Perfecting Patient Bed Flow in the Emergency DepartmentMoreira, Kim-Sun 01 January 2017 (has links)
Emergency department (ED) crowding is a serious problem in the United States. Crowding in the ED can result in delays that may negatively affect patient outcomes and increase the cost of care. The purpose of this project was to understand strategies that can help to improve patient flow in the ED. The plan-to-do-study act model for process improvement influenced this project. Secondary data were collected for a 2-month period to determine the impact of workflow processes (patient boarding time in ED, surge capacity and workflow processes including the impact of ancillary departments) on the movement of admitted patients from the ED to the inpatient units. Descriptive statistics were used to provide numerical summaries, frequencies, and percentages for the identified variables. The findings were consistent with an increased length of stay and longer ED boarding of patients due to the workflow process. Resulting recommendations included standardized calls for report on admitted patients within 30 minutes, timely discharge of patients, collaboration with attending physicians to facilitate evaluation of patients and orders, modification of staffing roles to ensure adequate staff, and identification of staff transporters to ensure timely transport of patients to their rooms. The findings helped to inform the development of a Bed Utilization Policy. The policy has been shared with the organization with the recommendation to implement and further evaluate to help manage bed flow. Development of utilization strategies that contribute to facilitating throughput will promote positive social change by providing nurses with the tools to help prepare for and respond to unexpected increases in patient volume. Improving efficiency with flow can help to improve patient care, timeliness, and safety.

En bransch som använder digitala system för att minska manuellt arbete, fysiska säljprocesser och ökad försäljning. Digital Sales Room är en ny digital helhetslösning som behandlar hela säljprocessen från första kundkontakt till stängt affär. I ett Digital Sales Room finns det olika funktioner som tillsammans skapar ett värde för både användaren och kunden. Eftersom det är en ny digital lösning är det viktigt att undersöka till vem ett Digital Sales Room anpassas till. Syftet med uppsatsen var att forska kring värdet av funktionerna i ett Digital Sales Room som är mest värdefullt för säljare, säljchefer och VD. Vidare ta reda på om ett Digital Sales Room är attraktivt för försäljningsföretag. Digital Sales Room är en ny kategori inom försäljning som innehåller ett antal olika funktioner för att effektivisera företags säljprocesser. Det kan brytas ner i olika funktioner: Dokumentutskick, redigera efter utskick, Videofunktion, chattfunktion och digital signering. Från en kvantitativ enkätstudie på fem företag med olika roller sammanställdes resultatet i olika diagram som även kodades och analyserades. Analysen utfördes genom en jämförelse mot litteraturstudien samt hur de olika rollerna ansåg varje funktion värdefull för sin arbetsroll. Därifrån identifierades var värdet i ett Digital Sales Room fanns och hur helheten kan fungera för ett försäljningsföretag. Studien resulterade i att det finns ett stort värde av ett Digital Sales Room för de olika rollerna där vissa funktioner var mer värdefulla än andra. En stor majoritet ansåg att helheten med ett Digital Sales Room som värdefullt för deras roll. / Digitalization helps organizations in Sweden to evolve. A Digital Sales Room is a new category that helps efficiently the whole sales process. From first customer interaction to signed agreement. There are a couple of functionalities in a Digital Sales Room that creates value for both the user and the customer. Because it's a new digital solution, the importance of discovering who a Digital Sales Room is adapted for is highly relevant. The purpose of this essay was to study about what functionality of a Digital Sales Room that is most valuable for sales reps, sales managers and CEO:s. Thereby to discover if a Digital Sales Room is attractive to sales oriented organizations. Since a Digital Sales Room is a new digital solution, we are breaking down the system into smaller functionality, which is: Document send out, edit after send, video integration, chat function and digital e-signing. From a quantitative survey on five different titles, we gathered the results and created diagrams that also were analyzed. The analysis of this study was made to compare the results with the literature on the different roles to find out what different functionality is valuable for each role. Therefore, the most valuable part of a Digital Sales Room was identified and how the whole system can be used in a sales-oriented company. The conclusion is that a Digital Sales Room generate a value for the different roles where some functions are more valuable than others. The majority considered the whole totality of a Digital Sales Room valuable.

Syftet med denna uppsats var att undersöka hur manliga mellanchefer handlade, i sin strävan att förena arbetsliv och familjeliv. Två tidskrävande delar av männens liv, som båda ställer krav, och påverkar dem, i deras möjlighet till handlande. Metoden vi valt är kvalitativ och vi genomförde intervjuer med fem mellanchefer, i Stockholmsområdet. De fick under intervjun berätta om sitt liv som pappa och chef. Vi såg att samhällets normer, och företagens policys, påverkade männen i deras handlande. För att bemöta de krav som dessa yttre faktorer ställde på dem skapade de strategier. Vi såg att de hade olika många strategier att ta till, beroende på vilka krav de ställdes inför. Hur många strategier de kunde välja mellan påverkades av deras handlingsutrymme. Vi såg också att i vissa situationer agerade männen lika när de ställdes inför liknande problem, oberoende av varandra. Vi fann även stöd för att männen, i större utsträckning än sina fruar, fick betalt för det arbete de utfört. / The purpose of this study was to investigate how male middle managers acted, in their efforts to balance work and family life. These two, time-consuming, parts of men's lives, both demand and influence them, in their ability to act. The method we have chosen is qualitative and we conducted interviews with five middle managers, in the Stockholm area. During the interview they got to talk about their roles as fathers and managers. We saw that social norms and business policies affected the men in their actions. To meet the requirements that these external factors put on them, they created strategies. We saw that they had a different number of approaches to choose from, depending on the demands they faced. How many strategies they could choose from were affected by their room for maneuver. We also saw that in some situations, when they were faced with similar problems, the men acted similarly, independently. We also found support for the fact that the men, to a greater extent than their wives, got paid for the work they did.

Inledning: Arbetsrummet är något som psykodynamiskt skolade psykoterapeuter medvetet förhåller sig till och använder sig av på olika sätt i professionen. Syftet med den här studien var att belysa några psykoterapeuters tankar om betydelsen av arbetsrummets funktion och utformning. Frågeställningarna utgår ifrån vad utformningen på arbetsrummet har för betydelse för den enskilde terapeuten och den psykoterapeutiska processen. Metod: I studien användes en kvalitativ metod och fem legitimerade psykoterapeuter med psykodynamisk inriktning intervjuades. Resultatet visar att de intervjuade psykoterapeuterna har mycket kunskap kring betydelsen av det terapeutiska rummet och dess utformning. De anpassar sig efter rummets arkitektur och försöker få till inredningen på bästa sätt för sig och patienten. De är måna om att patienten har det bra. Möblerna och framförallt fåtöljerna är viktiga i arbetet. De använder sig medvetet av textilier, konst, belysning och naturligt insläpp av ljus samt utsikt genom fönster. Ostördheten anses vara det viktigaste och de byter ogärna rum. Psykoterapeuterna ser sig som en del av rummet de arbetar i och rummet är en del av helheten. Den psykoterapeutiska processen påverkas alltså av arbetsrummet. Diskussion: Den enskilde psykoterapeuten använder mycket tid till att fundera ut vad som fungerar bäst i ett arbetsrum inför mötet med patienten och det krävs en flexibilitet för att kunna möta olika patientgrupper och olika typer av verksamheter. Trots det är det något som man inte pratar så mycket om eller lyfter fram i det psykoterapeutiska arbetet. Det är inte ovanligt att man är flera som delar på samma rum, såväl terapeuter som andra verksamheter och att man därför tillfälligt behöver byta rum. Det finns forskning kring vårdmiljö och vårdrummets påverkan på läkningsprocess, men ingen aktuell forskning på det psykoterapeutiska arbetsrummets utformning. Det kan finnas en risk att rummet glöms eller prioriteras bort i samband med inrättandet av olika verksamheter inom kommuner och landsting. / Introduction: Primarily psychodynamic trained psychotherapists relate to and use the workroom in various ways in their profession. The aim of the study was to highlight some psychotherapists thoughts about the importance of function and design in a workroom. The questions are focused on how the room’s interior design is of significance to the individual therapist and the psychotherapeutic process. Method: The study is based on a qualitative approach and five certified psychotherapists with a psychodynamic orientation were interviewed. The result shows that the interviewed psychotherapists have much knowledge on the importance of the therapeutic space and its’ design. They adapt the architecture and try to arrange the interior of the room in the best way for themselves and the patient. Making sure the patient is comfortable. The furniture and especially the armchairs are important in the process. They use textiles, art, lighting and the natural light and different views provided by windows. To be undisturbed is considered most important and they are reluctant to change rooms. The psychotherapists see themselves as part of the room they work in and the room as part of the whole. The room thus affects the psychotherapeutic process. Discussion: The individual psychotherapist uses a lot of time to figure out what works best in an office before meeting the patient and he or she requires a flexibility to meet different patient groups and different types of activities. Nevertheless, it is something that one does not talk so much about or highlight in psychotherapeutic work. It has become common to share rooms with other therapists as well as other activities, leading to a need for temporarily changing rooms. There is research on environmental health and care room influence on the healing process, but no current research on the psychotherapeutic workroom design. There may be a risk that the workroom is forgotten or is not prioritized when establishing various health care-businesses in the county and municipalities.

The aim of the thesis was to describe the main differencies between an interrogation of adults and children. The thesis is composed of six chapters, an introduction and a conclusion. The first chapter is focused on concept "a child" in the Czech legal order and International agreements. The next chapter describes specifics and problems of individual evolutionary periods of children. The third chapter explains the interrogation in a general meaning. The interrogation is included and modified in more statutes. I focused on provisions of a statute relating the interrogation in Czech law, especially in criminal law. Next part of this chapter deals with the interrogation and a criminalistics. How to prepare for the interrogation, the stages of the interrogation and tactics of its implementation. I also mentioned the forensic psychology which helps and supplements the criminalistics with its own methods and findings. The fourth chapter is an analysis of a testimony, in a first part in general meaning and the second part describes a process of formation of the testimony and the factors which can influence this process from an event to a record by an interrogator.
