Sjuksköterskans och den sexuellt utsatta kvinnans upplevelse att möta varandra i vården. : En litteraturstudie / The nurse's and the sexually abused woman's experiences of their meeting in healthcare : A literature review

Bakgrund: Sexuellt våld gentemot kvinnor är ett globalt hälsoproblem som bidrar till ett stort lidande. Att som sjuksköterska tillföra en god och positiv attityd gentemot kvinnor som blivit utsatta för sexuella övergrepp är viktigt för att skapa en trygghet mellan den utsatta och sjuksköterskan. En viktig del för sjukvårdspersonalen är träning som ökar kunskapen för att hitta hjälpsamma metoder och verktyg vid möte av dessa sexuellt utsatta kvinnor. Syfte: Studiens syfte är att belysa utifrån befintlig forskning, sjuksköterskans och den sexuellt utsatta kvinnans upplevelser av att mötas i vårdrelation. Metod: En litteraturstudie som baserades på 10 kvalitativa artiklar som besvarade författarnas syfte granskades och analyserades. Resultat: Utifrån de kvalitativa artiklarna valdes två huvudrubriker; sjuksköterskans upplevelse samt kvinnans upplevelse. Inom sjuksköterskans upplevelse delades de in i ytterligare två underrubriker; sjuksköterskans perspektiv och sjuksköterskans kunskap. Här belystes hur sjuksköterskorna upplevde att möta kvinnorna som blivit utsatta för sexuella övergrepp och vikten av rätt kunskap för att ge en god vård. Inom kvinnans upplevelse delades huvudrubriken upp i två underrubriker; kvinnans upplevelse av att bli bemötta i vården av SANE och kvinnans upplevelse av att utveckla PTSD. Här belystes kvinnors positiva upplevelser i att möta en SANE sjuksköterska samt hur sexuella övergrepp påverkar kvinnors psykiska hälsa och utveckling av PTSD. Konklusion: Mötet mellan sjuksköterskan och den utsatta kvinnan var psykisk påfrestande för båda parterna. Sjuksköterskan är i behov av rätt kunskap kring sexuellt utsatta kvinnor och starka känslor väcks till liv för sjuksköterskan. Med rätt kunskap kände sig sjuksköterskorna mer trygga i sin roll samt förmedlade en trygghet till den utsatta kvinnan.   Nyckelord: Kunskap, psykisk ohälsa, sexuellt utsatta kvinnor, sjuksköterska och upplevelse. / Background: Sexual violence against women is a global health problem that contributes to huge amount of suffering. As a nurse adding a good and positive attitude towards women who have been sexually abused is important in creating a sense of security among themselves. An important part of the healthcare personnel is training that increases the knowledge to find helpful methods and tools when meeting these sexually abused women. Aim: The purpose of the study was to highlight based on existing experience the nurse's and the sexually abused woman's experiences of meeting in a healthcare relation. Method: A literature study based on 10 qualitative articles answering the author´s purpose was reviewed and analysed. Results: Based on the qualitative articles, two main headings were selected; the nurse's experience as well as the woman´s experience. Within the nurse's experience, they were divided into two additional sub-headings; the nurse's perspective and the nurse´s knowledge, here it was highlighted how the nurses experienced meeting the women who have been subjected to sexual abuse and the importance of the right knowledge to provide good care. Within the women's experience, the main headings was divided into two sub-headings; the woman's experience of being treated in the care of SANE and the woman's experience of developing PTSD. Here, women´s positive experiences in meeting a SANE nurse were highlighted as well as how sexual abuse affects women's mental health and the development of PTSD. Conclusion: The meeting between the nurses and the affected woman was mentally stressful for both parties. The nurse is in need of adequate knowledge about sexually abused women and strong emotions are brought to life for the nurse. With the adequate knowledge, the nurses felt more confident in their role and created a sense of security to the exposed woman.  Keywords: Experience, knowledge, mental ill health, nurse and sexually assaulted women.

The purpose of this study is to increase knowledge about how counselors at youth centers deal with emotional burdens like to working with sexually exposed young people. Previous studies have shown that working with trauma can lead to secondary traumatization. There is a particular risk of secondary traumatization if you encounter repeated trauma stories. Counselors at youth centers meet young sexually exposed people for treatment talks.Through semi-structured qualitative interviews with six counselors from six different youth centers the study has observed several coping strategies that the counselors use. These strategies can be categorized into two different themes; individual coping strategies and coping strategies that the organization offers. The individual strategies can be categorized into emotion focused coping strategies and problem focused strategies. The study showed that the councelors mostly used emotion focused strategies like acceptance, distancing, avoiding and social support. The study also showes that the councelors used problem focused strategies like self care, having meaningful leisure time, demand-reducing strategies and setting limits. The study also showes that counselors don´t know of any target documents or routines for managing emotional stress at work.
