Background: Telephone counseling is a multifaceted profession, which makes it possible to perform nursing care without meeting the care seeker physically. Nurses supports, assesses, guides and provides care seekers with self-care advice, which is of great importance to avoid health problems and complications for the care seekers. Aim: The aim of this study was to describe nurses' experiences of telephone counseling within health centers and 1177 Vårdguiden. Method: A method aimed to contribute to evidence-based care based on the analysis of qualitative research described by Friberg (2012), was used. Eleven articles were analysed and resulted in four categories, with fourteen sub-categories. Results: The four categories that emerged were; the importance of own knowledge and security, to face ethical dilemmas, stress and job demands and the use of decision support. The lack of visual contact requires specific knowledge by the nurses and communication with healthcare seekers from other cultures and with third parties to some extent created uncertainty. The nurses must be quick and do right in limited time. Decision support was perceived as both an obstacle and complement. Conclusion: Nurses´ experiences of working with telephone counseling was described as stressful, demanding but also positive and challenging. In order to best perform telephone counseling it required experience, vast competence and ability to apply knowledge. It was considered important to use oneself as a main asset and to be aware of strengths, weaknesses and reactions in different situations.

Geometry-based stochastic physical channel modeling for cellular environments

Telecommunication has experienced significant changes over the past few years and its paradigm has moved from wired to wireless communications. The wireless channel constitutes the basic physical link between the transmitter and the receiver antennas. Therefore, complete knowledge of the wireless channel and radio propagation environment is necessary in order to design efficient wireless communication systems. This PhD thesis is devoted to studying the spatial and temporal statistics of the wireless channel in cellular environments based on a geometry-based stochastic physical channel modeling approach. Contributions in this thesis report include the following: ??? A new physical channel model called the eccentro-scattering model is proposed to study the spatial and temporal statistics of the multipath signals in cellular environments. ??? Generic closed-form formulas for the probability density function (pdf) of angle of arrival (AoA) and time of arrival (ToA) of the multipath signals in each cellular environment are derived. These formulas can be helpful for the design and evaluation of modern communication systems. ??? A new Gaussian scattering model is proposed, which consists of two Gaussian functions for the distribution of scatterers around base station (BS) and mobile station (MS) and confines these scatterers within a scattering disc. ??? The effect of mobile motion on the spatial and temporal statistics of the multipath signals in cellular environments is discussed. Three motion scenarios are considered for the possible trajectory of the mobile unit. Furthermore, two different cases are identified when the terrain and clutter of mobile surrounding have additional effect on the temporal spread of the multipath signals during motion. ??? The physical channel model is employed to assess the performance of a RAKE receiver in cellular environments. ??? Comparisons between uniform scattering and Gaussian scattering, which are the two assumptions for the distribution of scatterers usually used in the derivation of the pdf of AoA, are also presented. ??? An overview of earlier physical channel models and comparisons between these models and with the proposed model are presented.

Crisis telephone counselling : an exploratory study of outcomes

This exploratory study examined outcomes of single telephone counselling calls, with a self-selected sample of 38 clients from a family counselling agency. The sample was interviewed by telephone at two days and six weeks after the call to assess crisis counselling outcomes in affect, identification of the problem and action taken, and client equilibrium. Repeated measures of the 'perceptual concordance' of counsellor and client were taken over a period of six weeks, assessing levels of concordance between client and counsellor about perceptions of counselling, and client equilibrium. The sufficiency of a single counselling session was also assessed. 'Perceptual concordance' was suggested by reduced client stress, high levels of agreement between client and counsellor in the identification of the problem and agreed action, and satisfaction with the counselling. Positive indicators of restored client equilibrium included a maintenance of lower stress levels, changes in behaviour, improvement in perceptions of the seriousness of the problem, satisfaction with life and with the counselling. Further research of equilibrium as a concept, and an indicator of crisis resolution is warranted. The sufficiency of a single session of counselling was supported by 56% of clients. The variety of services used by clients as an outcome of the counselling suggests that it could be beneficial for telephone counselling agencies to offer a follow-up call. Verifying the agreed action and assessing equilibrium could be useful indicators of effective telephone counselling.

<p>The aim of the study was to describe what district nurses, who work with telephone- advice, document in patient record, to be able to develop their documentation. The Authors have examined 50 patient records. The examination tool is collected from “Lokal anvisning för hälso- och sjukvården I Södra Älvsborg”. The audit areas was record keeping, review and planning, realization, individual nursing and the patients participation, information, education and agreement. The sample was stratified to avoid examining only one district nurses´ documentation. The sample was from, the five first and the five last telephone-advices that was documented in patient record, the five first days in July 2006. The result of the audit shows a lack of documentation in nursing record with telephone-advice, for example the patients problems is missing, it can lead to a poorer security of the patient. The Authors conclusion is that the district nurses who works with telephone-advice needs education in documentation, to increase and improve security of the patient in nursing and treatment.</p> / <p>Syftet med studien var att beskriva vad distriktssköterskor dokumenterar i samband med telefonrådgivning för att kunna utveckla distriktssköterskors omvårdnadsdokumentation vid telefonrådgivningssituationer. Författarna har granskat totalt 50 patientjournaler. Granskningsdokumentet som använts är hämtat ur ”Lokal anvisning för Hälso- och sjukvården i Södra Älvsborg”. Den omfattar granskning av journalföring, bedömning och planering, genomförande, individuell omvårdnad och den vårdsökandes delaktighet och information, undervisning och överenskommelse. Urvalet var stratifierat, de fem första och de fem sista telefonrådgivningarna de fem första dagarna i juli månad 2006, för att undvika att granska enbart en distriktssköterskans dokumentation. Resultatet av granskningen visar på bristande dokumentation i omvårdnadsjournalen i samband med telefonrådgivning inom samtliga granskningsområden t ex så saknas dokumentation om den vårdsökandes aktuella behov. Det kan leda till att patientsäkerheten försämras. Författarnas slutsats är att distriktssköterskor som arbetar med telefonrådgivning har behov av utbildning vad gäller dokumentation i samband med telefonrådgivning för att förbättra och öka patientsäkerheten i vård och behandling.</p>
