equitable use of school and corporate resources, and the indirect and inadvertent consequences when schools rely on commercialism and sports stardom to sell the meritocratic value of getting an education to a generation of students. The dissertation addresses the implications of the rise of corporate philanthropy within the context of economic austerity in public education. A multi-disciplinary review of research, drawing on four bodies of literature, considers the assumptions underlying counter-related discourses about corporate involvement in the public sector: 1) Corporate Social Responsibility (CSR), 2) CSR as Greenwashing (i.e. disinformation disseminated by a firm to present misleading public images of corporate responsibility), 3) Public-Private Partnerships (PPPs) in education, and 4) PPPs as privatizations in education. The constant comparative method was used throughout to analyze multi-modal data from an ethnographic case study of one city's cross-sector collaboration with the NBA, including participant observations, review of news and media, and extended field interviews with thirty district leaders, school administrators, teachers, counselors, and coaches in three K-8 schools. Collaborative and partnership opportunities in the area of research and development for paediatric antiretroviral drugs for low income countries

This research was motivated by the urgent need for global health institutions like the World Health Organization and UNITAID to adopt an informed, market based approach to engaging with the research and development pipeline for drugs that treat children infected with the HIV virus. As the market size for these products declines over the next decade, the usual incentives for pharmaceutical and biotech companies to invest in the development of new drugs and new formulations of existing drugs is likely to dwindle. Innovated solutions are needed if a business case is to be made that addresses this important public health need. The objectives of the research include firstly, describing the public health need for research and development into paediatric Antiretroviral drugs; secondly describing the various stakeholders and their interests; and finally exploring and indentifying potential collaborative / partnership opportunities that can be employed to address the existing public health need while satisfying the various stakeholder interests at play.

Public-Private Partnership : countries' attractiveness and the risk of project failure

The primary objective of this thesis is to analyse the public private partnership (PPP) framework for infrastructure development in developing countries across the six regions of the world. The thesis utilises the World Bank's private participation in infrastructure (PPI) dataset for the period 1980–2014, and examines three thematic areas. The first comprises of an exploratory analysis of the PPI dataset. The second research area focuses on the relationship between countries' attractiveness for PPPs and the characteristics of the countries, including: macroeconomic and market; fiscal constraints; regulatory and governance; and experience in PPPs, by utilising the Zero-Inflated Negative Binomial and Cragg's Double Hurdle models in an attempt to model private investors' decision to engage in PPPs as separate participation and consumption decisions. The third research area employs the methodology of survival analysis to investigate the risk of failure of PPP projects based on the allocation of residual facility ownership between the partners. The thesis's primary contributions include the utilisation of a wider and more informative range of econometric methodologies which have not been previously applied to the PPI dataset, and for the first time also, provides a framework to select an appropriate structure for PPPs that will enhance project survival. A key finding of the thesis is that private investors prioritise macroeconomic and market variables, such as price stability over regulatory and governance variables, such as corruption, in their determination as to which country to engage in PPPs. Contrary to previous research, corruption was found to be of no consequence to private investors who wish to engage in PPPs even for developing countries. Another key finding is that PPP projects which confer residual ownership on the public sector have lower risk of failure than those for which such ownership is conferred on the private sector. Evidence also suggests that the size of the project and the participation of multilateral institutions in PPPs also affect the risk of project failure.

New decision support systems for Public Private Partnership projects

It is well established that one of the key contributing factors to the growth of countries’ economies is the existence of appropriate economic and social infrastructures. The setup of these infrastructures and their operation or delivery of public services associated with them have proved to be a costly and often unaffordable business for most governments. Therefore, private sector participation in the delivery of public services has been opted for by governments under Public-Private Partnerships (PPPs). Additional reasons for the public sector to choose a PPP framework to deliver public services include the public sector representatives’ lack of sufficient technical knowledge and specifications required by most projects; the required amount of investment to implement such projects is too high to be affordable by the public sector alone; the increasing difficulties of predicting and controlling the uncertainties that typically govern the phases of such projects including the level of actual demand for the related services by the time the project is operational; the level of bureaucracy in the public sector and its implications on the time frame of decision-making processes; the relatively low efficiency in project execution by the public sector; and most importantly the fact that, in practice, these difficulties are often better managed by the private sector who seem to possess both the financial and the executive capabilities to undertake such projects Currently, there is a gap in the game theory literature on modelling PPP related decisions and issues. This thesis contributes to reducing such gap by proposing three novel and practice-oriented game theoretical models along with algorithms for solving them, namely an ordinal game for the shortlisting of potential private sector partners; two non-cooperative dynamic games for negotiation with shortlisted private sector partners and selection of private sector partner to award the PPP contract to; and a non-cooperative dynamic game for financial renegotiation post-PPP contract award. In sum, each novel game addresses an important problem faced by the private sector during the typical stages of the life cycle of PPPs, namely private sector partner selection at the tendering stage; negotiation between the public sector and shortlisted bidders to select the candidate to whom to award a PPP contract to at the post-tendering stage; and financial re-negotiation between the public sector and the selected private sector partner at the post-contract award stage.

Strategies to Sustain Public Private Partnership: A Lebanese Agency Case Study

Four public private partnerships have been created in Lebanon to fulfill the promises of better public value and accelerated economic development for sustainable business development. The problem is some business owners embark on public private partnership projects without following known documented strategies that ensure business sustainability. The purpose of the single case study was to explore the strategies business owners used to sustain public private partnership businesses in Lebanon. The conceptual framework included the theory of X-efficiency and the new public management model. The Northern Lebanon public private partnership was chosen for the study. All 7 business owners participated through interviews for data collection. The emergent themes from the interviews were compared and contrasted across participants' responses and were cross referenced with the academic literature and printed agency reports. Data interpretations were triangulated through member checking. The business owners identified 7 specific strategies to monitor the agency's work. The top 3 strategies were proper selection of partners, the need for a strong technical director, and hiring of professional staff. Three additional strategies noted were the articulation of a clear mission and vision, followed by the development of bylaws and the identification of international best practices. Holding monthly partners' meetings to discuss emerging needs was the last strategy identified for consistent follow up and forward movement of the businesses. The findings over time could promote social change in Lebanon by revealing how municipalities can partner with the private sector and nongovernment organizations to reduce poverty, create jobs, and ensure local economic development.
